신경작용제 해독제의 약리기전 및 연구개발 (Investigation of the Pharmacological Mechanisms and the R&D of Medical Countermeasures Against Nerve Agent Poisoning)

  • Nerve agents are irreversible inhibitors of the cholinesterase enzyme. Exposure causes a progression of toxic signs, including hypersecretions, fasciculations, tremor, convulsions, respiratory distress, epileptiform seizures, brain injuries and death. A combined regimen of prophylaxis and therapy is the most effective medical countermeasure for dealing with the threat of nerve agent poisoning to military personnel. In this paper, the author investigated the updated technologies regarding various pre- and post-treatment drugs for nerve agents detoxification which are under development in several countries including Korea. Some characteristics of active ingredients in the formulations of drugs, their action mechanisms, and effectiveness were analyzed. Additionally, part of experimental data on the transdermal patch studied in ADD using beagle dogs was introduced.

소아에서 aminoglycoside의 1일 1회 요법 (Once daily dosing of aminoglycoside in children)

  • Aminoglycosides are frequently used antibiotics in children. The multiple daily dosing (MDD) in infants and children is twice or three times daily depending on age. Recent studies in adults have shown that once daily dosing (ODD) maximizes the bactericidal activity and might minimize the toxicity of antibiotics. So, I reviewed many studies about efficacy, toxicity and cost effectiveness of ODD of aminoglycosides in children. Most studies suggest that ODD compared with MDD of aminoglycosides is theoretically more efficacious and has no higher toxicity in infants and children. But, the total number of patients included in the studies is not large. Multi-center, controlled prospective studies are required in larger numbers of infants and children to determine the efficacy and safety of the ODD regimen in children before ODD of aminoglycosides can be recommended for routine use.

반코마이신과 프로베네시드의 약물동태학적 상호작용 (Pharmacokinetic Interaction of Vancomycin and Probenecid in Rabbits)

  • This study was attempted to investigate the pharmacokinetic interaction of vancomycin (10 mg/kg, i.v.) and probenecid (7.5. 15, and 30 mg/kg, oral) in rabbits. The area under curve (AUC) of plasma vancomycin concentration was significantly increased (p<0.01) in rabbits when the probenecid was coadministrated. Volume of distribution (Vd) was significantly decreased (p<0.05) in rabbits coadministrated with probenecid (15 and 30 mg/kg) and total body clearance (CLt) was decreased significantly (p<0.05. p<0.01) in rabbits coadministrated with probenecid (7.5, 15 and 30 mg/kg). There was significant correlation between AUC and probenecid dose. From the results of this experiment, it is desirable to adjust dosage regimen of vancomycin for reduction of side or toxic effect when the probenecid is coadministered in clinical practice.

BK virus-associated hemorrhagic cystitis after pediatric stem cell transplantation

  • Hemorrhagic cystitis is a common stem cell transplantation-related complication. The incidence of early-onset hemorrhagic cystitis, which is related to the pretransplant conditioning regimen, has decreased with the concomitant use of mesna and hyperhydration. However, late-onset hemorrhagic cystitis, which is usually caused by the BK virus, continues to develop. Although the BK virus is the most common pathogenic microorganism of poststem cell transplantation late-onset hemorrhagic cystitis, pediatricians outside the hemato-oncology and nephrology specialties tend to be unfamiliar with hemorrhagic cystitis and the BK virus. Moreover, no standard guidelines for the early diagnosis and treatment of BK virus-associated hemorrhagic cystitis after stem cell transplantation have been established. Here, we briefly introduce poststem cell transplantation BK virus-associated hemorrhagic cystitis.

메밀급여가 Streptozotocin유발 당뇨쥐의 장기무게 및 당질과 지질대사에 미치는 영향 (Effects of Buckwheat on Organ Weight, Glucose and Lipid Metabolism in Streptozotocin-Induced Diabetic Rats)

  • The present study was undertaken to evaluate the effects of 50% buckwheat diet on the body weight, organ weight, urine albumin, urine glucose, plasma glucose and plasma lipid in normal rats and diabetic rats treated with streptozotocin(STZ). The food intake, body weight, the level of urine glucose in diabetic buckwheat groups were not significantly different with diabetic control group. The level of urine albumin was lower in raw and steam buckwheat group than in the diabetic control group. Compared to the normal control group, liver and kidney weights were heavier in the diabetic groups. Pancreas weight was heavier diabetic buckwheat groups than in normal and diabetic control groups. Fasting plasma glucose level of diabetic buckwheat groups significantly decreased by 18-37% compared with the diabetic control group. Plasma triglyceride level of diabetic buckwheat groups significantly decreased by 34-50% compared with the diabetic control group. Plasma total cholesterol level of diabetic buckwheat groups decreased by 15-27% compared with the diabetic control group. The level of HDL-cholesterol was not affected by buckwheat diet. These results indicate that buckwheat is an effective therapeutic regimen for the control of metabolic derangements in diabetics.

푸로푸라놀롤과 안지오텐신 차단제와의 체내 상호작용 (Pharmacokinetic Interaction of Propranolol and Angiotensin Inhibitor in Rabbits)

  • Effect of an angiotensin inhibitor (AAS; loading dose of 25, 50, $100{\mu}g/kg$ and maintenance dose of 12.5, 25,$50{\mu}g/ka/hr$) on the pharmacokinetics of propranolol (4 mg/kg i.v.) was studied in rabbits. Plasma concentrations and relative bioavailability of propranolal increased upto 127-175% by AAS coinjection. The urinary excretion of propranolol decreased by AAS. Dosage regimen of propranolol should be adjusted when it is administered with AAS.

  • This paper was attempted to study on the effect of metoclopramide or propantheline on the absorption rate and bioavailability and pharmacokinetic parameter of fluorouracil in rats and rabbits. The result are as follows; Metoclopramide increased the absorption rate of fluorouracil but propantheline decreased in situ experiment with rat small intestine. Metoclopramide increased the blood level and relative bioavailability and absorption rate constant of fluorouracil in rabbits. Prolonged the peak blood level (tmax) and decreased the absorption rate constant (Ka) but did not affect the blood level and relative bioavailability of fluorouracil in rabbits. As a matter of fact, it is considerd that the coadministration of metoclopramide or propantheline is more desirable than the single administration of fluorouracil for available dosage regimen.

  • Headaches are common in children and become more common and increase in frequency during adolescence. There are various causes of headaches. The majority of cases are considered as primary and include migraine and tension headaches. The rational evaluation of headache begins with careful history. Migraine is genetically determined recurrent pain syndrome accompanied by neurological and gastrointestinal features, involving interaction of external triggers and internal pathophysiology and the causes of considerable disability to suffers. Establishing the correct diagnosis is essential for successful treatment. Treatment of pediatric migraine includes an individually tailored regimen of both nonpharmacologic and pharmacologic measures.

돼지감자 및 치커리 섭취가 흰쥐의 지질대사에 미치는 영향 (Effects of Jerusalem Artichoke and Chicory on Lipid Metabolism in Rats)

  • This study was carried out to determine the effects of Jerusalem artichoke (JA) powder , JA extract and chicory extract on lipid metabolism in SD rats. The experimental groups were divided into 4 groups ; control, JA powder JA extract and chicory extract. The animals were fed ad libitum each of the experimental diets for 3 weeks. After 3 weeks, the wet weights of cecum were significantly increased in rats fed JA powder and chicory extract. Cecal contents were slightly increased in all experimental groups. Serum HDL-cholesterol, HDL-cholesterol/total cholesterol ratio and atherogenic index were significantly increased in the chicory extract group. Serum triglyceride, total cholesterol and LDL-cholesterol levels were not different among the diet groups. Although the feeding of chicory extract significantly lowered total lipid of liver, there was no difference in levels of triglyceride and total cholesterol. The content of fecal lipiid and cholesterol were significantly higher in the Ja extract and chicory extract group than other groups. Fecal bile acid was significantly increased in the chicory extract group. These results indicate that chicory extract is an effective regimen for improvement of lipid metabolism in SD rats.

허혈/재관류 환경하에서 청폐사간탕의 염증 관련 반응 억제 효과 (Inhibitory Effects of Chungpesagan-Tang on Ischemia/Reperfusion-Induced Inflammatory Responses In vitro)

  • Chungpesagan-Tang (CST), a Korean traditional prescription composed of oriental medical herbs, has been used successfully to improve human health and regimen. This study was designed to examine the inhibitory effects of CST against in vitro ischemia/reperfusion-induced inflammatory response. We have characterized the production of prostaglandin $E_2$ and arachidonic acid during ischemia/reperfusion in the human neuroblastoma SK-N-MC and human monocytic macrophage U937 cells and the inhibitory effect of CST on these inflammation-related substance formation has been found out in this study. This result suggested that CST used in this experiment reinforced antiinflammatory potentials and protected cells against ischemia/reperfusion-induced inflammatory resopnse.
