목적: 본 연구는 수술 전 방사선화학요법을 시행 받은 국소진행된 직장암환자에서 치료 전 얻어진 직장암 생검조직 내에서의 cathepsin D(CD) 및 p53의 발현이 갖는 예후적 인자를 포함한 임상적 의의에 대하여 알아보고자 하였다. 대상 및 방법: 1993년 7월부터1999년 6월까지 국소진행된 직장암으로 수술 전 방사선화학요법을 시행 받은 127명의 환자 중 근치적 수술을 시행 받았고 수술 전 생검 조직의 파라핀 블록을 얻을 수 있던 89명의 환자를 대상으로 하였다. 방사선치료는 하루 1.8 Gy씩 주 5회, 25회에 걸쳐 전 골반 부위에 45 Gy까지 조사한 후에 원발종양 부위에만 5.4 Gy 추가 조사하였다. 화학요법은 5-Fluorouracil과 Leucovorin을 5일간 일시 정주하였고 방사선치료기간 1주 및 5주째 투여하였으며 방사선치료 종료 후 약 6주경에 근치적 절제술을 시행하였다. Mouse 단클론 항체인 CD와 p53을 이용하여 면역염색을 시행하였다. CD 및p53이 종양세포 및 종양기질세포에 10% 이상 발현 시 양성으로 판정하였다. 결과: 종양세포내 CD 발현은 57명(64%)이 양성이었고 종양기질세포내 양성은 71명(80%)이었다. p53의 종양세포내 발현은 35명(39%)이 양성을 보였다. 성별, 환자나이, 종양의 운동성, 종양의항문 연으로부터의 거리, 임상적 TNM 병기에 따라 종양세포 및 종양기질세포내 CD 발현 및 p53 발현의 통계적 차이가 없었다. 종양세포와 종양기질세포내 CD발현 사이에 양적인 상호 연관성이 있었다(p=0.01). 종양세포내 CD발현과 p53의 발현은 통계적인 상호관련성이 없었다(p=0.79). 단변량분석 및 다변량분석 상 5년 전체생존율 및 무병생존율에 대한 예후인자로서 종양세포 및 종양기질세포내 CD 발현과 p53은 통계적 유의성이 없었다. 그러나 종양세포 및 종양기질세포내 CD 발현에 따른 하위집단 생존율 분석상 종양세포 및 종양기질세포내 CD 발현이 음성인 경우가 5년 전체생존율이 75%로서 가장 높았으며 종양기질세포내에만 CD가 발현된 경우는 5년 전체생존율 29%로서 가장 낮았다(p=0.01). 결론: 수술 전 방사선화학요법을 시행 받은 국소진행된 직장암 환자에서 치료 전 직장암 생검조직 내에서의 p53 발현은 유의한 생존예후인자는 아니었으며 CD가 종양세포에는 발현되지 않고 종양기질세포에만 발현 시 전체생존율에 불량한 예후인자였다.
The purpose of this study was to identify nursing interventions performed by pediatric care unit nurses. For data collection this study used the taxonomy of Nursing Intervention Classification (NIC : 486 nursing intervention) which was modified by McCloskey & Bulecheck(2000). The 419 nursing interventions were selected by panel group, which consist of pediatric clinical experts and nursing scholars. The data were collected 104 nurses of pediatric care unit. There were 158 nursing interventions identified as being used at least monthly 50% or more of the nurses. The 32 nursing interventions were used at least daily, indication a set of core interventions unique to pediatric care unit practice. The most frequently used nursing interventions were 'Medication administration: intravenous' & 'Medication administration: oral'. The 27 nursing interventions were rarely performed by 90% or more of the nurses. The rarely used interventions were 'Ostomy care' & 'Rectal prolaps management'. The domain received the highest mean score was Health System, followed by Physiolocal: complex, Physiolocal: Basic, Safty, Community, Family, Behavior domain. These findings will help in building of a standardized language for the pediatric care units and enhance the quality of nursing care.
Due to the limited bioavailability of $[D-Ala^6]$LHRH from nonparenteral transmucosal sites of administration, enhancement of mucosal permeability by coadministration of several protease inhibitors and/or penetration enhancers were studied in rabbit mucosa. As a reliable bioassay method for $[D-Ala^6]$LHRH, ovulation-inducing effect were measured after vaginal administration in the rat. The permeation of $[D-Ala^6]$LHRH through the mucosal membrane of rabbit mounted on George-Grass diffusion cells were examined in the presence of polyoxyethylene 9-lauryl ether (POE), ${\beta}$-cyclodextrin$({\beta}-CyD)$ or ethylene diamine tetra acetate disodium salt(EDTA). The vaginal membrane showed higher permeability of $[D-Ala^6]$LHRH than the rectal and nasal membrane. POE and ${\beta}-CyD$ showed a small promoting effect on the membrane permeation of $[D-Ala^6]$LHRH, but EDTA showed significant enhancement. Ovaluation was enhanced by the coadministration of sodium laurate(0.5%), a protease inhibitor but was not enhanced by EDTA, a penetration enhancer. On the other hands, coadministration of sodium tauro 24,25 dihydrofusidate(1%) and EDTA(2%) enhanced the ovulation inducing-effect 2.8 times. These results suggest that the vaginal administration of $[D-Ala^6]$LHRH with STDHF or sodium laurate as a protease inhibitor, and EDTA as a penetration enhancer, may become an elective method for transmucosal delivery of $[D-Ala^6]$ LHRH.
In this study, the effect of daily short-distance walking exercise on milk production and metabolic status of the lactating dairy cows was investigated. The experiment was conducted with 10 lactating dairy cows comprising 2 groups (5 dairy cows/group), which were walking activity and non-walking activity. The walking activity (WA) group lactating dairy cows walked outdoors for 1 km/d, and the non-walking activity (non-WA) lactating dairy cows were maintained within a cowshed from $18^{th}$ April to $30^{th}$ May. The activity volume of the neck was not significantly different between the groups, but the activity volume of the leg was higher in WA group compared with non-WA (p<0.05). The rectal temperature was higher in the WA group ($38.6^{\circ}C$) compared with non-WA ($38.2^{\circ}C$) (p<0.05). No significant differences in dietary NE intake, milk production and milk composition were found between the groups. The plasma cortisol concentration was higher in WA group ($2.14{\mu}g/dl$) compared to non-WA ($0.95{\mu}g/dl$) (p<0.05). However, plasma cortisol level of WA dairy cows was significantly decreased as compared with non-WA cows. On the contrary, the level of plasma melatonin was increased in WA cows than that of non-WA dairy cows. It may be interpreted as a change in physical fitness. From this study, we suggest that walking activity can be improved energy balance. Further investigation is needed to determine whether different combinations of distance, rate or doing in morning or afternoon will stimulate energy balance.
This study was to decided the proper garment pressure level on the human body parts. Six volunteers (female: 30-40years) put on the same types of bands, a brief, and a non-woven gown. Garment pressure was measured in regular order with the elastic band on the human body parts such as the upper arm, the waist, the thigh, and the calf. At the same time, physiological responses such as the skin blood flow rate on 2 fingers, 7 different skin temperatures, rectal temperature, heat rates, and subjective responses about the pressure sensation, thermal sensation, and humidity sensation were measured and inquired. The results were as follows; 1. The thicker subcutaneous fat thickness, the higher the mean garment pressure on pressurizing the upper arm(p<.001). Also the thicker subcutaneous fat thickness. the thicker the upper arm circumference. 2. Heart rates increased pressured the upper arm and decreased pressured the waist, the thigh, and the calf. The higher the garment pressure, the higher heart rates on all body parts were pressured. Especially lean subjects showed higher physiological load than others. 3. On pressurizing the upper arm, heart rates, rectal temperature, and mean skin temperature were higher than without pressured state and pressured other body parts.4. The proper garment pressure levels were decided 30gf/$cm^2$ for fat people, 20gf/$cm^2$ for others on the upper arms and 24gf/$cm^2$ on the calf.
The study was carried out to develop useful formulation for omeprazole(OMP) through OMP-ethylendiamine complex(OMPED), and the pharmaceutical properties of formula were tested to find out the difference in vivo behaviors of formulations between the free and complexed OMP. Oral and suppository dosage forms were also formulated and the dissolution profiles and pharmacokinetic parameters were measured to observe the difference in bioavailability between the free and complex form, and the correlation between dissolution rate and bioavailability was evaluated. The results are summarized as follows; In the case of formulation for oral administration, the release of OMP from enteric OMPED pellets was found satisfactory to the requirement standard and no decomposition of OMP in the pellets was found in acidic solution. Therefore the enteric OMPED pellets are anticipated to be a stable formulation. The release of OMP from OMPED tablet with chitosan as excipient and coated with cellulose acetate phthalate was found to be significantly retarded. The results of bioavailability test for OMP and OMPED tablets with lactose-excipient showed that the AUC value of OMP tablet was $116.89\;{\mu}g\;{\cdot}\;min/ml$, that of OMPED tablet was $161.10\;{\mu}g\;{\cdot}\;min/ml$, respectively. The reason why was thought that OMP decomposes more readily in body than OMPED, and the AUC of the tablet with chitosan-excipient and coated with cellulose acetate phthalate was most enhanced. In the case of bioavailability for suppositories with OMP, $OMP-{\beta}\;-cyclodextrin$ complex and OMPED, the AUC of OMPED suppository was most increased. From the above results, it is thought that the more stable and bioavailable oral or rectal dosage forms could be developed by using the OMPED as a potential OMP complex.
Objective: The objective of the study was to determine the relationship between plasma and salivary cortisol concentrations in beef cattle that were subjected to handling prior to sampling. Methods: Twenty-one Nguni cows of three age categories; 5 to 7 yr (n = 7), 8 to 10 yr (n = 6), and 11 to 13 yr (n = 8) were handled for five consecutive weeks. In the pen, a human avoidance test was performed and cattle responses to restraint in the chute and crush were observed. In addition, rectal temperature readings were taken and, faecal samples were collected and analysed for glucocorticoid metabolites. Through the handling and restraint process, excretory and vocalisation behaviour, as a sign of stress were observed and recorded. Thereafter, six cows were randomly selected and subjected to an adrenocorticotropic hormone (ACTH) challenge. Blood and saliva samples were extracted to determine cortisol concentrations. Results: Repeated handling affected (p<0.05) faecal glucocorticoid metabolites, rectal temperatures, avoidance distance, crush scores as well as urination and defaecation behaviour. Acclimation to handling was variable based on each respective parameter. Saliva cortisol concentrations increased and decreased significantly (p<0.001). A peak value of $136.78{\pm}15.869nmol/L$ was observed 30min after administration of ACTH, from a baseline value of $8.75{\pm}15.869nmol/L$. Plasma cortisol concentrations did not differ (p>0.05) across the time of sampling. A low and insignificant correlation (r = 0.0131, p>0.05) between plasma and saliva cortisol was therefore observed. Conclusion: We conclude that if beef cows are subjected to handling prior to sampling, a weak relationship exists between plasma and salivary cortisol levels.
5-amino salicylic acid (5-ASA) is a standard therapy for the treatment of mild to moderate forms of inflammatory bowel diseases (IBD) whereas more severe forms involve the use of steroids and immunosuppressive drugs. Hyaluronic acid (HA) is a naturally occurring non-sulfated glycosaminoglycan that has shown epithelium protective effects in experimental colitis recently. In this study, both 5-ASA (30 mg/kg) and HA (15 mg/kg or 30 mg/kg) were administered rectally and investigated for their potential complementary therapeutic effects in moderate or severe murine colitis models. Intrarectal treatment of moderate and severe colitis with 5-ASA alone or HA alone at a dose of 30 mg/kg led to a significant decrease in clinical activity and histology scores, myeloperoxidase activity (MPO), TNF-α, IL-6 and IL-1β in colitis mice compared to untreated animals. The combination of HA (30 mg/kg) and 5-ASA in severe colitis led to a significant improvement of colitis compared to 5-ASA alone. Combined rectal therapy with HA and 5-ASA could be a treatment alternative for severe cases of IBD as it was the only treatment tested that was not significantly different from the healthy control group. This study further underlines the benefit of searching for yet unexplored drug combinations that show therapeutic potential in IBD without the need of designing completely new drug entities.
The effect of garlic extract and vitamin C mixture on the various cancer cell lines in vitro and in vivo have been examined. Proliferation of human colon cancer (HT-29), human rectal cancer (HRT-18) and human hepatoma (HepG2) cells was inhibited by garlic extract and vitamin C, respectively. Based on the cytotoxic activity, mixture of garlic extract and vitamin C was demonstrated to possess a synergistic growth inhibition on HT-29, HRT-18 and HepG2 cancer cells. Mixture of garlic extract and vitamin C significantly arrested G2/M phase cells in the HepG2 cell cycle. Oral administration of mixture of garlic extract and vitamin C to sarcoma-180 tumor-bearing mice prolonged survival time compared to that of control group. These results suggested that addition of vitamin C enhances anticancer activity of garlic extract in vitro, and mixture of garlic extract and vitamin C has antitumor effect in vivo.
This investigation was designed to determine the release of fenbufen from suppositories and their bioavailability in rabbits. Suppositories containing fenbufen were made by the fusion method with Witepsol H-15, Wecobee and PEG 1540 base. Displacement value, weight variation, content uniformity, melting point and melting time were determined for preformulation of the fenbufen suppositories. The release rates were determined with the KP dissolution apparatus and with cellophane tube dialysis device and were increased in order of PEG 1540, Witespol H-15 and Wecobee. The bioavailabilities of fenbufen after rectal administration were also increased in order of PEG 1540, Witespol H-15 and Wecobee.
