Surgical Treatment of Pancreatic Trauma in Children (소아의 외상성 췌장 손상의 수술적 치료)

  • Debates exist about the appropriate treatment for pancreatic trauma in children. We intended to examine the safety of the operation of pancreatic trauma in children. This is a retrospective study of 13 patients, younger than 15, who underwent surgery for pancreatic trauma, between 1993 and 2011 in Seoul National University Children's Hospital. Medical records were reviewed for mechanism of trauma, clinical characteristics, radiological findings, operation and outcomes. Organ injury scaling from the AAST (American Association for Surgery of Trauma) was used. All injuries were caused by blunt trauma. Patients with grade III, IV, and those who were difficult to distinguish grade II from IV, underwent surgery due to severe peritonitis. Three patients with grade II were operated for reasons of mesenteric bleeding, tumor rupture of the pancreas, and progression of peritonitis. Distal pancreatectomy was performed in 10 patients and subtotal pancreatectomy and pylorus preserving pancreaticoduodenectomy in 1 patient each. The remaining one underwent surgical debridement because of severe adhesions. The location of injury, before and after operation, coincided in 83.3%. The degree of injury, before and after the operation, was identical in all the patients except for those who were difficult to tell apart grade II from grade IV, and those cannot be graded due to severe adhesion. Postoperative complications occurred in 23.1%, which improved with conservative treatment. Patients were discharged at mean postoperative 12(range 8~42) days. Even though patients with complications took longer in time from diagnosis to operation, time of trauma to operation and hospital stay, this difference was not significant. In conclusion, When pancreatic duct injury is present, or patient shows deterioration of clinical manifestation without evidence of definite duct injury, or trauma is accompanied by other organ injury or tumor rupture, operative management is advisable, and we believe it is a safe and feasible method of treatment.

A Study on the Intake Status of Energy Drinks and Related Factors of University Students in Yeungnam Region (영남 지역 대학생들의 에너지 음료 음용 실태 및 영향 요인)

  • This study examined the intake status of energy drinks and related factors among university students in Yeungnam region, Korea. A total of 456 students participated in the study by a self-administered questionnaire. About 26% of the respondents never took energy drinks, whereas about 30% took them previously, but do not take them now. Almost 45% answered that they have taken them up to now. The reasons for first intake of energy drinks were 'necessity for myself' followed by 'curiosity', 'recommendation of friends', and 'seeing advertisement', and Chi-square test showed that there was a significant difference according to dwelling status. About half of the respondents took them since high school, and the main reason for energy drink intake was 'to stay awake'. About 45% of the respondents indicated that they increased study hours after taking energy drinks, whereas almost 40% answered that they had little effect on them. Over half of the respondents experienced side effects after energy drink intake. About one-fifth of the respondents mixed the energy drinks with alcoholic beverages to make them taste better. In addition, in the result for the multinomial logistic regression analysis, gender, grade, dwelling status, smoking status, alcohol drinking status, perception of effect of energy drinks on health, and perception of benefit level of caffeine were the predictors for the respondents who never took energy drinks in comparison with the respondents who have taken them up to now. On the other hand, for the respondents who took them previously but do not take them now compared with those of having taken them up to now, the predictors were dwelling status and smoking status. These results imply that dietary education from childhood regarding knowledge and side effects of energy drinks should be offered.

Actual State of Korean 'Geese Mothers' in New Zealand (뉴질랜드 거주 기러기 어머니의 생활 실태 연구)

  • The purpose of this study was to investigate the actual state of Korean 'geese mothers' in New Zealand. The analysis was peformed for 66 Korean mothers, who had moved temporarily to New Zealand for their children's education. The data have been analyzed by frequencies, percentiles and content analysis. The summarized results of this study are as follows. Most of the mothers were living in New Zealand temporarily for one to three years with one or two children. They hoped that their children will gain better and wide-spread opportunities because of studying abroad. The mothers expected their children to become well prepared as internationalized persons, able to achieve a satisfying occupation with high standard abilities. New Zealand's education system and their migration were highly satisfactory in the mothers' opinions. Most of the them contacted with their husbands who remained in Korea, through e-mails and phone calls, at least once in a week. Most of their friends were Koreans. They listed differences of language, mental values, and personalities as the reasons for having a larger number of Korean friends. The communication with the friends in New Zealand were being done by phones and e-mails mainly. Although there are difficulties for them to communicate others in English, they preferred to enjoy leisure time outside other rather than stay in house. And they gave themselves positive rates on adapting to the new environment, New Zealand.

A Study on Dining out Behaviours of Fast Foods - Focused on Youido Apartment Compound in Seoul - (패스트 푸드의 외식행동(外食行動)에 관한 실태조사(實態調査) - 여의도아파트단지(團地)를 중심(中心)으로 -)

  • To determine dining out behaviours of fast foods at five different fast food restaurants of Youido apartment compound in Seoul, a survey was conducted of 460 customers ranging in age from 7 to 29 year, from October 1 to 5th, 1986. Findings are summarized follows: The reasons the customers the fast food restaurants by the highest percentage were the following in the order: 'the atmosphere in which fast food is eaten is enjoyable for the companionship', 'convenient to dining', 'the surroundings and dining equipment are pleasant and hygienic', and 'to be able to stay as long as I want'. The majority of the customers visited the fast food restaurants with their friends between 1pm and 5pm. The fast foods purchased by the customers were mostly for snack rather than a full meal. Preference was a major factor in food selection from available meun items. Ice cream, Kentucky fried chicken, rolled rice with laver I rice cake stew, fried Mandoo were ranged high on the list of liked foods, in contrast, lower preference was for noodle soup, small red bean porridge, pinenut porridge, and persimmon punch.

Research on Gender Specification and Their Visual Preferences at Department Store Display Space - Target Department Store Space - (백화점 매장공간의 성별 탐색 특성과 주시경향에 관한 연구 - 백화점 매장 공간을 대상으로 -)

  • Observation about space is looked steady in an instant, but in continuous movement, one's observation unconsciously stays at different points. In department store, customer actually observes around the store for buying, not focusing on certain point. By studying customer's movement and observation feature, buying desire and interest can be found. For analysis of the different searching-features according to the continuous-observation depending on sex, the study is set up to record movements of customers at women in Department store. The following are the findings. First, Men observed 0.2-0.4 units more in I-II section which are assumed as predominant. The result shows that men can focus on more section (around +0.4%) and longer (around +5.7%) than women do. Second, the same feature of observation depending on sex is that both men and women observe left and right section while keep focusing on middle section. Third, according to the fact that right-focused observation magnificently occurred in the image curved to right, the Space-composition has influenced on the observation of both men and women on the space. Forth, excessive number of display can cause avoidance of observation. Moreover, observation does not stay on the coverage due to wall or post, but is attracted to the brand name. As brand name causes right-focused observation in the image [(8)], brand name can be one of the reasons to attract observation in women apparel store. To sum up, this study is noticeable as it researches about continuous-observation. Furthermore, verifying the result that the composition of space and the placement of products can cause big differences in the observation feature is meaningful outcome.

Dual Earner Couple's Interdependent Retirement Decision - Focusing on Spouses'Involuntary Retirement - (부부은퇴의 상호의존성 연구 - 배우자의 은퇴여부 및 은퇴자발성을 중심으로 -)

  • This study examined the interdependent retirement decision between husbands and wives. Since most of retirees in Korea retired by involuntary reasons such as health problems, it purposes to test whether the involuntary retirement of husbands(wives) influences the retirement decision of wives(husbands). Using data from 4th Korean Longitudinal Study of Ageing Panel, it conducted probit and multivariate probit analysis on 1,038 dual-earner couples aged 51~76 to test interdependence of couples' retirement. The results showed that husbands who have retired wife were more likely to retire. It also proved that involuntary retirement of spouse is a significant predictor of early retirement decision, though the direction of the effect is reversed by gender. Wives were more likely to retire when husband were retired involuntarily, while husbands were more likely to stay in labor market. These findings helped to understand the retirement decision of aged couples in Korea and brought suggestion on labor market and care policies for aged couples.

Energy Drink Consumption Status and Associated Factors among Male and Female High School Students in Deajon Area (대전 지역 남녀 고등학생의 에너지음료 섭취 실태 및 섭취 관련 요인)

  • The purpose of this study was to determine factors related to the consumption of energy drinks among male and female high school students in Daejeon. The research data, derived from the self-administered questionnaire method, was collected from 664 students in fifteen high schools during the spring of 2016. A total of 542 complete questionnaires were analyzed (response rate: 79.8%). Approximately 73% of the students self-reported having consumed energy drinks, with a greater percentage of male (as opposed to female) students self-reporting as having done so. The most common reasons given for the consumption of energy drinks were to stay awake (54.8%), the good taste of the drink (28.0%), to concentrate during studying (17.2%), and to relieve fatigue (16.9%). The adverse effects were palpitation (59.3%), insomnia (35.6%), and experiencing difficulty in waking up (30.5%). More than two in three (67.8%) students who experienced adverse effects still consumed energy drinks. The average level of health consciousness was lower than 3 out of 5 points. The results of the logistic regression analyses indicated a positive relationship between monthly allowance (OR=1.01 for male and female students) and the consumption of energy drinks by both male and female students. Among the male students, freshmen (OR=0.23) were less likely to have consumed energy drinks than juniors. Male students' sleeping hours (OR=0.65) and perceived school life satisfaction scores (OR=0.63) were negatively associated with the consumption of energy drinks. In the case of female students, study hours (OR=0.83) and energy drinks consumption were negatively related. These factors affecting energy drinks consumption could be considered in the development of dietary education programs aimed at protecting high school students from the adverse health impacts of energy drinks.

A Study on Court Ladies' tasks and Costume in the Joseon Dynasty (조선시대 궁녀의 직무와 복식에 관한 연구)

  • Court ladies' tasks were largely divided into two. First, if there is a royal family event, such as the royal family's wedding or feast, they followed the rules and regulations of work division that was defined in "Gyeonggukdaejeon". Secondly, during normal days, they were divided into Jimil (至密), Chimbang (針房), Subang (繡房), Sesugan (洗手間), Saenggwabang (生果房), Naesojubang (內燒廚房), and Oesojubang (外燒廚房) and took charge of female work that was needed daily at the palace including serving, cooking, laundry, needlework, embroidery, cleaning, and nurturing. This organization was operated by different palaces such as the daejeon(king's palace), daebijeon(queen dowager's palace), junggungjeon(queen's palace), or sejagung(prince's palace). Court ladies were selected among female slaves of the government office or naesusa(a ministry of royal household properties). Although commoners were forbidden to be selected, they sometimes became court ladies voluntarily for financial reasons or because of the will of their parents. Court ladies had different appearances according to the division they belonged to. The court girls of Jimil, Chimbang, and Subang wore saeangmeori while court girls from other places had braided hairstyles. At Jimil, they wore all different kinds of chima(skirt) and jeogori(jacket). Usually, court ladies wore navy chima, jade jeogori, green gyeonmagi(top jacket) with the uyeomeori hairstyle. When working in the evening, they wore pink jeogori or yellow-green jeogori, navy chima with the jojimmeori hairstyle for easiness to stay up all night and work. Navy chima was worn by court ladies. If there was a royal family event upper court ladies wore uyeomi, black or green wonsam, and stewards wore garima and dangui.

Assessment and Methods of Nutritional Support during Atropinization in Organophosphate and Carbamate Poisoning Cases (아트로핀으로 치료하는 유기인계와 카바메이트 중독 환자의 영양지원 평가와 방법)

  • Purpose: Atropine is an antidote used to relieve muscarinic symptoms in patients with organophosphate and carbamate poisoning. Nutritional support via the enteral nutrition (EN) route might be associated with improved clinical outcomes in critically ill patients. This study examined the administration of nutritional support in patients undergoing atropinization, including methods of supply, outcomes, and complications. Methods: A retrospective observational study was conducted in a tertiary care teaching hospital from 2010 to 2018. Forty-five patients, who were administered with atropine and on mechanical ventilation (MV) due to organophosphate or carbamate poisoning, were enrolled. Results: Nutritional support was initiated on the third day of hospitalization. Thirty-three patients (73.3%) were initially supported using parenteral nutrition (PN). During atropinization, 32 patients (71.1%) received nutritional support via EN (9) or PN (23). There was no obvious reason for not starting EN during atropinization (61.1%). Pneumonia was observed in both patient groups on EN and PN (p=0.049). Patients without nutritional support had a shorter MV duration (p=0.034) than patients with nutritional support. The methods of nutritional support during atropinization did not show differences in the number of hospital days (p=0.711), MV duration (p=0.933), duration of ICU stay (p=0.850), or recovery at discharge (p=0.197). Conclusion: Most patients undergoing atropinization were administered PN without obvious reasons to preclude EN. Nutritional support was not correlated with the treatment outcomes or pneumonia. From these results, it might be possible to choose EN in patients undergoing atropinization, but further studies will be necessary.

Is a Fast-track Critical Pathway Possible in Gastric Cancer Surgery? (위암 수술 후 조기회복을 위한 임상진료 지침서의 활용이 가능한가?)

  • Purpose: The postoperative hospital stay after gastric cancer surgery is usually 8 to 14 days. The main reason for a prolonged hospital stay may be the 3 to 4 day period of postoperative starvation. The aim of this study is to investigate the feasibility of a critical pathway for early recovery after gastric cancer surgery, and this pathway included early postoperative feeding. Materials and Methods: One hundred three consecutive patients who underwent gastric resection and reconstruction for gastric cancer from October 2007 to June 2008 at St. Mary's Hospital were prospectively enrolled in a fast-track critical pathway. The pathway included minimal preoperative procedures, optimal pain relief, proper fluid administration, early mobilization and early enteral nutrition at postoperative 1 day. The exclusion criteria were determined preoperatively, intraoperatively and postoperatively. Results: Of the 103 patients, 19 patients were excluded for preoperative (5), intraoperative (7) and postoperative (7) reasons. Eighty-four patients (81.6%) were included in the fast-track critical pathway. Sixty-eight (88.6%) of 84 patients were discharged at the planned 8 day after surgery during the initial period of the study, and the postoperative hospital stay was shortened up to 6 days during the more recent period. Postoperative complications occurred in 22 patients (26.2%), and these were gastrointestinal track-related complications in 6 cases (3 cases of ileus, 1 case of stasis and 2 cases of leakage) and infection-related complications in 8 cases. There was no statistical difference in the incidence of morbidity according to the clinic-operative features that included age, gender, stomach resection and lymphadenectomy. Conclusion: The fast-track critical pathway with using the available exclusion criteria was a valid option for patients who underwent gastric cancer surgery.
