요추부 골밀도 측정 시 장내 변화가 골밀도에 미치는 영향 (The Influences of Bowel Condition with Lumbar Spine BMD Measurement)

  • 골밀도는 골다공증 진단에 중요하게 이용되고 있고, 치료 및 예방에 중요한 지표가 되고 있다. 그러나 골밀도는 피검자의 상태, 골밀도 측정기, 검사자 등에 의해 정밀도의 오차가 발생할 수 있다. 골밀도에 미치는 다양한 요인 중 요추 부위에 실질적인 영향을 일으킬 수 있는 장내가스, 음식, 물을 통하여 환자의 상태에 따라 어떻게 변화하는지 알아보고자 하였다. Aluminium spine phantom(ASP)을 이용하여 수조의 물 높이 변화와 가스의 유무에 따른 골밀도의 변화를 알아보았다. 또한 자원자를 대상으로 물이 증가하거나 음식물 증가에 의한 골밀도의 영향을 알아보았다. Aluminium spine phantom을 통한 골밀도 측정에서 수조의 물 높이가 증가함에 따라 골밀도가 감소하여 통계적으로 유의한 변화를 관찰하였다(p=0.026). 가스의 유무에 따른 골밀도의 유의한 차이가 없었다(p=0.587). 자원자를 대상으로 한 연구에서는 음식물의 유무에 따른 골밀도가 유의한 차이가 없었으며(p=0.812), 물의 유무에 따라서도 골밀도의 유의한 차이가 없었다(p=0.618). 따라서 요추부 골밀도 측정에서 골의 경계를 인지하는데 어려움이 없다면, 환자의 금식여부나 대장 내시경 검사 후에 시행하는 골밀도 검사는 골밀도에 큰 영향을 끼치는 인자가 아님을 알 수 있었다.

일반촬영 검사 시 검사안내 유인물의 유용성에 관한 고찰 (A Study on the Usefulness of an Examination Information Printout during Ordinary X-Ray Examinations)

  • In a normal x-ray examination, the cooperation of a patient in positioning greatly influences the improvement of work performance, the quality of the images and the reduction in time spent on the examination. An attempt was made to discover the connection between an examination information printout and the effects of the examination by informing the patient before the examination on the examination positions in order to increase the level of cooperation by the patient and by surveying the degree of satisfaction of the patient during the examination proceedings. An examination information printout was prepared on five positions that patients consider to be difficult while receiving normal x-ray examinations and were shown to 257 male and female patients between the ages of 30 to 79 who had visited this hospital between January 1 to February 29, 2012 or to a person who had accompanied the patients before the examination to research the degree of satisfaction regarding the printout. In addition, the examination explanation printout was given to Radiologic Technologists with over 1 year experience in this hospital to use while taking the x-rays and the degree of satisfaction was surveyed and it was examined if the increase of the degree of satisfaction had an effect on the reduction in the examination time. The patients who utilized the examination information printout revealed results of approximately a 7.8% higher degree of satisfaction among the patients who had previous experience receiving x-ray examinations compared to those were having an x-ray for the first time. In the age groups between 30 to 70 years of age, the age group in the 30's revealed the lowest level of satisfaction at approximately 69% and the group in the 50's showed the highest degree of satisfaction of 87.94%. In the survey of the Radiologic Technologists, 85% responded that they were satisfied. In the question that stated "Will the examination information printouts reduce the time involved in unnecessary conversations with the patients?," the responses showed a 95% degree of satisfaction. The degree of satisfaction was high over the examination information printout by both the patients and the Radiologic Technologists. Although the effects of examination time reduction varied based on the ages of the patients and physical conditions that they were in and could not be given measurement values, it was able to be seen through the survey that reduction of the unnecessary communication between the patient and the Radiologic Technologists contributed to the increase in the degree of satisfaction. As a result, both the patients and the Radiologic Technologists had an increased degree of satisfaction over the examination information printout and it was possible for improvements to be made in the quality of medical service provided as well. Nevertheless, it is considered that a more developed manual in terms of quality and quantity must be produced using a more systematic approach and design.

방사선촬영 중 제어실 문의 열린 방향에 따른 차폐효과 (Shielding Effect according to the Direction of Control Room Door Opening during Radiography)

  • 방사선촬영검사 시에는 제어실 문을 닫고 촬영하도록 권고하고 있으나 응급 환자나 소아 환자 등의 방사선 촬영검사 시에는 발생할 수 있는 긴급 상황에 대한 신속한 조치 수행을 위해서 불가피하게 제어실 문을 열어 놓은 상태에서 촬영해야 하는 경우가 있다. 또한 일반 단순촬영의 경우에도 환자들의 대기시간 단축 등을 고려할 때 촬영시 제어실 문을 열어 두는 것이 더 효율적일 수 있다. 이러한 상황을 감안하여 제어실 문을 열어 놓고 촬영업무를 수행하는 경우에 방사선작업종사자에 대한 방사선피폭선량을 평가하여 방사선피폭을 최소화하면서 업무의 효율을 올릴 수 있는 방안을 모색하고자 본 연구를 수행하였다. 실제 선량률은 유리선량계를 이용하여 측정하였으며, 제어실 문이 열린 상태에서 촬영 시 방사선작업종사자 위치에서 의 선량률은 주당 100 mR(약 0.88 mGy)을 약 2.3배 초과하는 2.02 mGy가 측정되었다. 이 결과를 통해 문이 열린 경우에는 상당량의 산란선이 작업종사자의 피폭선량을 증가시킴을 알 수 있었으며, 제어실 문이 열린 상태에서 촬영할 경우 작업종사자의 과다피폭 가능성이 매우 높다는 것을 확인하였다. 또한 몬테카를로 전산 모의실험을 수행하여 동일한 조건에서도 제어실 문의 방향에 따라 방사선작업종사자가 위치한 곳의 선량률을 대략 1/100까지 감소시킬 수 있음을 확인하였다. 결론적으로, 제어실 문을 열고 촬영하더라도 제어실 문의 방향에 따라 동일한 차폐효과를 기대할 수 있으므로 방사선차폐시설을 함에 있어 이에 대한 고려가 요구된다.

핀홀콜리메이터를 사용한 핵의학영상기기의 순환적 영상 재구성을 위한 비동일 시스템 모델 개발 (Development of Unmatched System Model for Iterative Image Reconstruction for Pinhole Collimator of Imaging Systems in Nuclear Medicine)

  • 핵의학 영상 기기 중 SPECT시스템은 촬영목적에 따라 다양한 콜리메이터를 사용하며 영상 재구성을 위해서는 각 콜리메이터의 기하학적 특성을 반영하는 것이 필요하다. 본 연구에서는 이 중 핀홀 콜리메이터를 사용한 핵의학 영상기기의 영상 재구성에 관한 연구를 수행하였으며 특히 핀홀 콜리메이터 영상 재구성시 발생하는 샘플링 문제를 제거하는 방법에 대한 연구를 수행하였다. 순환적 영상 재구성 방법의 투사(projection)와 역투사(back-projection)시 각기 다른 방식으로 구축된 시스템 모델을 반영한 비동일 시스템 모델 방식을 개발하여 최대우도 기댓값최대화(maximum likelihood expectation maximization, ML-EM) 알고리듬에 적용하였다. 설계한 재구성 알고리듬을 성능을 검증하기 위해 geant4 application for tomographic emission(GATE) 시뮬레이션 툴을 이용하여 핀홀 콜리메이터의 디지털 팬텀 시뮬레이션을 수행하고 이를 이용하여 기존방식과 제안한 방식의 재구성 알고리듬에 대한 비교평가 연구를 수행하였다. 그 결과 본 연구에서 제안한 비동일 시스템 모델 사용 영상 재구성 방법은 동일 시스템 모델을 사용한 순환적 재구성 알고리듬에 비해 효과적으로 샘플링 문제를 제거할 수 있는 것을 확인 할 수 있었다. 본 연구에서 제안한 영상 재구성 방법은 다양한 콜리메이터에 확대적용 되어 사용 될 수 있을 것으로 기대된다.

저강도 레이저와 초음파의 동시 적용이 골절 치유 촉진에 미치는 효과 (The Effects of Low-Intensity Ultrasound and Laser on Healing of Bone Fracture)

  • The purpose of study was to evaluate effects of low-intensity ultrasound and laser on healing of bone fracture. Twenty fracture patient were selected for this study(fourteen males, six females. mean aged 44.8) fracture area was humerus, tibia, forearm bones. The obtain result are as follows. 1. The Prognostic Factors Related to Traumatic Brain Stem Injury

  • Objective : This study was conducted to assess the clinical significance of traumatic brain stem injury (TBSI) reflected on Glasgow Coma Score (GCS) and Glasgow Outcome Score (GOS) by various clinical variables. Methods : A total of 136 TBSI patients were selected out of 2695 head-injured patients. All initial computerized tomography and/or magnetic resonance imaging studies were retrospectively analyzed according to demographic- and injury variables which result in GCS and GOS. Results : In univariate analysis, mode of injury showed a significant effect on combined injury (p<0.001), as were the cases with skull fracture on radiologic finding (p<0.000). The GCS showed a various correlation with radiologic finding (p<0.000), mode of injury (p<0.002), but less favorably with impact site (p<0.052), age (p<0.054) and skull fracture (p<0.057), in order of statistical significances. However, only GOS showed a definite correlation to radiologic finding (p<0.000). In multivariate analysis, the individual variables to enhance an unfavorable effect on GCS were radiologic finding [odds ratio (OR) 7.327, 95% confidence interval (CI)], mode of injury (OR; 4.499, 95% CI) and age (OR; 3.141, 95% CI). Those which influence an unfavorable effect on GOS were radiologic finding (OR; 25.420, 95% CI) and age (OR; 2.674, 95% CI). Conclusion : In evaluation of TBSI on outcome, the variables such as radiological finding, mode of injury, and age were revealed as three important ones to have an unfavorable effect on early stage outcome expressed as GCS. However, mode of injury was shown not to have an unfavorable effect on late stage outcome as GOS. Among all unfavorable variables, radiological finding was confirmed as the only powerful prognostic variable both on GCS and GOS.

  • Radiologic imaging is important for evaluating extrahepatic bile duct (EHD) cancers; it is used for staging tumors and evaluating the suitability of surgical resection, as surgery may be contraindicated in some cases regardless of tumor stage. However, the published general recommendations for EHD cancer and recommendations guided by the perspectives of radiologists are limited. The Korean Society of Abdominal Radiology (KSAR) study group for EHD cancer developed key questions and corresponding recommendations for the radiologic evaluation of EHD cancer and organized them into 4 sections: nomenclature and definition, imaging technique, cancer evaluation, and tumor response. A structured reporting form was also developed to allow the progressive accumulation of standardized data, which will facilitate multicenter studies and contribute more evidence for the development of recommendations.

Three Cases of Rare Anatomic Variations of the Long Head of Biceps Brachii

  • In general, the long head of the biceps brachii originates from the superior glenoid labrum and the supraglenoid tubercle, crosses the rotator cuff interval, and extends into the bicipital groove. However, rare anatomic variations of the origins of the long head have been reported in the past. In this report, we review the clinical history, radiologic findings, and arthroscopic identifications of 3 anatomic variants of the biceps tendon long head. As the detection of long head of biceps tendon pathology during preoperative radiologic assessment can be difficult without prior knowledge, surgeons should be aware of such possible anatomic variations.

Comparison of Clinical and Radiologic Characteristics between Anthracofibrosis and Endobronchial Lung Cancer

  • Background: Endobronchial lung cancer (EBLC) and bronchial anthracofibrosis (BAF) share similar symptoms and radiological findings. The aim of this study was to describe clinical and radiological differences between BAF and EBLC, both of which were confirmed by bronchoscopy. Methods: This was a retrospective study of patients with BAF or EBLC from 2008 to 2014. Data were derived from a bronchoscopy registry made since January 1, 2008. Clinical and radiological characteristics of both diseases were analyzed. Results: Among 3,214 patients who underwent bronchoscopy, 167 and 117 patients were enrolled in BAF and EBLC groups, respectively. BAF occurred more predominantly in older non-smoking female patients with a higher chance of tuberculosis (38.3%) than EBLC (6.0%). Cough, sputum, and dyspnea were common symptoms reported for both groups. Bronchoscopic findings revealed that BAF lesions were more common in multiple lobar bronchi (85.0%) or bilateral bronchi (73.7%). Radiologic findings revealed that bronchial stenosis was the most commonly found lesion in both groups (49.1% and 78.6%, respectively). Rates of peribronchial calcification and bronchial wall thickening were higher in the BAF group. The number of patients with lymph node calcification was also higher in the BAF group. Conclusion: Results of this study demonstrated characteristics of clinical and radiologic findings of BAF and EBLC. Increasing the awareness of both diseases may help clinicians differentiate these two diseases from each other, thus avoiding unnecessary invasive diagnostic procedures.
