Simple Radiographic Analysis of Chronic Shoulder Pain in Patients 50 Years and Older (만성 견관절 동통을 가진 50세 이상 환자의 단순 방사선 사진 분석)

  • Purpose: To evaluate a usefulness of the simple radiograph in the patients with chronic shoulder pain 50 years and older. Material and method: 1152 patients with chronic shoulder pain and 100 asymptomatic individuals were involved in this study. All patients were 50 years and older. We excluded patients who had a history of fracture or dislocation. Radiographic interpretation was performed on a shoulder AP view, an axillary view and a supraspinatus outlet view. For statistical analysis, a chi-square test was performed. A p value of <0.05 was considered statistically significant. Results: Abnormal radiologic findings were identified in 369(32%) out of 1152 patients with a shoulder pain: greater tuberosity sclerosis, acromial sclerosis, subacromial osteophytes are common abnormal radiologic findings. A rotator cuff tear or impingement syndrome was identified on a final diagnosis in 61(85.2%) out of the 76 patients with radiologic abnormalities in both greater tuberosity and acromion (p<0.05). Abnormal radiologic findings were identified in 18% of the asymptomatic individuals. Conclusion: Simple radiographic analysis is an important primary diagnostic tool in patients (50 years and old) with chronic shoulder pain.

Moderating Effect of Emotional Intelligence on the Relationships Between Job Demand and Burn-out of Radiologic Technologists (방사선사의 직무요구와 소진간의 관계에서 감성지능의 역할)

  • The purpose of this study is to investigate the relationships between job role and exhausting metal status of radiologic technologists. The results showed as follows: First, job demand(role conflict, recognition demend, work demend) of radiologic technologist highly correlated to its exhausting mental status. Second, emotional intelligence moderated the relation between on the relationship between job demand(recognition demend) and burn-out(decline of success). We concluded that radiologic technologist's intervention for burn-out have positive effect on improving organizational performance. Further studies are needed to develop and test the effect of intervention programs for emotional intelligence and burn-out control.

Clinical, Radiologic, and Endoscopic Manifestations of Small Bowel Malignancies: a First Report from Thailand

  • Background: The symptoms of small bowel malignancies are mild and frequently nonspecific, thus patients are often not diagnosed until the disease is at an advanced stage. Moreover, the lack of sufficient studies and available data on small bowel cancer makes diagnosis difficult, further delaying proper treatment for these patients. In fact, only a small number of published studies exist, and there are no studies specific to Thailand. Radiologic and endoscopic studies and findings may allow physicians to better understand the disease, leading to earlier diagnosis and improved patient outcomes. Objective: To retrospectively analyze the clinical, radiologic, and endoscopic characteristics of small bowel cancer patients in Thailand's Siriraj Hospital. Materials and Methods: This retrospective analysis included 185 adult patients (97 men, 88 women; mean age = $57.6{\pm}14.9$) with pathologically confirmed small bowel cancer diagnosed between January 2006 and December 2013. Clinical, radiologic, and endoscopic findings were collected and compared between each subtype of small bowel cancer. Results: Of the 185 patients analyzed, gastrointestinal stromal tumor (GIST) was the most common diagnosis (39.5%, n=73). Adenocarcinoma was the second most common (25.9%, n = 48), while lymphoma and all other types were identified in 24.3% (n = 45) and 10.3% (n = 19) of cases, respectively. The most common symptoms were weight loss (43.2%), abdominal pain (38.4%), and upper gastrointestinal bleeding (23.8%). Conclusions: Based on radiology and endoscopy, this study revealed upper gastrointestinal bleeding, an intra-abdominal mass, and a sub-epithelial mass as common symptoms of GIST. Obstruction and ulcerating/circumferential masses were findicative of adenocarcinoma, as revealed by radiology and endoscopy, respectively. Finally, no specific symptoms were related to lymphoma.

  • Nowadays, digital Radiography (DR) systems are widely used in clinical sites and substitute the analog-film x-ray imaging systems. The resolution of DR images depends on several factors such as characteristic contrast and motion of the object, the focal spot size and the quality of x-ray beam, x-ray scattering, the performance of the DR detector (x-ray conversion efficiency, the intrinsic resolution). The DR detector is composed of an x-ray capturing element, a coupling element and a collecting element, which systematically affect the system resolution. Generally speaking, the resolution of a medical imaging system is the discrimination ability of anatomical structures. Modulation transfer function (MTF) is widely used for the quantification of the resolution performance for an imaging system. MTF is defined as the frequency response of the imaging system to the input of a point spread function and can be obtained by doing Fourier transform of a line spread function, which is extracted from a test image. In clinic, radiologic technologists, who are in charge of system maintenance and quality control, have to evaluate or make routine check on their imaging system. However, it is not an easy task for the radiologic technologists to measure MTF accurately due to lack of their engineering and mathematical backgrounds. The objective of this study is to develop and provide for radiologic technologists a medical system imaging evaluation tool, so that they can measure and quantify system performance easily.

A Study Concerning of Servant Leadership of Radiotechnologist (방사선사의 서번트 리더십에 관한 고찰)

  • This study aimed to look at servant leadership that general radiotechnologist perceive from the servant leadership perspective and based on this to suggest ways to improve not only organizational effects of radiotechnologist who work clinically but also their working conditions. A population of 290 radiotechnologist who work at hospital in Pusan was the subject of this study and a survey was conducted to them. The analysis for the collected data used SPSS/PC+Win13 version and one-way, ANOVA was carried out to verify differences between groups. Servant leadership according to background factors showed relatively higher values among unmarried than married, twenties or more in terms of age, and nuclear medicine department in terms of the work department than other groups. Regular positions in terms of work types and university hospitals in terms of hospital types showed high scores, and as the motive for being a radiotechnologist, many considered job prospects. Hospitals should improve the organization's ability and performance by managing human resources efficiently. According to this study, servant leadership that radiotechnologist serve the community based on true prestige with basic honesty and trust as a member of fair community is a new model of true leadership that the future society requires.

Report of Radiologic Education Effect Case in First-year Students at University Using Concept Map (개념 지도를 이용한 저학년 대상의 방사선학 교육 효과 사례 보고)

  • The purpose of this study was to apply a concept map to the first-year students of radiologic science and report its effects. The concept map is a visual representation of a major concept and related linking statements. Concept maps are useful tools for students to construct and organize content they have learned. The subjects of this study were first-year grade and at one university in Chungbuk, Korea. They were divided into active and passive participant groups in the class. And they were evaluated the educational effects such as satisfaction, fidelity, learning achievement, and interest before and after using the concept map. As a result, the passive participant group significantly increased the educational effect except for satisfaction, and the active participant group significantly increased the educational effect in all variables (p<0.05). These results showed that concept mapping, which induces first-year grade students to participate in class, could be helpful in radiologic education. It is expected to be used as basic data in various radiologic educational methodology studies in the future.

  • Neurogenic weakness, ataxia and retinitis pigmentosa (NARP) syndrome is a rare maternally inherited mitochondrial disorder. Radiologic findings in NARP syndrome are varied; they include cerebral and cerebellar atrophy, basal ganglia abnormalities, and on rare occasions, leukoencephalopathy. This article describes an extremely rare case of NARP syndrome mimicking mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS).

  • After the outbreak of the SARS-CoV2 virus that causes COVID-19, it spreads around the world with the number of infections and deaths rising rapidly caused a shortage of medical resources. As a way to solve this problem, chest X-ray diagnosis using Artificial Intelligence(AI) received attention as a primary diagnostic method. The purpose of this study is to comprehensively analyze the detection of COVID-19 via AI. To achieve this purpose, 292 studies were collected through a series of Classification methods. Based on these data, performance measurement information including Accuracy, Precision, Area Under Cover(AUC), Sensitivity, Specificity, F1-score, Recall, K-fold, Architecture and Class were analyzed. As a result, the average Accuracy, Precision, AUC, Sensitivity and Specificity were achieved as 95.2%, 94.81%, 94.01%, 93.5%, and 93.92%, respectively. Although the performance measurement information on a year-on-year basis gradually increased, furthermore, we conducted a study on the rate of change according to the number of Class and image data, the ratio of use of Architecture and about the K-fold. Currently, diagnosis of COVID-19 using AI has several problems to be used independently, however, it is expected that it will be sufficient to be used as a doctor's assistant.

Analysis of Pineal Cyst ; Clinical and Radiological Characteristics (송과체 낭종의 임상적, 방사선학적 특성 분석)

  • Objective : Object : A 7-year retrospective clinical and MRI study was reviewed to evaluate the characteristics of pineal cyst. We evaluated results of clinical and radiologic change of pineal cyst during follow up period. Patients and Methods : From 1992 to 1999, 50 patients with pineal cyst were visited for evaluation. Follow up clinical information was obtained from patients or clinical record through phone conversation. also radiologic findi-ngs of pineal cyst in mid-sagittal MRI were reviewed in detail. Results : Pineal cysts were usually asymptomatic and detected incidentally during evaluation of other diseases. Sagittal MRI is the most useful diagnostic test. The radiologic findings of pineal cyst were isointensity on T1WI comared to CSF and slightly high signal intensity on T2WI. Cyst wall was variably enhanced on Gad-enhanced T1WI. The average size of cyst was $13.9{\times}13.0mm$. The pineal cyst was classified into five groups according to previous report. Long term behavior of these lesions are not apparent, but in our study they showed no specific changes in clinical and radiological aspects during 25 months. Conclusion : Although long-term follow up results are needed, the pineal cyst are considered as normal variants. In our 25 months follow up results, there was no rapid change of cyst both clinically and radiologically. Therefore, frequent radiologic evaluation seems unnecessary except type III pineal cyst.

Noncontrast Computed Tomography-Based Radiomics Analysis in Discriminating Early Hematoma Expansion after Spontaneous Intracerebral Hemorrhage

  • Objective: To determine whether noncontrast computed tomography (NCCT) models based on multivariable, radiomics features, and machine learning (ML) algorithms could further improve the discrimination of early hematoma expansion (HE) in patients with spontaneous intracerebral hemorrhage (sICH). Materials and Methods: We retrospectively reviewed 261 patients with sICH who underwent initial NCCT within 6 hours of ictus and follow-up CT within 24 hours after initial NCCT, between April 2011 and March 2019. The clinical characteristics, imaging signs and radiomics features extracted from the initial NCCT images were used to construct models to discriminate early HE. A clinical-radiologic model was constructed using a multivariate logistic regression (LR) analysis. Radiomics models, a radiomics-radiologic model, and a combined model were constructed in the training cohort (n = 182) and independently verified in the validation cohort (n = 79). Receiver operating characteristic analysis and the area under the curve (AUC) were used to evaluate the discriminative power. Results: The AUC of the clinical-radiologic model for discriminating early HE was 0.766. The AUCs of the radiomics model for discriminating early HE built using the LR algorithm in the training and validation cohorts were 0.926 and 0.850, respectively. The AUCs of the radiomics-radiologic model in the training and validation cohorts were 0.946 and 0.867, respectively. The AUCs of the combined model in the training and validation cohorts were 0.960 and 0.867, respectively. Conclusion: NCCT models based on multivariable, radiomics features and ML algorithm could improve the discrimination of early HE. The combined model was the best recommended model to identify sICH patients at risk of early HE.
