Postal Dosimetry Audits for the Domestic Medical Linear Accelerator

  • Purpose: The objective of this study is to perform Postal dosimetry audits for medical linear accelerators in radiation therapy institutions using glass dosimeters and Gafchromic film reading systems and postal dosimetry audit procedures, and to evaluate radiation therapy doses and mechanical accuracy in medical institutions. Methods: Photon output measured and analyzed using a standard phantom for measuring photon output dose using a glass dosimeter for medical linear accelerators. Mechanical accuracy was measured and analyzed using software for film measurement. Results: Measurement and analysis of photon beam output dose using a standard phantom glass dosimeter for photon beam output dose measurement was completed. All tolerance doses were within 5%. Mechanical accuracy measurement and analysis using a standard phantom for verifying the mechanical accuracy of linear accelerator (LINAC) using a Gafchromic film were completed, and all results were shown within tolerances (2 mm or less). Conclusions: In this study, Postal dosimetry audits were performed on the output dose and mechanical accuracy of photon beams (207 beams) for 106 LINACs from 48 institutions. As a result of corrective action and re-execution, it was confirmed that all engines met the acceptable standard within 2 mm in the linear accelerator.

  • Diagnostic X-ray system is general and basic medical equipment to be used in mostly medical organizations, but being bombed of radioactivity is a big weak point when irradiates a X-ray to the human body so that ICRP restricted the radiation exposure tolerance of the human body. In order to reduce being bombed, the many research and development is now advanced. A lots of diagnostic X-ray machines have currently used due to the increase of occurrence efficiency of X-ray and precisely the output control by using the inverter which is a high speed switching semiconductors. For getting the confidence of the X-ray machine, the same radiation occurrence, same evaluation, and same irradiation condition are necessary when evaluates X-ray irradiation. It is the most important part for the accuracy of the test result and the patient safety. This paper has produced the high voltage occurrence system of full-wave rectification method by using the LC resonance inverter, and evaluated the irradiation reproducibility in order to use it in diagnosis of the patient.

  • The knowledge on the etiology of breast cancer has advanced substantially in recent years, and several etiological factors are now firmly established. However, very few new discoveries have been made in relation to occupational risk factors. The International Agency for Research on Cancer has evaluated over 900 different exposures or agents to-date to determine whether they are carcinogenic to humans. These evaluations are published as a series of Monographs (www.iarc.fr). For breast cancer the following substances have been classified as "carcinogenic to humans" (Group 1): alcoholic beverages, exposure to diethylstilbestrol, estrogen-progestogen contraceptives, estrogen-progestogen hormone replacement therapy and exposure to X-radiation and gamma-radiation (in special populations such as atomic bomb survivors, medical patients, and in-utero exposure). Ethylene oxide is also classified as a Group 1 carcinogen, although the evidence for carcinogenicity in epidemiologic studies, and specifically for the human breast, is limited. The classification "probably carcinogenic to humans" (Group 2A) includes estrogen hormone replacement therapy, tobacco smoking, and shift work involving circadian disruption, including work as a flight attendant. If the association between shift work and breast cancer, the most common female cancer, is confirmed, shift work could become the leading cause of occupational cancer in women.

Quantifying rice spikelet sterility on Vietnamese cultivars (Oryza sativa L.) under high temperature and shading condition

  • During grain filling period, rice is affected by many environmental factors; including temperature, water, radiation and soil nutrition condition. In future climate, greater shading and heat tolerance will be required in rice. In this study, the effect of shading and high temperature on spikelet sterility was conducted on fourteen Vietnamese cultivars. Field experiments were studied in 2015 and 2016 to evaluate the response of Vietnamese cultivars under high temperature during grain filling stage. The high temperature and shading were applied by closing two sides of growth chamber and covered by a black cloth (50% reduced solar radiation) under the field condition after the first cultivar heading. The sterility increased significantly under high temperature and shading. The highest percentage sterile spiketlets was observed in 'Jasmine 85' (71.7%) under shading and in 'OM4900' (53.4%) under high temperature in 2015 and 2016, respectively. Among the treatments, the percentage of sterile spekelets in Vietnamese cultivars under shading was highest which was 54.9% and 41.8% in 2015 and 2016, respectively. Yield components reduced significantly in both of shading and high temperature. Corresponding with significantly decrease in yield components, the yield in high temperature and shading decreased strongly in both 2015 and 2016.

  • Ceramics of quaternary garnets (Gd,Y)3Al2Ga3O12 doped with Ce, Tb have been fabricated and evaluated as prospective materials for indirect energy converters of α-and β-voltaic. Samples were characterized at excitation with an X-ray source and an intense 150 keV electron beam and showed good temperature stability of their emission and tolerance to irradiation. The role of X-rays accompanied the α-particle emitting in the increase of the conversion efficiency is clarified. The garnet-type structure of the matrix in the developed materials allows the production of quality crystalline mass with a light yield exceeding that of the commonly used YAG: Ce scintillator by a factor of two times.

Development of a Method to Measure the Radiation Isocenter Size of Linear Accelerators and Quantitative Analysis of the Radiation Isocenter Size for Clinac 21EX Linear Accelerator (선형가속기 방사선 중심점의 크기 측정 방법 개발과 Clinac 21EX 선형가속기의 방사선 중심점 크기 분석)

  • A method to get a size of the radiation isocenter of linear accelerators using star-shot images was presented and a computer program was developed to automate the method. Accuracy of the method was verified. The developed program was used to measure sizes of the radiation isocenters for a Clinac 21EX (Varian, USA) using data of quality assurance (QA) performed from June 2008 to December 2010. To calculated the size of radiation isocenter, positions of two points on each central ray of the star-shot image were found and the equation of the central ray was determined using the positions of two points. Using the equations of central rays the radius of the minimum circle intersecting all the central rays, which is one half of the size of radiation isocenter, was calculated. The program measured x-intercepts and y-intercepts of the central rays within errors of 0.084 mm and sizes of radiation isocenters within 0.053 mm. All the errors were less than the spatial resolution of star-shot images 0.085 mm. The radiation isocenter sizes of Clinac 21EX were $0.33{\pm}0.27mm$, $0.71{\pm}0.36mm$, $0.50{\pm}0.16mm$ for collimator, gantry and couch respectively. During the measurement period all the measured sizes were less than 2.0 mm and within tolerance. The developed program could calculate the size of radiation isocenters and it would be helpful to routine QA.

The Evaluation of Radiation Therapy and Combined-modality Therapy for Non-small-cell Lung Cancer in Elderly (고령의 비소세포성폐암 환자의 방사선 및 병용치료에 대한 효과 평가)

  • [ $\underline{Purpose}$ ]: To compare radiation therapy alone to combined modality therapy about survival rate and tolerance of elderly patients ($70=or{\geq}$) with non-small-cell lung cancer (NSCLC). $\underline{Materials\;and\;Methods}$: Between 1998 and 2002, 57 patients given radiation therapy due to NSCLC (Stage III) were analysed retrospectively. Radiation therapy alone (RT), concurrent chemoradiation (CRT), and sequential chemoradiation (SCRT) was done to 33, 16 and 8 patients, respectively. Patients' median age was 74 (range $70{\sim}85$). Male and female are 51 patients and 6 patients, respectively. 23 patients were stage IIIa and 34 were stage IIIb. Patients' characteristic distribution of RT and CRT was not significantly different except mass size that RT has a bigger than CRT. The fraction size of radiation therapy was 1.8 Gy in CRT and $1.8{\sim}3\;Gy$ in other groups. Total radiation dose was $51{\sim}63\;Gy$ according to the fraction size. If the prescribed total radiation dose was successfully irradiated, we stated that it was completion of radiation therapy. $\underline{Results}$: 52 patients were dead. Median period of radiation therapy was as follow: RT, 35 days, CRT, 60.5 days and SCRT, 35 days. Overall median survival time (MST) was 10.1 months. The 1 yr- and 2 yr-overall survival rate was 39.8% and 17.6%, respectively. MST of RT, CRT and SCRT was 8.9, 8.2 and 11.7 months, respectively. The 1 yr survival rate of RT, CRT and SCRT was 38.4%, 37.5% and 50% (not significant). Patients given incomplete radiation therapy were 12 (RT, 5 CRT, 6 SCRT, 1). N stage (p=0.081) and the difference of treatment methods (p=0.079) were the factors affecting incompletion of radiation therapy, but it was not significant. In case of combined-agents chemotherapy, 4 of 8 ceased radiation therapy. T stage ($T{\geq}3$), mass size (${\geq}5\;cm$), Karnofsky performance scale (${\leq}70$) and completion of radiation therapy were the prognostic factors in uni- and multi-variate analysis. $\underline{Conclusion}$: In elderly patients with NSCLC, radiation therapy alone was a treatment method with similar survival period compared with other methods. Generally, patients given radiation therapy alone was tolerable to a treatment. Before planning concurrent chemoirradiation in elderly patients with NSCLC, physicians pay attention to a selection of patients and chemotherapy agents considering general condition and toxicity.

Applicability of Daily Solar Radiation Estimated by Mountain Microclimate Simulation Model (MT-CLIM) in Korea (MT-CLIM 프로그램을 이용한 일별 일사량 추정의 국내 적용성 검토)

  • Accuracy of daily solar radiation estimated from a Mountain Microclimate Simulation Model (MT-CLIM) was assessed for seven observation sites with complex topography in Uiseong County. The coefficient of determination ($R^2$) between the observed and the estimated daily solar radiation was 0.52 for 7 sites for the study period from 1 August to 30 September 2009. Overall, the MT-CLIM overestimated the solar radiation with root mean square error (RMSE) of $3.83MJ\;m^{-2}$ which is about 25% of the mean daily solar radiation ($15.27MJ\;m^{-2}$) for the study period. Considering that the pyranometer's tolerance is ${\pm}5%$ of standard sensor, the RMSE of MT-CLIM was too large to accept for a direct application for agricultural sector. The reliability of solar radiation estimated by MT-CLIM must be improved by considering additional ways such as using a topography correction coefficient.

Development of Tomotherapy couch device capable of yaw-directional correction (Yaw방향의 보정이 가능한 Tomotherapy couch device의 개발)

  • Objectives : A self-made "Tomotherapy couch device" capable of correcting the Yaw direction was fabricated and evaluated for its usefulness. Materials and Methods : "Tomotherapy couch device" capable of correcting the Yaw direction is made of rigid fibreboard with a flexural strength of $200kg/cm^2$. CBCT Image from Novalis Tx and Iso-Align Phantom from MED-TEC were used to evaluate the physical accuracy. The treatment plan was designed using Accuray $Precision^{TM}$ and In House Head and Phantom. Accuray $PrecisionART^{TM}$ and $Precision^{TM}$ was used to evaluate dose. Results : Evaluation results, the self-fabricated device accurately corrected the setup error, Target dose was within 95 %~107 % of all. In order to directly evaluate the OAR dose according to the Yaw change, the absolute dose was measured. As a result, when the error in the Yaw direction was $3^{\circ}$, the specific OAR showed a maximum difference of 18.4 %. Conclusion : "Tomotherapy couch device" capable of correcting the Yaw direction can be manufactured at a lower cost compared to the effect, and it can prevent the patient's MVCT image dose for re-imaging. Accurate radiation therapy without errors can be performed.
