건강한 여성 근로자에서 Apolipoprotein E와 혈중 지질 농도와의 관련성 (The Association Between Apolipoprotein E Genotype and Lipid Profiles in Healthy Woman Workers)

  • Objectives: Plasma lipid profiles and Apolipoprotein E (ApoE) are established risk factors for cardiovascular disease (CVD). The knowledge of lipid profile may estimate the potential victims of cardiovascular disease before its initiation and progression and offers the opportunity for primary prevention. The most common ApoE polymorphism has been found to influence plasma lipid concentrations and its correlation with CVD has been extensively investigated in the last decade. Methods: The ApoE polymorphism and its influence on plasma lipid were investigated in healthy woman workers. The information on confounding factors was obtained through a self-administered questionnaire and ApoE polymorphism was investigated using PCR. Results: The relative frequencies of alleles E2, E3 and E4 for the study population (n = 305) were 0.127, 0.750 and 0.121, respectively. ApoE polymorphism was associated with variations in plasma HDL-cholesterol lipid profile. In order to estimate the independent effects of alleles E2 and E4, as compared with E3, on lipid profile, multiple regression was performed after adjustment for confounding variables such as age, BMI, blood pressure, education status, insulin, fasting glucose, HOMA-IR, menopause. ApoE2 had a negative association with HDL cholesterol and ApoE4 had a positive association with LDL cholesterol. Conclusions: This study identified that the ApoE and CVD risk factors contribute to the lipid profiles, similar to other studies. The analysis including dietary intake and other gene in further studies may help to identify clear effects on lipid profiles as risk factor for CVD.

방사선 사고와 급성 방사선 증후군 (Radiological Accident and Acute Radiation Syndrome)

  • In mass casualty situation due to radiological accidents, it is important to start aggressive management with rapid triage decisions. External contamination needs immediate decontamination and internal contamination should be treated with special expertise and equipment to prevent the rapid uptake of radionuclides by target organs. Acute radiation syndrome shows a sequence of events that varies with the severity of the exposure. More severe exposures generally lead to more rapid onset of symptoms and severe clinical findings. After the massive exposure, various systems of the body reflect their severe damages that can lead to death within hours or up to several months. The disease progression has classically been divided into four stages: prodromal, latent, manifest illness, and recovery or death. Three characteristic clusters of symptoms including the hematopoietic syndrome, the gastrointestinal syndrome and the cerebrovascular syndrome are all associated with the acute radiation syndrome. The standard medical management of the patients with a potentially survivable radiation exposure includes good medical, surgical and supportive measures. Specific treatment with cytokines and bone marrow transplantation should be considered. The management of internal contamination is much the same as the treatment of poisoning. The standard decontamination should be applied to reduce uptake, and the chelating agents can be administered to enhance the clearance of radioisotopes. Radioactive iodine ($^{131}I$) as one of the nuclear fission products can increase the incidence of thyroid cancer in children. Potential benefit of potassium iodide prophylaxis is greater especially in neonates, infants and small children.

의료용 Cyclotron의 방사선안전관리 행위 예측모형 (A Prediction Model for the Radiation Safety Management Behavior of Medical Cyclotrons)

  • 방사성동위원소(RadioIsotope) 생산시설인 Cyclotron의 방사선안전관리는 방사선작업종사자뿐만 아니라 일반 이용자들의 방사선에 의한 건강장해를 예방하기 위해 중요한 부분이므로 방사선안전관리 행위에 영향을 미치는 예측모형을 도출하여 방사선안전관리 행위수준을 높이는데 필요한 근거자료를 제공하고자 하였다. 문헌고찰, 현장관찰, 전문가 포커스 토의를 통하여 도출되고 내용타당도가 확보된 설문지를 사용하여 국내 Cyclotron이 설치된 24개 허가기관의 방사선안전관리자를 대상으로 2008년 1월 2일부터 30일간 조사하여 다음과 같은 결과를 도출하였다. 방사선안전관리에 관한 경로는 1단계 생산준비, 2단계 RI생산, 3단계 합성, 4단계 분배, 5단계 품질관리, 6단계 운반용기 포장, 7단계 운반 단계로 지정하였다. 방사선피폭의 우려가 가장 높다고 생각하는 단계는 분배 15명(62.5%), 시설 설치부터 운영까지 가장 힘든 업무는 인허가 관련'과 설치시설 및 생산장비확보'가 각 9명(37.5%), 방사선피폭 관련 문제가 발생한 단계는 '합성과 분배'각 4번(30.8%)으로 높게 나타났다. 방사선안전관리 행위수준은 4점 만점을 기준으로 최소 2.42점, 최고 4.00점, 평균 $3.46{\pm}0.47$점으로 나타났다. 방사선안전관리 행위와 직업에 대한 복지후생(r=0.529)이 유의수준 5%에서 통계적으로 유의한 상관관계가 있는 것으로 나타났다. 방사선안전관리 행위에 영향을 미치는 변수를 기준으로 예측모델을 도출한 결과 일반적 특성, 기관 특성, 자기효능감은 통계적으로 유의한 경로를 나타내지 않았고 직업특성 중 복지후생이 방사선안전관리 행위에 영향을 미치는 것으로 나타났다. 따라서 방사선안전관리 행위수준을 높이기 위해 직업적 특성 중 복지후생 수준을 향상시킬 수 있는 전략이 필요하다고 본다. 본 연구는 최근 설치가 급격히 증가하는 Cyclotron의 방사선안전관리에 대한 국내 전수조사로 최초 수행하여 기초자료를 제공하는데 의미가 있다고 본다.

방사선종양학과에 종사하는 치료방사선사의 감정노동에 미치는 요인에 관한 연구 (Study for Effective Factors of Emotional Labor in Radiation Therapist of Radiation Oncology Department)

  • The objectives of this study were to investigate the degrees of emotional labor, the effects to organizational effectiveness and the significant factors influencing organizational effectiveness in radiation therapy professional. The subjects of this study were the radiation therapists working at the hospital in Seoul and Gyungi. The self-reported questionnaires were administered to 205 radiation therapists and the data were analyzed with SPSS 19.0 package. The level of emotional labor of single or female subjects were revealed higher than married or male. The professionalism of job has significant correlation with emotional labor. Significant explanatory variables of the degrees of emotional labor included organization culture, distress and self-leadership. These three variables demonstrated the explanatory power of 13.9% of the emotional labor. Based on the findings, emotional labor of radiation therapist was correlated with occupational professionalism. These results suggest that the intervention program intervening emotional labor should be continued to develop in order to improve the effective self-leadership, organization culture and distress factor.

방사선안전을 위한 보건/비보건계열의 의료방사선 인식도 분석 (A Study on recognition for Medical radiation of health and non-health for radiation safety)

  • This study implemented the survey and analysis of the exposure to radiation by using the questionnaire targeting H Health College, located in Daejeon from September 1st, 2014 to October 15th. A total of 400 copies of the questionnaire was distributed and among them, 385 copies, excluding 15 omitted ones (total collection ratio: 96.3%), were used for the analysis. The analysis results are as follows. For the harmfulness of radiation for diagnosis, the average of the health-related was 3.15 and the average of the health-unrelated was 2.82, which the health-related students recognized the harmfulness of radiation for diagnosis higher (p<.001). The necessity of radiation for diagnosis was appeared higher from the health-related students as the average of the health-related was 3.07 and 2.52 for the average of the health-unrelated (p<.001). The recognition on the prevention of the exposure to radiation was higher from the health-related students as the average of the health-related was 3.13 and 1.84 for the average of the health-unrelated (p<.001). From this study, the necessity of using radiation from the medical field and the recognition on its harmfulness appeared to have a big difference between the health-related and health-unrelated. For such, the accurate understanding of the recognition on radiation and the education to improve recognition on radiation are considered to be required.

사고 대응 작업자 피폭선량 평가 (Dose Assessment for Workers in Accidents)

  • To effectively and safely manage the radiation exposure to nuclear power plant (NPP) workers in accidents, major overseas NPP operators such as the United States, Germany, and France have developed and applied realistic 3D model radiation dose assessment software for workers. Continuous research and development have recently been conducted, such as performing NPP accident management using 3D-VR based on As Low As Reasonably Achievable (ALARA) planning tool. In line with this global trend, it is also required to secure technology to manage radiation exposure of workers in Korea efficiently. Therefore, in this paper, it is described the application method and assessment results of radiation exposure scenarios for workers in response to accidents assessment technology, which is one of the fundamental technologies for constructing a realistic platform to be utilized for radiation exposure prediction, diagnosis, management, and training simulations following accidents. First, the post-accident sampling after the Loss of Coolant Accident(LOCA) was selected as the accident and response scenario, and the assessment area related to this work was established. Subsequently, the structures within the assessment area were modeled using MCNP, and the radiation source of the equipment was inputted. Based on this, the radiation dose distribution in the assessment area was assessed. Afterward, considering the three principles of external radiation protection (time, distance, and shielding) detailed work scenarios were developed by varying the number of workers, the presence or absence of a shield, and the location of the shield. The radiation exposure doses received by workers were compared and analyzed for each scenario, and based on the results, the optimal accident response scenario was derived. The results of this study plan to be utilized as a fundamental technology to ensure the safety of workers through simulations targeting various reactor types and accident response scenarios in the future. Furthermore, it is expected to secure the possibility of developing a data-based ALARA decision support system for predicting radiation exposure dose at NPP sites.
