Hospice Medicine and Nursing Ethics (호스피스의료와 간호윤리)

  • The goal of medicine is to contribute to promoting national health by preventing diseases and providing treatment. The scope of modern medicine isn't merely confined to disease testing, treatment and prevention in accordance to that, and making experiments by using the human body is widespread. The advance in modern medicine has made a great contribution to valuing human dignity and actualizing a manly life, but there is a problem that has still nagged modern medicine: treatment and healing for terminal patients including cancer patients. In advanced countries, pain care and hospice medicine are already universal. Offering a helping hand for terminal patients to lead a less painful and more manly life from diverse angles instead of merely focusing on treatment is called the very hospice medicine. That is a comprehensive package of medical services to take care of death-facing terminal patients and their families with affection. That is providing physical, mental and social support for the patients to pass away in peace after living a dignified and decent life, and that is comforting their bereaved families. The National Hospice Organization of the United States provides terminal patients and their families with sustained hospital care and home care in a move to lend assistance to them. In our country, however, tertiary medical institutions simply provide medical care for terminal patients to extend their lives, and there are few institutional efforts to help them. Hospice medicine is offered mostly in our country by non- professionals including doctors, nurses, social workers, pastors or physical therapists. Terminal patients' needs cannot be satisfied in the same manner as those of other patients, and it's needed to take a different approach to their treatment as well. Nevertheless, the focus of medical care is still placed on treatment only, which should be taken seriously. Ministry for Health, Welfare & Family Affairs and Health Insurance Review & Assessment Service held a public hearing on May 21, 2008, on the cost of hospice care, quality control and demonstration project to gather extensive opinions from the academic community, experts and consumer groups to draw up plans about manpower supply, facilities and demonstration project, but the institutions are not going to work on hospice education, securement of facilities and relevant legislation. In 2002, Ministry for Health, Welfare & Family Affairs made an official announcement to introduce a hospice nurse system to nurture nurse specialists in this area. That ministry legislated for the qualifications of advanced nurse practitioner and a hospice nurse system(Article 24 and 2 in Enforcement Regulations for the Medical Law), but few specific plans are under way to carry out the regulations. It's well known that the medical law defines a nurse as a professional health care worker, and there is a move to draw a line between the responsibilities of doctors and those of nurses in association with medical errors. Specifically, the roles of professional hospice are increasingly expected to be accentuated in conjunction with treatment for terminal patients, and it seems that delving into possible problems with the job performance of nurses and coming up with workable countermeasures are what scholars of conscience should do in an effort to contribute to the development of medicine and the realization of a dignified and manly life.

Awareness of Good Death and Attitudes toward Terminal Care among Geriatric Hospital Nurses (노인요양병원 간호사의 좋은 죽음인식과 임종간호태도)

  • Purpose: We conducted a descriptive correlational study to determine a relationship between nurses' awareness of good death and attitudes toward terminal care, which in turn could be used as basic data for improvement of the quality of terminal care at geriatric hospitals. Methods: From April 3, 2013 through April 22, 2013, data were collected from 230 nurses working at geriatric hospitals. Results: Nurses' attitudes toward terminal care showed no significant correlation with awareness of good death, but it was positively correlated with a sense of closeness, a subfactor of awareness of good death. There was negative correlation between emotions regarding a deathbed, a subfactor of attitudes of nurses in charge of terminal patients, and awareness of good death. We found positive correlation between terminal care performance and awareness of good death. Conclusion: This study warrants the need for nursing education catered to characteristics of geriatric hospitals and development of diverse intervention strategies to help them to attain a positive attitude toward death by familiarizing themselves with the concept of good death and enhancing job satisfaction.

A Study on Operational Status and Management Satisfaction of Dental Laboratories (치과기공소 운영실태 및 경영만족도 조사)

  • Purpose: The purpose of this study is to suggest direction for dental technicians and management of dental laboratories to contribute to the improvement of oral health of citizens with high-quality dental prosthesis by researching operational status and management satisfaction of dental laboratories. Methods: In order to investigate operational status and management satisfaction of dental laboratories, this study conducted a questionnaire survey by mailing questionnaires to heads of dental laboratories designated as clinical training workshops of Department of Dental Technology of G University and a total of 158 questionnaires were used for analysis. Results: As for gender, males were 142 persons (89.9%) and females were 16 persons (10.1%). For location of the laboratories, number of laboratories in metropolitan cities was 94 (59.5%) while small and that in mid-sized cities was 64 (40.5%). As for the satisfaction with job, those who have managed dental laboratories for 23-30 years ($1.92{\pm}77$), transact with 7-12 dental clinics ($1.70{\pm}75$), had monthly income of 4-8 million won ($1.74{\pm}.80$) and had 8-10 employees ($2.33{\pm}.68$) were the highest in their satisfaction. As for satisfaction with management, those who have managed their business for 15-20 years ($2.86{\pm}.52$), transact with more than 20 dental clinics ($2.00{\pm}.04$), had monthly income of less than 4 million won ($2.00{\pm}.85$) and had less than 3 employees ($2.00{\pm}.62$) were the highest in their satisfaction. As for monthly income, those with less than 2 million won were 24 persons (15.2%), 2-4 million were 64 persons (40.5%), 4-6 million were 40 (25.3%), 6-8 million were 14 (8.9%) and over 8 million won were 16 persons (10.2%). As for number of employees, those with less than 3 employees were 32perosns (20.3%), 4-7 were 82 (51.9%), 8-10 were 26 (16.5%) and more than 11 were 10 persons (6.3%). As for severance payment, 84 persons practiced it according to Labor Standard Act (53.2%) and 34 practice it to a proper degree (21.5%) while 20 did not practice it at all (12.7%) and 20 persons did not respond to the question (12.7%). Conclusion: Although dental laboratories have rapidly expanded in number from 473 in 1986 to 2,400 in 2015, dental technician circles are facing difficulty in their management due to fierce competition and low profitability. Therefore, in order to improve this problem, it is necessary to readjust prices for dental prosthesis to realistic ones.

An Integrative Review on Nursing Studies Related to Humor (유머와 관련된 국내 간호연구의 통합적 고찰)

  • Purpose: The purpose of this paper was to describe the attributes of nursing with humor based on an integrative review and to present basic information for developing humor intervention programs for children. Methods: Using combinations of the terms 'nursing' and 'humor' as key words, the researchers searched four electronic databases to relevant identify studies. Thirteen studies were selected through full text screening of related research published in academic journals from January 1993 to April 2013. Results: Through in-depth discussion and investigation of the relevant literature, three components emerged; 1) using instruments of coping for stress relief, 2) symptoms relief through pleasant feeling, and 3) job satisfaction and work improvement of nurse using humor. Conclusion: The results of this study suggest that humor is valuable as a nursing intervention for children and the possibility of bringing about positive results in nursing care if nurses utilize the sense of humor as fully as they can. Therefore, it is critical for nurses to develop a diversity of humor interventions for children as well as measurements to test quality and quantity of humor used in clinical pediatric settings.

Factors Affecting the Re-participation and Recommendation of Overseas Taekwondo Teaching Experiences of Taekwondo Majoring College Students (태권도전공 대학생의 해외 지도 활동 재참여 및 타인추천에 영향을 미치는 요인)

  • The study aims to find out the influence factors to intention of re-participation and intention of recommendation to overseas Taekwondo teaching experience. To accomplish this purpose, the subjects of this study consist of 316 people who have experienced Taekwondo teaching in overseas within the last five years. Descriptive analysis, one-way ANOVA, and hierarchical regression analysis were used to analyze data. 1) there was a significant difference in the intention of re-participation and intention of recommendation to overseas Taekwondo teaching experience according to the family income level, the period of experience, the satisfaction of curriculum, and the satisfaction of the field. 2) activity factors and interpersonal factors had an effect on intention of re-participation and intention of recommendation to overseas Taekwondo teaching experience more than personal factors. If this is reflected in the reality, students who majoring in Taekwondo will be able to have a higher quality overseas experience, which will ultimately contribute to the expansion of the job market and further development of Taekwondo internationalization.

A Study on Practice of Protective Actions for Medical Information - A comparison between hospital administrators and occupational therapists - (의료정보 보호행동 실천에 관한 연구 - 병원행정관리자와 작업치료사를 비교 -)

  • Attempts were made in this paper to compare the practice of protective actions for information of patients' medical treatment between hospital administrators who do not make direct contact with patients and occupational therapists who usually do. The comparison between jobs in charge showed that occupational therapists did not practice much protective actions for information of patients' medical treatment ($3.52{\pm}.809$) compared to hospital administrators ($3.92{\pm}.724$), even though the former had received regular education about protection, management and supervision of patients' medical information more often ($3.17{\pm}1.129$) than the latter ($3.16{\pm}1.037$). In spite of the fact that occupational therapists were exposed frequently to the danger of revealing medical information in the process of their job performance through talks and communications with patients, they displayed relatively little concern for and awareness of keeping information of medical treatment from being leaked by them. It is thus suggested to promote awareness of medical staff to protect medical information by means of flexible educational system for each occupational group, periodical monitoring, continuing public relation, training and quality control for protection of medical information, as well as routine self-examination of such practice.

A Study on Mental Health of Single Aged Persons in Home Perceived by Daughter-in-law (재가 독신노인을 부양하는 주부가 인지하는 노인의 정신건강에 관한 연구)

  • Nowaday, there have a lot of changes in the demands of the aged persons. Their problems also came to the fore with diverse forms under the influences of industrialization, urbanization and nuclear family. To make the matter worse, the aged population is mounting rapidly. Also, such structure as nuclear family is widely disseminating uncomfortable to the aged. People is mainly being guided by self interest above everything else. Indeed, they had, all together, bad effects on our traditional value system regarding 'respect for the aged and devotion to patients'. It seems unfortunately obvious that the family responsibility is gradually weakening to support the old who is a dependent family. The result is that the aged must have suffered all sorts of hardships in lightenning psychological, physical and economical difficulties. First, to grasp the situations and conditions supporting for single aged persons from each view of psychological, emotional, family-relational, healthy, social and economical standpoints, and second, to analyze their own recognition levels thinking of their health conditions and the relationships between the supporting environments of old family dependants and their psychological healths and then finally, to propose suggestions being able to be helpful for living comfortably in an old age and thereby, building up good family relation. The statistical techniques used to analyze 115 respondents living in Puchun city are frequency, $x^2$ test, t-Jest, ANOVA, Pearson's Correlation Coefficiency and Regression analysis (SPSS package), pertnent to prove the hypothesis suggested in this paper. Of course, it is needless to say that more data are needed on this point. However, several main research findings can be summarized as follows: First, the better single aged persons may be in the habit of eating a meal and the higher they may think of their physical health conditions and movement, the more they want to participate in economic activities to be free from economical dependence upon their children and to overcome lonliness. Second, single aged women appear to have had higher ability to take care their health for themself than single aged men do. It is why signle aged women do not, in general, have big problems to manage their health. But, as shown in this paper, single aged person"s were more liable to the diseases of the aged and, thereby, requiring special medical treatment badly to be healthy. Third. single aged persons revealed potential desires to free themself from socio economic dependence upon their children even in simple labor Job which can draw a monthly salary of about W200, 000. Fourth, they are generally satisfied with their children's filial piety toward them. Nonethless, most of them appear to be reluctantly dependent upon their children and live lonly lives very much. Fifth, they seem to have some hesitation in expressing their candid opinions as that then are some others along with family environmental factors for psychological and emotiona stability. Accordingly, it is safe to conclude by saying that much attention should' be paid no only to socio-economic supports and better medical services for the aged but also to political supports of the society and towards their children for the aged's emotiona support, for improving the quality of their lives in old age and promoting efficiency in suporting for old family dependants.

  • This means that development of industrial society has brought pains not only to workers themselves but also to their family due to various disasters, while it changed us to be affluent and improved the quality of life. The purpose of this study is to make the preliminary data for understanding the efficiency of the educations through examination of the existing state in performance about education programs for supervisors, which are in-house educations performed by the company itself and consignment educations performed by outside professional education institutions, in industrial safety and health education programs for prevention of industrial disasters and analysis on survey research of supervisors working at Anyang area to find out whether or not the in-house education and consignment education in education programs for supervisors have statistically significant differences in their ages, classification by industry, the completion number of educations, the number of regular employees regarding duties of supervisor determined by the Industrial Safety and Health Act. As the result of study, this study resulted in that consignment education performed by professional education institutions would be more helpful for improvement of job competence than in-house education performed by company itself. This can be interpreted that consignment education performed by professional education institutions has reacted more flexibly to continuous revisions of the Industrial Safety and Health Act and also more faithful to demand survey by consumers not being tied down by the conventionality for continuous development and study of the curriculum than that of in-house education.

A Study about The Motivational Theory-Based Types of Security Guard's Martial Art Training on Job Performance (경호원의 무도수련이 직무수행에 미치는 영향에 대한 실증분석)

  • This study aims to find out what affects on performance of private security service guards' duty regarding the security service martial arts. To achieve its goal, object of this study was set as private security service guards of private security service enterprise. Here are some conclusions that have been found. First of all, it was revealed that Hapgido, Judo, Taekwondo, kumdo and fencing are in order of the martial arts that help security service the most. Second, the relation of the ability to perform private security service guards' duty between the security service martial art of security guards and the types of them appeared that ones who had been trained were found to be more affirmative than ones who had not, and showed a significant difference. On the other hand, there was no significant difference found on the type of performance of private security service guards' duty. However, ones with a longer career seemed to be more affirmative on the side of work quality and duty, and it also showed a significant difference statistically. complishment relation between the security service martial art of security guards and the types of them appeared that ones who had been trained were found to be more affirmative than ones who had not, and showed a significant difference. Seventh, the relation of mental and the types of them appeared that ones who had been trained were found to be more affirmative than ones who had not, and showed a significant difference. On the other hand, there was no significant difference found on the type of performance of private security service guards' duty.

The effect of university students' approach to health information on improvement of health behavior (대학생들의 건강정보 접근이 건강행태 개선에 미치는 영향)

  • The goals of this study are understanding the state of improvement of university students' health behavior who are about to graduate and have to consider their health for getting a job according to the way of access and achieving health information and suggesting effective way to improve health behavior. 224 people who responded to the survey which was conducted at some universities in Kyungpook area are selected as a subject of final analysis. The analysis result of internet searching was more meaningful than the analysis result of TV programs or books regarding health. And the older grade(B=.103, p=.007), the more access to health information(B=.137, p=.008) and the higher score of existing health behavior(B=.431, p=.000) the score of improvement of health behavior was increased. Internet searching for health information is the most popular thing because information can be easily get after searching what you want immediately. TV program regarding health has better quality than the information on the internet because of the appearance of health professionals giving health information on TV but it is difficult to watch TV program whenever you want. However the separation between books, TV program and internet is obscured in that the books and TV program can be turned into replay and e-book on internet.
