Objective: To establish a cost-effective and easily available phantom for training residents in ultrasound-guided fine needle thyroid nodule targeting punctures. Materials and Methods: Tofu, drinking straws filled with coupling gel, a urine tube, and 21-gauge needles were used to generate a phantom thyroid with nodules for training. Twelve radiology residents were involved in the study. The puncture success rates were recorded and compared before and after phantom training using the Wilcoxon signed-rank test. Results: On ultrasonography, tofu mimicked the texture of the thyroid. Drinking straws filled with coupling gel mimicked vessels. The urine tube filled with air mimicked the trachea, and 21-gauge needles mimicked small nodules in the transverse section. The entire phantom was similar to the structure of the thyroid and surrounding tissues. The puncture success rates of radiology residents were significantly increased from 34.4 ± 14.2% to 66.7 ± 19.5% after training (p = 0.003). The phantom was constructed in approximately 10 minutes and materials cost less than CNY 10 (approximately $ 1.5) at a local store. Conclusion: The tofu model was cost-effective, easily attainable, and effective for training residents in ultrasound-guided fine needle thyroid nodule targeting punctures in vitro.
This study was performed from July to December 1974 to investigate the rates of contamination of distilled water for injection and local anesthetics. 222 vials were sampled from 7 different places of St. Mary's Hospital The samples were cultured for isolation of microorganisms. The results were as follows; 1. Of 222 vials, 11 were Contaminated by 6 types: Staphylococcus epidermis, Bacillus subtilis, Mima polymorph a, Alcaligenes faecalis, Herellea vaginicala and pepto- streptococcus. 2. 5(9.1 %) of 55 vials from surgical wards, 2 (4.2 %) of 48 vials from surgical OPD, and 4 (12,l %) of 33 vials from pediatric wards were contaminated 3. Between rates of contamination of distilled water for injection and local anesthetics and the number of punctures, there was no significant relationship. (CR=1.42 P〉0.1) 4. There was no significant relationship (CR=1.02 P〉0.1) between the rates of contamination of vials and the duration of the first and the last punctures. 5. The ratios of contamination between 75% alcohol and 2% phenol used for disinfection of rubber lids of vials did not show significant difference.
Purpose: The purpose of this study was to examine the puncture incidence of surgical gloves in an operational setting. Methods: The 277 surgical gloves were used collecting at one general hospital located in Seoul from February 1, 2011 to April 3. The data were analyzed by $x^2$ test, Fisher's exact test, and multiple logistic regression. Results: In the dental and orthopedic department, the puncture incidence were significantly higher than others. The time of wearing gloves demonstrated significant difference: The group 181-240 minutes had a significantly higher than those with under 90, 91-180, or over 240 minutes. The influencing factors were as follows. Compared to that of the 2nd assistants, the puncture incidence rates of the surgeons were 9.91 times, scrub nurses were 8.39 times higher respectively. The participants in work experience under 1 year showed a 4.58 times higher than those with over 7 years. In addition, compared to the puncture incidence rate of neurosurgery department, the 17.41 times in cardio-thoracic surgery, 13.89 times in dental surgery, 4.93 times in gynecology, and 4.97 times in orthopedics higher respectively. Conclusion: There is a need for training operational room personnels to occasionally exchange the gloves even during the procedure and to use double surgical gloves.
A case of an iatrogenic spinal arteriovenous fistula with progressive paraplegia in a young woman is reported. The fistula was eventually created after repetitive lumbar punctures performed in the process of spinal anaesthesia. Her symptoms were progressed to paraplegia over a period of 2 years. The digital subtraction angiography demonstrated a single-hole fistula, involving the anterior spinal artery and vein. The lesion was occluded by embolization with immediate improvement. The potential mechanism is discussed.
We performed the autologous epidural blood patch (AEBP) for the relief of headache and other related symptoms following dural punctures of the lumbar region during 8 years from 1981 to 1988. The total of 37 patients with the patching consisted of 9 cases in 3007 of spinal anesthesia, 12 accidental dural punctures in 4283 cases of lumbar epidural anesthesia, 12 cases in 4747 of epidural analgesia for back pain control, 3 cases of myelography and a case of diagnostic lumbar puncture. The headaches were relieved completely in 35 cases following the first AEBP and the remaining two were also relieved following the second AEBP. We think that the AEBP for postdural-puncture headache is the treatment of choice.

Lightning arresters are deteriorated by repetition of protective operation against overvoltages or impulse currents in environments of its use. If a deteriorated arrester is left in power lines, it can lead to an accident such as a line to ground fault even in a normal system. Therefore, it is necessary to eliminate the deteriorated arrester in advance by checking the soundness of arresters on a regular basis, and to ensure the reliability of power systems by preventing accidents. Various deterioration diagnostic techniques and devices are suggested, and most of which measure leakage current components as an indicator of arrester ageing. However, the techniques based on the magnitude of leakage current measure simply RMS or peak value of leakage current components and do not provide detailed information needed in the diagnosis. In this study, we found that the wave height distributions of the total leakage currents are remarkably changed or a new wave height are produced with the progress of arrester deterioration. To propose a new technique for the diagnosis, we designed a leakage current detection unit and an analysis program which can measure leakage current magnitudes and analyze wave height distributions. From the experimental results, we confirmed that the proposed technique by analyzing the wave height distribution can simply diagnose the mode of defects such as a partial damage and an existence of punctures in arresters as well as deterioration of arresters.

Objective : The primary aim of this investigation was to explore the nature of dura mater biomechanics following the introduction of puncture defect(s). Methods : Twenty-eight dura mater specimens were collected during autopsy from the department of forensic medicine of the authors' institution. Specimens were divided randomly into one of four groups : group I (cranial dura mater; n=7), group II (cranial dura mater with one puncture defect; n=7); group III (cranial dura mater with two puncture defects; n=7), and group IV (cranial dura mater with three puncture defects; n=7). Results : The mean${\pm}$standard deviation tensile strengths of the dura mater were $8.35{\pm}3.16$, $8.22{\pm}3.32$, $7.13{\pm}1.77$, and $6.94{\pm}1.93MPa$ for groups I, II, III, and IV, respectively. There was no statistical difference between all groups. A single, two or more punctures of the dura mater using a 20-gauge Quincke needle did not affect cranial dura tensile strength. Conclusion : This biomechanical study may contribute to the future development of artificial dura mater substitutes and medical needles that have a lower negative impact on the biomechanical properties of dura mater.
Background/Aims: Endoscopic ultrasound (EUS)-guided hepaticogastrostomy (EUS-HGS) performed at the intrahepatic bile duct segment 3 (B3) is widely used for biliary drainage. Although performing post-puncture procedures is easier in the intrahepatic bile duct segment 2 (B2) when using a conventional oblique-viewing (OV) EUS scope, this method may cause transesophageal puncture and severe adverse events. We evaluated the safety and efficacy of B2 puncture using a novel OV-EUS scope. Methods: In this single-center retrospective study, we prospectively enrolled and collected data from 45 patients who consecutively underwent EUS-HGS procedures with a novel OV-EUS scope between September 2021 and December 2022 at our cancer center. Results: The technical success rates of B2-EUS-HGS and EUS-HGS were 93.3% (42/45) and 97.8% (44/45), respectively. The early adverse event rate was 8.9% (4/45) with no cases of scope changes or transesophageal punctures. The median procedure time was 13 minutes (range, 5-30). Conclusions: B2-EUS-HGS can be performed safely with the novel EG-740UT (Fujifilm) OV-scope without transesophageal puncture and with a high success rate. B2-EUS-HGS using this novel OV scope may be the preferred strategy for EUS-HGS.
The genus Philanthus Fabricius, 1790 in Korea are reviewed, and two species, Philanthus triangulum and P. coronata, are treated. The former is new to Korea. This species is easily separated from congeners by dense punctures in propodral enclosure and tri-forked marking in lower face. DNA barcoding test is supportive in our identification as well as conspecitficity of Korean materials showing variation in size and coloration. The observed data on flower associations of this species specified with domestic localites and date are separately provided. The current status of Philanthus coronatus that has been the only representative of the genus but forgotten for a lengthy time in Korea is discussed.
Spinal subdural hematoma (SSDH) is an extremely uncommon condition. Causative factors include trauma, anticoagulant drug administration, hemostatic disorders, and vascular disorders such as arteriovenous malformations and lumbar punctures. Of SSDH cases, those that do not have any traumatic event can be considered cases of nontraumatic acute spinal subdural hematoma, which is known to have diverse clinical progress. Treatment typically consists of surgical decompression and cases in which the condition is relieved with conservative treatment are rarely reported. We report two nontraumatic acute spinal subdural hematoma patients who were successfully treated without surgery.
