농어촌의료(農漁村醫療)서비스 개선사업(改善事業)에 대한 보건소(保健所) 공무원(公務員)의 인식도(認識度) (Opinion Survey of Health Center Officers on Rural Health Service Improvement Project)

  • This study was performed investigate the opinion of civil servants in Health center about Rural Health Service Improvement Project The survey by mail was carried out for 447 servants of 25 health centers in Kyungsangbuk-do and the data were collected through self-administered questionnaires to servants about need, participation, concern, and comprehension for the project and satisfied with current facility and equipment of health center. The results were as follows. Generally considered, 48.2% of the improved health center servants was satisfied with health center building and 14.0% or 24.1% of the improving or unimproved center was. About the location of health center, 37.7% of the improved health center servants was satisfied, 25.9% of the unimproved center was. Of the improved health center servants, 43% was satisfied with the medical equipment but in unimproved place, the dissatisfaction was appeared higher than any other place. 49.7% of respondents was participated in making out the Rural Health Service Improvement Project. 50.6% was interested in this project. In the improved area. 65.5% of health center servants replied that the mayor's or county executive's concern about this project was high and 46.5% in councilors but in the unimproved area. their concern was low. About the contents of the project. 24,6% of the servants in the improved center, only 15.2% in unimproved center replied that they had known well. After making out the plan, 13.6% of respondents was unsatisfied with this plan and 17.1% replied that the estimating method of selecting the project area was not good. After the improvement of institution and equipment, 86.1% of health center servants answered that the medical service provided by health center would increase but 59.2% replied that the residents' utilization rate of private medical facility would decrease. The servants of the improved health center replied that the recognition about the developing will of health service(91.2%), the efficiency(91.2%), the quality of health and medical service(93.0%), the amount of health project(91.2%) were improved. In health center which had already improved the institution and equipment, 88.5% of servants replied that the residents' utilization for health center was increased. So, this project should be continuously carried out for health center and health center must develope new project to fit region condition.

장애인보조기구센터의 공적급여 연계 현황과 시사점 연구 (The Study on the Present Status and Implications of Public Benefits linkage in Assistive Technology Center)

  • 본 연구는 현재 운영되고 있는 광역 보조기구센터의 공적급여 연계 현황을 분석하고 발전 방향을 제시하는데 목적이 있다. 이를 위해 각 보조기구센터가 공적급여 사업에서 연계하고 있는 서비스 단계 및 주요 연계 부분들을 파악하였고, 지역사회에서 실시하고 있는 수리사업의 현황과 연계 현황 그리고 각 서비스에 필요한 공급 업체 연계에 대한 현황을 조사하였다. 이를 통해 나타난 결과는 공적급여는 보조기구 정보제공 수준에서의 연계가 가장 많았다. 수리사업은 모든 센터들이 실시하지만 경 정비 수준의 수리를 일반적으로 수행하였고, 지역사회 전문 수리 업체와 연계하여 서비스를 제공하는 실정이었다. 공급 업체 연계는 지역별로 차이가 많이 나타났다. 이를 통해 전문센터에 보다 다양한 보조기구의 확충을 통한 공적급여 정보제공의 질적 향상과 지속적인 공적급여 지원 기관과의 긴밀한 교류가 필요하며, 연계의 효과성을 분석하여 정책 수립의 근거를 확보할 필요가 있다.

보건소의 물리치료서비스 접근성에 관한 연구 (A Study on the Accessibity of Physical Therapy Service in Public Health Center)

  • Every citizen has a right to get the least health care service for their living and should be guaranteed the right of health which is specified in the constitution of Korea. Therefore, health care service should realize the equity and accessibility of service by the equitable distribution which is needed for health care. The objects of this study were the patients who was treated by the physical therapy in health care center(two of them located in cities and others are in agricultural areas). These are the results of our study. First, the age, job, sex and experience of former treatment of objects showed no meaningful statistic difference in the part of convenience of health care center, where as the system of health scurity showed highly meaningful difference. Second, The age, job and system of health care of objects showed no meaningful difference in the part of attitude and appreciation to therapist, where as the experience of former treatment in other center showed highly meaningful difference. Third, The age, job and sex of objects showed no meaningful difference in the part of the appreciation to results of service, where as the system of health scurity and the experience of former treatment in other center showed meaningful difference. To offer the effective physical therapy, the government of province or nation should invest the large amount of financial fund continuously and make the plan of efficient operating system for the physical therapy service room of public health center which is the most nearly located to native citizen. The problem of distance, low reliability to public service, public relation and reliabity should be improved to raise the utility of physical therapy room. And also, the physical therapy room should be opened in health subcenter to offer the condition of the native citizen's healthy life.

내러티브 탐구를 통한 일 대학 간호학생들의 보건소실습 경험 연구 (A Study on Nursing Students' Experience during Clinical Practice at a Public Health Center)

  • Purpose: The purpose of this study is to understand nursing students' experiences during clinical practice at a public health center. Method: This research used narrative inquiry far data collection. From April 2005 to June 2006, data collection was conducted by open-ended interview, questionnaire and close observation. The participants, who were student nurses, were willing to take part in this study. Results: On the basis of these data, the experiences of clinical practice at public health center were: 1) when the student nurses begin clinical practice at public health centers for the first time, most of the students feel fearful, nervous and stressed. They also mentioned having a hard time being polite to clients and the staff. 2) The students had new experiences at the health public center compared with clinical practice. Especially, the student nurses who were determined to be good nurses were doing home visiting care service. Not only did they have the opportunity to confirm their identity as nurses, but also the students change their career course from clinical nursing to public health nursing. 4) They reflected on themselves after home visiting care service. Conclusion: On the basis of these findings, the following recommendations are made. 1) Data collection and analysis are needed, net only through the narrative method, but also through other various qualitative methods. 2) Comparative study is necessary to enhance clinical experiences through the analysis of the interfering factors and the original experiences.

지역 보건소 구강보건실 활성화를 위한 치과위생사 역할 정립에 관한 연구 (A study on the role of dental Hygienist for revitalization of Dental Health class in Community Health Center)

  • The purpose of study was to offer devices to activate the dental health class of community health center and to evaluate the present programs and to propose adequate guidelines for future public dental health program of dental health care in health center. For this study, the mail quastionnaire survey was carried out from the 116 dental hygienists who are working in community health center. Present condition and direction of public dental health service are as follows: dentist's office was 90% by area and work department. Dental health department was equipped in 91.7% of 'public health center', but 'health branch office' was 57.9%. Dental hygienist education condition of Public health center was the most frequency in 'At large city'. 'Have no entirely' of dental health education number of times was 35.8% in 3 years. That is 44.5% in supplement insturction. Most Dental hygienist's business was most 'teeth-sealant' and 'Old man false teeth prosthetic dentistry business'. Therefor, The Obstacle factors of dental health service activity were 'manpower tribe(average 3.92)', and next 'lack of understanding and support insufficiency of law(average 3.47)'. Curriculum for educational practice should be also designed for brightening the dental health service business. The most important thing for dental health service is 'expanding and improving the facilities Legal system' and next 'Opportunity enlargement and activation that can take dental hygienist's residency'.

We suggest what government need to do to support SME's, which includes establishing a one-stop service center, outsourcing of public service, and triggering strategy In order for the government support to be effective, it should serve SME's needs continuously and professionally.

일개 보건소 방문간호사업의 업무 분석 (A Study for Reorientation of Home Care Service at Community)

  • The purpose of this study is to evaluate the community based home care service, and make reorientation for better service. The data was collected from the public health center, which was operated for one year, 1997. This case is evaluated and reoriented according to five elements of public health care system; system of resources for public health, organization and administration, health care delivery system of financing, management. In resources for public health, available health personnels are 15 physicians, 17 nurses, 11 nurse aides and 2 other persons. One professional health personnel take care of 609 clients, The equipments used for elderly and the disabled are 6 wheelchairs, 4 walkers, 1 hairwashers and 30 viberations. But these equipments are not enough to deal all clients. In organization and administration, planning and setting goals for community home care are made by the director, supervisor of family department and public health nurse. So there is no regular commitee for home care services in this community. The form of delivery of health care is focused on preventive health care. The important works of public health nurse are health education, preventive care for hypertension, D.M. and vaccination of communicable desease. In finaning system, funds come from central government(8.3%), local goverment(16.7%) and health center itself(3.8%), The services consist of health education, vaccination, clinical test and equipment. There are several local volunteers, which are local hospitals, a college, a christian association, a catholic association, a drivers association and a disabled association. The volunteer groups give physical and mental support to the clients. In management, this health center has three evaluation methods. One is done by local government, one is done by health center itself, and the other is done by clients with questionair. But the evaluation tools are deferent between agency. Home care services must be planned and evaluated. This public health center has to have more personnel, equipments, education for professional kowledge and meetings with community volunteer agencies.

1차 보건의료사업의 비용-효과분석을 통한 보건소 기능의 확대 방안 연구 (A Study on Improvement of Health Center's Function through Cost-Effectiveness Analysis in Korea)

  • The aim of this study sets out to discover a desirable form of public centers among the alternative ones and make a health center model. Especially, this study attempts; (i) to investigate factors that affect the performance of health centers; (ii) carry out cost-effectiveness analysis (CEA) for the various type of health centers; (iii) identify innovative strategies to increase the use of health center. Cost-effectiveness analysis is used to compare the performance of all the centers. The following is taken to create the index. Wi = Ti x Mi x Eij (Wi: weight for service item I, Ti : time spent for service I, Mi ; number of health personnel involved in service I, Eij : years of schooling for personnel j in providing service I). As a result of these analyses, policy options as follows are recommended; (i) proper manpower, especially public health physician (oriental medical doctor), should be enough to provide health care adequately; (ii) facilities ad equipments in the health center should be provided sufficiently. (iii) the utilization of health centers should be raised by active operation of mobil service, community participation and health education program. Ultimately health centers in public sector are to be fostered for the promotion of health care by enhancing the financial and quality, continuity and efficiency of health services.

맞춤형방문건강관리사업의 지역사회 네트워크 탐색 (Network Analysis to Describe Service Link for Customized Visiting Health Care Program)

  • Objectives: The nurse visiting health service named Customized Visiting Health Care Program(CVHCP) requires the service innovations incorporating community support into a local service network. The purpose of this study was to assess the community network in CVHCP and inform improvement in this network. Methods: We used Social Network Analysis(SNA) in one CVHCP at H city. Network links were generated by self-administered questionnaires by the 14 community resource centers who quantified their links to all other 25 agents on the list. Links were analyzed by a dichotomous scale for any experience of collaboration and a scored scale of 0 to 3 for level of collaboration using UCINET v6. Results: A list of 14 agents was generated, and local network was dominated by the Public Health Center and a local welfare center named Unlimited Care Center(UCC). According to centrality score, UCC was the most prominent agent, and Public Health Center was the most influential agent, being a link in the pathway flow between other agents for 9.5% of contribution. CVHCP scored lower rank of prominent with 30.8% of other agents reported referring to it. Conclusions: Social network analysis provides a useful network description for informing and evaluation service network improvement in maximizing its service for the CVHCP.

공중보건한의사의 진료여건에 관한 조사연구 (A Survey on the Medical Conditions of Public Health Oriental Medical Doctors)

  • Public health oriental medical doctor has played a great role in providing oriental medical treatment and oriental medical health program with public health medical services, the basic infrastructure, however, is not sufficient. In this study, the researcher surveyed the treatment working or service condition of public health oriental medical doctor. 1. The payment, allowance of doctors are fixed upon the law and guideline according to the financial status of local government. The branch of public health center has more support like an official residence with expenses. 2. The public health center mainly has assitants and ratio of full-timer is more than the branch public health center without any assistants if any, they are temporary employee 3. The public health center has 5.22 beds while the branch has 3.14 beds. The daily average number of patient for public health center is 15.01 while the branch has 8.7 More than half of outpatients are over sixties with musculoskeletal disease. 4. Regarding the traveling clinic, the public health center put into more operation than the branch. The 3rd year serving public doctor gives negative about the traveling clinic much more than 1st and 2nd year serving public doctor. The treatment service condition of public doctor of the public health center is better than the branch doctor, but we are on the point of role-changing as health promotion and preventive treatment to bring up motivation, education and competence strengthening for execution the local-bound health program.
