일반병원직원의 호스피스 완화의료 교육 요구도에 관한 융합 연구 (A Convergence Study on the Necessity of General Hospital Workers Hospice Palliative Care Education)

  • 이 연구는 호스피스 완화의료 비 지정병원 직원 중 간호조무사와 일반직원을 대상으로 임종 및 호스피스 완화의료 교육 요구도를 융합적으로 파악하고자 하였다. 조사기간은 2016년 9월 22일부터 10월 21일까지로, 대전 충남지역의 10개 병원 호스피스 완화의료 비지정병원 직원 158명을 대상으로 조사하였다. 구체적인 연구 결과는 다음과 같다. 호스피스 완화의료 교육 내용의 이론과 실무분야 교육 중요도에서는 연구 대상자 모두 이론보다 실무에 더 높은 중요도를 가지고 있다고 답했다. 이상의 연구결과가 호스피스 완화의료 비지정병원직원 중 특히 간호조무사를 대상으로 업무 특성과 요구도에 맞는 맞춤식 호스피스 완화의료 교육 프로그램 개발을 위한 기초적인 자료로서 도움이 되기를 기대한다.

민간종합병원과 지방의료원의 인건비 투자효율성 비교분석 (Comparative Analysis of Value Added to Personnel Expenses between General Hospitals and Regional Public Hospitals)

  • Objectives : This study compared Value added to personnel expenses between 12 private general hospitals and 12 regional public hospitals, based on location and size, to examine industry competitiveness in terms of management. Methods : From 2011 to 2015, the value added and value added to personnel expenses were calculated by year. and a SPSS statistical program was used to determine and influential factors between private general hospitals and regional public hospitals. Results : The total value added to personnel expenses was 26.85 percent lower than general hospitals compared to regional public hospitals. The product category most influenced by the value added to personnel expenses was Stationeries & expendables at general hospitals and Outsourcing at regional public hospitals. Conclusions : Regional public hospitals have relatively low value added to personnel expenses compared to the general hospitals. Therefore, it is necessary for hospital management to reexamine gross revenue relative to total manpower.

의료 종사자의 감정노동이 직무스트레스에 미치는 영향: 설립형태와 직종의 조절효과 (Influence of Emotional Labor on the Job Stress of the Contact Department in a General Hospital Moderation Analysis of Foundation and Occupation)

  • Objectives : The rapidly changing consumer-centric and customer-oriented nature of the medical environment results in significant cognitive load. We aimed to clarify the situation of emotional labor and job stress among hospital employees and seek policies and hospital management for employees' emotions. Methods : The study was conducted through a questionnaire about emotional labor and job stress among 554 individuals working in Seoul, in 9 national, public, and private hospitals. Results : The results of the emotional labor and job stress questionnaire showed statistically significant differences in surface behavior and job stress; both had higher values in employees from the private hospitals than employees from public hospitals. Conclusions : This study found that the stress of emotional labor is a serious problem in government medical institutions. In addition, these institutions need to provide internal customer satisfaction through the hospital ombudsman and harmonize work and healing programs by including plans for improvement.

  • ln general, hospital information system should provide interoperability hat usually and operate independence of other HIS. This study proposes a new HIS paradigm that can be implemented within standard HL7 Interface engine and clinical data repository (CDR). We have developed an alternative architecture relying on agent solutions with distributed queries to heterogeneous databases. This architecture creates a very fine and flexible repository that can handle queries with the bases of standard HL7 messaging structure. Deploying Agent solutions to manipulate autonomy of storage management and sociality for communication with open world is another issue that keeps this system from reinventing existing wheels in medical informatics. This study the first attempt to construct CDR based private clinic. We used the information stored in the clinical patient record system of the internal medicine private hospital which is used rational database. We were searched increasing the 1,000 data entry from 1,000 to 10,000. By the result, experimental CDR showed highly efficient performance more than 6,000. In the future, the CDR can be further extended for clinical information among private hospitals estranged from EHR (Electronic Health Records).

병원식사 부적응으로 인한 식사 거부 환자의 영양관리 (Nutrition Care of Diet - refusal Patients having Difficulties in Adapting to Hospital Diet)

  • The purpose of this study was to produce proper nutrition and foodservice through positive intervention for diet-refusal patients who has difficulties in adaptation to hospital diet, and to contribute to hospital management with the focus of fast recovery. The survey was conducted using questionnaires between July 18 and August 17, 2000. The subjects were consisted of 24 diet-refusal patients who were hospitalized in St. Mary's Hospital except the patients hospitalized for the purpose of operation or pre-therapy tests. Also chart reviews were conducted. The statistical analysis of data was done by SAS/win 6.11 package to determine descriptive analysis and paired t-test and Pearson's correlation. The summary of study results is as follows : 1. Most of the subjects was receiving chemotherapy. The leading place for offering private food was from patient's home. Subjects worried about delayed recovery from illness due to nutritional problems caused by extreme fasting. The problems were in order of anorexia, nausea and vomiting during therapy process. 52.4% of subjects ate hospital diet after positive intervention. 2. Major reasons of hospital diet-refusal patients' dissatisfaction were in order of expenses, dissatisfaction, serum total protein levels were significantly lower(p<.05). 3. Serum Alb(p<.05), Hb(p<.05), Hct(p<.05) and TLC(p<.001) levels showed significantly negative correlations with fasting duration. With these results, it is concluded that a major cause of malnutrition among patients is to last fasting.

일개 지방사립대학병원에서의 중증외상팀 운영경험 (Experience with Operating a Trauma Team at a Local Private University Hospital)

  • Purpose: This hospital has operated a trauma system of the inclusive trauma system under the sponsorship of this hospital and with financial support from the government from 2011, and it has been designated as a specialized trauma center (candidate) since November 2008. Therefore, this emergency medical center evaluated the influence of the inclusive trauma system on the course of healing and on the results for trauma patients within the region. Methods: The medical records of all patients who were registered as trauma patients from among those who visited the emergency medical center of this hospital from April 2009 to May 2012 were retrospectively reviewed. The monthly and the annual averages of important indices, such as the time in the emergency room and preventable mortalities, were calculated, and patterns of change were sought. The preventable mortality rate was calculated by using the Trauma Injury Severity Score (TRISS) for each patient. Results: The total number of patients registered from April 2009 to May 2012 was 601, and male patients accounted for a larger proportion(432 males(71.88%) vs. 169 females(28.12%)). Their average age was 46.2 years, the average Revised Trauma Score (RTS) was 5.74 points, and the average Injury Severity Score (ISS) was 26.99 points. The preventable mortality rate during the entire period, which was calculated using the TRISS, appeared lower than the preventable mortality rates reported in past studies in the Republic of Korea. Conclusion: These results for the operation of a new trauma system are limited in that they are only for a local private university hospital. However, results show greater changes and developments in and out of the hospital due to multilateral endeavors by the trauma team and the hospital. These endeavors include increased communications among the departments and development of a complementary patient registration system.

기독병원과 일반병원의 조직문화 특성에 따른 조직성과 분석 (Analysis on Organization Performance Based on Hospital Culture)

  • Our study attempt is to see organizational performance according to the differences between types of hospital cultures. To determine theoretical relationship between the organizational culture and the performance, we select two hospitals in Pusan which are different in the purposes and shapes of establishment. We surveyed their members on a questionnaire based on the type of the organizational culture of the two institutions, analyzed, and review its organizational effectiveness. For the survey with questionnaires, which had been preliminary studied to raise its validity, question forms were distributed to 528 persons in April, 1999 based on the self-responses and recollected within 48 hours. The recollection rate was high(89.96%) and the quantity of questionnaires used for our final analysis was 430(81.44%). The Cronbach Coefficient Alpha of the questionnaires was 0.742. Regarding statistical techniques for analysis of the written materials, dispersion analysis(ANOVA) was adapted to test the organizational effectiveness of the two hospitals having the different organizational cultures, and Pearson Correlation was applied to determine correlations was among all variables. T-test was performed to test organizational effectiveness based on the differences in the extent of sharing the culture, organizational committment and work satisfaction between the two health institutions. From our analysis, we obtain the following conclusions. First, concerning with organizational culture of the two hospital, one of which is a christian hospital and the other is a private foundation hospital, the former is conservative and human-oriented but the latter focuses on renovation and accomplishment. Second, the private establishment has a relatively higher organizational effectiveness that the religious hospital as a result of analyzing the extent of sharing culture, organizational committment and work satisfaction. Third, it has been found that the correlations between the extent of the sharing culture and the organizational committment, the extent and work satisfaction, and the committment and the satisfaction are respectively positive influencing organizational effectiveness, especially work satisfaction. Fourth, cultural factors by which the christian hospital is affected more positively including human relations among its members, belief, its idea of establishment, tradition, work responsibility, power, and wage. On the other hand, factors such as director's leadership, personnel management, wage, hospital regulations and department managers' management ability have been seen as negative influences in order. And fifth, for the private foundation hospital human relations among its members, wages, work responsibility, director's leadership and department managers' management ability were positive in their sequence while wages, personnel management, hospital regulations, welfare and department managers' management ability were considered as negative influences in order. As these results of this study, the higher extent of sharing organizational culture, the more increasing in both organization committment and work satisfaction, the higher the effectiveness. Although it was somewhat difficult to generalize the results whose subjects were the two hospitals only, it was obvious that organizational culture was an important influential factor of organizational effectiveness. It is questionable that the extent of sharing organizational culture, organizational committment and work satisfaction as variables affecting the effectiveness have their validity, but this study has its significance in that it provided an approaching to evaluate the organizational culture of individual hospitals making allowances for such variables related to the general activities in its hospital. We hope the results of the study could be useful for the managerial strategies of the institutions.

  • This is to improve the complicate process of the existing CD copy flow in Department of Radiology. It is also to improve the medical service of the hospital to secure private information of patients. Survey: The patients who ask for CD copies in the Department of Radiology of this hospital(for 6 months in 2010). Time: Comparing the total amount of time of the previous and the modified CD copy flow. Using the SPSS 12.0 program for testing significance of independent two-samples t-test. Private information: Introducing the identification-system based on the medical law to supplement the security of personal information of patients. There was manifold complaints due to receipt and issuing the prescription in Admission and Discharging office and outpatient department. The time has been approximately 50% reduced from 70 mistunes to 32 mistunes due to simplifying the routes. The security of the personal information has been supplemented by let patients submit documentary evidence based on the medical law. The service in the hospital has been improved as a result of simplifying the whole process of the routes and receipt process. Therefore, the each stage of waiting time has been decreased. In addition, the personal information of the patients has been more secured.

일개 종합병원의 민간 건강검진 수검자의 검진이용 특성, 건강행태 및 건강관리 수준 분석 (Analysis of Utilization Characteristics, Health Behaviors and Health Management Level of Participants in Private Health Examination in a General Hospital)

  • 본 연구의 목적은 2차 종합병원급 건강검진센터를 이용한 민간 종합검진 환자의 검진 이용특성과 이들의 건강행태 및 건강관리 수준을 분석하는 것이다. 이를 위해 대전지역 일개 2차 종합병원급 건강검진센터의 2011년 20,696명의 민간 건강검진 수검자를 대상으로 이들의 2001년에서 2011년까지 11년간의 수검자료 150,501건을 분석하였다. 민간 종합검진 수검자의 검진군 분류를 위한 군집분석은 K-means기법의 z-score표준화 방법을 이용하여 분류하였으며, 정기/비정기 검진 분류모형 개발을 위해 로지스틱회귀분석, 의사결정나무, 신경망 분석을 이용하였다. 개발된 비정기 검진군 분류 모형에 따라 신규 검진군 중 비정기 검진군이 될 확률이 높은 1,000명을 추출하여 고객관리사업 대상자로 하였다. 분석결과, 수검자는 신규 검진군, 정기 검진군, 비정기 검진군으로 분류하였다. 신규 검진군은 30대가 많고, 신장질환 의심자의 비율이 높았다. 정기 검진군은 남자, 이상지혈증 의심 비율이 높았다. 비정기 검진군은 흡연율과 운동부족 비율이 높았고, 빈혈 및 당뇨의심 비율이 높았다. 의사결정나무 분석결과 비정기 검진환자의 특성에 영향을 미치는 변수로는 성별, 연령, 거주지, 운동, 빈혈, 이상지혈증, 당뇨, 비만, 간질환 등이었다. 특히 여자 수검자로서 빈혈 검사는 정상, 운동을 하지 않는 군이면서 비만이 의심되는 수검자의 비정기적 수검율은 71.4%에 달하였다. 이러한 연구결과 토대로 맞춤형 고객관리 사업을 진행한다면 건강검진센터 효율적인 운영에 기여할 수 있을 것이다.

우리나라 현행 한의사 진료가운에 대한 조사연구 (Study on the Present Gown of Oriental Medical Doctors in Korea)

  • This study was carried out to survey the contentment of the oriental medical doctors to the present gown which they wear. The objects were the oriental medical doctors who work for the university hospitals and the private clinics. The subjects were mainly the evaluation of the relation to the image of oriental hospital and the gown, the evaluation of the present their gowns and the evaluation on the improvement of their gowns. The collected data were analysed by the frequency, cross-table and $\chi^2$ using SPSS program. The results showed that the oriental medical doctors who work for the university hospital evaluated the difference of the image more important than those who work for the private clinics. Also the former was more than the latter on the necessity of the difference in the gowns between the western and oriental medical doctors. The oriental doctors regarded the tradition as of great importance in the improvement of their gown. More than 70% of them wanted to separate their gown for the spring-summer from that for the autumn-winter.
