A Case Report of Primary Ovarian Insufficiency Treated with Korean Medicine Treatment (한의치료로 호전된 조기난소부전 환자 치험 1례 보고)

  • Objectives: The purpose of this study is to report the case of Korean medicine treatment on primary ovarian insufficiency. Methods: The patient in this case was 29-year-old female who was diagnosed with primary ovarian insufficiency. She had irregular menstruation and hypomenorrhea more than 6 months. She also suffered from hot flash and vaginal dryness. She was treated over 1 year with Korean medicine treatment, such as herbal medicine, acupuncture, and pharmacopuncture. We assessed the clinical symptoms, menstrual status and serum hormone level during the treatment. Results: After treatment, symptoms of primary ovarian insufficiency were relieved, level of serum FSH decreased and level of serum E2 increased. We maintained the treatment over 1 year and kept follow-up measurements of serum hormone level. Conclusions: This study shows that Korean medicine treatment can be effective in treating primary ovarian insufficiency. The report suggests the long treatment procedure for primary ovarian insufficiency.

Assessment of Quality of Life in 26 Patients with Primary Hyperhidrosis before and after Oriental Medicine Treatment (일차성 다한증 환자 26명의 한의학적 치료 후 삶의 질 변화에 대한 임상적 평가)

  • Backgrounds and Objectives: Primary hyperhidrosis deeply affect a patient's quality of life, resulting in impairments of daily activities, social interactions and occupational activities. The objectives of the study were to assess quality of life(QOL) in patients with primary hyperhidrosis before and after treatment with oriental medicine. Methods : Between December 2006 and July 2007, 26 patients underwent oriental medicine treatment. which consisted of herb medicine. acupuncture and iontophosis (if palmar/plantar hyperhidrosis) treatment. QOL questionnaires comprised of dermatology life quality index (DLQI) and visual analogue scale (VAS) were employed before and after treatment Results : All 26 patients were evaluated. The treatment controlled primary hyperhidrosis in 84.6% of cases. The mean of DLQI score before treatment was 16.65 after treatment mean DLQI score was 8.18. The mean of VAS before treatment was 91.35 after treatment mean VAS was 49.92. QOL in patients with primary hyperhidrosis significantly improved after oriental medicine treatment. The improvements in QOL were similar in patientswith palmar/plantar, head, and systemic hyperhidrosis. Conclusions : Oriental medicine treatment leads to significantly improved QOL in patients with primary hyperhidrosis. Further studies are needed to evaluate the objective efficacy in patients with primary hyperhidrosis before and after oriental medicine treatment.

Spinal Dural Arteriovenous Fistulas : Clinical Experience with Endovascular Treatment as a Primary Therapeutic Modality

  • Objective : The aim of this study was to evaluate the efficacy of endovascular therapy as a primary treatment for spinal dural arteriovenous fistula (DAVF). Methods : The authors reviewed 18 patients with spinal DAVFs for whom endovascular therapy was considered as an initial treatment at a single institute between 1993 and 2006. NBCA embolization was considered the primary treatment of choice, with surgery reserved for patients in whom endovascular treatment failed. Results : Surgery was performed as the primary treatment in one patient because the anterior spinal artery originated from the same arterial pedicle as the artery feeding the fistula. Embolization was used as the primary treatment modality in 17 patients. with an initial success rate of 82.4%. Two patients with incomplete embolization had to undergo surgery. One patient underwent multiple embolizations, which failed to completely occlude the fistula but relieved the patient's symptoms. Spinal DAVF recurred in two patients (one collateral development and one recanalization) during the follow-up period. The collateral development was obliterated by repeated embolization, but the patient with recanalization refused further treatment. The overall clinical status improved in 15 patients (83.3%) during the follow-up period. Conclusion : Endovascular therapy can be successfully used as a primary treatment for the majority of patients with spinal DAVFs. Although it is difficult to perform in some patients, endovascular embolization should be the primary treatment of choice for spinal DAVF.

  • Objectives: This paper was aim to report the effects of Gamisoyosan-gagambang (加味逍遙散加味方) on the Primary Dysmenorrhea patients. Methods: Two patients who had been suffering from primary dysmenorrhea were enrolled in this study. They received oriental treatment such as herbal medicine. We gave them Gamisoyosan-gagambang several times. During the treatments, we requested them to visit hospital once every two weeks. If they could not come to hospital, we called them to take follow-up. The progress of symptoms was evaluated by visual analogue scale(VAS). Results: After treatment, most symptoms were improved and also VAS was reduced. Conclusions: This clinical cases indicate that Gamisoyosan-gagambang is effective in treatment of the primary dysmenorrhea patients. More clinical data and studies are requested for the treatment of primary dysmenorrhea.

Nasopharyngeal cancer found after treatment of unknown primary cancer in the head and neck (두경부 원발부위 불명암에서 치료 후 발견된 비인두암)

  • Despite adequate diagnostic work-up, unknown primary carcinoma(UPC) of the head and neck cannot be detected in approximately 2- 3% of patients.(1,2) There are several explanations for a cervical metastasis in the absence of a primary tumor. Here in, we report 2 patients, who were diagnosed with nasopharyngeal cancer after treatment of unknown primary cancer of the neck. Both patients had radical neck dissections and chemoradiation therapy, but 1 patient showed nasopharyngeal cancers 4 years after treatment and the other patient at 9 months after treatment for the unknown primary cancer. Therefore, we report 2 cases of nasopharyngeal cancer, which were diagnosed after treatment of unknown head and neck primary site.

Survival and Recurrence Rate after Treatment for Primary Spinal Sarcomas

  • Objective : We have limited understanding on the presentation and survival of primary spinal sarcomas. The survival, recurrence rate, and related prognostic factors were investigated after treatment for primary sarcomas of the spine. Methods : Retrospective analysis of medical records and radiological data was done for 29 patients in whom treatment was performed due to primary sarcoma of the spine from 2000 to 2010. As for treatment method, non-radical operation, radiation therapy, and chemotherapy were simultaneously or sequentially combined. Overall survival (OS), progression free survival (PFS), ambulatory function, and pain status were analyzed. In addition, factors affecting survival and recurrence were analyzed : age (${\leq}42$ or ${\geq}43$), gender, tumor histologic type, lesion location (mobile spine or rigid spine), weakness at diagnosis, pain at diagnosis, ambulation at diagnosis, initial treatment, radiation therapy, kind of irradiation, surgery, chemotherapy and distant metastasis. Results : Median OS was 60 months, the recurrence rate was 79.3% and median PFS was 26 months. Patients with distant metastasis showed significantly shorter survival than those without metastasis. No factors were found to be significant relating to recurrence. Prognostic factor associated with walking ability was the presence of weakness at diagnosis. Conclusion : Primary spinal sarcomas are difficult to cure and show high recurrence rate. However, the development of new treatment methods is improving survival.

Chemically Enhanced Primary Treatment at D Wastewater Treatment Plant (화학적 처리에 의한 1차 하수처리장의 처리효과 개선 -현장시험을 중심으로-)

  • This study examined the feasbility of upgrading D waste water treatment plant which treats incoming wastewater by primary sedimentation only. By adding 20-40 ppm of Hi-PAX into the outlet of the aerated grit chamber, BOD and SS removal efficiences were improved from 29% and 36% to 53 % and 73%,respectively. However, chemically enhanced primary treatment failed to meet the upcoming wastewater quality standard(BOD and SS <20 mg/l) consistently. This was suspected to result from the deteriorated plant return stream. The wastewater treatment by chemical treatment should have increased the amount of the sludge to be removed as the sludge production was increased. Chemically enhanced primary treatment is anticipated to consistently produce effluent of the 1996 standards quality by adjusting the amount of the sludge to be removed. Besides BOD and SS removal, chemically enhanced primary treatment resulted in the improved T-P removal from 30% to 64-74%. However, such benefit was not observed in T-N removal. Improved T-P removal will be expected to help control water pollution in Masan bay.

  • Because of the intensified environmental problems such as climate change and resource depletion, sewage treatment technology focused on energy management has recently attracted attention. The conversion of primary sludge from the primary sedimentation tank and excessive sludge from the secondary sedimentation tank into biogas is the key to energy-positive sewage treatment. In particular, the primary sedimentation tanks recover enriched biodegradable organic matter and anaerobic digestion process produces methane from the organic wastes for energy production. Such technologies for minimizing oxygen demand are leading the innovation regarding sewage treatment plants. However, sewage treatment facilities in Korea lack core technology and operational know-how. Actually, the energy potential of sewage is higher than sewage treatment energy consumption in the sewage treatment, but current processes are not adequately efficient in energy recovery. To improve this, it is possible to apply chemically enhanced primary treatment (CEPT), high-rate activated sludge (HRAS), and anaerobic membrane bioreactor (AnMBR) to the primary sedimentation tank. To maximize the methane production of sewage treatment plants, organic wastes such as food waste and livestock manure can be digested. Additionally, mechanical pretreatment, thermal hydrolysis, and chemical pretreatment would enhance the methane conversion of organic waste. Power generation systems based on internal combustion engines are susceptible to heat source losses, requiring breakthrough energy conversion systems such as fuel cells. To realize the energy positive sewage treatment plant, primary organic matter recovery from sewage, biogas pretreatment, and co-digestion should be optimized in the energy management system based on the knowledge-based operation.

Hypertension knowledge and Treatment compliance of Hypertensive Patients -Comparative analysis between groups treated at Primary Health Care Post in "H" Gun (country) and at other private Clinics·Hospitals. - (고혈압 환자의 고혈압지식 및 치료지시이행 -H군 관내 보건진료소와 병·의원 이용그룹 간 비교분석-)

  • Purpose: To determine the knowledge and treatment compliance with hypertension, directions for the aspects of implementation, and related factors to patients with hypertension using Primary Health Cares and Private Clinics Hospitals. Method: General characteristics and High blood pressure-related properties(17 questions), Hypertension knowledge measures(20 questions), and Treatment Compliance(22 questions) were used. Result: 1. The patients of Private Clinics Hospitals had statistically significantly higher drinking habits than Primary Health Care centers. The patients of Private Clinics Hospitals had statistically significantly higher rate of no family history of hypertension than Primary Health Care centers. 2. Primary Health Care centers had statistically significantly higher knowledge of hypertension than Private Clinics Hospitals. Primary Health Care centers had statistically significantly higher treatment compliance than Private Clinics Hospitals. Conclusion: Both Primary Health Care centers and Private Clinics Hospitals are high medication compliance but low lifestyle compliance with hypertension. We need to recognize the importance of lifestyle compliance, to apply proper programs and to provide therapists' aggressive intervention.

A Retrospective Study on the Effect of Pulp Treatment on the Exfoliation of Primary Teeth (유치 치수치료가 치아 탈락에 미치는 영향에 관한 후향적 연구)

  • The aim of this study was to retrospectively analyze the effects of pulp treatment on exfoliation of primary teeth and its related factors. In this study, 167 teeth of 97 patients aged 6 months to 12 years who were treated with pulp treatment at Dankook Dental Hospital were selected, and information related to pulp treatment and tooth loss was collected. The included subjects were 72 primary anterior teeth (43.1%) and 95 primary posterior teeth (56.9%), of which 56 were males (57.7%) and 41 females (42.3%). The mean follow-up period was 106.1 ± 38.7 months, and the mean age at pulp treatment was 34.8 ± 15.4 months for primary anterior teeth and 69.1 ± 25.1 months for primary posterior teeth. Unilaterally pulpectomized teeth were significantly exfoliated earlier than the same tooth on the opposite side (p < 0.05). Also, in the case of teeth with periapical lesions, despite pulp treatment, the probability of extraction due to infection has been increased on primary anterior teeth (p < 0.05), but not on posterior teeth (p > 0.05). Pulpectomized teeth were lost earlier, an average of 7.8 months for primary anterior teeth and 8.5 months for primary posterior teeth. Early loss of the primary tooth can lead to space loss and premature eruption of the successor, so this can be considered when planning or performing treatment of the primary tooth.
