Background: The surgical extraction of the third molar is the most frequently encountered procedure in oral and maxillofacial surgery and is related with a variety of complications. This study examined the efficacy of intentional partial odontectomy (IPO) in the third molars which have no periapical lesions and are located near important anatomical structures such as inferior alveolar nerve. Methods: Seven patients (four males, three females, $39.1{\pm}11.6years$), who received IPO to reduce the risk of inferior alveolar nerve injury (IANI), were followed long-term. The treated teeth were horizontally impacted third molars in the mandibular left (n = 5) or mandibular right (n = 4) areas and were all ankylosed with the surrounding alveolar bone. During the IPO, the bone around the crown was removed to expose the crown, and then the tooth was resected at cement-enamel junction (CEJ). Any secondary trauma to the healthy root was minimized and remained intact after primary suture. Results: The mean follow-up time was $63.2{\pm}29.8months$, and all sites showed good bone healing after the crown removal. Also, sensory abnormality was not found in any patients after IPO. In one patient, the bone fragments erupted 4 months after IPO. In other patient, an implant placed on second molar site adjacent to the third molar that received IPO was explanted about 2 years after the patient's persistent discomfort. Conclusions: In case where high risk of IANI exists, IPO may be chosen alternatively to surgical extraction to reduce the risk of nerve damage.

The purpose of this study was to evaluate the shearbond strength and resin-dentin interface of three different dentinal bonding systems in primary and permanent teeth. Thirty extracted human primary molars and premolars, which were non-carious and free of obvious defect, were selected for this study. All specimens were divided into six groups with two groups allocated for each of the three dentinal bonding system(All-bond 2, Scotchbond Multi-Purpose, Gluma bonding system). After completion of bonding composite to dentin using each tested dentin bonding system, bond strength measurement and histological observation were performed. The results are as follows: 1. All-bond 2 and Scotchbond Multi-Purpose, A good quality hybrid layer was identified, the morphology of which could be equated with the zone of H-E and Brown-Brenn staining. In Gluma bonding system, hybrid layer was very thin, and separated from the solid polymer. 2. All-bond 2 had the highest mean shearbond strength, followed by Scotchbond Multi-Purpose and Gluma bonding system in both primary and permanent teeth. There was no statistically significant difference between All-bond 2 and Scotchbond Multi-Purpose. Statistically significant difference could be found between Gluma bonding system and the other two groups(p<0.05). 3. The fracture patterns observed were mainly the mixture of adhesive failure and dentin dettachment pattern in All-bond 2 and Scotchbond Multi-Purpose while adhesive failure prevailed in Gluma bonding system.

Early loss of the primary maxillary second molar can lead to complications in which mesial drift of the adjacent first molar (M1) can disturb eruption of the succedaneous second premolar (P2). This study reports two cases of space loss for P2 caused by early exfoliation of its predecessor. After the eruption of the first premolar, the Jones jig appliance was used to distalize M1 and regain space for the eruption of P2. The appliance was further utilized to align the palatally erupted P2 into the dental arch. In both cases, the space and corrected position of P2 were well maintained. Early exfoliation of the primary second molar caused by mesial encroachment of M1 is a common phenomenon, and pediatric dentists should attend to this during routine examinations. An appropriate intervention should be initiated when the primary second molar is lost during the mixed dentition period. If used with careful anchorage control, the Jones jig appliance can effectively resolve this problem.

Impaction is defined as the cessation of the eruption of the tooth caused by a clinically or radiographically detectable physical barrier in the eruption path or by the ectopic position of tooth germ. Besides the third molars and the maxillary canines, the most common impacted tooth is the second premolar. The overall frequency of premolar impaction has been reported to be 0.5%. In some cases, orthodontic traction and surgical repositioning may be indicated. When impacted second premolar is involved with prolonged retained second primary molar, extraction of primary molar and space maintenance lead to eruption of second premolar. In these cases, all patients visited to department of pediatric dentistry of Kyungpook National University Hospital for the chief complaint of unerupted second premolar. Extraction of prolonged retained second primary molar and space management are tried for spontaneous eruption of impacted second premolar tooth. The results were as follows: 1. When impacted second premolar is involved with prolonged retained second primary molar, minimal treatment via elimination of primary molar leads to successful results. 2. Proper space management and periodic radiographic examination are required before eruption of second premolar. 3. Sufficient time must be allowed for confirm of tooth movement before orthodontic traction or surgical repositioning. 4. The result is more successful in incomplete root development.

Premature loss of maxillary primary incisors often results from early childhood caries (ECC), trauma. While space maintenance in the posterior region is an important consideration when there is early loss of primary molars, the anterior segment appears to be stable, even with the early loss of several incisors, once the primary canines erupt. However, collapse of anterior arch integrity is evident in cases where incisor teeth are in a crowded dentition prior to extraction or lost before the eruption of the primary canines. So, when early loss of maxillary primary incisors, the aim of restoration is esthetics, speech problem, oral habit such as tongue thrusting than space maintenance. This paper reports that the esthetic problem due to premature loss of maxillary incisors can be successfully resolved by soldered open-faced stainless steel crown.
Purpose : This study evaluated the impact of the presence and aspect of mandibular third molars to the mandible angle fracture or condyle fractures in Korean. Materials and Methods : A retrospective study was designed for patients attending the division of Oral and Maxillofacial Surgery, Kang-dong sacred heart hospital for treatment of mandibular fracture from January 2006 to September 2010. The primary variable was the presence of mandibular third molar and the secondary variable was the aspects of third molar impaction. Mandibular third molars were classified by the impaction depth and the available space as Pell & Gregory system. Outcome variables were the presence of mandibular angle fracture or condyle fracture. Also the source of trauma, age, sex were studied. Hospital charts, radiographs were used for study. Statistic analysis was done with descriptive statistics, the X2-test, linear-by-linear association. P value under 0.05 was considered significant statistically. Results : The number of involved patients was 86. The ratio of male to female patients was about 9:1 for angle fracture and 7:3 for condyle fracture. The most common source of trauma was assault for angle fracture and fall down for condyle fracture. The presence of mandibular third molar increased frequency of angle fracture and decreased condyle fracture with larger impaction depth. But available space of mandibular third molar did not show high association with angle or condyle fractures. Conclusion: Preventive extraction of mandibular third molar is recommended for patients with high risk of angle fracture. Male patients at their third decade or martial artists, police officer could be the case. But it is not recommended for patients with low risk of angle fracture and high risk of condyle fracture relatively. Elder female patients without any symptom on their third molar could be the case.

Ectopic eruption of the first permanent molar is an abnormal positioning of this tooth, causing a premature resorption of the distal surface of the second primary molar. It occurs in approximately 3~4% of the population and the maxillary arch is usually affected. While 66% of ectopically erupting permanent molars are corrected spontaneously without treatment (i.e. a reversible type), active treatment is necessary for irreversible ectopic eruption cases. The treatment modalities have been divided into two categories: interproximal wedging and distal tipping. Interproximal wedging is indicated for minimal impaction and when the impaction is severe, distal tipping techniques are required. Although much has been written about treatment modalities on unilateral ectopic eruption of the first permanent molar, few reports mention bilateral ectopic eruption cases. In this report, two cases of bilateral ectopic eruption of the first permanent molars in young patients are presented. We describe the use of a modified bilateral Halterman appliance for correction of these cases and the clinical results were satisfactory.

Ectopic eruption is out of a normal position by local eruption disturbance in the developing permanent molar. The prevalence of ectopic eruption is reported to be the between 2 and 6%, most often associated maxillary first permanent molar whereas, the occurrence for the mandibular is quite rare. The etiologic factors of ectopic eruption are inadequate arch length, lack of growth in the posterior region of the jaw, mesially inclined eruption path of first permanent molars, abnormally large first permanent molars, hereditary factor and a stainless steel crown which has been improperly restored. Ectopic eruption can be treated by the use of brass wire, separating elastics, distal disking and Humphrey appliance and the use of removable appliance and cervical traction headgear after extraction of the second primary molar. This case was that lower right first permanent molar was mesially tilted state by locking on the stainless steel crown of a lower right second primary molar. The stainless steel crown was removed and Humphrey appliance was set. Like this case, ectopic eruption could be happened by the stainless steel crown which improperly restored. In restoration of the stainless steel crown, selection of proper size, trimming and contouring are very important.
Purpose: This study examined the clinical success rate of Mg titanate implants (M Implant system, Shinhung, Korea), which employ a Mg coating method, by evaluating the marginal bone loss and implant stability using radiographs and Osstell$^{(R)}$, over a 1 year. Materials and methods: The locations of the implants placement were divided into 4 areas; the maxillary and mandibular premolars and molars. In the maxilla, 8 and 9 implants were inserted in the premolar and molar areas, respectively. In the mandible, 11 and 51 implants were inserted in the premolar and molar areas. Marginal bone loss and ISQ of all implants (79) were measured after insertion, mounting the prosthetic appliance, and 1, 3, 6, and 12 months after loading. The marginal bone loss was measured from the radiograph using XCP bite, which was customized, and the implant stability measured using Osstell$^{(R)}$. Fisher's exact test (${\alpha}$=.05) was used to compare the success rates of each region. Results: The mean marginal bone loss for the upper and lower jaws were 1.537 mm and 1.172 mm. The mobility showed a non-significant reduction or increase according with time. The success rates were accounted for 94.12% and 98.39% in the upper and lower jaws; the premolars and molars were accounted for 100% and 96.67%. The two cases of early failure resulted from failure of primary stability during implant insertion. The late failures were not observed for 1 year after adding a loading to the implants. Conclusion: The Mg titanate implant showed good primary stability and good clinical results in both healing and function.

In clinical pediatric dentistry, we have many chances to encounter the white spot like incipient enamel lesions on the mesial surfaces of the 1st molars with direct vision, especially just after the 2nd primary molars were exfoliated. But it was thought highly desirable to assess if these lesions are properly and effectively managed yet. This study aims at surveying the prevalence of incipient lesions on the mesial surfaces of the 1st molars in children through direct observation and examining the suitability of adhesive sealing on them as a pilot trial in searching for their proper management. 1. Among the 124 mesial surfaces of the 1st molars examined, 34% were sound, 53% had incipient carious lesions and 13% had cavitated lesions. 2. In the sectional views of the specimens, 20% showed microleakage after thermo-cycling and it was thought not recommendable as a permanent method. Therefore in order to effectively fight against the incipient caries lesions in children‘s permanent teeth, it was thought proper not to rely on any one method, but to perform reinforcing oral hygiene and promotion of remineralization in combination with therapeutic sealing which is stronger in short-term sealing effect. Although therapeutic sealing has been considered as the core in minimally invasive concept to treat the white spot lesions, its long-term clinical trials have not been suggested. Continuous research is strongly required for making this approach to acquire permanent nature, especially in regards of proper pretreatment and high molecular materials deeply penetrable into enamel.
