A Philological Study on Poisoning of Mind-tranquilizing Herbal Medicines(安神藥) (안신약(安神藥)의 독성(毒性)에 관한 문헌적 고찰)

  • Objectives & Method : We investigated toxicity, poisoning symptoms, poisoning treatment and prevention against poisoning of mind-tranquilizing herbal medicines(安神藥) in order to use herbal medicines accurately. Result : Cinnabaris(朱砂), Zizyphi Spinosae Semen(酸棗仁), Polygalae Radix(遠志), Ganoderma(靈芝) and Polygoni Multiflori Ramulus(夜交藤) may give rise to some side effects or toxic symptoms in mind-tranquilizing herbal medicines(安神藥). The representative methods of poisoning treatment in western medicines are washing out the stomach, promotion of vomiting, causing diarrhea, supplies of grape sugar and symptomatic treatment, etc. The representative methods of poisoning treatment in oriental medicine take advantage of herbs. And Oriental medical doctor should meet symptoms as patients call for attention. In order to prevent against poisoning of mind-tranquilizing herbal medicines(安神藥), the patients should keep usage, dosage and notes and oriental medical doctors should do processing drugs. Conclusion : We should pay attention to clinical using of Cinnabaris(朱砂), Zizyphi Spinosae Semen(酸棗仁), Polygalae Radix(遠志), Ganoderma(靈芝) and Polygoni Multiflori Ramulus(夜交藤) in mind-tranquilizing herbal medicines(安神藥).

Result:Ginseng Radix(人參), Codonopsis Pilosulae Radix(黨參), Panacis Quinquefolii Radix(西洋參), Astragali Radix(黃芪), Atractylodis Rhizoma Alba(白朮), Dioscoreae Rhizoma(山藥), Dolichoris Semen(白扁豆), Glycyrrhizae Radix(甘草), Jujubae Fructus(大棗) and Mel(蜂蜜) may give rise to some side effects, allergic reaction or toxic symptoms in restoratives for invigorating qi(補氣藥). The representative methods of poisoning treatment in western medicines are stopping medication, washing out the stomach, promotion of vomiting, causing diarrhea, supplies of grape sugar and symptomatic treatment, etc. The representative methods of poisoning treatment in oriental medicine take advantage of various herbs. Examination of Pesticide Poisoning Deaths Statistics in Korea and Precautionary Measures Against Pesticide-Poisoning (국내 농약중독 사망사고 현황분석과 사고 예방대책)

  • The data on pesticide poisoning death toll during 2000-2005 was obtained from Korean National Statistical Office and analysed to find out any precautionary measures against pesticide poisonings. Pesticide poisoning death toll showed slightly increasing trend with year up to about 3500 in 2005. Most of the pesticide poisoning death cases were intentional self-poisoning accounting for more than 80% of total pesticide poisoning death. The pesticide most often implicated in poisoning death were paraquat dichloride, a non-selective herbicide and organophosphate/carbamate insecticides. The regional distribution of pesticide poisoning death showed urban 21.6% and rural area 78.4%. As for the occupation of the dead, agricultural, forestry and fishery workers accounted for 27.4% and non-agricultural workers was 72.6% of pesticide poisoning death. Therefore, government need to take some precautionary measures to reduce the number of pesticide poisoning cases. It is strongly recommended that pesticide regulatory authorities encourage and train the agricultural workers to use safe storage boxes and enforce strict regulation for the prevention for the prevention of highly toxic pesticides sales to non-agricultural workers.

  • Paraoxonase, an antioxidant enzyme, exclusively located on HDL is well known for both hydrolysis of organophosphate and prevention against LDL oxidation. It have been reported that PON1 decrease its activity under oxidative stress and that PON1 activity is lower in subject with higher vulnerability to organophosphate poisoning. The aim of our study is to examine the effect of oxidative system on paraoxon-hydrolzing activity and to elucidate the plausible mechanisms responsible for the decline of HDL-asscociated PON1 activity in vivo system.   • Salmonella is one of the major pathogenic bacteria that cause food poisoning. This study investigated whether heat-killed as well as live Lactobacillus protects host animal against Salmonella infection. Live and heat-killed Lactobacillusacidophilus was administered orally to Sprague-Dawley rats for 2 weeks before the rats were inoculated with Salmonella. Rise in body temperature was moderate in the group that was treated with heat-killed bacteria as compared to the Salmonella control group. The mean amount of feed intake and water consumption of each rat in the heat-killed bacteria group were nearly normal. The number of fecal Salmonellae was comparable between the live and the heat-killed L. acidophilus groups. This finding shows that L. acidophilus facilitates the excretion of Salmonella. Moreover, the levels of pro inflammatory cytokines, including tumor necrosis factor (TNF)-alpha and interleukin (IL)-1 beta, in the heat-killed L. acidophilus group were significantly lower when compared to the levels in the Salmonella control group. These results indicate that nonviable lactic acid bacteria also could play an important role in preventing infections by enteric pathogens such as Salmonella.

  • The optimum condition for the extraction of antimicrobial substance from Lithospermi radix was investigated. For the purpose of obtaining basic data for the development of natural preservatives and for the prevention of food poisoning accidents, its antimicrobial activity was tested against several kinds of saprophytic microbes and food poisoning bacteria. The optimum condition for extraction of antimicrobial substance was to steep Lithospermi radix into 95% ethanol for 24 hours at room temperature. Antimicrobial activity was observed at the pH range $5.0{\sim}8.0$, but its activity became stronger at acidic condition. The result of ion exchange chromatography was showed that the antimicrobial activity of anionic portion was more apparent than that of cationic portion. The antimicrobial activity against Gram positive bacteria was stronger than that of Gram negative bacteria and the growth of food poisoning bacteria such as S. aureus and V. parahaemolyticus was inhibited in the concentration of 0.1% for 48 hours. As for mold and yeast, the growth of some kinds of these organisms was inhibited in the concentration of 0.1 % for 48 hours and the growth of nearly all the fungi was inhibited in the concentration of 0.15% for 96 hours.

Making the Difference in Occupational Health: Three Original and Significant Cases Presented at ICOH Congresses in the 20th Century

  • Background: The aim of this study is to illustrate the historical role of the International Commission on Occupational Health (ICOH) congresses as an arena where national and international occupational medicine can dialogue and as the first example of scientific transferability of the research and prevention results that have had such an impact on global public health. Methods: We used the ICOH Heritage Repository, in which ICOH congress proceedings (from the first congress in Milan in 1906 to the last congress, held in Dublin in 2018), are organised in an orderly way, updated and easily accessible according to open access logic. Results: We describe studies by three physicians who submitted significant scientific work to ICOH congresses, one on the battle against ancylostomiasis (Volante, 1906), the second (Quarelli, 1928) on carbon disulphide poisoning, and the third (Viola, 1969) on the carcinogenicity of vinyl chloride monomer. Priority is given to Italian cases, on account of the authors' obvious familiarity with the issues. Conclusion: The visibility offered in ICOH conferences and their published proceedings has boosted the international spread of their findings, contributing to the scientific transferability of the research results and influencing the development of policies and prevention interventions that have had a great impact on global public health.

Comparison of Antimicrobial Activity of Electrolyzed Water Using Various Electrodes against Biofilm of Oral Pathogens

  • Biofilms of oral microbes can cause various diseases in the oral cavity, such as dental caries, periodontitis and mucosal disease. Electrolyzed water generated by an electric current passed via water using a metal electrode has an antimicrobial effect on pathogenic bacteria which cause food poisoning. This study investigated the antimicrobial activity of electrolyzed waters using various metal electrodes on the floatage and biofilms of oral microbes. The electrolyzed water was generated by passing electric current using copper, silver and platinum electrodes. The electrolyzed water has a neutral pH. Streptococcus mutans, Porphyromonas gingivalis and Tannerella forsythia were cultured, and were used to form a biofilm using specific media. The floatage and biofilm of the microbes were then treated with the electrolyzed water. The electrolyzed water using platinum electrode (EWP) exhibited strong antimicrobial activity against the floatage and biofilm of the oral microbes. However, the electrolyzed water using copper and silver electrodes had no effect. The EWP disrupted the biofilm of oral microbes, except the S. mutans biofilm. Comparing the different electrolyzed waters that we created the platinum electrode generated water may be an ideal candidate for prevention of dental caries and periodontitis.

Comparison and Improvement of Domestic and Foreign Regulations for the Prevention of Suffocation Accidents (국외 질식재해 예방규정 비교를 통한 국내 규정 개선방안)

  • Objectives: 'Confined space' was only defined in the Safety and Health Regulations as a place where oxygen deficiency and hydrogen sulfide must be dealt with at the time of the initial enactment (1982). The danger of fire and explosion were added in 2003. We will compare and review the regulations related to confined space work under the current safety and health rules alongside regulations in other countries and prepare a plan to improve the system through enhanced clarity and execution. Methods: In a comparison of systems for the prevention of suffocation in confined spaces in major countries (Germany, United States, Japan) different concepts of the definition of confined spaces in different countries apparently due to differences in each country's legal implementation system, accident analysis methods, the status of safety and health implementation in workplaces, the precautions against actual confined space work, and the definition of confined spaces were found to be not much different between Korea and the other foreign countries. Results: In the case of Germany and the United States, a confined space is defined as a contextual concept rather than a place, so more careful attention is needed from operators or enclosed space managers as it is often necessary to judge the actual workplace. In the case of Korea and Japan, the interior of the place is mainly defined as a place, especially in the case of Japan, which concentrates on oxygen deficiency and hydrogen sulfide poisoning. Conclusions: For measures to improve regulations on the prevention of suffocation accidents in Korea, I would like to propose three major measures to improve the system in the rules on domestic industrial safety and health standards. It is necessary to prepare and provide a guide to ensure that the 18 types of confined spaces currently defined as confined spaces are clearly understood by field management supervisors or workers.
