소아환자의 Midazolam의 정맥투여 용량에 따른 진정 효과에 관한 비교연구 (THE COMPARATIVE STUDY ON THE SEDATIVE EFFECT OF DIFFERENT INTRAVENOUS MIDAZOLAM DOSAGES FOR PEDIATRIC DENTAL PATIENTS)

  • 소아환자는 치과치료시 다양한 방법의 진정요법이 요구된다. 정맥내 투여방법은 약효의 발현이 빠르고, 진정의 심도 및 시간을 술자의 의도대로 조절할 수 있다는 장점이 있다. Midazolam은 현재 의과 및 치과영역에서 성인의 진정요법에 가장 널리 쓰이며, 심혈관계와 호흡계에 대한 영향이 적은 진정 약제이다. 그러나 midazolam을 이용한 정맥내 진정요법의 소아치과에서의 사용에 대한 연구와 보고는 적은 편으로, 이에 본 연구에서는 소아환자에서의 midazolam을 이용한 정맥내 투여시 효과적이고 안전한 초기 투여용량에 관한 연구를 하고자 한다. 진정요법으로 2회 이상의 구치부 치료가 필요한 건강하고 비협조적인 행동을 보이는 16명의 환자를 대상으로 하였으며 평균연령은 $54.7{\pm}10.7$개월, 평균 몸무게는 $18.1{\pm}3.0kg$이었다. 술전 투약으로 0.3mg/kg, 최대 5mg의 midazolam을 근육내 투여 후 30~50%의 $N_2O-O_2$를 병용투여한다. 이중 맹검법에 의해 모든 환자는 두 번의 내원 중 임의로 한번은 0.1mg/kg(I군)을, 다른 한번은 0.2mg/kg(II군)의 초기 용량의 midazolam을 정맥내 투여하여 치료하였고, 추가투여시에는 초기용량의 1/2을 투여하였다. 치료시 환자의 생징후를 측정하였고, 행동양상은 Ohio State University Behavioral Rating Scale과 Automated Counting System을 사용하여 평가하였다. 술자는 Houpt가 제시한 기준을 응용하여 임상적 치료의 성공과 실패를 평가하였다. 몸무게에 대한 총 투여용량은 I군에서 0.16mg/kg, 2군에서 0.24 mg/kg 이었다. 생징후의 경우 두 군 모두 정상범위 내에서 안정된 상태를 보였고, 통계적으로 유의한 차이를 보이지 않았다(p>0.05). 행동양상평가에서 II군에서 바람직한 행동양상(Quiet)의 비율이 높았고, 임상적 성공률은 II군에서 높았으나, 두 군간 통계학적 유의차는 보이지 않았다(p>0.05). 작용 발현시간은 II군에서, 회복시간은 I군에서 빨랐고, 약물 투여 횟수는 I군에서 많았으나, 두 군간 통계학적으로 유의한 차이를 보이지 않았다(p>0.05).

  • 한국형발사체 3단은 추력기 자세제어시스템에 의해 3단 엔진 점화 시점부터 위성 분리, 위성과 충돌을 막기 위한 회피기동까지 롤 및 3축 자세제어가 수행된다. 추력기 자세제어시스템은 추력기를 작동시킬 때 추진제를 소모하므로, 적정 추진제 충전은 임무 성공에 있어 중요하다. 그러므로 한국형발사체의 비행 중 추력기 자세제어시스템의 추진제 소모량이 얼마인지 추정할 수 있는 수단이 필요하다. 본 연구에서는 추력기 자세제어시스템에서 획득할 수 있는 압력, 온도 데이터를 바탕으로 추진제 소모량을 추정할 수 있는 에너지 관계식을 개발하였다. 개발된 관계식을 검증하기 위해 On-board 시스템과 유사한 시스템을 구성하여 시험을 했고, 추진제 소모량 추정식과 증류수를 사용한 시험 결과를 비교분석하였다. 또한 오차 분석을 통해 예측 결과의 신뢰성을 판단하였다. 마지막으로 시스템 수준 운용시험의 추진제 소모 결과도 나타내었다.

임플랜트 지지 보철물에서 고정체의 식립위치와 각도에 따른 삼차원 유한요소법적 응력분석에 관한 연구 (THREE DIMENSIONAL FINITE ELEMENT STRESS ANALYSIS OF IMPLANT PROSTHESIS ACCORDING TO THE DIFFERENT FIXTURE LOCATIONS AND ANGULATIONS)

  • Statement of problem. The implant prosthesis has been utilized in various clinical cases thanks to its increase in scientific effective application. The relevant implant therapy should have the high success rate in osseointegration, and the implant prosthesis should last for a long period of time without failure. Resorption of the peri-implant alveolar bone is the most frequent and serious problem in implant prosthesis. Excessive concentration of stress from the occlusal force and biopressure around the implant has been known to be the main cause of the bone destruction. Therefore, to decide the location and angulation of the implant is one of the major considering factors for the stress around the implant fixture to be dispersed in the limit of bio-capacity of load support for the successful and long-lasting clinical result. Yet, the detailed mechanism of this phenomenon is not well understood. To some extent, this is related to the paucity of basic science research. Purpose. The purpose of this study is to perform the stress analysis of the implant prosthesis in the partially edentulous mandible according to the different nature locations and angulations using three dimensional finite element method. Material and methods, Three 3.75mm standard implants were placed in the area of first and second bicuspids, and first molar in the mandible Thereafter, implant prostheses were fabricated using UCLA abutments. Five experimental groups were designed as follows : 1) straight placement of three implants, 2) 5$^{\circ}$ buccal and lingual angulation of straightly aligned three implants, 3) 10$^{\circ}$ buccal and lingual angulation of straightly aligned three implants. 4) lingual offset placement of three implants, and 5) buccal offset placement of three implants. Average occlusal force with a variation of perpendicular and 30$^{\circ}$ angulation was applied on the buccal cusp of each implant prosthesis, followed by the measurement of alteration and amount of stress on each configurational implant part and peri-implant bio-structures. The results of this study are extracted from the comparison between the distribution of Von mises stress and the maximum Von mises stress using three dimensional finite element stress analysis for each experimental group. Conclusion. The conclusions were as follows : 1. Providing angulations of the fixture did not help in stress dispersion in the restoration of partially edentulous mandible. 2. It is beneficial to place the fixture in a straight vertical direction, since bio-pressure in the peri-implant bone increases when the fixture is implanted in an angle. 3. It is important to select an appropriate prosthodontic material that prevents fractures, since the bio-pressure is concentrated on the prosthodontic structures when the fixture is implanted in an angle. 4. Offset placement of the fixtures is effective in stress dispersion in the restoration of partially edentulous mandible.

  • 혼합치열기의 소아환자의 경미한 하악 전치부의 총생을 해결하는 문제와 하악 제 2 유구치의 조기 상실로 인한 하악 제 1 대구치의 근심경사 그리고 이에 따른 제 2 소구치의 맹출 공간 부족 등을 해소하기 위한 여러 가지의 방법들이 시도되어왔지만 저마다 그 술식의 적용범위나 방법 등에는 많은 어려움 들이 있어왔다. 이 연구의 목적은 이런 선택의 어려움을 가만하여 실제 임상에서 보다 간편하고 환자의 불편감을 줄여줄 수 있는 한 방법으로 Lip Bumper를 이용한 술식을 소개하고 그 적용례를 통하여 성공과 실패 사례들을 소개함으로써 임상가들에게 보다 유익한 정보를 주기 위함이다.

흉강경하 흉부 교감신경간 절제술을 시행한 본태성 다한증 환자의 임상적 고찰 (Clinical Evaluation of Thoracoscopic Sympathectomy in Hyperhidrosis)

  • Background: Essential hyperhidrosis is a condition with excessive sweating, which may be localized in any parts of the body. Thoracic sympathectomy has been a surgical procedure for the management of hyperhidrosis. Methods: We studied 30 ASA I and II patients suffering from severe hyperhidrosis. Bilateral upper thoracoscopic sympathectomy of $T_{2-4}$ was performed in 30 patients under general anesthesia. Anesthesia was induced with 2.5% thiopental sodium 5 mg/kg and succinylcholine chloride 1 mg/kg and was maintained with enflurane 1~2 Vol% and $N_2O-O_2$ mixture adjusted to maintain $SpO_2$ greater than 96%. During anesthesia, invasive arterial pressure, heart rate, EKG, $SpO_2$ and capnography were monitored. Skin temperature was measured with thermister probes attached to the index finger of each hand. An increase in temperature after cautery confirmed success of the sympathectomy. Results: There were 14 men and 16 women whose ages ranged from 16 to 46 years old (mean age 22.2). Of these patients, 13 patients had complained of palm-sole hyperhidrosis, 9 of palm-sole-axilla hyperhidrosis, 4 of palm-sole-face hyperhidrosis and 4 of palm-sole-axilla-face hyperhidrosis. The provocative factors of excessive sweating were tension and stress from interpersonal relationships. There was positive familial history in 37%. The most common complication was compensatory hyperhidrosis in 23 patients comprising 76%. Other complication included peumothorax (4 patients), hemothorax (1 patient), ipsilateral Horner's syndrome (1 patient) and paresthesia of right arm (1 patient). The degree of satisfaction was graded as good, fair and poor with 15, 12 and 3 patients, respectively. Conclusions: Thoracoscopic sympathectomy with VATS is an efficient, safe and minimally invasive surgical procedure for essential hyperhidrosis.

Experimental investigation on No-Vent Fill (NVF) process using liquid Nitrogen

  • For a long-term space mission, filling process of cryogenic liquid propellant is operated on a space vehicle in space. A vent process during transfer and filling of cryogenic propellant is needed to maintain the fuel tank pressure at a safe level due to its volatile characteristic. It is possible that both liquid and vapor phases of the cryogenic propellant are released simultaneously to outer space when the vent process occurs under low gravity environment. As a result, the existing filling process with venting not only accompanies wasting liquid propellant, but also consumes extra fuel to compensate for the unexpected momentum originated from the vent process. No-Vent Fill (NVF) method, a filling procedure without a venting process of cryogenic liquid propellant, is an attractive technology to perform a long-term space mission. In this paper, the preliminary experimental results of the NVF process are described. The experimental set-up consists of a 9-liter cryogenic liquid receiver tank and a supply tank. Liquid nitrogen ($LN_2$) is used to simulate the behavior of cryogenic propellant. The whole situation in the receiver tank during NVF is monitored. The major experimental parameter in the experiment is the mass flow rate of the liquid nitrogen. The experimental results demonstrate that as the mass flow rate is increased, NVF process is conducted successfully. The quality and the inlet temperature of the injected $LN_2$ are affected by the mass flow rate. These parameters determine success of NVF.

  • 본 논문에서는 윗면에 균일한 압력을 받는 외팔보의 굽힘 변형과 고정 자유 지지된 무한길이 스트립의 폭 방향 굽힘변형을 위에 언급된 이론을 적용하여 살펴보고자 한다. 먼저 기본 지배방정식들을 요약하여 변형률의 1차항까지 나타내며 각 경우에 대해 변형을 중심선에 상대적인 단면의 변위와 단면의 회전 그리고 병진을 나타내며 각 경우에 대해 변형을 중심선에 상대적인 단면의 변위와 단면의 회전 그리고 병진을 나타내는 도심의 변위로 분해하고 도심에 상대적인 변위는 Michell에 의한 평판의 해와 St. Venant에 의한 봉의 해를 이용한다. 가정된 변위장으로부터 응력을 구한 다음 적절한 조건 하에서 국부평형방정식을 구하여 전체평형방정식을 유도한다. 또한 이로부터 각 단면의 회전과 중심선의 변위가 구해질 수 있음을 보인다.

  • Friedensreich Hundertwasser , born in Vienna 1928, is a painter, an architect and an environmentalist. His architecture is radically different from the traditional , straight -lined, functional architecture practised, for example, by the Bauhaus Masters. From the very beginning , he has tried by various means to show that radical change in thinking are necessary , possible and realized. Hundertwasser's goal in this architecture is to make and give human dwelling in harmony with nature. his architecture is characterized by avoidance of straight lines, " Window Right", irregular arrangement of windows, onion dom, a wealth of colours and such environmentally -friendly consideration as planting of roofs and realizing of "tree-tenants". hundertwasser sees houses as evolving creations which are to be shaped by their inhabitants. He hates Uniformity. The irregularity is always the guiding principle in his architecture. The Hundertwasser's architeucture express his fundamental belief that we may not simply rob nature of her resources, but that we must also return territory to nature which we habe stolen from her. Roofs, terraces and courtyards planted with vegetation and " tree-tenants" build bridges between man and nature. They influse the architecture with vitality and romanticism and are a statement of a quality rather than a standard of living. It is quite possible to value the architecture of Hundertwasser as an important inspiration, as a sort of turning point in the thoughts and actions of city planners. It's success could be healing shock for the international architectural community, which is hypnotized by technology and abused by the shortsighted interests of ren.tability . Under this pressure, today's architects trend to reduce people to a mere part of the " Dwelling machine", without acknowledging that in the long run such machines eat away at people's souls.uch machines eat away at people's souls.

부분 설절제술을 받은 환자에서의 연마면 인상 및 Direct Metal Laser Sintering 을 이용한 총의치 제작 증례 (Complete denture making in a patient of partial glossectomy using polished surface impression taking and direct metal laser sintering method: A case report)

  • 성공적인 총의치를 위해서는 유지, 안정, 지지 세가지 요소가 필수적으로 요구된다. 또한 심한 치조제의 흡수나 수술에 의한 반흔으로 인해 하악 설측변연 형성이 어려운 총의치 제작시에는 무교두 치아배열, 연마면 인상, 무게 보강 내부금속 삽입, 의치 접착 크림의 사용 등의 다양한 고려사항이 필요하다. 본 환자의 경우 심한 치주염으로 심하게 골흡수가 된 무치악 치조제를 가졌고, 또한 설암으로 인한 설절제술로 인해 연조직의 문제를 가진 환자이다. 부가적인 유지와 안정을 얻기 위해 연마면 인상과 내부 보강 금속구조물의 삽입 그리고 최소 가압 인상 등의 방법을 적용하여 보다 나은 임상적 결과를 나타내었다. 또한 원하는 3차원적 형태를 정확하게 부여하고 제작시 복잡한 기공과정을 줄인 금속 구조물을 제작하기 위해 최소 가압 인상 채득과 Direct metal laser sintering이 사용되었다.
