A STUDY ON THE VOLUMES AND FORMS OF THE PALATE FOR DECIDUOUS AND PERMANENT DENTITION (유치열과 영구치열의 구개 형태에 관한 연구)

  • The purpose of this study was to clarify the palatal forms through palatal curvatures of right to left, anterior to posterior direction, and difference of right and left palatal volumes in the primary and permanent dentition. Samples were consisted of normal occlusion of both dentition(50 males and 50 females each). Their upper plaster casts were used, measuring points were decided, through 3-dimensional laser scanning(3-D Laser Scanner, DS4060, LDI, U.S.A.), and fitting standard horizontal plane were made for measuring the height and sectioned volumes of palate. The results were as follows: 1. Palatal volume and height were greater at the right side of the palate in the primary and permanent dentition of male and female, but there was no significance. 2. Palatal height was greater in male compared to female, especially, there was significant difference at intercuspid, inter-second premolar area in the permanent dentition(P<0.05). 3. To the height of A-P direction of mid-palatal area, the highest point was 20mm in the primary dentition, 30mm in the permanent dentition from interdental papilla of central incisors. 4. Palatal height of inter-cuspid and inter-second premolar became shallow and broad, high and broad each, compared to inter-deciduous canine and inter-second deciduous molar.

  • Statement of problem: There have been a few studies about unsplinted implant retainted maxillary overdenture. Purpose: The purpose of this study was to examine the effect of different position of implant for 2 implants-retained maxillary overdenture. Materials and methods: Three-dimensional finite element models were used to reproduce an edentulous human maxilla with an implant-retained overdenture. Two implants in the canine tooth positions on both side and in the second premolar tooth positions on both side models were examined. Axial loads of 100 N were applied to the occlusal surface at the right first molar tooth positions. Maximum stress at the implant-bone interface and stress at the cortical bone surface just under the loading point were observed. Results and conclusion: Within the limits of this study, maximum stresses were concentrated around implant of canine position at loading side. The second premolar area was thought to be more favorable to distribution of stress on mucosa, alveolar bone and implants than canine area for maxillary overdenture.

The effect of early membrane exposure on exophytic bone formation using perforated titanium membrane (천공형 티타늄 막의 조기 노출이 수직 골 형성에 미치는 영향)

  • This study was performed to evaluate the effect of membrane exposure on new bone formation when guided bone regeneration with perforated titanium membrane on atrophic alveolar ridge. The present study attempted to establish a GBR model for four adult beagle dog premolar. Intra-marrow penetration defects were created on the alveolar ridge(twelve weeks after extraction) on the mandibular premolar teeth in the beagle dogs. Space providing perforated titanium membrane with various graft material were implanted to provide for GBR. The graft material were demineralized bovine bone(DBB), Irradiated cancellous bone(ICB) and demineralized human bone powder(DFDB). The gingival flap were advanced to cover the membranes and sutured. Seven sites experienced wound failure within 2-3weeks postsurgery resulting in membrane exposure. The animals were euthanized at 4 weeks postsurgery for histologic and histometric analysis. The results of this study were as follows: 1. There was little new bone formation at 4 weeks postsurgery. irrespectively of membrane exposure. 2. There was significant relationship between membrane exposure and bone graft resorption(P<0.05), but no relation between membrane exposure and infiltrated connective tissue. 3. There was much bone graft resorption on DFDB than ICB and DBB. 4. The less exposure was on the perforated titanium membrane, the more dense infiltrated connective tissue was filled under the membrane when grafted with ICB and DBB. but there was no relationship between the rate of membrane exposure and the percentage of infiltrated connective tissue area and no relationship between the percentage of the area in the infiltrated connective tissue and in the residual bone graft. Within the above results, bone formation may be inhibited when membrane was exposed and ICB and DBB were more effective than DFDB as a bone graft material when guided bone regeneration.

INFLUENCE OF CAVITY SIZE AND RESTORATION METHODS ON THE CUSP DEFLECTION IN COMPOSITE RESTORATION (복합레진 수복시 와동의 크기 및 수복 방법이 교두 굴곡에 미치는 영향)

  • The aim of this study was to measure the cusp deflection during composite restoration for MOD cavity in premolar and to examine the influence of cavity dimension, C-factor and restoration method on the cusp deflection. Thirty extracted maxillary premolar were prepared to four different sizes of MOD cavity and divided into six groups. The width and depth of the cavity were as follows. Group 1; $1.5{\;}{\times}{\;}1{\;}mm$, Group 2; $1.5{\;}{\times}{\;}2{\;}mm$, Group 3; $3{\;}{\times}{\;}1{\;}mm$, and Group 4-6; $3{\;}{\times}{\;}2{\;}mm$ respectively. Group 1-4 were restored using bulk filling method with Z-250 composite. However, Group 5 was restored incrementally, and Group 6 was restored with an indirect resin inlay. The cusp deflection was recorded at the buccal and lingual cusp tips using LVDT probe for 10,000 seconds. The measured cusp deflections were compared between groups, and the relationship between the cube of the length of cavity wall/the cube of the thickness of cavity wall ($L^3/T^3$). C-factor and cusp deflection or % flexure ($100{\;}{\times}$ cuspal deflection / cavity width) was analyzed. The cusp deflection of Group 1-4 were $12.1{\;}\mu\textrm{m},{\;}17.2{\;}\mu\textrm{m},{\;}16.2{\;}\mu\textrm{m}{\;}and{\;}26.4{\;}\mu\textrm{m}$ respectively. The C-factor was related to the % flexure rather than the cusp deflection. There was a strong positive correlationship between the $L^3/T^3$ and the cusp deflection. The cusp deflection of Group 5 and 6 were $17.4{\;}\mu\textrm{m}{\;}and{\;}17.9{\;}\mu\textrm{m}$ respectively, which are much lower value than that of Group 4.

Study of endodontic working length of Korean posterior teeth (한국인의 구치부 근관작업장에 관한 연구)

  • Objectives: The aim of this study was to investigate average working lengths of Korean posterior teeth and evaluate validity of endodontic file length. Materials and Methods: The endodontic working length of the posterior teeth of 670 Korean patients were measured than each mean value and standard deviation were investigated than the frequency deviation and standard deviation per each length were calculated. Results: Among the canals of premolar, 66.5% of canal length was marked under 20 mm by endodontic working length and 95.4% could be measured under 22 mm and Among the canals of molars, 95.5% of canal length was marked under 20 mm endodontic working length. Conclusions: With the result of measurement of endodontic working length of premolars of Korean, it suggested that 23 mm endodontic file is more proper than the 21 mm and 25 mm file on the market.

A CLINICAL AND RADIOGRAPHIC STUDY OF CONGENITALLY MISSING TEETH (선천성 결손치에 관한 임상 및 방사선학적 연구)

  • The clinical and radiographic features of 655 congenitally missing teeth were studied with full mouth periapical radiograms and/or pantomograms from 368 persons visited the Department of Oral Radiology, Infirmary of Dentistry, Kyung Hee University during January 1981 to December 1989. The obtained results were as follows: 1. The prevalence of congenitally missing teeth was revealed to be 8.75% in total examined persons, and there was a higher prevalence in females (9.5%) than in males (8.0%). 2. The most frequently missing teeth were mandibular second premolars (24.6%), followed by mandibular lateral incisors (21.7%), maxillary second premolars (16.2%), and maxillary lateral incisors (11.5%). 3. There was a higher prevalence in the mandible (60.3%) than in the maxilla (39.7%), and no significant differences between right (49.65%) and left (50.35%) side. 4. In number of congenitally missing teeth per person, 54.6% had one missing tooth, and 32.9% had two missing teeth. 5. In persons with one or two congenitally missing teeth, the most frequently missing tooth was mandibular lateral incisor, and the second premolar was the tooth most frequently missing in those persons with more than three congenitally missing teeth.

A RADIOGRAPHIC STUDY OF CEMENTOMA (백악종에 관한 방사선학적 연구)

  • This study was undertaken to document and better define this condition to help clarify this clinical and radiographic appearances by the analysis of clinical and radiographic features of cementoma. A study was made of a series of 104 cases with cementoma. This investigation of cementoma revealed the following features: 1. The female occurred in 73% of periapical cemental dysplasia and benign cementoblastoma, and 80% of cementifying fibroma. 2. 40% of periapical cemental dysplasia occurred in the fifth decades, and 73% of benign cementoblastoma during the second and third decades, while there was no age predilection in the cementifying fibroma. 3. 63% of periapical cemental dysplasia occurred in the mandibular anterior region. 91% of benign cementoblastoma and 80% of cementifying fibroma occurred in the mandibular premolar and/or molar region. 4. There were no cases complaining the associated clinical signs and subjective symptoms in the periapical cemental dysplasia, however the patient complained the pain in 36% of benign cementoblastoma and 40% of cementifying fibroma. 5. There were no cases expanding the cortical plates in the periapical cemental dysplasia, however 73% of benign cementoblastoma and all of 5 cases of cementifying fibroma showed the expansion of cortical plates. 6. Several radiographic features of the periapical cemental dysplasia were shown: a. 29% of the cases had multiple lesions. b. 53% of the cases were in the mature stage. c. During the osteolytic stage, the alveolar lamina dura was lost in 89% of the cases.

Assessment of the Measurement Method of the Bone Mineral Density on Cu-Equivalent Image (구리당량 영상작성에 의한 골밀도계측방법의 평가)

  • Purpose : The effects of step numbers of copper wedge and exposure on the coefficient of determination (r²) of the conversion equation to Cu-equivalent image and on the Cu-equivalent value (mmCu) and it's coefficient of variation measured at each copper step and the mandibular premolar area were evaluated. Method: Digital image analyzing system consisted of scanner, personal computer, and a stepwedge with 10 steps of 0.03 mm copper in thickness as reference material was prepared for quantitative assessment of the bone mineral density. NIH image program was used for analyzing images. Results : The film having moderately high film density showed the discrepancy between the real thickness and the measured Cu-equivalent value of each copper step. The Cu-equivalent image was dependent on the determinational coefficient of the conversion equation than the coefficient of variance of the measured value. Conclusion : Obtaining conversion equation with high coefficient of determination and proper film exposure are supposed to be neccessary for quantitative assessment of bone density. Multiple steps in the range of the corresponding copper thickness to the bone density of the area to be measured should be prepared.

A comparison of subtracted images from dental subtraction programs (디지털공제프로그램간의 디지털공제영상 비교)

  • Purpose: To compare the standard deviation of gray levels on digital subtracted images obtained by different dental subtraction programs. Materials and Methods: Paired periapical films were taken at the lower premolar and molar areas of the phantoms involving human mandible. The bite registration group used Rinn XCP equipment and bite registration material, based on polyvinyl siloxane, for standardization. The no bite registration group used only Rinn XCP equipment. The periapical film images were digitized at 1200 dpi resolution and 256 gray levels by a flat bed scanner with transparency unit. Dental digital subtraction programs used for this study were Subtractor (Biomedisys Co., Korea) and Emago (Oral Diagnostic Systems, The Netherlands). To measure the similarities between the subtracted images, the standard deviations of the gray levels were obtained using a histogram of subtracted images, which were then analyzed statistically. Results: Subtracted images obtained by using the Emago program without manual selection of corresponding points showed the lowest standard deviation of gray levels (p<0.01). And the standard deviation of gray levels was lower in subtracted images in the group of a bite registration than in the group of no use of bite registration (p < 0.01). Conclusion: Digital radiographic subtraction without manual selection of reference points was found to be a convenient and superior method.

THE CLINICAL AND RADIOLOGIC CONSIDERATION OF CEMENTIFYING AND OSSIFYING FIBROMA OF THE JAWS (악골에 발생한 백악질 및 골화성섬유종의 임상 및 방사선학적 고찰)

  • The purpose of this study was to know the proper diagnosis and to establish the treatment plan of cementifying and ossifying fibroma in the jaws through the clinical, raiological, and histopathologic considerations. The authors compared and analyzed the c1inicoradiologic features of the thirteen cases of cementifying and ossifying fibroma, diagnosed at the Dental college hospital in Yonsei university, Seoul, Korea, during the period from 1980 to 1995. The obtained results were as follows : 1. Cementifying and ossifying fibroma occured in the mean age, 44 years, ranged from 29 to 65 years and the male to female ratio was approximately 1:5. 2. Swelling was the most common frequent presenting complaints. Other reported symptoms included pain, tooth mobility and asymptom. 3. The frequency of the lesions was twelve cases in the mandible and one case in the maxilla. And eleven of thirteen cases were distributed on the premolar and molar region. 4. Radiologically, eight of thirteen cases were well defined lesions, five cases were relatively well defined lesions. And nine of thirteen cases were mixed lesions, three cases were radiopaque lesions, and only one case was purely radiolucenct lesion. 5. Histologically, seven of thirteen cases were classified ossifying fibroma, four cases were cemento-ossifying fibroma, and two cases were cementifying fibroma.
