2008~2009년 국민건강영양조사를 활용한 45~60세 한국여성의 폐경 여부에 따른 건강인자 특성 (Characteristics of the Health Factors in 45~60 Year Old Korean Women related to Menopausal Stages - Based on 2008~2009 Korean National Health and Nutrition Examination Survey -)

  • 본 연구는 2008~2009년도 국민건강영양조사 자료를 이용하여 45세~60세 중년여성의 식사의 질을 포함한 다각적인 건강 행태를 파악하고자 폐경 전 후의 인구사회학적인 특성, 신체적, 정신적, 혈액학적, 식사의 질의 특성을 분석하여 건강 요인과 폐경과의 관계를 분석하고자 하였으며, 그 결과는 다음과 같았다. 1) 조사대상자 중 폐경 후 여성이 폐경 전 여성에 비해 연령이 유의적으로 높았고(p < 0.001), 교육수준(p < 0.001)과 월 소득(p < 0.01)은 유의적으로 낮았다. 2) 체지방률(p < 0.001)과 허리둘레(p < 0.001)는 폐경 후 여성이 폐경 전 여성에 비해 유의적으로 높았다. 3) 혈액학적 지표 특성 중 혈당(p < 0.05), 총 콜레스테롤(p < 0.001), LDL-콜레스테롤(p < 0.001), 중성지방(p < 0.001), GOT(p < 0.001), GPT(p < 0.001)의 농도가 폐경 후 여성이 폐경 전 여성에 비해 유의적으로 높았다. 4) 삶의 질은 폐경 전 여성이 폐경 후 여성에 비해 유의적(p < 0.01)으로 높은 것으로 나타났으나, 불안/우울 정도와 스트레스 수준은 두 그룹 간 차이가 없었다. 5) 식사의 질은 평균 영양소 적정섭취비(MAR)가 0.78로 다소 낮았으며, 폐경 전 후 각 군을 비교 한 결과에서는 비타민 A, 비타민 $B_1$, 비타민 $B_2$, 나이아신의 NAR 값이 폐경 후 여성에서 폐경 전 여성에 비해 유의적으로 낮았다(p < 0.05). 6) 중년여성의 연령, 허리둘레, 체지방률, BMI는 혈당, 총 콜레스테롤, LDL-콜레스테롤, 중성지방과 모두 양의 상관성을 보였다. 이상의 결과를 종합할 때, 폐경 후 여성의 비만도가 폐경 전 여성에 비해 다소 높은 것으로 나타났으며, 혈중 지질 농도 또한 폐경 후 여성에서 유의적으로 높은 수치를 나타내어 폐경 후 여성의 비만 및 혈중 지질 관리가 필요한 것으로 나타났다. 또한 칼슘, 칼륨, 비타민 $B_2$ 등의 비타민, 무기질 영양상태가 한국인의 영양섭취기준을 충족시키지 못하는 수준인 것으로 확인되었으며, 이러한 양상은 폐경 후 여성에서 더욱 뚜렷하게 나타나 앞으로 기대 수명이 증가하면서 폐경 후 의 삶이 증가된다는 점에서 볼 때 폐경 후 여성의 세밀한 건강관리와 식사의 질적 보완을 위한 체계적인 건강 및 영양교육 프로그램 구축이 사회적으로 반드시 필요할 것으로 사료된다.

The Effects of Low-Calorie Diet with Raw-Food Formula on Obesity and Its Complications in the Obese Premenopausal women

  • Recently interests on raw-food diets are rapidly increasing in relation to chronic diseases prevention in Korea, but studies on raw-food diets have been hardly performed by nutritionists. This study was performed to investigate the effects of low-calorie diets using a raw-food formula in the form of freeze-dried powder on obesity and its complications in the obese women (body mass index (BMI) $\geq$ 25kg/㎡) for eight weeks. Forty premenopausal women (mean BMI 28.04kg/㎡, mean age 28.33years old) participated in this diet intervention, and were controlled by eating 1 regular meal, 1-2 snacks and 2 raw-food formula (140kcal/pack) meals a day within the 1500-1300kcal ranges. Anthropo-mentric measurements, body compositions, physical exercise, and obesity-related risk factors were assessed before (the initial), during (the 4th week) and after (the 8th week) the study. All the data was analyzed by paired t-test, repeated measures ANOVA, and nonparametric rank test at p<0.05 level. Obesity was significantly increased during this study, and it was decreased in weight (-4.59%, p<0.000), BMI (-4.56%, p<0.000), body fat percent (-6.18%, p<0.000), fat mass (-10.19%, p<0.000), waist and hip circumferences(-5.69%, p<0.000 and -2.55%, p<0.000) and WHR (-3.24%, p<0.000). Energy expenditure of physical exercise was increased as much as 70kca1/day during the study (p=0.000), but it did not have any correlations with weight loss and changes of body compositions. Biochemical measurements including blood triglyceride(p <0.006) and leptin(p<0.000) levels were significantly decreased, LDL cholesterol level was increased(p<0.05), but all the blood lipid levels were in the normal ranges. Fatty liver echogenicity and menstrual irregularity were improved after the diet intervention(p<0.000 and p<0.034). In conclusion, this B-week low-calorie diet intervention using raw-food formula was effective for obese premenopausal women in reducing obesity and its risk factors so as not to proceed towards comorbidities. However, the variation of blood lipid levels should be observed for a longer Period.

이소플라본의 투여가 골질량이 감소된 저체중과 정상체중 여대생의 골밀도 및 골대사 지표에 미치는 영향에 관한 연구 (A Study of Soy Isoflavone Supplementation Effect on Bone Mineral Density and Bone Metabolism Markers in Female College Students with Low Bone Mass)

  • To investigate the effects of isoflavone supplementation on both bone mineral density and hormone variation in premenopausal women who had decreased bone mass, the 24 subjects were divided into two groups: one was the underweight group, consisting of 13 subjects, and the other was the normal weight group consisting of 11 subjects. For each group, we investigated the effects of isoflavone supplementation of 90 mg/day on both bone mineral density and hormone variation during 3 menstrual cycles. Anthropometric measurements, dietary recall, and analyses of blood and urine were assessed from baseline to post-treatment. The results were as follows: The average age of the underweight group was 21.8 years old and that of the normal weight group was 23.2 years old. The comparative results for the two groups at baseline were as follows: Onset of menarche, menstrual cycle, and menstrual length were not significantly different between the groups. Serum protein, total, HDL-, LDL-cholesterol, triglyceride, Ca, P, Mg, Cu, and Zn level were not significantly different between the groups. Serum estradiol, SHBG, LH, and FSH level were also not significantly different between the groups. Lumbar spine BMD by T scores of the underweight group was significantly lower than that of the normal weight group. Serum osteocalcin, urinary DPD, and urinary pH were not significantly different between the groups. The comparative results for the two groups at post-treatment were as follows: From baseline to post-treatment, the intake of energy, nutrients and isoflavone in food did not significantly change in either group. Serum protein, total cholesterol, HDL-, LDL-cholesterol, and triglyceride levels did not significantly change in either group. Serum Ca, Cu, and Zn levels were significantly lower in both groups and serum Mg level significantly decreased only in the underweight group. Serum estradiol levels were significantly lower in both groups, but serum SHBG, LH, and FSH levels did not significantly change in either group. Lumbar spine BMD by T score of the underweight group significantly increased to 15%, but that of the normal weight group did not significantly change. Serum osteocalcin of the underweight group significantly increased to 28%, while that of the normal weight group significantly increased to 40%. Urinary DPD of the normal weight group significantly increased to 12%. The results show that the BMD of the underweight group was lower than that of the normal weight group. Therefore, the underweight group had a disadvantage in obtaining maximum bone mineral density. The results also show that isoflavone supplementation during 3 menstrual cycles was effective in increasing the bone mineral density of the lumbar spine and affected bone metabolism markers in premenopausal underweight women. Therefore, it can be concluded that sufficient intake of isoflavone could be helpful in preventing decreases in bone mass among premenopausal women, especially underweight women.

유방암 환자에 있어서 폐경상태에 따른 위험인자의 상관성 연구 (A Study on Relationship to Risk Factors according to Menopausal Status in Breast Cancer)

  • It is important to identify modifiable risk factors for breast cancer, because the breast cancer is one of the major causs of mortality among women. Some reported that obesity is a risk factor for breast cancer, but the results are not constant. Many risk factors are related to the duration of estrogenic stimulation of the breast. In general, early menarche and late menopause are positive risk factors. Human breast cancer has different characteristics according to the status of menopause(premenopause and postmenopause). In premenopausal women, about 60% of circulating estrogen is from the ovaries in the form of estradiol, and the remaining 40% is estrogen formed primarily in the adipose(fat) tissue via aromatization of androstenedion from the adrenal glands. After menopause this adipose cell production of estrone is the main source of estrogens and the level of estrone is maintained approximately at premenopausal levels. This study was undertaken to determine the role of body size and body mass index by status of menopause in development of breast cancer using retrospective case/control study. From March 1991 to February 1997 at the Wonkwang University Hospital, the breast cancer cases(n=72) and controls(n=86) were selected. By statistical analysis method, regression analysis, paired T-test and multiple logistic regression were done to estimate the influenced factors same as height, weight, BMI, age at menarche and age at menopause. The following results were obtained : 1. In premenopausal women, age at menarche was showed comparatively high correlation coefficients and BMI was described prominently highly in postmenopause. 2. At the results of multiple regression analysis, age at monarch, BMI and weight were showed as significant variables. In this method, critical factor ($R^2$) was 0.054. 3. Paired samples T-test was undertaken to test mean difference between two groups of cases and controls. The result of test performance showed a significant difference. 4. In comparison with women whose weight less than 50 kg, the ORs for the upper 5th group was 1.82(95% confidence interval). The heaviest women had a higher risk(OR=1.14, 95% confidence interval $1.12{\sim}1.31$, p=0.005). Higher body mass index was significantly associated with increased risk of premenopausal breast cancer (OR=1.01, 95% confidence interval $1.08{\sim}l.18$, p=0.05).

A Comparative Study of the Iron Nutritional Status of Female College Women according to Bone Mineral Density

  • The purpose of this study was to investigate the relationship between nutritional status of iron and bone minernl density in premenopausal women. In the study, we classified the subjects into osteopenia (-2.5-I, n=29) groups according to their lumbar spine bone mineral density. Anthropometric measurements, dietary intake analysis and blood biochemistry measurements were performed on the subjects. The average ages of those in the osteopenia and normal groups were 22.2 yrs and 23.0 yrs, respectively, with no significant difference. The average body mass index (p<0.05) of those in the osteopenia group (19.6) was significantly lower than that of the normal group (21.3). The mean protein intake of those in the osteopenia group was significantly lower than that (p<0.05) the subjects in the normal group. The osteopenia group consumed a significantly lower amount of iron (p<0.05) and non-heme iron (p<0.05) compared to the normal group. The intakes of total food, vegetables and milk of those in the osteopenia group were significantly lower than those of the subjects in the normal group. The serum ferritin (p<0.001) level of those in the osteopenia group was significantly lower than those of the subjects in the normal group. In conclusion, a balance of iron status may be helpful in the prevention of bone mass loss in premenopausal young women.

폐경전후 여성의 삶의 질에 미치는 영향요인 (Influencing Factors on Quality of Life in Pre- and Postmenopausal Women)

  • Purpose: This study was to investigate the factors influencing quality of life and difference of quality of life in a postmenopausal group compared to premenopausal women. Method: The subjects consisted of 213 women from 40 to 64 years old. The data were collected from June 1, 2003 to June 31, 2004 using structured questionnaires which included questions relating to demographic and obstetric background and 4 scales(climacteric symptoms scale, marital satisfaction scale, sex-role attitude scale, quality of life scale). Result: Comparing pre- and postmenopausal women, significant statistical differences were demonstrated in climacteric symptoms and sex-role attitude. However, quality of life was not different between pre- and postmenopausal women. In premenopausal women, marital satisfaction($32.7\%$) and educational level($8.0\%$) were significant predictors to explain quality of life. Marital satisfaction($12.9\%$) was significant predictor to explain quality of life in postmenopausal women. Conclusion: No difference of quality in life between pre- and postmenopausal women provides information for changing traditional approaches of menopause according to physiological changes as illness. In addition the study showed that it is necessary to use marital satisfaction information when developing nursing interventions to promote the quality of life.
