Systemic Review : The Study on Missed Abortion (Pub Med 검색을 통한 계류유산의 최신 연구에 대한 고찰)

  • Purpose: The purpose of this study was to investigate about missed abortion pathophysiology, diagnosis, medical treatment and to research the trend of the study related to missed abortion. Methods: We referred a PubMed site by using search word of "missed abortion"(Limits: 3 Year, only items with abstracts, Human). Results: 37 journals with 49 papers were searched. Conclusion: 1. The study of missed abortion pathophysiology was the following. The first was that important pathologies such as molar pregnancy and placental trophoblastic disease can be diagnosed by routine histopathologic analysis of product of conception following first-trimester spontaneous miscarriages. The second was that coelomic fluid leptin concentration in missed abortion is higher than in normal. The third was that adenosine deaminae activity in serum and placenta of patients with anembryonic pregnancies and missed abortions was low. The forth was that Leptotrichia amnionii sp. nov. was the etiopathogenetic factor in missed abortion. 2. Transvaginal ultrasound assessment of irregular vaginal bleeding is effective in diagonosis of missed abortion. 3. There were medical therapy with misoprostol, mifepristone or anti progesterone for missed abortion. Misoprostol was administrated oral(sublingual) and vaginal.

  • Studies on the clinical significance with Amerlex $FT_{4}$ RIA kit observing the determination of $FT_{4}$ were investigated using a tracer as $^{125}I-T_{4}$ derivative which is not almostly bound to thyroxine binding globulin, etc. The results are followed; 1. $FT_{4}$ value($1,55{\pm}0.38ng/100ml$) of normal group was not accorded that of hyperthyroidism with Amerlex $FT_{4}$ RIA kit, and was higher than that of hypothyroidism. 2. $FT_{4}$ value was lower level in chronic-kidney disfunction syndrome whereas, it was normal in a cancer patient, a woman in pregnancy and a patient in TBG disfunction. 3. The value of this method is a good corelationship at that of equilibrium dialysis method. (r=0.931) 4. $FT_{4}$ value by this kit was linear relationship to those of the other kit (Gamma Coat and Liquisol), and the normal value of each methods was also similar. As mentioned above, this method is more simple and rapid, compared to the other method. Therefore, it was thought that this method is a very useful clinically.

  • Eight female Himalayan tahrs (Hemitragus jemlahicus) were estrus-synchronized, and transcervically inseminated with frozen-thawed semen in September, 2009, about 2 to 3 months earlier than their natural breeding season. Intravaginal progesterone-releasing devices were inserted into vaginas of six Himalayan tahrs on September 7, and the other two on September 8 to suppress luteal function of ovaries. The devices had been placed deep inside the vagina prior to withdrawal on September 23. A day before CIDR removal, a combination of PMSG 400 IU and hCG 200 IU was intramuscularly injected. Forty hours later, frozen-thawed semen was transcervically inseminated. Pregnancy diagnosis was performed 39 days later by analyzing progesterone level of serum. Every treatment was done under anesthesia inducted by xylazine injection. In conclusion, vaginal discharge of cervical mucus, hormonal changes induced by implant-typed or muscularly injectable hormones and widening of cervix enough to insert an insemination gun into uterine body were achieved in non-breeding season. Moreover, the first inseminated Himalayan tahr, 36 hours after CIDR removal was assumed to be pregnant but the fetus may have been lost due to the use of anesthetic drug.

  • Vitamin D is an essential component of bone and mineral metabolism; its deficiency causes growth retardation and skeletal deformities in children and osteomalacia and osteoporosis in adults. Hypovitaminosis D (vitamin D insufficiency or deficiency) is observed not only in adults but also in infants, children, and adolescents. Previous studies suggest that sufficient serum vitamin D levels should be maintained in order to enhance normal calcification of the growth plate and bone mineralization. Moreover, emerging evidence supports an association between 25-hydroxyvitamin D (25[OH]D) levels and immune function, respiratory diseases, obesity, metabolic syndrome, insulin resistance, infection, allergy, cancers, and cardiovascular diseases in pediatric and adolescent populations. The risk factors for vitamin D insufficiency or deficiency in the pediatric population are season (winter), insufficient time spent outdoors, ethnicity (non-white), older age, more advanced stage of puberty, obesity, low milk consumption, low socioeconomic status, and female gender. It is recommended that all infants, children, and adolescents have a minimum daily intake of 400 IU ($10{\mu}g$) of vitamin D. Since the vitamin D status of the newborn is highly related to maternal vitamin D levels, optimal vitamin D levels in the mother during pregnancy should be maintained. In conclusion, given the important role of vitamin D in childhood health, more time spent in outdoor activity (for sunlight exposure) and vitamin D supplementation may be necessary for optimal health in infants, children, and adolescents.

  • Gestational choriocarcinoma is a highly malignant tumor which arises from the trophoblast of human pregnancy. This tumor develops early pulmonary metastasis and the most common pattern of pulmonary metastasis is discrete multiple nodules. But occasionally solitary pulmonary metastasis occurs. Authors presented three cases of choriocarcinoma presented with different types of solitary pulmonary metastases with review of literatures. We emphasize the importance of careful obstetric history taking and screening of serum gonadotropin level in differential diagnosis of solitary pulmonary lesion especially among women who are from areas of high prevalence of trophoblastic neoplasia.

  • Wolf-Hirschhorn syndrome (WHS) is caused by a deletion of the short arm on chromosome 4 and is characterized by multiple congenital abnormalities, growth and mental retardation. In this case report, we performed amniocentesis for the chromosome analysis on a 25-year-old pregnant woman at 16 weeks of gestation whom we suspected of Edward's syndrome by the triple test of maternal serum and ultrasonography. The result of analysis revealed a karyotype of the fetus with 46,XY,del(4)(p15) by trypsin Giemsa's banding technique. With the result, we were able to diagnose the fetus as having WHS. As such, after therapeutic termination of the pregnancy, we confirmed WHS through the sampling of tissue by both trypsin Giemsa's banding and fluorescence in situ hybridization (FISH) method. To determine the origin of the WHS, we further tested the karyotypes of the parents. As parental karyotypes were found to be normal, we determined the case of the fetal WHS to be de novo.

Mammary Gland Indices at the End of Lactation in the Superovulated Javanese Thin-Tail Ewes

  • Thirty lactating Javanese thin-tail ewes (12 ewes had been injected, prior to mating, with 700 IU pregnant mare serum gonadotropin, and 18 ewes with saline as a control) were used to evaluate the effect of superovulation on milk production during lactation and mammary chemical indices at the end of lactation. Thirteen ewes (9 control and 4 superovulated ewes) were fed at low and the other 17 ewes (9 control and 8 superovulated ewes) were fed at high quality ration. Superovulated ewes, either fed at low or high quality ration, had dramatically higher milk yields (57%). At the end of lactation, superovulated ewes had higher mammary dry fat-free tissue, mammary DNA concentration, total mammary DNA and RNA contents than nonsuperovulated ewes. Superovulation did not affect mammary RNA and collagen concentrations, and total collagen content. Ration quality did not significantly increase milk production during lactation and mammary chemical indices at the end of lactation. The observed increase in milk production in the superovulated ewes was probably due to the increased mammary secretory cell number and their synthetic activities during lactation as a result of the increased endogenous hormonal stimulation of mammary growth and development during pregnancy.

  • This study was performed in order to evaluate the sero-epidemiological status of toxoplasmosis in pregnant Korean women. Among 5,175 sera and 750 amniotic fluid samples obtained from pregnant women, 41 serum samples $(0.79\%)\;and\;10 (1.33\%)$ amniotic fluid samples tested positive for IgG antibodies by ELISA. Fifty one cases showing a score more than 0.25 on ELISA were tested for PCR reaction against the SAG1 gene. Only one case of the 51 ELISA positive cases exhibited a positive reaction on all tests. This case had a history of acute nephropyelitis during early pregnancy, but fortunately, had delivered a phenotypically healthy baby. In this study, the seroprevalence of toxoplasmosis in pregnant women was found to be comparatively low, consistent with previous reports from Korea. However our trials, performed with a variety of diagnostic tools, were considered to be useful for the precise diagnosis of congenital toxoplasmosis.

  • Objectives: The purpose of this study is to report the effects of Korean medical treatment in ovarian disorder patients resistant to controlled ovarian hyperstimulation. There are some case-reports about effectiveness of the Korean medical treatment to treat ovulation disorder and become pregnant, but cases are still deficient. Methods: The patient was diagnosed possibility of premature ovarian failure by serum hormone assay and underwent hyperstimulation to freeze oocyte. But ovary had no response to hyperstimulation. She was treated by Korean medical treatment, such as herbal medicine, acupuncture and moxibustion. Results: After the treatment, the patient recovered from irregular ovulation & menstrual cycle and became pregnant and gave birth. Conclusions: This case shows the Korean medical treatment might be effective in ovarian disorder patients resistant to hyperstimulation.

  • Hog cholera serum neutralizing antibody of piglets and sows were titrated by means of END SN method. The piglets of a variety of ages, precolostrally immunized with LOM living HC vaccine were subjected to the test. The sows were vaccinated with lapinized living HC vaccine after 25 days from the parturition. Throughout the studies the following results were obtained and summarised. 1. Hog cholera antibody titers of inbred sows immunized with lapinized living HC vaccine after 25 days from parturition were high except Hampshire group(Table 2). 2. Sows, different stage of the pregnancy or the day of parturition, and of 3 way crossed, that were immunized with lapinized living HC vaccine have shown no significant difference on HC antibody titer(Table 2, 3). 3. HC antibody titers of piglets, immunized with a single dose of LOM HC vaccine before feeding colostrum, were high in case of the younger group(1 week) compare to the older(7 week) (Table 4). 4. The piglets that were booster immunized with LOM HC vaccine at the age of 7 weeks have shown an inconsistent antibody titers(Table 5).
