A Study on the Awareness of Health and the Utilization of Primary Health Care in Rural Areas (일부 농촌지역주민의 보건의식과 일차의료 이용 실태에 관한 조사)

  • This study was to examine the awareness of health and the utilization of primary health care in rural areas. The data were obtained from self-administered questionnaire conducted 450 parents of Mi Gum md Su Dong middle school students in Nam yang Ju city, Kyung Gi-Do, Korea, from December 15 to 20 in 1993. The results were as follows : 1) Among the causes of disease, 'insufficient health care' was the highest(39.1%), and 'bad environment'(28.9%), 'complexity of life style'(17.8%) in next order. 2) In the priority between 'daily farm work' and 'primary health care', only 45.6% of respondents answered that primary health care is more important than the daily farm work. The 29.8% of respondents answered 'daily farm work', and the 23.1% answered 'the equal of the both'. 3) The 63.6% of the respondents recognized correctly, the meaning of primary health care. And the rate of information source in primary health care were 'TV and Radio'(42.2%), 'medical facilities'(23.3%), and 'newspaper and magazine'(11.3%) in order. 4) In the choice rate of medical facilities for primary health care, 'drug store' was the highest(34.9%), and 'local private clinic'(34.7%), 'health (sub)center'(15.8%), 'hospital'(10.2%) in next order. 5) The 53.5% of the respondents had experienced to visit the health (sub)center more than once. And the disfavorite reasons of health (sub)center were 'insufficient equipment'(36.7%), 'inavailable time to visit'(26.9%), and 'poor treatment'(9.1%). 6) Among the preference of the physicians for primary health care, 'specialist' was the highest rate of the respondents(54.2%), and 'general practitioner'(32.4%), and 'family doctor'(9.8%) in next order. The major obstacles in utilizing the medical facilities for primary health care were 'daily farm work'(41.6%), 'distance'(21.1%) and 'medical cost'(10.4%) in order. 7) The weakened reasons in health (sub)center function were 'insufficient medical equipment'(44.4%), 'the lack of resident's understanding for health (sub)center'(21.8%), and 'short thought of duty in health (sub)center personnel'(16.9%) in order.

Evaluations of the Space Dose and Dose Reductions in Patients and Practitioners by Using the C-arm X-ray Tube Shielding Devices Developed in Our Laboratory

  • The present study used a digital angiography x-ray device to measure the space dose and exposure dose of patients and practitioners using x-ray tube shielding devices developed in our laboratory. The intent of the study was to reduce the space dose within the test room, and to reduce the exposure dose of patients and practitioners. The patient and practitioner exposure doses were measured in five configurations in a human body model. The glass dosimeter was placed on the eye lenses, thyroid glands, left shoulder, right shoulder, and gonads. The beam was collimated at full size and at a 48% reduction for a comparative analysis of the measurements. The space dose was measured with an ion chamber at distances of 50 cm, 100 cm, and 150 cm from the x-ray tube under the following conditions: no shielding device; a shielding device made of 3-mm-thick lead (Pb) [Pb 3 mm shield], and a shielding device made of 3-mm-thick Pb (outside) and 3-mm-thick aluminum (Al) (inside) [Pb 3 mm+Al 3 mm shield]. The absorbed dose was the lowest when the 3-mm-thick Pb+3-mm-thick Al shield was used. For measurements made with collimated beams with a 48% reduction, the dose was the lowest at $154{\mu}Gy$ when the 3-mm-thick Pb+3-mm-thick Al shield was used, and was $9{\mu}Gy$ lower than the measurements made with no shielding device. If the space dose can be reduced by 20% in all situations where the C-arm is employed by using the x-ray tube shielding devices developed in our laboratory, this is expected to play an important role in reducing the annual exposure dose for patients, practitioners, and assistants.

  • As a nursing practice involves nurses'actions in a specific context of health care, this study has focused on exploring the espoused theories in nursing practice within the action science perspectives. Espoused theories are the belief, principles, and rationale expressed by the practitioner as guiding her/his actions in a situation of practice. The data were analysed qualitatively and 25 elements of espoused theories of nursing action were identified and clustered into 6 categories. The 25 elements of espoused theories are as follows: The clinical nurse worked in wholistic and individual nursing, focussed on the patient's needed, comfort and supportive nursing (5 theories of nursing goal); excellent skills, knowledge based, assessment and data collection, explaining, educating or a scientific basis(6 theories of nursing intervention): advocacy, value oriented, treatment, accountability and commitment(4 theories of nursing ethics); human respect, partnership, trust(3 theories of patient-nurse relationship) : knowledgable, accumulated clinical experiences and personally lived experiences, positive perspectives(4 theories of nurse), role of intervention, rewarding, peer relationship(3 theories of situations). The above mentioned espoused theories are similar to that of nursing textbooks which students learned through basic nursing education and almost the same as the Acts ofa Nurse in Korean. However, we are doubtful whether nurses actually do as they think. Therefore, it is recommended to review the theories-in-use in order to find any discrepancies between the espoused theories and the reality of nursing actions.

The Medical Study about the Development of Pediatrics for Song(宋) Dynasty (양송시기(兩宋時期)의 소아과학(小兒科學)에 관한 연구)

  • The Song Dynasty(宋代) period gained the summit in the developmental phase of the history in pediatrics. In 1076, Taeuiguk(太醫局) was founded and Sobangmaek(小方脈) was the one of nine branch in Taeuiguk(太醫局). As a result, educational agency and system was arranged in the field of pediatrics. At the same time, pediatrics have furnished with foundation to develop independent field in medical science. Especially, Byeonjeungnonchi(辦證論治) system is established in pediatrics science according to the compilation of "Soayakjeungjikgyeol(小兒藥證直訣)" by Jeoneul(錢乙). Children's doctors, Jeoneul(錢乙), Jinmunjung(陳文中) and Donggeup(董汲) arranged clinically theories, so they made that age the peak of the pediatrics. Looking at the developmental part of theory in pediatrics of the Song Dynasty period, Jeoneul(錢乙), who was the representative children's doctor in Song Dynasty period, insisted that children had formed Ojang-yukbu(五臟六腑) in the physiological part but they were not perfect like adult's. Therefore they had to consider the poor part in children's body, and then they had to cure them. 'Byeonjeung(變蒸)' was considered to be normal physiological phenomenon. Also he insisted that Yukeum(六淫), 'Pung, Han, Seo, Seup, Jo, Hwa(風, 寒, 暑, 濕, 燥, 火)' and 'Gyeong, Pung, Gon, Cheon, Heo(驚, 風, 困, 喘, 虛)' are the main cause of the children's five vital organ's pathology Like this, his academic thought, which lay emphasized on children's spleen and stomach and Ojangbyeonjeung(五臟辦證) main principles, affected on doctors, Donggeup(董汲), Yeomhyochung(閣孝忠), Yubang(劉肪), Jang-wonso(張元素), Idong-won(李東垣), Manjeon(萬全). Bangje(方劑) fields of pediatrics had developed so fast and "Taepyeongsunghyebang(太平聖惠方)" contained about 2689 kinds of children's prescription. According to this, there are so many kinds of medicinal shape, for example 'Hwan, San, Go, Dan(丸, 散, 膏, 丹)', which cured children' s disease. Through this, we find that the shape of medicine about the children's diseases had developed to the more simple direction, taking account of the children's clinical trait.

Analyzing the Importance and Priority of Safety Information in Walking Tourism: Focusing on Open Data Utilization (도보여행 안전정보의 유형별 중요도 및 우선순위 도출: 오픈데이터 활용을 중심으로)

  • The tourism industry is now changing to smart tourism, which maximizes tourists' overall tourism experience with the use of advanced mobile technologies and emphasizes the utilization of tourism information. Despite the quantitative expansion of the tourism industry, there is a lack of academic and practical discussion on tourism safety. Especially, in the context of walking tourism, tourists are more likely to be exposed to natural or social disasters and emergencies. Therefore, it is necessary to build a system that can provide walking tourists with safety information not only on dangerous factors which are anticipated to be confronted during a walking trip in advance but also on specific dangers in real time. Under the circumstances, this study seeks to identify the types of tourism safety information that can be offered by using publicly available open data, drawing on the safety information framework on the walking tourism that is presented in Choi et al. (2017)'s study. More specifically, this study focuses on the use of open data which is provided by the Korean government. Furthermore, this study verifies the types of safety information that are most urgently needed in walking travel situations. Specifically, this study aims to derive the importance and priority of each type of safety information for a walking trip by applying the analytic hierarchy process (AHP) analysis. For this, we collected 35 questionnaires from walking tour operators (practitioners) and walking tourists. The main results are as follows. First, natural disaster information is the most important factor in the top-level factor of safety information for walking tourists, followed by social disaster, life safety, and exhibition (security crisis) information. Second, information on natural disasters, environmental pollution, and weather is considered to be important at the sub-level factor. Lastly, the noteworthy result of this study is that the importance of each type of safety information varies depending on the walking tour operators (practitioners) and the walking tourists. That is, there is a recognition difference between the operator (practitioner) and the user in the importance and priority of the safety information of the walking trip. Therefore, it is necessary to develop policies and services reflecting the opinions of potential users when providing safety information so that the most importantly recognized information can be provided first.

A Study on Substantiation Methodology for Extension of Time in International Mega-Project Claims (해외 대형건설공사 공기지연클레임의 공기연장기간 입증방법)

  • Recently many Korean large-sized construction companies as well as specialty contractors have been expanding their business in the overseas market to mitigate impacts from long-term stagnation in the domestic market. Such as a result, it has been arisen as a major issue that contractors effectively manage the delays which have been commonly faced with in international mega-projects. Therefore it can be said that the ability of the delays management leads a success of project. This paper presents a review of literature for delay analysis methods such as time impact analysis(TIA) and raises practical problems to rationally substantiate the duration of extension of time(EoT) in international mega projects which have wide scope of works as well as complicated and detailed contract baseline programs by taking theoretical approaches. We suggest the two improvement concepts to substantiate the EoT claims which are effectively applicable to practical projects. The first is to sort out dominant delays among numerous delays in advance of applying a delay analysis. The second is to set up the delay sections with the advanced concept for analysing delays' effects by TIA. This study performed a case study on an actual international mega-project. to confirm the effectiveness of improvement concepts that this practical approach is applicable for the assessment of EoT. In conclusion, this paper supposes to provide practitioner in charge for an EoT claim with an advanced substantiation methodogy when they not only analyse the construction delays by using TIA, but also negotiate with stakeholders about with the assessment of EoT.

EFFECT OF RESIN MATRIX ON DEGREE OF CONVERSION AND FRACTURE TOUGHNESS OF DENIAL COMPOSITES (기질레진의 조성에 따른 복합레진의 물리적 성질에 관한 연구)

  • Current composites are made with dimethacrylate monomers and silane-treated silica microfillers, either alone or with silane treated glass fillers The main reasons for clinical failure of dental composites are secondary caries, wear and fracture. Most of practitioner want to get a composite which is more tougher under occlusal stress, less polymerization contraction, and better handling properties in application clinically. The aim of this study was to investigate the influence of resin matrix with various flows on the physical proper-ties such as fracture toughness and degree of conversion of the experimental resins. It was hypothesized that flexible or tough resin composites can be designed by judicious choice of monomer composition Various flow resin matrices containing Bis-GMA, UDMA, and TEG-DMA were made by altering the pro-portion of the monomers. After the unfilled resins were light-cured for different light intensity, the fracture toughness(K$_{1c}$) was measured according to ASTM standard using the single edge notched geometry, and degree of conversion(DC) was measured by FTIR. And experimental composites were formulated with variations in the proportion of silanated quartz and strontium glass fillers as 60, 75, and 77wt%. Also, the physical properties of composites with various filer contents were evaluated as same manner. All resulting data were compared by ANOVA/Tukeys test at 0.05 level. The results were as follows; 1. The degree of conversion of high flow resin containing less Bis-GMA was higher than that of low flow unfilled resin 2. While the degree of conversion of unfilled resin was increased according to light intensity for polymerization, there was no significant increase with moderate and high light intensity. Also, the fracture toughness was not increased by high light intensity. 3. The fracture toughness was high in the low flow unfilled resin containing higher contents of Bis-GMA. 4. There was a significant increase for fracture toughness and a tendency for degree of conversion to be reduced when the content of fillers was increased. 5. In the experimental composites, the flow of resin matrix did not affected on the fracture toughness, even, which was decreased as increase of viscosity. These results showed that the physical properties of a dental composite could be attributed to the flow of resin matrix with relative content of monomers. Specific combination of resin monomers should be designed to fulfil the needs of specific indication for use.

  • Current business environment asks the fundamental changes about business security. The essences of these changes are that the security management of today's global business is important task of managers and the security practitioner is professional who needs very specialized education and training with business thinking. Rapid process of globalization of global village tore down the business limit that was restricted on the geological areas' limitation. Rapid business environments' change that is driving depends on development of science and technology with globalization needs new paradigm to keep business continuity. With the process of globalization, Korea, which importance is gradually increasing in the national economy, has trade dependent economic system, which keeps power of national economy through trade, so Korean economic tendency is accelerating. To keep competitiveness in global market, new strategy that is different with existing domestic business management is necessary. That is, capacity of coping with outside risk in domestic business management is established in some degree, but business activities in foreign countries faces at numerous unexpected risks that differ from country to country such as difference with the custom, changes of corporate governance etc. To cope with these new risks effectively, new paradigm for business risk is necessary. Especially, flexibility of thinking like new paradigm is necessary to cope with new security risk effectively. To cope with security risk that occurs in the new business environment effectively and competes against international company in global market, company management and members' changes of cognition about security and innovative changes in security policy is necessary. In the basement of these changes, there is expansion of business security tasks, improvement of report line, enhancement of professionalism and status of security officers, variation of hands-on workers and increasing of investment to the security etc.

Outcomes for Employment of a Trauma Clinical Nurse Specialist in the Treatment of Trauma Patients (외상환자의 치료에서 외상 전문간호사 도입 결과)

  • Purpose: Ongoing treatment and care, as well as initial stabilization, are required for trauma patients. With increasing number of sickest trauma patients and shortage of surgeons, the need for advanced practice nurse to provide and coordinate trauma care has been greater. The purpose of this study is to analyze the effect of hiring a trauma clinical nurse specialist and its influence on the treatment of trauma patients. Methods: Based on the employment of the clinical nurse specialist in December 2010, the patients were divided into two groups: patients admitted from January 1, 2010 to November 30, 2010 and patients admitted from December 1, 2010 to December 31, 2011. Retrospectively, data were collected using electronic medical records. The general characteristics, clinical courses, and ICU re-admission rates, collaboration (transfers to other departments and collaborative surgery) were compared. Results: To have a clinical nurse specialist on the trauma team resulted in a statistically significant reductions in the length of general ward hospital stay (p<0.05), the ICU re-admission rate, (p<0.03), the lead-time before transfer to other departments (p<0.05). Conclusion: The clinical nurse specialist, as a professional practitioner, improved the quality of treatment through early detection and management of problems. In addition, as a coordinator, the clinical nurse specialist maintained a cooperative relationship with multi-disciplinary medical personnel. The trauma clinical nurse specialist contributed to the treatment of trauma patients positively through a decrease in ICU re-admission rate and length of hospital stay.

A Study on the Medical Ethics Education at Colleges of Korean Medicine (한의대 교과목으로서 의료윤리에 관한 고찰)

  • Objectives : According to the data from the 2016 Yearbook of Traditional Korean Medicine, 10 out of 12 medical schools of Korean medicine are offering medical ethics. Medical ethics has become essential in Korean medicine education, but there has been no agreement on the content of education yet, so initial discussions are necessary with respect to the content and methods of education. Methods : In this study, basic data were collected by searching papers, reports, books, and media articles on medical ethics related to Korean medicine education, and by studying the website of medical schools of Korean medicine nationwide. Based on the collected data, the status of medical ethics lectures were determined and compared with the current state of medical ethics lectures by western medical schools. The contents suitable for medical ethics education at medical schools of Korean medicine were discussed. Results : The topics of the medical ethics include: the basic concepts of medical ethics, the ethics of birth, the ethics associated with genetics, the ethics associated with death, and the ethics regarding doctor-patient interaction, the ethics of medical research, medical rationing ethics, ethics between medical staffs, medical law and ethics, philosophical base of medical ethics, ethics of doctor as professional, and moral personality formation of doctor. The contents of medical ethics in traditional Korean medicine reflected views on the human body and life based on "Huangdi's Internal Classic"and medical ethics from the viewpoints of Buddhism and Confucianism. Conclusions : Medical schools of Korean medicine are medical training institutions, and medical ethics education is essential to become a Korean medicine doctor as professional worker, medical practitioner, and biomedical researcher. There is no fundamental difference in the basic principles of medical ethics in both western and Korean medicine, and there are differences in contents depending on the clinical practice. The contents of medical ethics on clinical practice should be modified for Korean medicine doctors, and traditional Korean medicine ethics would be set up upon that. In the national licensing examination, medical ethics needs to be added as one part so that all the ethical problems related to the clinical situation can be solved.
