Properties of the Measures to Assess Oxaliplatin-induced Peripheral Neuropathy: A Literature Review (옥살리플라틴 유도 말초신경독성 측정도구의 고찰)

  • Purpose: The purpose of this study is to provide a comprehensive overview of the various measures available for assessment of oxaliplatin-induced peripheral neuropathy (OXLIPN) and to evaluate the measurement properties of each assessment tool. Methods: A systematic review was conducted to identify existing measures for OXLIPN found in the databases of PubMed, Cochrane Library, Embase, RISS and KoreaMed. The quality of the 24 identified tools was evaluated based on their properties of measurement including content validity, internal consistency, criterion validity, construct validity, reproducibility, responsiveness, floor-ceiling effects and interpretability. Results: Ten (41.7%) of the 24 tools were identified as specific measures for assessing OXLIPN and the most popular type of measures were clinical grading systems by clinicians (58.3%) and only 29.2% of measures were identified as patient reported outcomes. The most frequently used tool was National Cancer Institute-Common Toxicity Criteria (NCI-CTC), but the validity of NCI-CTC has not been reported appropriately. Overall, the Neuropathic Pain Symptom Inventory (NPSI) received the best psychometric scores, and the Chemotherapy-induced Peripheral Neuropathy Assessment Tool (CIPNAT) and Functional Assessment of Cancer Therapy/Gynaecologic Oncology Group-neurotoxicity-12 (FACT/GOG-Ntx-12) followed NPSI. Conclusion: To select appropriate measure, evidences should be accumulated through the clinical use of tools. Therefore, practitioner and researchers are urged to report relevant statistics required for the validation of the currently used measures for assessment of OXLIPN.

  • This study suggested cooperative governance cases and improvement strategies between local welfare centers and urban regeneration support centers, centered on urban revitalization areas around Seoul Station, by applying FGI. Based on the analysis of the contents of FGI participating in the cooperative governance process, it was proved that the common role and identity between the local welfare center and the urban regeneration support center were regional based activities. And the advantage of constructing collaborative governance was analyzed by four ways. In addition, this study presented improvement strategies from a microscopic and macro perspective. The results of this study suggest that the change in the curriculum of social workers' education and the expansion of the scope of the practitioner in the Urban Regeneration Business Act propose.

  • The CCC-r chart is more effective than traditional attribute control charts for monitoring high-quality processes. In-control process parameters are typically unknown and should be estimated when implementing a CCC-r chart. Phase II control chart performance can deteriorate due to the effect of the estimation error. In this paper, we used the standard deviation of average run length (ARL) as well as the average of ARL to quantify the between-practitioner variability in the CCC-r chart performance. The results indicate that the CCC-r chart requires larger Phase I data than previously recommended in the literature in order to have consistent chart in-control performance among practitioners.

  • With the improved cure rates for childhood malignant conditions in the past decade, late effects of cancer therapy must be recognized to minimize their impact on the quality of life in long-term survivors. Chemoradiation therapy is a major part of pediatric oncology treatment and is implicated in causing tooth agenesis, microdontia, root shortening, early apical closure, and coronal hypocalcification. Dental development may be affected by illness, trauma, chemotherapy, or radiation therapy at any point prior to complete maturation. Treatment given during the first 3.5 years of life was more likely to affect the dental lamina and crown formation and result in a small tooth. Dental treatment affected by chemoradiation damage to developing teeth includes orthodontic tooth movement, prosthetic abutment consideration, periodontal health, space maintenance, requirement for home fluoride regimens to protect hypomineralized teeth, and enodontic procedures. Dental abnormalities are common in patients treated for cancer, and these children require aggressive dental follow-up. Meticulous surveillance may facilitate detection of abnormalities, enabling the dental practitioner to intervene earlier in promoting a more aggressive regimen of oral care, thus reducing the morbidity associated with dental sequelae of oncotherapy, specifically periodontal disease and malocclusion. In this case, we report microdontia of all permanent second premolar and second molar in an 8 year old boy treated with chemotherapeutic agents during period of active dental development(14 months to 38 months of age).

  • Usually Mylar strip and wedge or celluloid strip crown were used in conventional method of restoring proximal carious lesions of primary anterior teeth. But, these methods have some difficulties to place the matrix and wedge due to short crown lenth and interdental spacing of primary teeth. And it is difficult to form proper cavosurface margin due to the rigidity of matrix and inconvenience to support matrix by practitioner's hand in narrow oral cavity of child. This article describes an alternative technique for restoring carious lesions of primary anterior teeth using available straw. This method is very convenient and reduce chair time and it also allows good marginal adaptation.

  • Market orientation emphasizes the capability of a firm to learn customers, competitors, and inter-functional coordination and to use this market intelligence of creating superior value in the marketplace. In recent years, academic and practitioner interest has focused on market orientation and factors that engender this orientation in organizations. Although the merits of maintaining organizational learning have been extensively discussed in the literature, little studies examine the empirical link between market orientation and organizational learning which has a strong relation with it. The objective of this study is to assess the relationship between organizational commitment, market orientation, and organizational learning and presents more close a relational structure. The relationships between organizational commitment(OC), market orientation (MO), and organizational learning(OL) were analysed by structural equation modelling. a structure of OC-MO-OL is supported by our research and past literatures.

Developing the Joint Employment System of Occupational Health Nurse Practitioners for the Minor Enterprises in Korea (Part 2) - Comparison of Opinions between Employers and Occupational Health Nurses - (사업장 보건관리자 공동채용모형개발(II)-사업주와 간호사의 의견 비교-)

  • This study is to develop the model on the joint employment system of occupational health nurse practitioners for medium or small sized enterprises in Korea based on the comparison opinions between employers and occupational health nurses (OHN). The data were collected by questionnaires from Oct. to Nov. in 1997. The number of subjects was 210 OHNs and 176 employers, response rate was 47.1% for OHNs and 23.2% for employers. The SAS PC program was used for the descriptive statistics. The results were as follow: 1. More than three years career was preferred for qualification of OHN by employers and OHNs. OHNs preferred that all instruments and materials should be equipped in each industry, but employers preferred that they should be equipped by OHN. 2. For the limit number of industries and employees per one OHN, employers preferred average 2.25 industries and 307 employees, OHNs preferred average 3.0 industries and 448 employees. Both of them preferred two times visit per week for all day long. 3. OHNs wanted that the local office of Ministry of Labor could act as mediator, but employers preferred direct contact individually. For the average wage, employers preferred 887,000 won per month, but OHNs preferred 1,960,000 won per month. 4. OHNs preferred the Korean Association of Occupational Health Nurse as the recruit agency. Finally, employers and OHNs have the opinions that they can choose this system depending on the condition. There is in need of development of effective strategy for this new system such as manpower development, cost-benefit analysis, establishment of occupational health service standard, advertisement for employers, and government supported project.

Factors Influencing Cognitive Function According to Degree of Cognition in Community Dwelling Elders (재가노인의 인지 정도와 인지 영향요인)

  • Purpose: This study was done to identify differences in factors influencing cognitive function according to the cognition of community dwelling elders. Methods: A convenience sample of 565 community dwelling elders participated in this study and from May to June, 2010 trained research assistants collected data using structured questionnaires. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, correlation, and multiple regression with the SPSS Win 15.0 version. Results: The elders had a slightly low degree of cognitive function. Of the 565 elders, 41.1% were in the normal group for cognitive function and 58.9% in the mild impairment group. Although there were no significant differences in general characteristics between the groups, factors influencing cognitive function were different. Factors influencing elders in the normal group were leisure activity and moderately good health status. These variables accounted for 5.7% of the variance in cognitive function. Factors influencing elders in the mild impairment group were leisure activity, depression, and education level limited to elementary school graduation. These variables accounted for 19.9% of the variance cognitive function. Conclusion: This study provides comprehensive understanding of factors influencing cognitive function in elders. Differentiated nursing interventions according to degree of cognitive function are suggested.

Developing the Joint Employment System of Occupational Nurse Practitioners for the Minor Enterprises in Korea (part 1) (보건관리자 공동채용 모형 개발 (I) -산업간호사 의견을 중심으로-)

  • The occupational health services for the minor enterprises are poor comparing with that of large enterprises in Korea. In 1997, Korean government announced officially the law on joint employment system of occupational health practitioners to solve the problems. This study is to develop the model on the joint employment system of occupational nurse practitioners for minor enterprises in Korea based on the opinions of occupational health nurses (OH Ns). The data were collected by questionnares from October to November in 1997. The number of subjects was 210 occupational health nurses who had participated the mandatory nursing education program provided by the Korean Occupational Health Nurses Association(KOHNA). The response rate was 47.1%. The contents of questionnaires were general characteristics, occupational & job characteristics of OHNs, and the views on the joint employment system. The SAS-PC program was used for the statistical analysis. The results were as followed, (1) The proper number of industies was 3 and the appropriate number of workers was 448 for one joint employed occupational nurse practitioner (J EONP). (2) 44.7% of the OHNs wanted that the Ministry of Labor monitored the way and content of contraction b/w JEONP and minor emterprises, 82.6% of the OHNs wanted that KOHNA managed the employment informations. (3) The OHNs of occupational health agencies showed more positive attitude than the OHNs of private industries on the joint employment system. (4) 88.3% of the OHNs wanted the education training for JEONP. Especially in occupational hygiene and safety control. (5) The OHNs expected the role expansion of Occupational Health Nursing by JEONP system. At the same time they worried the adverse effects. For the successful execution of this system, it is necessary the cooperation among the Ministry of Labor, KOHNA, the minor enterprises, and the OHNs.

오행침자법(五行鍼刺法) 원리 및 임상응용에 관한 연구 - I 원리에 관한 연구

  • Objective: To review the theoretic basis of the Sa-Ahm 5 Element acupuncture devised about 360 years ago, papers and books were researched. Methods: Total of 59 books and papers ranging from ancient Huang Di nei jing to modern Bio Medical Acupuncture for Pain Management were researched to study the basic theory of it in relation to the 5 Shu points, Results: Gao-mu in Chinese Ming dynasty, for the 1st time, had used 5 Shu points based on creation cycle as tonification and sedation treatment respectively and named it as 'tonification and sedation treatment of self meridian' but since then, this method, without special reasons, has been rarely used until Sa-Ahm's new doctrine that include the concept of destruction cycle was asserted. Conclusions: Sa-Ahm 5 Element acupuncture is a method which uses 5 Shu points from the viewpoints of simultaneous tonification and sedation methods which are based on promotion and control cycles. Though it is nowadays mostly-used method in accordance with practitioner's points, it needs to be set guidelines by which to effectively practice Sa-Ahm acupuncture.
