구치부 복합레진의 적정 RADIOPACITY에 관한 연구 (A STUDY ON THE OPTIMAL RADIOPACITY OF POSTERIOR COMPOSITE RESINS)

  • The aim of this study was to compare the accuracy of radiographic diagnosis of secondary carious lesions adjacent to composite filling materials with different radiopacity. The level of radiopacity that is most compatible with the radiographic diagnosis of secondary caries was studied in a two part experiment. In the first part, the radiopacity of 6 posterior composites CBP, CF, HM, LF, PQ, P50), enamel and dentin were measured by desitometer and 6 posterior composites divided into 3 groups based on their level of radiopacity compared with enamel and dentin. In the seocnd part, class II composite fillings with or without secondary caries were made in extracted premolar and radiographs of the teeth were examined by 10 dentists to diagnose simulated carious lesion. The following results were obtained: 1. The radiopacity of 6 posterior composites varied between 1.76(PQ) and 6.78(P50)mm Al equivalent. 2. For 4 composites the radiopacity exeeded that of an equal thickness of enamel, and for two the radiopacity was lower than that of dentin. 3. The detection of secondary caries was facillitated when the radiopacity of a composite resin was similar to or slightly greater than that of enamel.

Bulk-fill 복합레진, 믿고 사용해도 될까?

  • Composite resin restorations in posterior teeth are increasing due to the aesthetic needs of patients and the development of materials. This trend will accelerate in line with domestic insurance policies. However, resin composites generate stresses due to their contraction during the polymerization process. To reduce the polymerization shrinkage stress of resin composites, incremental layering technique has been recommended for decades. This technique reduces stress at the cavity wall interface and allows a more efficient light curing of the material. Bulk-fill resin composites have been designed to simplify the restorative technique because they can be placed into cavities in a single increment of 4-5mm. The simplification of the operative procedures is desirable in clinical daily practice. In this context, bulk-fill resin composites are an attractive alternative for posterior restorations. However, a clearer understanding of the clinical performance of this relatively new class of materials in comparison to conventional resin composites is required. Based on previous studies, the aim of the current review was to present the clinical criteria for the use of bulk-fill composites in direct restorations of posterior teeth.

다양한 용액에 따른 Bulk-fill 복합레진의 색조안정성 평가 (Color Stability of Bulk-Fill Resin Composites after Immersion in Different Media)

  • 이 연구의 목적은 bulk-fill 복합레진과 전통적인 복합레진을 대상으로 다양한 용액과 시편의 두께에 따른 색조안정성의 차이를 평가하는 것이다. 전통적인 복합레진으로 Filtek™ Z250, Filtek™ Z350XT와, bulk-fill 복합레진으로 Filtek™ Bulk-fill Posterior Restorative, Tetric® N-Ceram Bulk Fill이 사용되었다. 각 복합레진 시편은 3개의 군으로 나누어 각각 증류수, 레드와인, 커피 용액에 침전시킨 뒤, 침전 1, 7, 28일에 색조 측정을 시행하여 각 시점과 초기 기준점의 CIE L*a*b*값의 색차(ΔE*)가 계산되었다. 모든 복합레진은 레드와인에서 가장 큰 색조변화가 나타났으며, 그 다음은 커피, 증류수 순서였다. Filtek™ Z350XT는 모든 용액에서 가장 큰 색조변화를 나타냈고, 다음은 Filtek™ Bulk-fill Posterior Restorative였다. 그 뒤로는 Filtek™ Z250과 Tetric® N-Ceram Bulk Fill이 유사한 수준의 색조변화를 나타냈다. Bulk-fill 복합레진에서 2 mm와 4 mm인 시편의 두께에 따른 색조변화의 차이는 나타나지 않았다.

Single-peak LED와 dual-peak LED의 출력 파장 차이가 복합 레진 미세 경도와 색 안정성에 미치는 영향 (Effect of the difference in spectral outputs of the single and dual-peak LEDs on the microhardness and the color stability of resin composites)

  • 연구목적: 여러 광개시제 시스템에 의해 활성화되는 복합 레진을 기존의 single-peak LED와 최신의 dual-peak LED로 광중합하였을 때의 누프 미세 경도와 수중 보관 후의 색 안정성 차이를 알아보기 위한 것이다. 연구 재료 및 방법: Camphorquinone이 광개시제로 포함되어 있는 Z100과 다른 광개시제가 포함되어 있는 것으로 알려진 Tetric Ceram과 Aelite LS Posterior를 유리판 위에서 테플론 주형(직경 8 mm, 두께 2 mm) 내로 충전하고, single-peak LED와 dual-peak LED로 광중합하였다. 중합 후 누프 미세 경도를 측정하였고 한 달 후 색 변화를 측정하였다. 광중합기간의 미세 경도와 색 변화 차이를 student t-test로 분석하였다. 결과: 모든 레진에서 dual-peak LED로 광중합하였을 때 미세 경도가 높게 나타났다. 색 안정성 역시 dual-peak LED로 광중합하였을 때 높게 나타났으나 Aelite LS Posterior에서만 통계학적으로 유의한 차이가 있었다. 결론: 다른 광개시제가 포함되어 있는 복합 레진을 dual-peak LED로 광중합한 경우 미세 경도와 색 안정성에 있어서 더 좋은 결과를 얻을 수 있었다.

  • The aim of study was to investigate the effect of flow, specimen geometry and adhesion on the measurement of linear polymerization shrinkage of light cured composite resins using linear shrinkage measuring device. Four commercially available composites - an anterior posterior hybrid composite Z100, a posterior packable composite P60 and two flowable composites, Filtek flow and Tetric flow-were studied. The linear polymerization shrinkage of composites was determined using 'bonded disc method' and 'non-bond-ed' free shrinkage method at varying C-factor in the range of 1∼8 by changing specimen geometry. These measured linear shrinkage values were compared with free volumetric shrinkage values. The viscosity and flow of composites were determined and compared by measuring the dropping speed of metal rod under constant load. In non-bonded method, the linear shrinkage approximated one third of true volumetric shrink-age by isotropic contraction. However, in bonded disc method, as the bonded surface increased the linear shrinkage increased up to volumetric shrinkage value by anisotropic contraction. The linear shrinkage value increased with increasing C-factor and approximated true volumetric shrinkage and reached plateau at about C-factor 5∼6. The more flow the composite was, reduced linear shrinkage was measured by compensation radial flow.

  • ;Dentistry has benefited from tremendous advances in technology with the introduction of new techniques and materials, and patients are aware that esthetic approaches in dentistry can change one's appearance. Increasingly. tooth-colored restorative materials have been used for restoration of posterior teeth. Tooth-colored restoration for posterior teeth can be divided into three categories: 1) the direct techniques that can be made in a single appointment and are an intraoral procedure utilizing composites: 2) the semidirect techniques that require both an intraoral and an extraoral procedure and are luted chairside utilizing composites: and 3) the indirect techniques that require several appointments and the expertise of a dental technician working with either composites or ceramics. But, resin restoration has inherent drawbacks of microleakage. polymerization shrinkage, thermal cycling problems. and wear in stress-bearing areas. On the other hand, Ceramic restorations have many advantages over resin restorations. Ceramic inlays are reported to have less leakage than resin restoration and to fit better. although marginal fidelity depends on technique and is laboratory dependent. Adhesion of luting resin is more reliable and durable to etched ceramic material than to treated resin composite. In view of color matching, periodontal health. resistance to abrasion, ceramic restoration is superior to resin restorationl. Materials which have been used for the fabrication of ceramic restorations are various. Conventional powder slurry ceramics are also available. Castable ceramics are produced by centrifugal casting of heat-treated glass ceramics. and machinable ceramics are feldspathic porcelains or cast glass ceramics which are milled using a CAD/CAM apparatus to produce inlays (for example, Cered. They may also be copy milled using the Celay apparatus. Pressable ceramics are produced from feldspathic porcelain which is supplied in ingot form and heated and moulded under pressure to produce a restoration. Infiltrated ceramics are another class of material which are available for use as ceramic inlays. An example is $In-Ceram^{\circledR}$(Vident. California, USA) which consists of a porous aluminum oxide or spinell core infiltrated with glass and subsequently veneered with feldspathic porcelain. In the 1980s. the development of compatible refractory materials made fabrication easier. and the development of adhesive resin cements greatly improved clinical success rates. This case report presents esthetic ceramic inlays for posterior teeth.teeth.

Knitted Glass Fabric 강화 복합레진을 사용한 고정성 치과보철물에 대한 적용성 평가 (A FEASIBILITY STUDY ON THE APPLICATION OF THE KNITTED GLASS FABRIC COMPOSITES TO FIXED PROSTHODONTIC RESTORATION IN DENTISTRY)

  • Current dental restorations present a relatively weak resistance to fracture. Owing to their unique mechanical properties, fibre-reinforced polymers are now being considered. Unidirectional or woven continuous fibres, made of glass, polyethylene, carbon or Kevlar, have been evaluated. This study focused on the use of glass fibre knitted fabrics to reinforce acrylate resins, in order to investigate the possibility to construct single crowns as well as three unit bridges. Some points affecting the final composite system were tested ; 1) static strength, with focus on the stress transfer under a occlusal contact point ; 2) modelling of a three nit bridge ; 3) fatigue strength as a posterior three unit bridge material. The study demonstrated that knitted fabric reinforcements are showing an interesting compromise between stiffness, static strength for single crown. For three unit bridge applications in the posterior arch, however knitted glass fabric reinforcements were not strong enough in fatigue An additional reinforcement in the posterior arch fixed partial denture design was recommended.

구치부 수복용 복합레진의 파괴특성에 관한 Acoustic Emission 연구 (ACOUSTIC EMISSION ANALYSIS FOR FRACTURE CHARACTERISTICS OF DENTAL POSTERIOR COMPOSITES)

  • Dental composite resin is a kind of the particle - reinforced composite material, and is widely used in recent dental restoration of anterior and posterior tooth region. The purpose of this study was to investigate the fracture behaviour according to volume fractions and external findings of the filler particles for better interpretation of the fracture characteristics of posterior dental composite resins by analytic method of fracture mechanics. The plane strain fracture toughness($K_{IC}$) and Acoustic Emission were determined with three - point bending test using the single edge notch specimen according to the ASTM - E399, and its analyzed data was compared with filler volume fractions derived from the standard ashing test and scanning electron fractographs of each specimen including the unfilled experimental resin as a control. The results were that the value of fracture toughness of the composite resin material was in the range from 0.85 MPa$\sqrt{m}$ to 1.60 MPa$\sqrt{m}$ and was higher than the value of the unfilled experimental resin, and the fracture behaviours dervied from Acoustic Emission analysis show prominent differences according to the volume fraction and the size of filler particles used in each composite resin. The degree of resistance against crack propagation seems to be increase and the fractographs demonstrate the high degree of surface roughness and irregularity according with the increase of fracture toughness value.

Survival rates against fracture of endodontically treated posterior teeth restored with full-coverage crowns or resin composite restorations: a systematic review

  • This systematic review aims to summarize the current clinical studies that investigated survival rates against fracture of endodontically treated posterior teeth restored with crowns or resin composite restorations. Literature search were performed using keywords. Publications from 1980 to 2016 were searched in PubMed, ScienceDirect, ISI Web of SCIENCE, MEDLINE, and SCOPUS. Included studies were selected based on inclusion and exclusion criteria. Three clinical studies were included: 1 randomized controlled trial and 1 prospective and 1 retrospective cohort studies. Pooled survival rates ranged from 94%-100% and 91.9%-100% for crowns and resin composite, respectively. The majority of teeth had no more than 3 surface loss of tooth structure. The studies included were heterogeneous, and were not appropriate for further meta-analysis. Current evidence suggested that the survival rates against the fracture of endodontically treated posterior teeth restored with crowns or resin composites were not significantly different in the teeth with minimum to moderate loss of tooth structure.

  • The purpose of present study was to evaluate the polymerization shrinkage stress and amount of linear shrinkage of composites and compomers for posterior restoration. For this purpose, linear polymerization shrinkage and polymerization stress were measured. For linear polymerization shrinklage and polymerization stress measurement, custom made Linometer (R&B, Daejon, Korea) and Stress measuring machine was used (R&B, Daejon, Korea). Compositers and compomers were evaluated: Dyract AP (Dentsply Detrey, Gumbh. German) Z100 (3M Dental Products, St. Paul. USA) Surefil (Dentsply Caulk, Milford, USA) Pyramid (Bisco, Schaumburg, USA) Synergy Compact (Coltene, Altstatten, Switzerland), Heliomolar (Vivadent/Ivoclar, Liechtenstein), and Compoglass (Vivadent Ivoclar/Liechtenstein) were used. 15 measurements were made for each material. Linear polymerization shrinkage or polymerization stress for each material was compared with one way ANOVA with Tukey at 95% levels of confidence. For linear shrinkage: Heliomolar, Surefil
