성인형 치주염 환자에 있어 manual probe의 Florida probe의 임상적 비교 (Clinical Comparison Of Manual Probe With Florida Probe In Adult Periodontitis)

  • The periodontal probe is a commonly used instrument to assess periodontal conditions. And so, there has been many studies to develop the accuracy and reproducibility of the periodontal probe. The purpose of this study was to compare two different periodontal probes for measurement reliability and time required to use in subjects with moderate periodontitis. It was done after evaluating reproducibility of probing depth by stent guiding for a Manual probe and a Florida probe in subjects with healthy periodontal condition. The results were as follows 1. In experiment to evaluate the reproducibility of probing depth by stent guiding for a Manual probe and Florida probe in subjects with healthy periodontal condition, there was no major significant difference between intraprobe and interprobe relationships. 2. There were reduced probing measuremint error by using the Florida probe for posterior teeth and by using the Manual probe for anterior teeth of subjects with moderate periodontitis. 3. At proximal area, there was higher measurement error by using the Manual probe than the Florida probe. 4. The mean of pocket depth measurement using Manual probe was signifi cantly higher than that using Florida probe(p<0.05). With increasing pocket depth, interprobe difference increased and reproducibility reduced. 5. There was no significant difference in time required to use between Manual probe and Florida probe(p<0.05). 6. There was slight probing measurement difference between Manual probe and Florida probe at different site, but both probes have similar degrees of reproducibility and similar time required to probe.

두경부 방사선치료시 접합 조사면의 선량분포 (Dose distribution at junctional area for head and neck radiotherapy)

  • 두경부 종양의 방사선치료에서 비대칭 콜리메이터를 이용한 이분조사는 두 조사면을 겹침없이 연결시켜 조사할 수 있는 효과적인 방법이고, 동일 방사선 조사면을 분리하여 X-선과 전자선을 인접시켜 조사하는 경우는 빈번히 사용되는 방법이다. 본 연구는 X-선의 이분조사면을 이용한 두 조사면의 인접면의 선량과, X-선과 전자선 조사면의 인접면에서 선량을 측정하여 임상에 적용할 수 있는 자료를 얻고자 하였다. X-선의 반쪽 조사면의 선량측정에서 하부 전방 단일 이분조사면의 선량분포는 Y축이 0 cm인 면(콜리메이터가 닫힌 면)에서 0.5 cm, 1 cm 거리에서는 각각 개방조사 선량의 93%, 95%, 전후방 이분조사의 경우 Y축이 0 cm인 면에서 0.3 cm, 0.5 cm, 1 cm거리에서는 각각 개방조사 선량의 92%, 95%, 98%로 선량감소를 보였다. X-선과 전자선의 조사면을 인접시켰을 때 깊이 0 cm, 0.5 cm, 1.5 cm, 2 cm, 3 cm에서의 두 조사면의 인접면에서의 선량 분포의 분석에서 X-선 조사면에서 선량 증가는 깊이 1.5 cm에서 폭 7 mm에 걸쳐 있었고 최고 6%의 증가를 보였으며 다른 측정 깊이에서는 선량증가가 허용범위 내에 있었다. 그리고 전자선 조사면쪽에서 선랑 감소는 전 측정 깊이 0.5 cm-3 cm에서 각각 폭이 1mm-12.5 mm에 걸쳐 4.5%- 30%의 주변부 보다 선량감소를 보였다. 본 연구에서 이분 조사면의 선량은 콜리메이터가 닫힌 면에서 감소함을 확인하였다. 그리고 X-선과 전자선을 인접시켜 조사 할 때 두 조사면의 인접면을 중심으로 X-선 조사면 쪽에서 선량증가, 전자선 조사면쪽에서 선량 감소를 보임을 확인하였다.

  • The balance ability significantly decreased in the elderly because of deterioration of the neural musculature regulatory mechanisms. Several studies have investigated methods of improving balance ability using real-time systems, but it is limited by the expensive test equipment and specialized resources. Recently, Kinect system based on depth data has been applied to address these limitations. Little information about accuracy/inaccuracy of Kinect system is, however, available, particular in motion analysis for evaluation of effectiveness in rehabilitation training. Therefore, the aim of the current study was to evaluate accuracy/inaccuracy of Kinect system in specific rotational movement for balance rehabilitation training. Six healthy male adults with no musculoskeletal disorder were selected to participate in the experiment. Movements of the participants were induced by controlling the base plane of the balance training equipment in directions of AP (anterior-posterior), ML (medial-lateral), right and left diagonal direction. The dynamic motions of the subjects were measured using two Kinect depth sensor systems and a three-dimensional motion capture system with eight infrared cameras for comparative evaluation. The results of the error rate for hip and knee joint alteration of Kinect system comparison with infrared camera based motion capture system occurred smaller values in the ML direction (Hip joint: 10.9~57.3%, Knee joint: 26.0~74.8%). Therefore, the accuracy of Kinect system for measuring balance rehabilitation traning could improve by using adapted algorithm which is based on hip joint movement in medial-lateral direction.

한국 젊은이의 안면미 선호경향에 관한 연구 : 얼굴의 측모평가를 중심으로 (A STUDY ON THE FACIAL ESTHETIC PREFERENCES AMONG KOREAN YOUTHS: ASSESSMENT OF PROFILE PREFERENCES)

  • This study was designed to assess profile preferences among Korean youths in the year 1992. Facial esthetics was evaluated by means of silhouette profiles, eliminating the influence of a number of aspects that may affect judgment when normal lateral photographs are used. The main points of preference to be clarified here are as follows. First, on facial convexity, Second, on nasion depth, Third, on mentolabial sulcus depth, Fourth, on the position of upper and lower lips, Fifth, on facial type according to Angle's classification of malocclusion, Sixth, on Song's tangents. The 54 subjects printed in questionnaire as black and white silhouettes were selected from 300 tracings from cephalometric radiographs of people whose age ranging from 11 to 20 years. Photographs of six female subjects were retouched by computer graphic software and printed in color and black/white photographs which were used for adaptation of eyes of participants in selecting profiles in silhouette. They constitute 2 questions. The 54 subjects were grouped as 22 questions, each of them composed of 6 subjects, according to the aspects to be clarified. Twenty four questions in total were asked to assess profile preferences. For the assessment, the profile line, the facial esthetic triangle, Song's tangents, and Angle's classification of malocclusion were introduced. The profile line is composed of 11 component points which are Trichion, Glabella, Nasion, Pronasale, Subnasale, Labrale superius, Stomion, Labrale inferius, Supramentale, Pogonion, and Gnathion. The facial esthetic triangle is composed of 3 tangents: A-tangent which is the tangent of dorsum of nose, B-tangent which is the line passing through Sn and Ls, and C-tangent which is drawn on the turning point of the curve which lies between mentolabial sulcus (Sm) and pogonion (Pg). Angle's classification has 3 types of malocclusion which are Class I, Class II, and Class III. Class II malocclusion is subdivided into Division 1 and Division 2. The participants of the survey were composed of 861 college students (448 male students, 413 female students) whose majors grouped as Fine Arts. Liberal Arts, and Natural Sciences, and whose mean age 21.8 years. The statistics program SPSS/PC + of SPSS Inc. was used to analyze answers of participants. Crosstabulation, Chi-square test, and Kendall test were done. The conclusions are as follows: First, Korean youths have a tendency to prefer the slightly convex face to the flat or concave face. Second, they prefer a moderately deep nasion. Third, they prefer a moderately deep mentolabial sulcus. Fourth, they prefer the position of lips which are near to Ricketts' E-line. The position of the upper lip which is slightly posterior to E-line is preferred. The upper lip which lies too far anterior or posterior to the lower lip is not perferred. Fifth, they prefer most, according to Angle's Classification of Malocclusion, Class I facial profile which has a slight inclination to Class II division 2. The order of preference is Class I, Class II division 2, Class III, and Class II division 1. Sixth, they prefer the type 2 and 3 of Song's tangents. The facial profile within which A-and B-tangent meet is preferred. The facial profile which has Cotangent that .meets with A-tangent slightly posterior to the crossing point of A-and B-tangent or that parallels with B-tangent is preferred.

Ricketts 분석법에 의한 혼합치열기 정상교합 아동의 두부방사선계측학적 평가에 대한 연구 (A STUDY ON CEPHALOMETRIC EVALUATION OF MIXED DENTITION CHILDREN WITH NORMAL OCCLUSION)

  • 부정교합의 진단과 치료를 위해서는 두개 및 안면부의 형태와 성장 양상에 대한 지식이 필수적이다. 그러나 소아치과 영역에서 필요한 혼합치열기 아동에 대한 연구는 성인의 그것에 비하여 많지 않음에 저자는 혼합치열기 정상교합 아동의 두부방사선학적 표준치를 얻고자 건치아동 선발대회의 후보중 보존적, 보철적, 교정적, 성형외과적 치료경험이 없고 연구모형 상에서 비교적 교합이 양호하며 안모 및 전신발육 상태가 정상으로 생각되는 아동 102명(남자 48명, 여자 54명)을 선정하여 측모 두부방사선규격사진을 촬영한 후, Ricketts 분석법에 의해 분석하여 다음과 같은 결론을 얻었다. 1. 전두개저 길이, 후안면고경, 하악체의 길이를 나타내는 항목에서 남자가 여자보다 유의하게 큰 값을 나타내었고(p<0.05), Porion은 남자가 여자에 비해 후방위치하고 있었다(p<0.01). 2. 안면의 깊이를 통해서 본 Pogonion은 남자가 더 전방위치하고 있었고(p<0.05), 하악하연평면각은 남자에서보다 여자에서 유의하게 컸으며(p<0.01), FH 평면에 대해 남자의 상악중절치가 여자에 비해 유의하게 전방으로 경사져 있었다(p<0.05). 3. 연령에 따라 유의하게 변하는 항목은 전두개저 길이, 상악대구치의 위치(p<0.01), 하악골체의 길이(p<0.05)였다. 4. 상악골의 고경, 안면의 깊이, 하악하연평면각, 상악돌출도등에서도 연령에 따른 변화는 보였지만 유의한 차이는 없었다(p>0.05).

인공와우 이식술 환자의 Cochlear View 촬영에 관한 연구 (A study on the Cochlear View in Multichannel Cochlear Implantees)

  • Cochlear implant poses a contraindication to the magnetic resonance imaging(MRI) process, because MRI generates artifacts, inducing an electrical current and causing device magnetization. CT is relatively expensive and the metal electrodes scatter the image. Post-implantation radiological studies using anterior-posterior transorbital, submental-vertex and lateral views, the intracochlear electrodes are not well displayed. Therefore, the authors developed a special view, which we call the cochlear view. The patient is sitting in front of a vertical device. Then the midsagittal plane is adjusted to form an angle of $15^{\circ},\;30^{\circ}$, and $45^{\circ}$ with the film. The flexion of the neck is adjusted to make the infraorbitomeatal line(IOML) is parallel with the transverse axis of the film. The central ray is directed to exit from the skull at point which is 3.0 cm anterior and 2.0 cm superior to the EAM(external auditory meatus). Results have shown that single radiography of the cochlear view provides sufficient information to demonstrate the position of the electrodes array and the depth of insertion in cochlear. Radiography of the cochlear view in angle of $45^{\circ}$ is an excellent image. The cochlear view gives the greatest amount of medical information with the least radiation and lowest medical cost. It can be widely used in all cochlear implant clinics.

Upper airway dimensions and craniofacial morphology: A correlation study using cone beam computed tomography

  • Objective: To determine the correlation between dentoskeletal parameters related to craniofacial morphology and the upper airway (UA) volume. Methods: Cone-beam computed tomography images of 106 randomly selected orthodontic patients were analyzed using NemoFab Ortho software. The dentoskeletal variables assessed were anterior facial height (AFH), posterior facial height (PFH), PFH/AFH ratio, hyoid position, maxillary width (MW), and palatal depth. The UA volume (evaluation in anatomical regions and as a whole) was also assessed using the same software. We also evaluated potential differences in UA variables between age and sex groups. The correlation between the dentoskeletal parameters and UA volume was calculated using the Pearson correlation coefficient (R). Analysis of variance and Student's t test were performed to assess differences between age and sex for UA variables. Statistical analyses were performed using SPSS software (version 26 for Windows). Results: This study found that PFH, AFH, and MW were the dentoskeletal parameters most strongly correlated with UA volume. However, the ANB angle did not show any significant correlation with UA volume. Additionally, differences in UA volumes were observed between age groups. Sex differences were found in both the "8-12" and "≥ 16" age groups for oropharyngeal and pharyngeal volumes. Conclusions: In conclusion, our findings indicate a significant correlation between UA volume and dentoskeletal parameters, particularly those related to facial height and MW.

한국인 만성 치주염 환자에서 치주질환 원인균의 동정 (Identification of putative periodontal pathogens in Korean chronic periodontitis patients)

  • Purpose: Specific bacteria are believed to play an important role in chronic periodontitis. Although extensive microbial analyses have been performed from subgingival plaque samples of periodontitis patients, systemic analysis of subingival microbiota has not been carried out in a Korean population so far. The purpose of this study was to investigate the prevalence of 29 putative periodontal pathogens in Korean chronic periodontitis patients and evaluate which pathogens are more associated with Korean chronic periodontitis. Material and Methods: A total of 86 subgingival plaque samples were taken from 15 chronic periodontits(CP) patients and 13 periodontally healthy subjects in Korea. CP samples were obtained from the deepest periodontal pocket (>3 mm probing depth[PD]) and the most shallow periodontal probing site ($\leq$3 mm PD) in anterior tooth and posterior tooth, respectively, of each patient. Samples in healthy subjects were obtained from 1 anterior tooth and 1 posterior tooth. Polymerase chain reaction (PCR) of 16S ribosomal DNA (rDNA) of subgingival plaque bacteria was performed. Detection frequencies(% prevalence) of 29 putative periodontal pathogens were investigated as bacterium-positive sites/total sites. Results: With the exception of Olsenella profuse and Prevotella nigrescens, the sites of diseased patients generally showed higher prevalence than the healthy sites of healthy subjects for all bacteria analyzed. Tanerella forsythensis (B.forsythus), Campylobacter rectus, Filifactor alocis, Fusobacterium nucleatum, Porphyromonas endodontalis and Porphyromonas gingivalis were detected in more than 80% of sites with deep probing depths in CP patients. In comparison between the sites (deep or shallow PD) of CP patients and the healthy sites of healthy subjects, there was statistically significant difference(P<0.05) of prevalence in T.forsythensis (B.forsythus), C.rectus, Dialister invisus, F.alocis, P.gingivalis and Treponema denticola. Conclusion: Our results demonstrate that the four putative periodontal pathogens, T.forsythensis (B.forsythus), C.rectus, P.gingivalis and F.alocis are closely related with CP patients in the Korean population.

3급 부정교합 환자에서 두개저 성장 양상에 따른 악골 성장 특성에 관한 연구 (Study on Characteristics of Maxillofacial Growth in Class III Malocclusion Patients by Cranial Base Growth)

  • Purpose: Craniofacial structure form results from the adaptation to morphologic and functional changes in their neighboring structures for a mutual balance. The purpose of this study is classification of maxillomandibular complex growth pattern follow by cranial base growth pattern. And this study is identifying the correlation between maxilla-mandibular complex growth pattern and orthodontic criteria. Methods: 142 Class III malocclusion patients had orthognathic surgery at Wonkwang University Dental Hospital during April 2004 to October 2010. Patients were divided into 4 groups and the correlation between cranial base and maxillomandibular growth patterns were evaluated. Results: There was a correlation between cranial base and maxillomandibular growth patterns. Positive relationships were found between the occlusal plane, Incisor mandibular plane angle, mandibular plane, positioning of pogonion and the saddle angle, indicating maxillary growth patterns. Negative relationships were found between SNA, SNB, maxillary incisor angle and saddle angle. Positive relationships were found between the ratio of the anterior and posterior cranium, positioning of pogonion and the percentage of cranial depth indicating mandibular growth patterns. Negative relationships were found between the occlusal plane, maxillary incisor angle, mandibular plane, mandibular angle and cranial depth. Conclusion: Cranial base and maxillofacial growth patterns were correlated and the classification should be adjusted before orthognathic surgery.

Study of frontal and ethmoid sinus of sinonasal complex along with olfactory fossa: anatomical considerations for endoscopic sinus surgery

  • The Functional endoscopic sinus surgery through transnasal approach is a common modality of treatment for disorders of the nasal cavity, paranasal air sinuses as well as cranial cavity. The olfactory fossa (OF) is located along the superior aspect of cribriform plate which varies in shape and depth. This variable measurement of the depth of OF is mostly responsible for greater risk of intracranial infiltration during endoscopic procedures in and around the nasal cavity. The morphology of frontal and ethmoid sinus (ES) vary from simple to complex. This cadaveric study is planned to improve the ability of the otolaryngologist, radiologist to understand the possible morphological variations and plan steps of less invasive "precision surgery" to have a safe and complication free procedures. A total of 37 human head regions were included in the study. For classification of OF, Modified Kero's classification was used. The size, shape and cells of frontal and ES were noted. We found, type II (60.8%) OF was more common followed by type I (29.7%) than type III (9.5%). The shape of frontal sinus was comma shaped (55.4%) followed by oval (18.9%) than irregular (16.2%). Most common two cells type of ES was seen in 50.0% of both anterior and posterior ES. Out of 74 ES, 8.1% of Onodi cells and 14.9% of agger nasi cells were seen.
