2020년 개정 진료 치침에 따른 과민성폐렴의 진단 (Diagnosis of Hypersensitivity Pneumonitis: 2020 Clinical Practice Guideline)

  • 과민성폐렴(hypersensitivity pneumonitis)은 기도를 통해 흡입된 항원물질이 세기관지와 폐포에 면역매개 염증병변을 일으켜 발생하는 간질성폐질환(interstitial lung disease)이다. 다양한 유기물질이 발생원인으로 작용할 수 있기에 환자의 흉부영상검사와 임상증상을 통해 과민성폐렴을 의심하고 직업 또는 주변 환경을 통해 노출되는 항원을 파악하는 것이 과민성폐렴 진단의 핵심이다. 하지만 다양한 임상증상과 진행 형태로 인해 간질성폐질환 환자의 진단에서 과민성폐렴을 정확히 감별할 수 있느냐는 풀기 쉽지 않은 주제이다. 이에 2020년 미국흉부학회, 일본호흡기학회 그리고 라틴아메리카흉부학회는 과민성폐렴 진단에 대한 새로운 임상진료지침을 발표하였다. 이번 임상진료지침은 과민성폐렴 진단에 있어서 흉부 고해상도 전산화단층촬영(high-resolution CT; 이하 HRCT)의 역할을 강조하며 과민성폐렴에 대한 새로운 분류 기준도 제시하고 있다. 본 종설은 흉부 HRCT 내용을 포함해 새롭게 소개된 과민성폐렴 진단에 대한 전반을 살펴보고자 한다.

방사선 조사후 관찰한 가토 폐의 병리학적 변화 (Pathologic Changes in the Rabbit lung Following Single Dose Irradiation)

  • The damage which radiation produces in tissues such as the lungs can be discussed at the molecular, biophysical, cellular, and organ levels. The cellular effects of irradiating the lungs are related to the histologic and clinical sequelae. In the present study the right lung of rabbits were exposed to single dose of 20 Gy of X-irradiation. Animals from each group were sacrificed monthly for 6 months postexposure. Sections of lung were examined by light microscopy(LM) and by transmission electron microscopy(TEM). Multiple exudative lesions were seen at 2 months after the 20Gy irradiation,and they progressed to a proliferative and then reparative fibrotic lesion by 6 months. Changes in epithelial lining of lung components, particulary the presence of type II pneumocytes were found by both LM and TEM. Capillary endothelial damages were less pronounced. The possible implication of cellular components in radiation pneumonitis and fibrosis is discussed.

Acute Respiratory Distress Due to Methane Inhalation

  • Inhalation of toxic gases can lead to pneumonitis. It has been known that methane gas intoxication causes loss of consciousness or asphyxia. There is, however, a paucity of information about acute pulmonary toxicity from methane gas inhalation. A 21-year-old man was presented with respiratory distress after an accidental exposure to methane gas for one minute. He came in with a drowsy mentality and hypoxemia. Mechanical ventilation was applied immediately. The patient's symptoms and chest radiographic findings were consistent with acute pneumonitis. He recovered spontaneously and was discharged after 5 days without other specific treatment. His pulmonary function test, 4 days after methane gas exposure, revealed a restrictive ventilatory defect. In conclusion, acute pulmonary injury can occur with a restrictive ventilator defect after a short exposure to methane gas. The lung injury was spontaneously resolved without any significant sequela.

방수 스프레이 흡입 노출로 인한 급성 호흡기 중독 사례 및 원인 고찰 (Comprehensive Review of Acute Respiratory Failure Following Inhalation Exposure to Waterproofing Agents)

  • Background: In Korea, a healthy 36-year-old man developed acute interstitial pneumonitis soon after inhaling a waterproofing spray which he had applied at home to his outdoor jacket. Objectives: The objectives of this study were to review cases of varying degrees of respiratory toxicity and poisoning in connection with the use of waterproofing spray and summarize major reasons for cases of poisoning. Methods: We searched articles reporting on a combination of a waterproofing agent and/or respiratory symptoms, including acute respiratory syndrome, lung injury, pneumonia, pulmonary toxicity, and respiratory disease. Results: We reviewed a number of cases of varying degrees of respiratory toxicity and poisoning resulting from inhalation of waterproofing spray containing fluorocarbon co-polymer, solvents and propellants reported in a variety of countries. The literature searches concluded that among the ingredients of waterproofing agents, fluorinated polymer may cause acute respiratory health effects. Conclusion: Environmental policy should be implemented in order to prevent consumers from using household and industrial products including waterproofing agents. In addition, a national surveillance system should be created to collect cases of poisoning caused by the use of consumer products.

우측 폐하엽의 폐쇄성 폐렴 (Obstructive Pneumonitis of right lower lung field)

  • A chest x-ray of 68 year old male showed pneumonic consolidation of right lower lung field with blunting of right cardiac border. Computerized tomography of chest revealed infiltrative mass with V-shaped calcification just below right main bronchus. This finding has to be made into differential diagnosis of numerous pulmonary diseases including, mycobacterial disease, neoplasm, lymphadenopathies, and foreign bodies. Initial bronchoscopic findings suggest endobronchial mass lesion on right intermediate bronchus but endobronchial biopsy fail to prove malignant cell or underlying illness. But repeated endobronchial biopsy shows metallic material in the right Intermediate bronchus and we remove it with alligator jaws forcep under bronchoscopy. He was well after discharge.

  • Imatinib mesylate, a selective inhibitor of BCR-ABL kinase activity, has demonstrated significant clinical efficacy in the treatment of chronic myeloid leukemia (CML) and gastrointestinal stromal tumors (GISTs). It has become the standard of treatment for these diseases. Although the toxicity profile of imatinib is superior to that of interferon or other cytotoxic agents, some adverse events including edema, gastrointestinal toxicities and hematologic toxicities are commonly observed in the patients treated by imatinib. We present two cases of imatinib induced interstitial pneumonitis during the treatment of a chronic phase of CML.

  • Pulmonary mucosa-associated lymphoid tissue-derived (MALT) lymphoma is a rare disease. This disorder is considered to be a model of antigen-driven lymphoma, which is driven either by autoantigens or by chronic inflammatory conditions. Low-grade B-cell MALT lymphoma may develop from a nonneoplastic pulmonary lymphoproliferative disorder, such as lymphocytic interstitial pneumonitis (LIP). A recent estimate predicts that less than 5% of LIP patients acquire malignant, low-grade, B-cell lymphoma. In Korea, there has been no previous report of malignant low-grade, B-cell lymphoma, acquired from LIP. Here, we present the case of a patient with LIP that developed into pulmonary MALT lymphoma, six years after diagnosis.

지속적인 고열을 동반한 폐렴양 결핵병변 1예 (A Case of Tuberculous Pneumonitis With Continuous High Spiking Fever)

  • A 33-year old male was admitted due to continuous high spiking fever for 2 months via local clinic. He had been diagnosed pulmonary tuberculosis at local clinic. However, spiking fever had not been controlled by anti-tuberculous medications. Chest PA showed confluent consolidation on right upper & mid-lung field. 5 anti-tuberculous regimens(Streptomycin, Isoniazid, Rifampin, Ethambutol, Pyrazinamaide) were administered initially and steroid therapy was followed for relieving toxic symptoms Very slowly resolved chest X-ray lesion and continuous fever suggested the possibility of misdiagnosis. After 60th hospital day, the chest X-ray lesion was resolved gradually and fever subsided almost completely. He was discharged on 76th hospital day with anti-tuberculous drugs and steroid(prednisolon), without any other problems except sustained mild fever.

  • Achalasia is a functional disorder of a short segment of the lower esophagus showing obstruction of the esophagogastric junction. Dysphagia. regurgitation and weight loss are outstanding features,however, complicated pulmonary troubles aspiration pneumonitis, bronchiectasis, lung abscess etc. --are sometimes more apparent than the disease entity in children though achalasia is rarely encountered in children. During the period of January, 1961, to May, 1968, the authors experienced 9 cases of achalasia of the esophagus in Chest Surgery Department,N.M.C. 1. Seven of nine were male and four were under 5 years of age. 2. So-called symptoms triad noted in almost every case, and 3 of 4 children showed recurrent attacks of pneumonitis. 3. Chest film showed widened superior mediastinum by dilated esophagus in 6 cases, and pulmonary infiltrations in 3 cases of children. 4. Preoperative diagnosis were achalasia,esophageal stricture by rodent-cidal ingestion and suggestive esophageal cancer in 7 cases,one case and remaining one case. respectively. 5. Modified Heller procedure was performed in all cases with definite diagnosis of achalasia at operation table in misdiagnosed 2 cases. 6. Immediate postoperative complications were 2 cases of wound infection and one case of atelectasis and no operative mortality encountered. 7. Seven of nine showed excellent result of operation, and good in one case. Remaining one case failed relieving obstruction and underwent interposition of ileocolonic segment with excellent result.
