Effectiveness of Non-Pharmacologic Interventions in Chemotherapy Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis (항암화학요법 관련 말초신경병증에 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed.   • When we see normal gait, gait cycle is seperated as stance phase and swing phase. It needs 6 determinant of gait of pelvic rotation, pelvic tilt, knee joint of stance phase, ankle and foot motion, ankle and knee motion, and pelvic movement to be accomplished. In addition, a joint and muscle action is accomplished biomechanically at the same time with its gait cycle. In oriental medicine, the relationships between chang-fu physiology and meridian physiology are summaried as follows ; ${\bullet}$ chang-fu physiology : Spleen manages the extremities. Liver manages soft tissues. Liver stores blood. Kidney stores essences. Abnormal gaits are due to three causes as following; first, physical defect secoud, pain third, nervous system or instability of muscle. In oriental medicine, we can know relationship in "atrophy, numbness, stroke, convulsion, muscular dystrophy of knee, rheumatoid arthritis, five causes of infantile growing defects, five causes of softening, sprain". Especially, atrophy is the most important symptom. Gait evaluation should be emphasized where a point can walk 8 feet to 10 feet considering stride width, stride length, the body weight center, stride number, flexion, extension, rotation of a joint as a standard factor. The point is we should find out something strange in a patient's side, front and back view. After that we should find out its cause as an index that we can observe abnormal findings in a joint and muscle.

The Experiences of Alternative Therapy with the Menopausal Symptom in middle-aged Women (중년여성의 갱년기 증상완화를 위한 대체요법 경험)

  • The primary purpose of this study was to describe psychological aspects of menopausal women who have experiences on alternative therapies, such as pomegranate tree fluid, life pack, evening primerose, zacro syrup, zacro pill, multi-vitamin, oriental medicine instead of estrogen. In order to have insights on the participants behaviors, this qualitative research was conducted. The participants in this study were six women who have been taking at least one of the alternative therapies to relieve the menopausal symptom. Data had been collected from November 2002 to May 2003. The interview questions were about motivations to seek the alternative therapy, menopausal symptoms, feelings or positive effects after the treatment. The interviews were recorded and transcribed into written documents. The method described by Giorgi was utilized for data analysis. The study results were as follows : 1. The participants sought the alternative therapy based on their own information or by friends' recommendations. The motivations of taking the therapy were to live younger and healthier by reliving menopausal symptoms including somatic symptoms, such as irregular menstruation, flushing sense, cold sweating, insomnia, dull headache, vagina dryness, cystitis after sexual intercourse, joint pain, nervousness, depression, skin dryness, and suppression of activity. 2. The participants expressed that they became more active and felt better with disappearing menopausal symptoms after about 2 to 3 weeks treatment. 3. The participants did not want to accept the fact that they had menopause, which caused avoidance of having medical diagnosis and medical treatment. They also did not want to let their spouses know about their symptoms. 4. The major reason of taking the alternative therapy was to avoid the side effects of estrogen therapy in spite of high costs of the alternative way. 5. The participants suggested that menopausal women needed to be educated about physical and psychological symptoms of menopause. They also needed to be informed about how to manage it. In conclusion, accurate information about foods and drugs utilized for the alternative therapy was needed. Medically proved methods to recognize positive effects and side effects were also necessary. These goals could be achieved by continuous medical observations and further research.

  • An 86 year old woman was admitted complaining of dyspnea and right pleuritic pain with a 5 week durations. A physical examination, chest X-ray, and diagnostic thoracentesis upon admission revealed findings consistent with severe pneumonia and empyema on the right lung. Despite the insertion of a chest tube and negative suction via Emersion pump, the continuous air leakage was sustained, and a bronchopleural fistula (BPF) was found on the chest-CT. A flexible bronchoscopic occlusion with an Endobronchial Watanabe Spigot (EWS) was performed after 56 days of admission. An 5 mm diameter EWS was successfully inserted into the anterior segmental bronchus of the right upper lobe by flexible bronchoscope. There was no aAir leakage detected after this procedure. The patient was discharged 30 days after the EWS occlusion.

  • Manual material handling(MMH)is major factor which causing physical injuries of worker at working area and frequency of low back pain(LBP) is increasing industrial accidents. Especially, working in bad circumstance such as farm, orchard, harbor loading and unloading, logging place and mining place which located in inclined slope can cause much possibility of hazard and absence of working balance can cause injuries of musculoskeletal system such as joint, bone, ligament. So, this study used EMG system to measure and evaluate muscle force information and fatigue of worker when lifting on slope. The result of measuring averaged integrated EMG(AEMG) shows multifidus muscle be used more than anything else in force. neck extensors are used at 15°, 20°frequently. generally the AEMG result shows multifidus muscle be used in force. Commonly, muscle fatigue of multifidus is higher than other muscle by analysis mean power frequency(MPF). The result of load sharing rate shows multifidus and erectorspinae which are deep spinal muscles is relatively high and neck extensor is low.

  • This study focused on the understanding the life of the elderly in the institution by phenomenological method. The participants were seven. who were 4 men and 3 women living in the elderly institution in Taegu. The data were collected through the indepth interviews and participant observation from June 20. 1999 to January 10. 2000 and analyzed by phenomenological analytic method by Giorgi. The structure of the meaning were as follows: Unhappy life: irresponsibility. self-centered thought. hardness. Irresistible life: misfortune. unhealthiness. Reading their son's intention. Bitter life: unfair treatment from their sons and daughters, betrayed feeling for their being thrown away. Outside oriented life: a fear of others' attention, wrong information about institutionalized life, maladjustment. Self-consolation life: comfortableness, convenience, economic merit of low cost. Dissatisfied life: discomfort from communal living. unkindness of the institution staffs, depreciated tendency to the elder people, irrational social security system. economic distress, physical pain, restrained feeling. Tenacity to their sons and daughters longingness for their sons and daughters, regretableness, waiting, Regretable life: remorse for their past life. agony, guilty, loneliness, grief, self-abandonment, self-depreciation, other residents' death in common. Inharmonious life with other residents complaint, conflict, ignorance, selfishness. Yearning life for opposite sex: sexual interest. Preparing for their life: control over their body and mind, consideration for others. A life with hope: blessing death, forever healthfulness, affiliation to their family. From the results of this study an education and consultation should be done as soon as possible to remove the negative recognition of the institutional life to the facility residents. the family and the future consumers of elderly institution. To minimize the maladjustment to facility life of residents, a new program and interventions for the new comer's are needed. The thesis with above results will widen the understandings of institutional residents and an important guidance for a better nursing care in elderly institutions of korea.

The Effect of Qigong on the Health Promotion in Post-mastectomy Women (기공(氣功)이 유방절제술을 받은 여성의 건강증진에 미치는 영향)

  • Objective : Qigong has been reported as an effective for post-mastectomy women. It consists of a series of postures combined with slow, smooth, graceful movements, and is considered a low intensity exercise. In this study, a specialist of Qigong developed a Qigong program for post-mastectomy women in a community. The object of this research is to evaluate the effect of Qigong on a subjective symptom scale, on the grade of discomfort and pain in everyday life, BMI, fat(%), muscular strength, shoulder joint movement, in post-mastectomy women. Methods : The study used a non-equivalent control group non-synchronized design. The experimental group received a Qigong 3 times a week for 12 weeks from September 7th to December 28th in 2009. The subjects were 40 patients are divided into 25 persons in experimental groups and 15 persons in contrastive group. The data were analyzed with descriptive statistics, chi-square test, t-test, Mann-Whitney U test. A P-value less than .05 was considered significant by 2-tailed test. All statistical analyses were performed with SPSS win(ver 12.0). Results : The results were as follows : The score in the subjective symptom(p=.040), and score of anxiety (p=.024), fat(%)(p=.007) were significantly decreased after Qigong program. The values for shoulder flexibility (left : p=.010, right : p=.008), and Muscular strength(grip power)(left : p=.021, right : p=.029) significantly increased after Qigong program. And Flexion(left ; p=.029), Extension(left : p=.001, right : p=.038), Adduction(left ; p=.001, right ; p<0.001) were also significantly increased after Qigong Conclusions : The Qigong is an effective health promotion program for post-mastectomy women.

Assessment of Nutritional Status and Factors Related to Smoking in Adolescent Males -II. Psychosocial Factors Influencing Smoking among Male High School Students- (남자 고등학생 흡연자의 영양상태 판정 및 흡연관련 요인분석 -II. 흡연과 관련된 사회심리적 요인에 대한 연구-)

  • The study purpose was to investigate psychosocial factors related to smoking among adolescent boys. The Theory of Planned Behavior provide the basis for the study. Twenty-five attitudinal beliefs, 9 normative beliefs and 20control beliefs were identified through questionnaire development. The data were analyzed using t-test and χ2-test. Thirty-three percent of 300 students were smokers. Most of the beliefs examined were significantly different between smokers(n=92) and nonsmokers(n=92). With respect to attitudinal beliefs, smokers responded less negatively on the items of bad health effects of smoking such as sore throat, headache, chest pain, risk of cancer and bad blood circulation(p<0.001), and decreased physical strength(p<0.05). Smokers believed less negatively on the items that smoking leads to bothering others, bad breath, yellow teeth and making them spend money(p<0.001). In contrast, smokers felt more positively on smoking as a means of stress management, relaxing, helping digestion(P<0.001). Smokers felt less pressure for not smoking from significant others. Especially mother, siblings, friends, girl friends, seniors of school were important sources of influence regarding subject's smoking. Smokers felt less confident in controlling the urge to smoke in several situations including; when they were with friends or asked to smoke by friends; after the meal, or drinking; when they were bored or stressed, upset, and when they felt unstable(p<0.001). Smokers also scored lower on specific skills to quit or control the urge to smoke as well as overall perceived control, compared to nonsmokers(p<0.001). These results suggest that interventions for adolescents incorporate diverse strategies to increase the perceived control over smoking in specific situations as well as overall perceived control, to help them realize and modify attitudinal beliefs, and to elicit support from significant others for not smoking.   • Purpose: Pnumosinus dilatans is a rare disease that one or more of the paranasal sinuses are dilatated without functional alteration. The most frequently involved sites are frontal and sphenoid sinus. Facial asymmetric contour is the most common signs and nasal obstruction and pain may be combined. The purpose of reconstruction of pneumosinus dilatans is two-fold, to re-establish a permanent pressure equilibrium of sinus and to correct the possible facial deformities. Methods: We present a case of a 24-year-old female with a 7-year history of protrusion of right malar region. Plane radiography and computed tomography detailed an abnormal expansion of the right maxillary sinus without thinning of bony wall, leading to diagnosis of maxillary pneumosinus dilatans. Surgical decompression and maxilloplasty were achieved by ostectomy of anterior wall of maxillary sinus and repositioning of removed bony fragment with miniplate. Results: Post operative course was uneventful without complication and malar height became symmetric by physical and radiologic examination. Conclusion: We corrected successfully pneumosinus dilatans of maxillary sinus by surgical decompression and maxilloplasty. For this case, we reviewed literature related to this topic.

Neurilemoma Localized in the Palmaris Longus Tendon with no Connection to the Major Nerve Trunk (주요 신경과의 연결이 없이 발생한 긴손바닥근의 신경초종)

  • Purpose: Neurilemoma is benign tumor of the nerve sheath which arises from Schwann cells. It is usually formed along the path of a peripheral nerve but is rarely separate from normal nerve fascicles. We experienced a patient with an isolated neurilemoma localized in the palmaris longus tendon with no connection to the major nerve trunk, which was in an unusual location and has never been reported. We report our case with the review of the literature. Methods: A 23-year-old female visited our clinic with mild pain on the mass at the flexor area of the right wrist which had been present for about one year. The physical examination revealed a $1{\times}1cm$ sized subcutaneous mass at the flexor area of the right wrist. Sonography and computed tomography showed an ovoid, superficial solid mass on the palmaris longus tendon. Upon surgical excision, a $1{\times}0.5cm$ sized mass attached to the palmaris longus tendon was found. The tumor had no connection with the median nerve and was detached easily from the palmaris longus tendon. Results: Histological examination demonstrated the mass to be a neurilemoma, which consists of spindle shaped cells with oval elongated nuclei arranged fascicles. No sensory dysfunction or evidence of recurrence was found during the 12 months of postoperative follow-up. Conclusion: We experienced a rare case of neurilemoma attached to the palmaris longus tendon with no connection to the major nerve trunk. We wish to emphasize its unusual location through our case and hope to expand our spectrum in exploring the upper extremity mass.
