Research Trends of Abnormal Postures in Korean Literature -Based on Abnormal Spinal Alignment in the Sagittal Plane- (자세이상과 관련된 국내 연구동향 분석 -시상면상의 척추 정렬 이상을 중심으로-)

  • Objectives The purpose of this study is analyzing internal research trends of abnormal postures and providing problems of researches forward. Methods 6 Korean databases were searched for articles of abnormal postures, irrespective of publication year and 30 research were systematically reviewed. An analytical method such as descriptive statistics and an acutal number and percentage was used. Results We collected 30 studies. 16 studies were published within the last four years. 2012 saw 7 studies, the highest number of studies. Clinical research is the major research method. 26 studies were clinical research. The studies can be categorized according to the characteristic of the research content such as research of related factors or physical characteristics, research of treatment or programs, etc. 16 studies were about research of treatment or programs, the most number of studies. 11 studies were about research of related factors or physical characteristics, and 3 studies were about other issues. Conclusions Since the connection among postural abnormality, pain, and musculoskeletal system disorder has been raising, it is important to conduct a continued, methodical study.

The Characteristics of Hwa-byung Patients Based on Hwa-byung Epidemiologic Data (화병역학연구 자료를 기반으로 한 화병 환자의 특성)

  • Objectives : The aim of this study was to investigate the characteristics of Hwa-Byung(HB) patients, draw the clinical key issue of HB, and provide the data as the basis for development of Clinical Guideline of HB. Methods : The study participants included 151 subjects who thought they have HB in 9 site. For all patients, we used HB epidemiologic study protocol, which include the Korean version of the Structured Clinical Interview for the DSM-IV Diagnosis (SCID-1), diagnostic criteria of HB, symptoms check list, pattern identification tool, psychiastric psychological questionaires, and Framingham coronary risk score(FRS). Results : 1. For all participants, 62 % of subjects were diagnosed with HB. These patients had comorbid DSM-IV diagnoses with depression (65%) and anxiety (27%). But 22% of these patients had only HB. 2. HB patients had various physical symptoms when visiting, were diagnosed as various disease, and undergone medical treatments in the past. Among the diagonsed illness, Gastronitestinal diseases (51.6%), psychiatric disorders(40.9%), endocrine diseases(39.8%) were being the most frequent. The frequent physical symptoms of HB patients reported were chest discomfort, head ache, pallpitation, frequent sigh, amnesia, shoulder pain, dry mouth, eye fatigue etc. 3. HB patients had high scores in psychologic questionares, CES-D, STAI, and STAXI. It means that HB patients might be low-level emotional stability. 4. Participant had negative opinion about the treatment of HB that it would be difficult or impossible (65.7%), but they neglect the need of medical treatment. Conclusions : This result shows that HB is comorbid with various psychiatric disease, but it has different physio-psychological symptoms from others and there were only HB patients. so we identify HB is independent disease. Moreover, there were the wrong perception of HB which blocking treatments. These problems support that the neccesity of development of Clinical Guideline of HB.

  • By implementing epoch-making policies for industrial promotion, the national economy has made a remarkable development. As a result of such economic growth, industrial accidents and occupational diseases have become a serious problem in Korean society. In the presidential order for the execution of the Korean Labor Standard Law, neuritis and other diseases stemming from health impairments due to vibrations in industrial processes are designated to be dealt with as vibration diseases. In the case of vibration disease, industrial accident compensation is not effectively paid. In order to investigate the vibration hazards of rock-drill operation, the authors studied the subjective symptoms and performed physical function tests on a total of 79 persons (vibration exposed group) who used rock-drills, and 39 persons (control group) who did not use rock-drills at anthracite mines. The results of the physical function test were as follosws : 1. The right hand was more affected by white finger than the left hand. 2. Independent variables such as duration of rock-drill operation, age, drinking and smoking were indentified as statistically significant factors for the occurrence of white finger. 3. In the pain sense threshold, the group with Raynaud's phenomenon showed a statistically higher level than that of the control group. 4. The skin temperature of the group with Raynaud's phenomenon was lower than that of the control group. The recovery time of skin temperature aftr cooling was delayed compared with the value of the control group.

A Study on The Health Status of Island Community People in Island (도서지역 주민의 건강상태에 관한 연구)

  • During the past 10 years, concern for community people's health has increased together with the changes. Public health policies and studies for community people's health, however, have influenced those in childmaternal health care to want more general coverage and studies of health and wellness. Particularly, the study of community people's health in the extent an island area is almost rare as that personal and the material benefits in this area are lacking of community people's work is large, and the basic elements of living, such as diet and elimination, are irregular due to the schedule of the tide. Thus, there are many potential health problems. In this regard, the study attempted to understand the health problems of island community people and to provide a basis for developing health promotion and health education programs. In collecting data for the study, face to face interviews were made through a structured questionnaire from October 1 to December 30, 1996. Collected data were analyzed with the SAS statistics program, descriptive statistics, t-test and ANOVA. Subjects' health status was examined by classifying into such categories as their health perception, complaints of health problem, related lifestyle, psychosocial health staus, the result of examination is as follows; 1. For subjects' health perception, 26.9% of the subject answered not sick, but not so healthy'; 30.9% thought they were healthy, while 22.9% answered that they were not healthy. 2. For the health problem complaints many complained of pains in their muscles and skeletal system, especially knee joint pain. Women's health problems related with breast and the reproductive system included 52.3% of cases doing breast self examination, while 56.55% received the cervical cancer screening test. In men's health problems, 44.2% of subjects answered that they have moderate to severe BPH(Benign Prostatic Hypertrophy) symptom. 3. There were statistically a significant difference in the degree of physical health according to marital status(p=0.0028), occupation(p=0.0442), income(p=0.0357). 4. For stress status, 17.2% was to need the intervention, 50.2% was to need observation. 5. The mean score of self-esteem was 27.7 showing a relatively high score. 6. For the rate of smoking, 37.7% used to smoke, while 28% used to take alcohol. 7. The rate of substance abuse was 45.9% of subjects. 8. Most of subjects' health behaviors included most of the acupuncture (52%). 9. The rate of subjects receiving comprehensive medical testing was 34.36% while 34.78% did after care managing behavior. 10. For the obesity grade, 53% is normal weight, low-weight 32.8%, obesity 33%. 11. For nutrition status, 78.7% illy balanced to need intervention of nutritional education. 12. For 78.7% of subjects, muscle strength and 40.7% of stretching were not good enough to need health education on physical exercises. Therefore, based on the results, appropriate health education programs need to be developed to promote health of community people on an island.

A Latent Class Analysis and Predictors of Chronic Diseases -Based on 2014 Korea National Health and Nutrition Examination Survey- (만성질환에 관한 잠재계층분석과 예측요인 -2014 국민건강영양조사를 중심으로-)

  • The aim of this study was to investigate the latent classes and predictors of chronic diseases such as hypertension, dyslipidemia, arthritis, thyroid disease, depression, atopy, allergy, and diabetes. The subjects of this study were Korean citizens who participated in the Korea National Health and Nutrition Examination Survey in 2014. Stratified cluster sampling method was used with a sample size of 7,550. Latent hierarchy analysis was applied to this data. Four classes were identified. Class 1 consisted of participants with hypertension and diabetes. Class 2 consisted of participants with atopy and allergies. Class 3 consisted of participants with dyslipidemia, arthritis, thyroid disease, and depression. Class 4 consisted of participants without any chronic diseases. In comparing Class 1 to Class 4, age, physical activity, self-management, obesity, and presence of high cholesterol were found to be significant. In comparing Class 2 to Class 4, gender, age, and education level were significant. When Class 3 was compared to Class 4, gender, age, pain and discomfort, as well as high cholesterol were found to be significant. Diabetes and hypertension should be treated as comorbid conditions, applying integrated treatments involving effective drug treatment, diet, and physical activity programs. Atopy was found to be strongly correlated with allergies. Thyroid disease was found to coexist with dyslipidemia and arthritis, along with having a strong correlation to depression. Age-appropriate preventive measures can help reduce the risk of chronic diseases.

Changes in job stress, self-efficacy, depression and health-related quality of life according to the degree of musculoskeletal symptoms for white-collar workers (일반 사무직근로자의 근골격계 자각증상 정도에 따른 직무스트레스, 자기효능감, 우울 및 건강관련 삶의 질 변화)

  • The study aims to evaluate job stress, self-efficacy, depression and health-related quality of life according to musculoskeletal symptoms of the ordinary white-collar worker. The subjects of the study were 83 workers who were working D Health Center and Dgu office of Gwangju Metropolitan City and they were voluntarily interviewed with a use of organized questionnaire. Except the responses from three subjects whose responses were insincere of 102 ones were decided for the final analysis. The results showed that there were significant differences in all job stress, self-efficacy, depression and health-related quality of life according to musculoskeletal symptoms. Depression, self-efficacy, job stress were significantly correlated; however, health related to quality of life was not. Therefore, as the results indicate that job stress, self-efficacy, depression and health-related quality of life can be affected by musculoskeletal pain for office workers, there is a need for the development of programs focused on the prevention of musculoskeletal discomfort.

Relationship between oral health quality of life and oral health-related factors in patients with successful dental implants (임플란트 완성 환자의 구강건강관련 삶의 질과 관련 요인)

  • Objectives : This study was purposed to estimate the level of oral health quality of life and the relationship between health-related factors and oral health quality of life in patients with successful dental implants. Methods : This questionnaire survey was conducted during the period from December 2009 to March 2010 with 126 patients confirmed successful dental implants in Busan. The oral health related quality of life and oral health-related factors were estimated with OHIP-14 and 8 oral health characteristics, respectively. Data analysis was performed with descriptive analysis, t-test, ANOVA, multiple regression analysis using SAS(ver 9.1) program. Results : The score of oral health quality of life in patients with successful dental implants was $12.25{\pm}7.82$. The scores of subscale of the oral health quality of life were $1.24{\pm}1.35$ in social disability, $1.60{\pm}1.37$ in handicap, $1.60{\pm}1.44$ in psychological disability, $1.70{\pm}1.38$ in functional limitation, $1.81{\pm}1.35$ in physical disability, $2.14{\pm}1.46$ in physical pain, $2.17{\pm}1.46$ in psychological discomfort, respectively. The related factors of oral health quality of life in patients with successful dental implants were absence of other type prosthesis and experience of tooth brushing education. Conclusions : The oral health related quality of life in patients with successful dental implants was relatively good condition. Social supports and chances for high quality denture and tooth brushing educations are needed to improve oral health related quality of life in patients with dental implants.

Arthroscopy for the Diagnosis and Treatment of Failed Trochleoplasty in a Dog (활차구 성형술이 실패한 개에서 관절경을 이용한 진단과 치료)

  • A 3-year-old castrated male Spitz dog (8.4 kg) was referred for hindlimb lameness. The dog had undergone corrective surgery for bilateral medial patellar luxation 6 weeks prior to presentation. Intermittent lameness of both hind limbs was evident on general inspection. Physical examination revealed moderate pain and crepitus upon flexion and extension of the stifle joints. Radiography revealed features consistent with distal deviation of the cartilage flaps and effusion in both stifle joints. Detachment and deviation of both osteochondral blocks were suspected based on the physical and radiographic findings. Arthroscopy confirmed failed osteochondral blocks (right $1.5cm{\times}0.9cm{\times}0.2cm$, left $1.3cm{\times}0.9cm{\times}0.1cm$), which were removed. After removal of the osteochondral blocks, the stepped trochlear grooves were located and debrided using an arthroscopic burr, to prevent patellar tracking disorder. The dog resumed normal ambulation without medial patellar luxation 2 weeks postoperatively, and there were no complications. Arthroscopy is an effective surgical modality for the diagnosis and treatment of canine joint disorders. It offers a magnified view of the joint, which allows accurate evaluation of intra-articular structures. In addition, arthroscopic surgery can reduce the postoperative complications associated with surgical wound.

Carpal Tunnel Syndrome among Packing Workers in A Rayon Manufacturing Factory (모 사업장 포장부서 근로자들에서 발생한 수근터널증후군에 대한 조사연구)

  • The carpal tunnel syndrome is one of the most common peripheral nerve entrapment syndromes. The typical symptoms are pain, numbness and paresthesia in the median nerve territory of the hand. Recently, it is widely recognized that occupational factor is regarded as the important cause of the carpal tunnel syndrome. Clinical study is performed in the 42 female workers who is repetitively working at packing department in a rayon manufacturing factory from November 1991 till March 1992. The study included a questionnaire, physical examinations, and the neurophysiological test. The summary of the results obtained was as follows : 1. Among 42 packing workers, 9 workers(21.4%) were diagnosed as carpal tunnel syndrome by electromyography. The affected side was bilateral in 4 workers(9.5%), right in 4 workers(9.5%), and left in one worker(2.4%). 2. Among 42 subjects, 28 workers(66.7%) complained the clinical symptoms related to carpal tunnel syndrome, 11 workers(26.2%) showed positive Phalen sign, and 7 workers(16.7%) showed positive Tinel sign. 3. Researchers regard electromyographic finding as the gold standard for diagnosis of carpal tunnel syndrome. The sensitivity and specificity of the clinical symptoms to diagnose the carpal tunnel syndrome were 0.89, 0.39 respectively. If the carpal tunnel syndrome is diagnosed by the combination of the positive findings of the symptoms and the physical examinations, either Tinel or Phalen sign, the sensitivity and specificity were 0.67 and 0.76 respectively. Considering above results, though this small number of workers is not adequate for epidemiologic conclusions, carpal tunnel syndrome seems to be an important occupational disorder among packing workers in a rayon manufacturing factory.

A Study of Spinal Curvature in Female and Male University Students (남녀 대학생의 척추만곡에 관한 연구)

  • This study examined the rates of spinal abnormal curvature and the correlation of the Body Mass Index (BMI), Low Back Pain (LBP) and spinal curvature by measuring scoliosis, kyphosis, and lordosis in university students. The study population included 67 male, 92 female university students, making a total of 159, in Wonju City. Spinal curvature was measured by an electrogoniometer in a computerized skeletal analysis system. Lateral curvature of spine of more than 10 degrees was considered as nonspostural scoliosis. The correlation of BMI, LBP and the spinal curvature was analysed by Pearson's correlation coefficient and t-test. The following results were obtained: 1. The overall incidence and rate of scoliosis in cases with a greater than 10 degree curve in males was an incidence of 8 and a rate of 11%. In females the incidence was 36 and the rate 39.2%. 2. The overall incidence and rate of kyphosis of less than 20 degrees in males was a rate of 9 and an incidence of 11.9%. In females, the rate was 5 and the incidence 5.4%. In kyphosis cases of more than 40 degrees, the male rate was 5 and the incidence 7.7%. For female the rate was 13 and the incidence 14.2%. 3. The overall incidence and rate of lordosis with curves of less than 20 degrees was a rate of 6 for males and an incidence of 9.0%. For females, the rate was 5 and the incidence 5.4%. In cases of more than 50 degrees lordosis, the female rate was 2 and the incidence 2.2%. There were no males in this category. 4. There was a negative correlation between kyphosis and BMI. The greater the kyphotic curve, the less the BMI in males (p<0.05). There was no significant BMI difference by gender in either scoliosis or lordosis. There was, however, a significantly decreased sacral angle in the female group with LBP. The results of this study cannot be generalized to the general population because the subjects were all from one university. The measurements were quite reliable because the angles determined by the Metrocom System were highly correlated with radiologic findings. This study shows the need for a regular screening system for spinal curvatures in university health examination procedures.
