  • Purpose: To describe the psychosocial adaptation, physical disability and social support, and to examine whether the physical disability and social support influence the psychosocial adaptation of women with Rheumatoid Arthritis(RA). Method: This survey was conducted with 102 women diagnosed as RA using a structured survey tool between April 12th and 30th 2004. Results: The Physical disability ranged from 0 to 51, the average was 9.89(${\pm}12.15$), appearing that less severe than previous studies. The social support ranged from 29 to 168, and the average was 91.73(${\pm}31.44$). The age, marital status, and monthly income were associated with patient's perceived social support. The psychosocial adaptation ranged from 77 to 186 and the average was 132.12(${\pm}24.13$). Entering physical disability and social support into the model significantly improved the prediction of psychosocial adaptation: 45.1% of the variance of psychosocial adaptation was attributed by the physical disability (Beta=-.325) and the social support (Beta=.204). Additionally, the religion (Beta=.231) and monthly income (Beta=.381) were significant predictors of the psychosocial adaptation. Conclusions: (1) Programs to improve physical disability of the clients are needed. (2) Marital status and age should be considered when the programs are developed. (3) More social support should be provided to the women with RA. (4) Adequate financial support is essential for the psychosocial adaptation of women with RA.

  • This study examined the effects of a physical therapy program on quality of life (QOL), and neck and shoulder disability in head and neck cancer patients. The program included neck and shoulder range of motion (ROM) exercises, massage, progressive strengthening exercises, and stretching exercises. Sixteen patients who were assigned to an experimental group performed physical therapy for 40 minutes three times a week for eight weeks. Fifteen other patients were assigned to a control group who did not performed the physical therapy program. The European organization for research and treatment of cancer (EORTC QLQ-C30) and head and neck (EORTC QLQ-H&N) instruments, and the Neck Disability Index (NDI) were assessed before and after the rehabilitation program. The 40-minute program consisted of a 10-minute ROM exercise for the neck and shoulder, a 10-minute massage and 15-minute of progressive resistance exercises, followed by a five-minute stretching exercises. Statistically significant differences were noted for changes in global health, physical function in the EORTC QLQ-C30 and cancer related symptoms in the EORTC QLQ-H&N35 (p<.05). The NDI also showed significant differences (p<.05). Physical therapy may therefore benefit the physical aspects and QOL and improve neck and shoulder disability in patients with head and neck cancer.

  • Background: The prevalence of neck pain and neck dysfunction is high in general population. However, there is little literature on the relationship and factors affecting neck pain and neck dysfunction. Objective: To investigate the correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction, and sleep quality in adults with chronic neck pain, and factors affecting neck pain and neck dysfunction. Design: Cross-sectional study Methods : The sample included 114 subjects, who had complained of chronic neck pain for more 12 weeks. We conducted a Pearson's correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction and sleep quality and a regression analysis of the related variables, thereby analyzing factors affecting neck pain and neck dysfunction. Results : In the present study, in adults with chronic neck pain, neck pain was positively correlated with the Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI)-Pain score, and SPADI-Total score (p<.05). The NDI was positively correlated with neck pain, SPADI-Pain score, and SPADI-Total score, as well as with Pittsburgh Sleep Qulity Index(PSQI-K) (p<.05). Among the factors affecting neck pain, shoulder disability as assessed by the SPADI was a significantly associated with neck pain, while shoulder pain and shoulder disability determined by the SPADI were identified as significant variables among the factors affecting neck disability. Conclusion : These results indicated that as neck pain worsened, shoulder pain and neck and shoulder dysfunction also increased, which suggested that shoulder disability affected neck pain. In addition, as the neck dysfunction increased, neck pain and shoulder pain and shoulder dysfunction increased, and sleep quality deteriorated, which suggested that shoulder pain and shoulder disability affected neck disability.

  • Most conventional instruments measuring disability rely on total score by simply adding individual item responses, which is dependent on the items chosen to represent the underlying construct (test-dependent) and a test statistic, such as coefficient alpha for the estimate of reliability, varying from sample to sample (sample-dependent). By contrast, item response theory (IRT) method focuses on the psychometric properties of the test items instead of the instrument as a whole. By estimating probability that a respondent will select a particular rating for an item, item difficulty and person ability (or disability) can be placed on same linear continuum. These estimates are invariant regardless of the item used (test-free measurement) and the ability of sample applied (sample-free measurement). These advantages of IRT allow the creation of invariantly calibrated large item banks that precisely discriminate the disability levels of individuals. Computer adaptive testing (CAT) method often requiring a testing algorithm promise a means for administering items in a way that is both efficient and precise. This method permits selectively administering items that are closely matched to the ability level of individuals (measurement precision) and measuring the ability without the loss of precision provided by the full item bank (measurement efficiency). These measurement properties can reasonably be achieved using IRT and CAT method. This article aims to investigate comprehensive overview of the existing disability instrument for back pain and to inform physical therapists of an alternative innovative way overcoming the shortcomings of conventional disability instruments. An understanding of IRT and CAT method will equip physical therapist with skills in interpreting the measurement properties of disability instruments developed using the methods.

  • According to the Ministry of Health and Welfare, the portion of physical disability was 51.4% which was the highest among the registered disabled in December 2015, followed by visual disability(10.1%), disability of the brain lesions(10.0%), hearing disability(10.0%). The aim of this study is to investigate the types of disability in patients with disabilities visiting the Pediatric Dentistry, Chonbuk National University Dental Hospital in 2016. Of the patients who visited the pediatric dentistry, 196 patients (7.5%) had disabilities, and 19 patients had two or more disabilities. Twelve patients (6.1%) had both disabilities of brain lesion and intellectual disorder. Of the 177 patients with only one disability, 54 patients (30.5%) had physical disorders and 123 patients (69.4%) had psychiatric disorders. In psychiatric disorders, intellectual disorder was the most with 98 patients (55.3%), followed by autistic disorder with 23 patients (12.9%). In physical disorders, disability of brain lesion was the most with 37 patients (20.9%), followed by speech disability with 8 patients (4.5%). In this study, as a result of analyzing the types of disability with the patients who visited the Pediatric Dentistry of Chonbuk National University Dental Hospital, most of them had either intellectual disorder, disability of brain lesion, or autistic disorder. Dentists need to understand their general and dental characteristics to treat children and adolescents with disabilities. Since the most chief complaint of the patients was dental caries, early interventions should be made by dentist to reduce the incidence of the disease.

  • This study has been done for the purpose of determining whether the positive association between social support and depression is attributable to an overall beneficial effect of support(direct effect) or to a process of support protecting persons from adverse effects of stressors such as life events, pain or physical disability (buffering effect). The sample consisted of 214 patients who were identified as the rheumatoid arthritis. The instruments used in this study were Depression Scale(CES-D), Perceived Social Support Scale, Life Events Questionnaire, AIMS Pain Scale, and Physical Disability Scale. The data were analysed by the use of t-test, ANOVA, Pearson Correlation Coefficient and Stepwise Multiple Regression. The results of this study are summerized as follows : 1. The 1st hypothesis, "The higher the life events degree, the higher the depression degree" was supported(r=.49, P=.0001). 2. The 2nd hypothesis, "The higher the pain degree, the higher the depression degree" was supported(r=.44, P=.0001). 3. The 3rd hypothesis, "The higher the physical disability degree, the higher the depression degree" was supported (r=.46, P=.0001). 4. The 4th hypothesis, "The higher the social support degree, the lower the depression degree" was supported(F=84.52, P=.0001). 5. The 5th hypothesis, "There will be different in the relationship between the degrees of life events and depression according to social support degree" was rejected (F=.29, P=.5928). 6. The 6th hypothesis, "There will be different in the relationship between the degrees of pain and depression according to social support degree" was supported (F=3.19, P=.0755). 7. The 7th hypothesis, "There will be different in the relationship between the degrees of physical disability and depression according to social support degree" was supported(F=5.69, P=.018). 8. The predictive variables for depression were the degrees for social support, life events, pain, and physical disability. 9. The depression degree showed a inverse correlation with social support degree (r=-.56, p=.0001). The social support degree showed a inverse correlation with the degrees of life events(r=-.22, p=.0007), pain(r=-.18, p=.0069) and physical disability(r=-.15, p=.0293). 10. The depression degree showed significant differences in the variables of sex (t=2.26, p=.025), educational level(r=.189, P=.006) and the number of treatment method (r=.201, P=.003). In conclusion, it was found that social support had the direct effect on depression and the buffering effect in each relationship between degrees of pain or physical disability and depression in patients with rheumatoid arthritis. So the researcher thinks that it is effective that nurses should provide these patients with social support to reduce depression in cases of having severe pain or physical disability.

  • The purpose of this study is to analyze the physical characteristic by directly measuring the wheelchair using disabled women. The subjects were 103 disabled women of wheelchair used women and between 20 - 55 years of age. The result of this study is as follow. There was a remarkable difference in the physical characteristic of wheelchair using disabled women due to their cause of disability. The cause of disability was classified into the 4 groups; poliomyelitis, spiral cord injury, muscular dystrophy, cerebral palsy. Poliomyelitis disability generally had a large horizontal area due to their strong upper body. People with spiral cord injury disability was shown to have the largest height, cervical height, waist back length, crotch length, knee length, The group of muscular dystrophy disabled people have the shortest length of body and also lean. The cebral palsy group of disabled people has an average length and height size body. A comparison of anthropometric measurements of wheelchair using disabled women with National Anthropometric Survey Korea(1997) was significant difference. People with poliomyelitis disability was shown to have a larger waist back length, neck point to breast point compared to normal women, but stature, crotch length was shorter compared to normal women. People with spiral cord injury disability had a similar in the vertical area. The group of muscular dystrophy and cebral palsy disabled people was short and smaller in general compared to a normal woman.
