Muscle tension dysphonia (MTD) is a voice disorder characterized by excessive tension of the laryngeal muscles during phonation. Voice therapy is the gold standard of treatment for MTD. However, patients with MTD do not always respond to voice therapy. Multidisciplinary approaches have been attempted to treat intractable MTD such as lidocaine instillation, lidocaine injection to recurrent laryngeal nerve, botox injection and excision of false ventricle using CO2 laser. Recently, injection laryngoplasty is suggested that assists in more efficient phonation and voice therapy to MTD patients. A patient with intractable MTD underwent lidocaine injection and injection laryngoplasty showed improved voice quality and remained stable until postoperative 3 months without any complications.
Hunminjeongeum had 17 initial consonant letters. Among them, five consonant letters, those are ㄱ (牙音, molar sound letter), ㄴ (舌音, lingual sound letter), ㅁ(脣音, labial sound letter), ㅅ (齒音, dental sound letter), ㅇ (喉音, guttural sound letter), were served as chief consonants. There was no argument that consonant letters were made by symbolizing the shape of vocal organs during phonation of them. It could be phoniatrically explained that all of five chief consonants were morphologically symbolized from left lateral view of vocal tract during articulation. Although 'ㄱ' was known as molar sound, it was not modeled the shape of molar tooth but modeled the shape of tongue at molar teeth bearing area. The same principle applies to 'ㅅ', and it was represented the shape of upper surface of anterior tongue instead of incisor teeth. 'ㄴ' was a lingual sound and directly shaped the shape of tongue. 'ㄷ' was made by addition of a stroke 'ㅡ' meaning hard palate above 'ㄴ'. 'ㅁ' was represented the shape of lateral view of anterior mouth. 'ㅇ' was looked like shaping left lateral view of laryngopharyngeal space.
Background and Objectives : Many students major in applied music singing have various difficulties in phonation when they sing or perform. But the studies about this cause are lack. The purpose of this study is whether singing voice therapy based on vocal music is effective or not to applied music singing students that have difficulties in phonation. Materials and Methods : Singing voice therapy program had been done to 8 students major in applied music singing during 7 weeks. We did laryngeal stroboscope, perceptual, acoustic and aerodynamic examination before and after singing voice therapy to this students. And we studied post examination results have differences or improvements compared to pre examination results. Results : Four male students don't have effective improvement after singing voice therapy but they shows slight improvement in aerodynamic examination values compared to pre examination values. Four female students shows improvement in aerodynamic examination values and pitch range increasing. Above all, one student's vocal nodules disappeared after singing voice therapy. Conclusion : Singing voice therapy based on music singing is very effective to the students major in applied music singing. Above all, their curriculum in applied music singing should include phonation training based on music singing before training perform and technic practices.
This research compared the features of respiration and phonation between practical musicology students and general male students, according to their smoking status. Participants of this research are 15 practical musicology male students attending ${\bigcirc}{\bigcirc}$ university and 16 general ${\bigcirc}{\bigcirc}{\bigcirc}$ university students. The participants, both non-smokers and smokers with 5-years of smoking history have no history of voice disease in any case and have normal cognitive functions. The results indicated that, first, there is not a notable difference in the respiratory activity status(FVC, FEV1, FEV1/FVC), regardless of major and smoking status. In MPT, even though there is no significant difference in accordance with their majors, considering smoking status, the smoker group was shorter than non-smoker group significant difference statistically (p<.01). Second, the divisions of participants' major did not show significant difference in Fo, jitter, shimmer, and NHR in the vowel prolongation task. However, the smoker group showed a significantly higher degree of jitter and shimmer than the non-smoker group (p<.05) as Fo and NHR shows no difference. In the case of VRP, maximum frequency and frequency range of the practical group are significantly higher than normal group statistically (p<.001). Moreover, although the difference of the minimum frequency shown at the statistic is not significant, practical group showed a higher tendency of frequency than normal group (p=.051). In conclusion, even though there is no difference in respiratory activity between the smoker group and non-smoker group, the MPT of the smoker group is shorter than that of non-smoker group. In addition, the smoker group showed a higher degree of jitter and shimmer than the non-smoker group. MPT is related to the valve action of vocal fold that passes through the glottis. Thus, it is interpreted that the smoker group has a lower quality of voice and valve action of the vocal fold. Also, the practical group has a higher degree of maximum frequency and frequency range than the normal group. This research can function as basic data for vocal characteristics for the majors in relation to the voice-specializing.
Background and Objectives : Vocal fold cyst is generally treated by surgical resection, it has a difference with vocal fold polyp, treated by conservative management first. Decrease in mucosal waves is known as main diagnostic criteria of vocal fold cyst. Sometimes there is a difficulty for diffrential diagnosis between cyst and polyp only by endoscopic examination. The purpose of the study is to identify the objective features of vocal cyst and polyp on the basis of voice analysis for the proper differential diagnosis, especially at high pitched phonation. Materials and Method : The voice analysis was done in 15 focal fold cyst patients and 42 vocal fold polyp. Parameters of perceptual assessment, acoustic and aerodynamic measure, and voice range profile were compared between two groups. Results : Vocal fold cyst patients showed significantly reduced MPT by acoustic and aerodynamic analysis, narrowed frequency-range and low maximun frequency by voice range profile analysis compared with vocal fold polyp patient. Maximun frequency 381 Hz is established for cut off value, differential diagnosis between cyst and polyp (ROC analysis, sensitivity 60%, specificity 68%). Conclusion : Voice analysis is helpful for differential diagnosis between vocal fold cyst and polyp, especially there is a difficulty for distinguish cyst from polyp at clinical situation by endoscopic examination. The result of decreased maximum frequncy at vocal fold cyst supports incomplete high-pitched phonation and falsetto regester at vocal fold cyst patients due to decreased mucosal wave, compared with vocal fold polyp patients.
Background : Cepstral analysis which is obtained from Fourier transformation of spectrum has been known to be effective indicator to analyze the voice disorder. To evaluate the voice disorder, phonation of sustained vowel /a/ sound or continuous speech have been used but the former was limited to capture hoarseness properly. This study is aimed to compare the effectiveness in analysis of cepstrum between the sustained vowel /a/ sound and continuous speech. Methods : From March 2012 to December 2014, total 72 patients was enrolled in this study, including 24 unilateral vocal cord palsy, vocal nodule and vocal polyp patients, respectively. The entire patient evaluated their voice quality by VHI (Voice Handicap Index) before and after treatment. Phonation of sustained vowel /a/ sample and continuous speech using the first sentence of autumn paragraph was subjected by cepstral analysis and compare the pre-treatment group and post-treatment group. Results : The measured values of pre and post treatment in CPP-a (cepstral peak prominence in /a/ vowel sound) was 13.80, 13.91 in vocal cord palsy, 16.62, 17.99 in vocal cord nodule, 14.19, 18.50 in vocal cord polyp respectively. Values of CPP-s (cepstral peak prominence in text-based speech) in pre and post treatment was 11.11, 12.09 in vocal cord palsy, 12.11, 14.09 in vocal cord nodule, 12.63, 14.17 in vocal cord polyp. All 72 patients showed subjective improvement in VHI after treatment. CPP-a showed statistical improvement only in vocal polyp group, but CPP-s showed statistical improvement in all three groups (p<0.05). Conclusion : In analysis of cepstrum, text-based analysis is more representative in voice disorder than vowel sound speech. So when the acoustic analysis of voice by cepstrum, both phonation of sustained vowel /a/ sound and text based speech should be performed to obtain more accurate result.
본 연구는 세 명의 연하장애환자들을 대상으로 후두관련기능의 개선을 목표로 한 치료적 노래부르기 중심의 음악중재를 시행하고, 각 사례 별로 관찰된 변화를 기술한 개별 사례연구이다. 중재는 개별 세션의 형태로 매 회기 30분, 주 3회씩 총 12회기 동안 진행되었으며, 시행된 프로그램은 호흡근 이완, 구강근육운동, 후두상승 유지를 위한 발성과 치료적 노래부르기 등의 순서로 구성되었다. 대상자들의 연하 및 말하기 기능, 삶의 질 등의 변화를 알아보기 위해 매 회기 후두교호운동속도, 최대발성시간, 음성 질 관련 항목 등을 측정하였고, 중재 전후로 설문을 통해 삶의 질을 평가하였다. 중재 과정에서 대상자의 기능적 특성에 따라 구분되는 변화의 양상이 관찰되었고, 중재 후에는 모든 대상자의 후두교호기능 및 음성 질 지표 측정값에 변화가 있었으며, 연하관련 삶의 질 평가점수도 증가하였다. 본 연구를 통해 치료적 노래부르기 중심의 음악중재프로그램이 다양한 원인에 의한 연하장애환자들의 후두관련기능 및 삶의 질 영역에 공통적으로 혜택을 줄 수 있다는 점과, 중재 시 대상자의 기능적 특성에 따라 고려해야 할 부분에 대해 구체적으로 알 수 있었다.
Background and Objectives Therapies have been reported to treat the glottal gap previously. However, these voice therapies showed the limits because many techniques focused only on one among breathing, resonance and phonation. In addition patients often have difficulties visiting hospital frequently. 'Gliding and humming' is vocal training technique that readjusts total vocal patterns such as breathing, resonance and phonation. This technique can be easily applied during short term sessions. The purpose of this study is to evaluate the efficiency of voice therapy with 'gliding and humming' for patients with glottic gap during short-term treatment sessions. Materials and Method Twenty-three patients with glottal gap were selected. Of all patients, 14 patients had sulcus vocalis and 12 patients had muscle tension dysphonia (MTD). Voice therapies were performed 1.9 sessions in average. GRBAS, jitter, shimmer, noise to harmonic ratio, semitone range, closed quotient_vowel and maximum phonation time were compared before and after the therapies. In addition, changes of glottal gap and MTD severity were evaluated. Results Statistically significant improvement was observed. MTD improvement was observed only among the patients with glottal gap improvement. Also sulcus vocalis group showed the statistically significant improvement. Conclusion 'Gliding and humming' was effective to the patients with glottic gap and sulcus vocalis. Also, among patients who have both glottic gap and MTD, the data suggests that voice therapy for glottic gap also makes improvement in MTD.
본 연구는 조기 양측 인공와우이식 아동과 건청 아동 간 파열음의 조음위치와 발성유형에 대한 성대진동 시작시간(voice onset time, VOT)의 차이를 살펴봄으로써, 조기 양측 인공와우이식을 통한 청각적 보상과 피드백이 청각장애 아동의 말 운동통제 능력에 미치는 영향을 살펴보고자 하였다. 대상자는 만 5세부터 10세 사이의 양측 인공와우이식 아동 15명과 건청 아동 15명을 선정하였다. 검사 과제는 조음위치 및 발성 유형을 고려한 9개 파열음의 VCV음절을 사용하였다. VOT 측정을 위해 Praat 프로그램을 사용하였으며, 정확한 측정을 위해 녹음된 음성, 스펙트로그램, 포먼트 정보를 함께 사용하였다. 삼원혼합분산분석 결과, 집단에 대한 주 효과는 유의하지 않았다. 조음위치에 대한 주 효과는 유의하였으며, 연구개음의 VOT값이 양순음과 치조음보다 유의하게 길었다. 발성유형에 대한 주 효과도 유의하였으며, VOT값은 격음, 평음, 경음 순으로 유의하게 길었다. 이러한 결과는 조기 양측 인공와우이식을 통한 양이 청취가 고도 이상의 난청 아동의 말 운동 통제 능력에 긍정적인 영향을 준다는 것을 시사한다.
This study examines the acoustic characteristics of Korean stops of two dialects, Seoul and Kyungsang, focusing on VOT(Voice Onset Time). 8 speakers of these two dialects were asked to read 590 words which contain the stops of different places of articulation and phonation types. The results showed that overall the VOTs of Kyungsang dialect were shorter than those of Seoul dialect. This was more prominent in lenis stops than in fortis or aspirated stops. It was also shown that there were significant VOT overlapping differences between the two dialects.
