Persistent Gingival Swelling and Fistula Obscured Horizontal Root Fracture: A Case Report

  • Horizontal root fracture (HRF) is a result of trauma to teeth and periodontium, which implies severe injury to cementum, dentin, and pulp. This is a rare case of HRF in the maxillary lateral incisor of a 62-year-old male who only presented persistent gingival swelling, fistula, and dull pain at first. An apical radiolucency of unknown origin turned out to be a result of hidden HRF at the coronal third level that was later visualized radiographically during endodontic treatment. The tooth was scheduled to be extracted upon the patient's agreement. The purpose of this report is to alert clinicians about the importance of diagnosing HRF through thorough clinical and radiographic examinations. Where there is persistent fistula without proper cause, HRF should be considered as a causative factor, and the diagnosis could be effective with aid of cone beam computed tomography, electronic root apex locator, as well as other clinical signs.

Korean Medicine Treatment Accompaning Myofascial Trigger-Point Accupuncture for Lower Urinary Tract Symptoms after Radical Prostatectomy : A Case Report (근치적 전립선 절제술 후 발생한 하부요로증상의 근막 트리거 포인트 침치료를 병행한 한의치료 1례)

  • Objectives: The purpose of this study is to report a case of Korean medicine treatment accompaning myofascial trigger-point accupuncture for severe & persistent lower urinary tract symptoms after radical prostatectomy. Materials and Methods: A prostate cancer patient with severe & persistent lower urinary tract symptoms after radical prostatectomy conducted 26 months ago, was treated with Korean medicine accompaning myofascial trigger-point accupuncture. The International Prostate Symptom Score(IPSS), the IPSS Quality of Life(IPSS-QoL) and the Overactive Bladder Symptom Score(OABSS) questionaires were administered to evaluate the effect of treatment for lower urinary tract symptoms. Results: After 5 month treatment with Korean medicine accompaning myofascial trigger-point accupuncture, IPSS, IPSS-QoL and OABSS progressed from 28 to 8, from 5 to 2 and from 12 to 4 respectively. Conclusion: This case study suggests that Korean medicine accompaning myofascial trigger-point accupuncture may contribute to treat severe & persistent lower urinary tract symptoms after radical prostatectomy.

Factors for persistent growth hormone deficiency in young adults with childhood onset growth hormone deficiency (소아청소년기 성장호르몬결핍증의 성인기 지속에 영향을 미치는 요인)

  • Purpose : Growth hormone (GH) replacement after retesting is necessary because impairment of body composition and cardiovascular health has been more severe in adult patients with persistent GH deficiency (GHD) from childhood to adulthood. This study aimed to investigate the factors for persistent GHD and define a highly probable group of persistent GHD in young adults with childhood-onset GHD. Methods : GHD was reassessed by insulin tolerance test (ITT) in 55 adult patients (39 males, 16 females) with childhood-onset GHD. Twelve patients presented with idiopathic GHD and 43 patients presented with organic GHD caused by tumors involving the hypothalamus-pituitary (H-P) region (n=33), other brain tumors (n=3), meningitis (n=3), leukemia (n=2) and others (n=2). Results : Forty-nine (89.1%) of 55 patients had persistent GHD. IGF-I was positively correlated with log of peak GH (r=0.57, P<0.001). There was no difference in the proportion of persistent GHD between idiopathic and organic GHD. The percentage of patients with persistent GHD was 40%, 80%, and 95.6% for patients with zero, one, two or more additional pituitary hormone deficiencies (PHDs), respectively (P=0.002). The probability of persistent GHD was higher in patients with diseases involving the H-P region (P=0.003). GHD persisted in 15 of 18 patients treated with cranial irradiation. Conclusion : We suggest that the probability of persistent GHD in adulthood was high in patients with 2 or more additional PHDs, and diseases involving the H-P region.

Application of HPV DNA Testing in Follow-up after Loop Electrosurgical Excision Procedures in Northern Thailand

  • Background: HPV DNA testing has been recently introduced as an adjunct test to cytology in the follow-up of patients after treatment for cervical lesions using the loop electrosurgical excision procedure (LEEP). The aim of this study was to evaluate the role of HPV testing in the detection of persistent or recurrent disease after LEEP in patients with cervical epithelial lesions in northern Thailand. Materials and Methods: Patients who underwent LEEP as a treatment for histological low-grade (LSIL) or high-grade squamous intraepithelial lesion (HSIL) or worse at Chiang Mai University Hospital between June 2010 and May 2012 were included. Follow-ups were scheduled at 6-month intervals and continued for 2 years using co-testing (liquid-based cytology and Hybrid Capture 2 [HC2]) at 6 months and 24 months and liquid-based cytology alone at 12 and 18 months. Results: Of 98 patients included, the histological diagnoses for LEEP included LSIL in 16 patients, and HSIL or worse in 82 patients. The LEEP margin status was negative in 84 patients (85.7%). At follow-up, 10 patients (10.2%) had persistent/recurrent lesions; 4 among LSIL patients (25.0%) and 6 in the group with HSIL or worse (7.3%). Only 2 of 82 patients (2.4%) with HSIL or worse diagnoses had histological HSIL in the persistent/recurrent lesions. Using histologically confirmed LSIL as the threshold for the detection of persistent/recurrent disease, cytology had a higher sensitivity than HC2 (90.0% versus 70.0%). At the 6-month follow-up appointment, combined cytology and HC2 (co-testing) had a higher sensitivity in predicting persistent/recurrent disease (80.0%) compared with that of cytology alone (70.0%) and HC2 (50.0%). Conclusions: After LEEP with a negative surgical margin, the rate of persistent/recurrent lesions is low. The addition of HPV testing at the 6-month visit to the usual cytology schedule may be an effective approach in the follow-up after LEEP.

Bronchoscopic Ethanolamine Injection Therapy in Patients with Persistent Air Leak from Chest Tube Drainage

  • Background: Chest tube drainage (CTD) is an indication for the treatment of pneumothorax, hemothroax and is used after a thoracic surgery. But, in the case of incomplete lung expansion, and/or persistent air leak from CTD, medical or surgical thoracoscopy or, if that is unavailable, limited thoracotomy, should be considered. We evaluate the efficacy of bronchoscopic injection of ethanolamine to control the persistent air leak in patients with CTD. Methods: Patients who had persistent or prolonged air leak from CTD were included, consecutively. We directly injected 1.0 mL solution of 5% ethanolamine oleate into a subsegmental or its distal bronchus, where it is a probable air leakage site, 1 to 21 times using an injection needle through a fiberoptic bronchoscope. Results: A total of 15 patients were enrolled; 14 cases of spontaneous pneumothorax [idiopathic 9, chronic obstructive pulmonary disease (COPD) 3, post-tuberculosis 2] and one case of empyema associated with broncho-pleural fistula. Of these, five were patients with persistent air leak from CTD, just after a surgical therapy, wedge resection with plication for blebs or bullae. With an ethanolamine injection therapy, 12 were successful but three (idiopathic, COPD and post-tuberculosis) failed, and were followed by a surgery (2 cases) or pleurodesis (1 case). Some adverse reactions, such as fever, chest pain and increased radiographic opacities occurred transiently, but resolved without any further events. With success, the time from the procedure to discharge was about 3 days (median). Conclusion: Bronchoscopic ethanolamine injection therapy may be partially useful in controlling air leakage, and reducing the hospital stay in patients with persistent air leak from CTD.

Structure-Model Verification of Yacht Participants' Motivation, Basic Psychological Need Satisfaction and Intentions of Persistent Participation (요트참가자들의 기본심리욕구와 운동참가동기 및 지속적 참여 의도의 구조모형 검증)

  • The purpose of this study was verily the structure model of yacht participants' motives for sport participation, satisfactions of basic psychological need and intentions of persistent participation. Their motives for sport participation and satisfaction of basic psychological desires are reported as variables involving so much in their intentions of persistent participation in the sport activity in which participants of Life Exercise take part. Therefore, a survey research was conducted by aiming at members of yacht foyers' groups and university yacht clubs, 267 copies of the questionnaire in total were used for the analysis. SPSS 12.0 and AMOS 7.0 were used for the processing of the data, and exploratory factor analysis, confirmatory factor analysis, reliability analysis, correlation analysis and covariance structure model analysis were also applied according to each study objective. The results of the study were as follows: Firstly, there were significant correlations among yacht participants' satisfaction of basic psychological desires, participation motives and intentions of consistent participation. Secondly, yacht participants' satisfaction of basic psychological desires was found to have significant and direct effects on their intentions of persistent participation while haying indirect effects through the medium of their motives for sport participation at the same time. Thirdly, their motives for sport participation had significant effects on their intentions of persistent participation. Fourthly, suggested by the study, the structural equation model of yacht participants' motives for sport participation, satisfaction of basic psychological desires and intentions of persistent participation was found statistically appropriate.

Temperament characteristics of children with persistent and recovered stuttering: A longitudinal study (말더듬이 지속된 아동과 회복된 아동의 기질 특성 비교: 종단연구)

  • The purpose of this study was to investigate the temperament characteristics associated with stuttering subtypes (persistent and recovered) over time and the relationship between those characteristics and stuttering severity. This four-year longitudinal study covered 41 preschool children who stutter (CWS) and 30 preschool children who do not stutter (the CWNS group). At the final visit, 27 CWS were classified as the Recovered group and 14 CWS were classified as the Persistent group. Using the Children's Behavior Questionnaire-Short Form, each participant's temperament characteristics were measured twice: at one year and two years after the initial visit. The three subscale scores (Extraversion, Negative Affectivity, and Effortful Control) and the 15 component scores were analyzed, and they were used for between-group and between-visit comparisons. The Persistent group showed a significantly higher Negative Affectivity subscale score at every visit than the Recovered and CWNS groups. Within this subscale, significant group differences were found in the 'Fear' and 'Anger/Frustration' components, demonstrating that the Persistent group scored higher than the Recovered and CWNS groups. There was no significant correlation between the subscale and component scores and the stuttering severity scores within the Persistent group at any visit. These results support the proposition that these two stuttering subtypes have different temperament characteristics; they also imply that temperament might be influenced by stuttering experience over time.

  • This paper deals with the design and fabrication of loop superconducting Persistent Current Switch(PCS) circuit. The self and mutual inductances of the circuits are calculated and compared to the measured values. The size of loop circuits was determined using the calculated inductances, and the fabrication processes of the superconducting persistent current switch are described.
