末梢性 眩暈을 主症狀으로 入院한 患者 1例에 對한 症例報告 (A Clinical Review of Peripheral Dizziness Patient-one case)

  • Introduction: Vertigo is one of the common symptoms that we can see often clinically. It is hallucination to motion of oneself or surroundings. Vertigo include not only simple whirling sensation but also leaning or falling down sensation. Particularly in vestibular neuritis, the principal symptoms is dizziness and accompanied by nystagmus, gait imbalance, nausea, vomiting. Cause of vestibular neuritis is inflammatory disease such as common cold. Objective: The aim of this study was to estimate the efficacy of oriental-treatment on vestibular neuritis patient Subjects: We diagnosed one patient who had severe vertigo and gait imbalance as "dizziness retention of phlegm"(담훈) and treat orientally. Conclusion: After oriental-treatment for 15 days, walk balance was improved and no more vertigo was appeared. We could assume that in the vertigo and gait inbalance due to vestibular neuritis, the acupuncture and herbal medicine can be used.

Isolated hemorrhage in the cerebellar vermis with vertigo and body lateropulsion to the contralesional side

  • There have been several reports of patients with isolated lesions of the cerebellar vermis presenting with clinical features similar to those of peripheral vestibulopathy. We report a case of small, isolated hematoma in the cerebellar vermis in a patient who presented with vertigo, ipsilesional nystagmus, and body lateropulsion to the contralesional side without the usual signs or symptoms of cerebellar dysfunction. Although they present with symptoms that mimic those of peripheral vestibulopathy, and brain computed tomography shows no abnormality, as there may be a small, isolated hematoma or infarction in the cerebellar vermis. Thus, brain magnetic resonance imaging should be performed in elderly patients with vascular risk factors.

  • Benign Paroxysmal Positional Vertigo(BPPV) is a common disorder that peripheral vertigo in clinic, it is provoked Paroxysmal vertigo according to positional change. A man, aged 75, suffered from attacks of vertigo when get out of bed, He complained vertigo, nausea and walking trouble So he visited our clinic Based upon the case history and position-provoked vertigo with characteristic findings on the Dix-Hallpike maneuver, we diagnosed as BPPV. Through only Shang-San-Huang(上三黃) of Dong Shi Xue(董氏穴) acupuncture therapy symptoms were vanished completely. This study was showed effect that operating only Dong-Shi acupuncture therapy without maneuver therapy and the other therapies to patient can not tolerate the Dix-Hallpike Maneuver. We need additional researches with methodological improvement and many clinical cases.

안구편위와 진탕을 동반한 현훈 환자 1례 (A Case of Vertigo with Heterophoria and Nystagmus)

  • Vertigo is a symptom, not a disease. The tenn vertigo refers to the sensation of spinning or whirling that occurs as a result of a disturbance in balance. It's sometimes referred to as a hallucination of motion. The cause of vertigo is very various and generally divided into peripheral and central. A patient presented with severe vertigo, nystagmus and heteophoria. She has taken the BanHaBaekChulChunMa-tang and treated by acupuncture. After 2 days, her symptoms become disappeared and about 1 week, all symptoms except heterophoria were disapperaed. And after 2 weeks, heterophoria was disappeared. In conclusion, we diagnosed her case as vestbular neuronitis by her clinical course, but we also considered her illness as vertebrobasilar insufficiency, TIA etc.

  • Vertigo is a subtype of dizziness in which there is an illusion of movement, most commonly rotation. Acute peripheral vestibulopathy is one of the most common clinical neurologic syndromes at my age with the acute onset of vertigo, nausea, and vomiting lasting for several days and not associated with auditory or neurologic symptoms. Most affected patients gradually improve over one to two weaks, but some develop recurrent episode. We treated two patients suffered from vertigo, nausea & vomiting, nystagmus, headache with herbal medicine, acupuncture, cupping-treatment. After treatment we observed the improvement. Based on this study, it is considered that oriental medical treatment can be applied to the management of vestibular neuronitis.

  • Benign paroxysmal positional vertigo(BPPV) is the most common peripheral vestibular disorder clinically. It causes dizziness or vertigo, lightheadedness, imbalance, and nausea. by a change of position of the head with respect to gravity. BPPV is very well explained by mechanical consequences of loose debris within the inner ear and oriental medical theory of vertigo resulting from Phlegm(痰). Therefore, for treatment of BPPV, we can consider not only oriental medical therapy but also Canalith Repositioning Procedure. We report four improved cases of dizziness patient diagnosed BPPV and Dam Hun(痰暈) treated by canalith repositioning procedure and herbal medication and acupuncture.

  • Aviation medical examiners should always keep two principles in mind when evaluating patients with vertigo: Does this disease render him or her incapacitated? And does this disease recur? Meniere's disease accompanies vertigo and hearing symptoms such as hearing loss and tinnitus and is particularly important because it can render the patient incapacitated and has recurrent consequences. In this case, we intended to investigate the course of a patient who was evaluated with symptoms of dizziness and tinnitus and to review the patient's judgment result according to the principles of aeromedical evaluation.

  • Damum(痰飮) is a kind of pathologic secretion products that excessive water is accumulated in a part of body. Damhun(痰暈) is the vertigo induved by Damum(痰飮). We experienced two clinical cases treated vertigo by orietal medicine(herbal medication, acupuncture). 33-years old male and 63-years old female patients had Damhun(痰暈), vertigo by Damum(痰飮). We administered Banhabaechulcheoma-tang(Pansiabaichutienma-tang) and Yeonggyechulgam-tang(Lingguishugna-tang) to each Patient, and two Patients improved vertigo and referred symptoms. In other words, this study is that We did not limited peripheral vertigo to vestibular system, made a diagnosis of Damum(痰飮) and treated vertigo by orietal medicine(herbal medication, acupuncture) referred to many medical practitioner of many generations. We suggest that digestive sypmtom(related to Damum(痰飮), abdominal examination and stool frequency are closely associated with vertigo and could be estimated prognosis of vertigo.

A Case of Isolated Nodular Infarction Mimicking Vestibular Neuritis on the Contralateral Side

  • Differentiating central vestibulopathy from more common vestibular disorders is crucial because it often necessitates different treatment strategies, and early detection can help to minimize potential complications. Isolated nodular infarct is one of the central brain lesions that can mimic peripheral vertigo. We present a case of isolated nodular infarct that had been misdiagnosed as vestibular neuritis on the contralateral side at the initial evaluation. The patient was successfully treated with anticoagulants and antihyperlipidemic agents. Clinicians should keep in mind that some causes of central vertigo mimic peripheral vestibulopathy at the early stage.
