Antibacterial Effect on Oral Normal flora of Phytoncide from Chamaecyparis Obtusa (구강 상주균에 대한 편백 피톤치드의 항균효과)

  • The present study was performed to observe the effect of phytoncide on oral normal microflora and the inhibitory effect of the surviving resident oral bacteria on F. nucleatum. In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). The surviving salivary bacterium were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against F. nucleatum, the isolates from the phytoncide-treated saliva were cultured with F. nucleatum. The results are as follows: 1. Among the 200 surviving resident oral bacterium, 70(35.0%) bacterium inhibit the growth of F. nucleatum on blood agar plates. 2. Among the 70 bacterium which inhibit F. nucleatum, Streptococcus salivarius was 41.3%(45/109), Streptococcus sanguinis was 28%.(7/25), Streptococcus mitis was 20%(3/15), Streptococcus parasanguinis was 33.3%(3/9), Streptococcus Alactolyticus was 100%(8/8), Streptococcus vestibularis was 28.6%(2/7) and Streptococcus sp. was 50%(2/4). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit F. nucleatum. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used to prevent and cease the progress of periodontal disease, halitosis. Thus it is expected to promote oral health.

Change of Stratification of Three Dimensional Culture by Gingival Keratinocytes & Fibroblasts (치은 각화상피세포와 섬유아세포를 이용한 삼차원적 배양시 중층화 동안의 변화)

  • Epithelial-mesenchymal interaction plays a important role in cell growth and differentiation. This interaction is already well known to have an importance during the organ development as well as cell growth and differentiation. However, in vitro experimental model is not well developed to reproduce in vivo cellular microenvironment which provide a epithelial-mesenchymal interaction. Because conventional monolayer culture lacks epithelial-mensenchymal interaction, cultivated cells have an morphologic, biochemical, and functional characteristics differ from in vivo tissue. Moreover, it's condition is not able to induce cellular differention due to submerged culture condition. Therefore, the aims of this study were to develop and evaualte the in vitro experimental model that maintains epithelial-mesenchymal interaction by organotypic raft culture, and to characterize biologic properties of three-dimensionally reconstituted oral keratinocytes by histological and immunohistochemical analysis. The results were as follow; 1. Gingival keratinocytes reconstituted by three-dimensional organotypic culture revealed similar morphologic characteristics to biopsied patient specimen showing stratification, hyperkeratinosis, matutation of epithelial architecture. 2. Connective tissue structure was matured, and there is no difference during stratification period of epithelial 3-dimensional culture. 3. The longer of air-exposure culture on three-dimensionally reconstituted cells, the more epithelial maturation, increased epithelial thickness and surface keratinization 4. In reconstitued mucosa, the whole epidermis was positively stained by anti-involucrin antibody, and there is no difference according to air-exposured culture period. 5. The Hsp was expressed in the epithelial layer of three-dimensionally cultured cells, especially basal layer of epidermis. The change of Hsp expression was not significant by culture stratification. 6. Connexin 43, marker of cell-cell communication was revealed mild immunodeposition in reconstitued epithelium, and there is no significant expression change during stratification. These results suggest that three-dimensional oragnotypic co-culture of normal gingival keratinocytes with dermal equivalent consisting type I collagen and gingival fibroblasts results in similar morphologic and immunohistochemical characteristics to in vivo patient specimens. And this culture system seems to provide adequate micro-environment for in vitro tissue reconstitution. Therefore, further study will be focused to study of in vitro gingivitis model, development of novel perioodntal disease therapeutics and epithelial-mensenchymal interaction.

MOLECULAR CLONING AND SEQUENCE ANALYSIS OF THE GENE FOR THE HEMIN-BINDING PROTEIN FROM Prevotella intermedia (Prevotella intermedia에서의 Hemin 결합 단백질 유전자의 분리 및 염기서열 분석)

  • Prevotella intermedia is one of the most frequently implicated pathogens in human periodontal disease and has a requirement for hemin for growth. This study has identified a hemin-binding P. intermedia protein by expression of a P. intermedia genomic library in Escherichia coli, a bacterium which does not require or transport exogenous hemin. The genomic library of P. intermedia was constructed into plasmid pUC18, transformed into Escherichia coli strain $DH5{\alpha}$, and screened for recombinant clones using heminbinding activity by plating onto hemin-containing agar. Approximately 5,000 recombinant E. coli colonies were screened onto LB-amp-hemin agar, single clone(pHem1) was exhibited a clearly pigmented phonotype. The 2.5kb insert DNA of pHem1 was determined by restriction enzyme mapping. Southern blot analysis of BamHI, BglII, EcoRI, HindIII and PstI-digested P. intermedia DNA indicated that single copy of the gene was present in the genome. Northern blot analysis revealed that the size of transcript was approximately 1.8 kb. The cloned gene contained a single ORF, consisting of approximately 850-residue amino acids. A BLAST search of the Institute for Genomic Research genes with similar nucleotide sequence revealed no significant similarity It needs further investigation to clarify the mechanisms of heme uptake in P. intermedia.

A Study on Ages, Learnings, Monthly Incomes and Occurrence of Dental Diseases among Industrial Workers in Korea (일부 남성 근로자들의 연령, 교육 수준 및 월 평균 수입과 구강병 발생양상에 관한 연구)

  • The purpose of this study was to develop more effective dental health education program and to reduce the occurrence of dental diseases among industrial men-workers in Korea. The questionaire and dental examination were given to 782 industrial men-workers who visited Asan Medical Center for the purpose of health examinations in 2000. The obtained results were as follows; 1. According to the results of logistic regression analysis, both ages (especially under 30's: P=0.027) and monthly incomes (especially under 1,000,000 won: P=0.000) show negative relationships with the occurrence of dental caries. 2. As the results of logistic regression analysis, ages shows positive relationship and monthly incomes shows negative relationship (only under 1,000,000 won: P=0.059) with the occurrence of missing teeth. 3. According to the results of logistic regression analysis of periodontal disease, the model shows no significance (P=0.117). 4. As the results of logistic regression analysis, both monthly incomes and ages show positive relationships with the occurrence of abrasion.

A Study on Oral Health Awareness and Oral Health Care Practice of the Industrial Workers (산업체 근로자의 구강보건인식 및 구강보건관리 실천에 대한 연구)

  • This purpose of this study was to analysis the relation of awareness and practices of oral health promotion. The subjects in this study were 133 workers who worked in Seoul, Gyeonggi province, and were at the age of 20 and up. The survey was conducted from January 16 to March 31, 2012. The collected data is statistically analyzed by SPSS. For the level oral health awareness of industrial workers, the awareness of prevention of tooth decay and periodontal disease was high, but that of necessity of oral hygiene supplies other than tooth brushing was low. For the level of oral health care practice, they showed the highest awareness for the importance of cleaning a tongue when brushing teeth, and the lowest for the importance of dental care. For oral health awareness according to the general properties, workers with average income of 2~3 million one showed higher oral health awareness than others. For oral health care practice, those who brush their teeth more than 3 times a day and who have visited the dentist during the last year had higher oral health care practice than others. For oral health awareness according to the level of interest in oral health and the status of oral health of the industrial workers, those who have higher interest in oral health turned out to have higher oral health awareness. The relation between oral health awareness and oral health care practice of the industrial workers showed a positive correlation, which indicates that the higher oral health awareness is, the higher oral health care practice is.

Comparison of Quality of Life for Oral Health between Dental Hygiene Students and Non-Dental Hygiene Students in Some Areas by Using Oral Health Impact Profile-14 (구강건강영향지수-14를 이용한 일부 지역 치위생 계열과 비치위생 계열 대학생들의 구강건강 삶의 질 비교)

  • The purpose of this study is to compare the quality of life for oral health between dental hygiene students and non-dental hygiene students in some areas and to analyze the factors affecting on it by using oral health impact profile-14 (OHIP-14). This study was carried out by surveys. Subjects for study were 248 undergraduate students, subjectively recognized oral health condition by subjects were periodontal disease and bad breath, and non-dental hygiene students recognized statistically significantly higher than dental hygiene students (p<0.001). Among the detail factors of OHIP-14, factor of decreased social ability was the lowest impact factor (p<0.01). The most influential factor on OHIP-14 was gender and age, grade of school and alcohol intake were influential in order (p<0.01).

The IL-1 Gene polymorphisms in Korean Children (한국인 아동에서의 IL-1 유전자 다변성 연구)

  • The severe form of chronic periodontitis(CP) has been reported to be strongly associated with the presence of allele 2 of composite IL-1B(+3954) and IL-1A(+4845) genetic polymorphisms(genotype positive). However, other studies have reported conflicting findings. These might have resulted from differences in ethnic background and disease entities. The aim of this study was to determine the distribution of IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN(VNTR) genetic polymorphisms in children as a future Korean population. The study population consisted of 92 children from the Dept. of Pediatric Dentistry, Chonnam National University Hospital. Genomic DNA was obtained from buccal swab. The IL-1A(+4845), IL-1B(+3954), and IL-1B(-511) genes were genotyped by amplifying the polymorphic region using multiplex polymerase chain reaction(PCR), followed by restriction enzyme digestion and gel electrophoresis. IL-1 RN(VNTR) polymorphism were then evaluated by PCR amplification and fragment size analysis in agarose gel. The allele 2 frequency was 41.3%, 4.3%, 47.8%, and 9.9% for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN respectively. The frequency of genotype with allele 2 carriage for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN was 77.1%, 7.6%, 63.0%, and 15.2% respectively. The allele 2 frequency in IL-1B(+3954) was significantly higher in female than in male population(p<0.05). The negative association was shown between the presence of allele 2 in IL-1B(-511) and in IL-1B(+3954), and the carriage rate of IL-1B(+3954) allele 2 tended to lower in IL-1B(-511) allele 2(P=0.056). Only 7.3% of children carried the composite genotype of IL-1A(+4845) and IL-1B(+3954). These results suggest that the polymorphism of IL-1B(+3954) and the positive composite genotype was relatively rare in Korean population.

Detection of periodontal disease related bacteria from the implant-abutment interface in oral cavity (구강내 임플랜트-지대주 연결부에서 치주질환관련 세균의 검출)

  • Statement of the problem: Implant systems result in gaps and cavities between implant and abutment that can act as a trap for bacteria and thus possibly cause inflammatory reactions in the peri-implant soft tissues. Purpose: Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythia, Treponema denticola, and Aggregatibacter actinomycetemcomitans, related to implant-abutment interface microleakage. Material and methods: Samples were taken from 27 subjects with sterilized paper points and were transported in $1{\times}PBS$. The detection of periodontopathogens were performed by polymerase chain reaction with species-specific primers based on 16S rDNA. Results: Our data showed that the detection rate of P. gingivalis and P. intermedia in implant fixture was 59% and 82% in patients respectively. Detection rate of P. gingivalis and P. intermedia in implant crevice was 44% and 82% in patients. Detection rate of P. gingivalis and P. intermedias in tongue was 82% and 82% in patients. Conclusion: Current implant systems cannot safely prevent microbial leakage and bacterial colonization of the inner part of the implant.

SURGICAL APPROACH TO THE INFRAOCCLUDED TEETH BY USING SPACE REGAINING TREATMENT (저위교합 유구치에서의 공간확장술을 이용한 외과적 접근)

  • Infraclusion may be defined as teeth that stop their relative occlusal movement in the dental arches during or after the period of active eruption and then remain under the occlusal plane. Delayed exfoliation, malocclusion, increased susceptibility to dental caries and periodontal disease of both the neighboring teeth and retained molar, and dislocation of the successor are the consequencces of infraclusion of primary molars. Therefore, early diagnosis and appropriate treatments are necessary. The therapeutic approach of the infracluded teeth varied from preservation to extraction. The teeth with simple infraclusion without any signs of interference with occlusal and jaw development may be examined periodically with follow-up check and radiographically. However, if the infracluded tooth interferes with normal eruption of successor or shows any sign of delayed resorption, or the tipping of adjacent teeth or supraeruption of opposing teeth is expected, the teeth inflicted should be extracted and appropriate measures should be provided in order to maintain the normal development of occlusion and dentition. The adjacent teeth which have been collapsed over a infracluded deciduous teeth can disturb the arch length perimeter. In such cases, surgical approach might be necessary, although it would be difficult when teeth are severly leaned. However, an easier surgical access have been obtained by space regaining procedures, in young patients whose arch length has been shortened due to the infracluded teeth.

Full Mouth Rehabilitation with a Overlay Denture (피개의치를 이용한 지적장애인의 완전구강회복)

  • This clinical report presents the case using overlay dentures to treat a patient with cleft lip and palate and malocclusion. This patient requires vigorous treatment such as orthodontic treatment and combined orthodontic/oral surgery procedures. Overlay denture can be used as an alternative treatment option. Intraoral and radiographic examinations were done. The vertical dimension of occlusion was evaluated for proper prosthetic procedures and the surveying in the diagnostic cast was done. Considering the tissue and teeth undercut, the alterations of teeth shape were done. Final preparation and impression was performed. After the evaluation of vertical dimension and occlusion with wax-denture, the dentures were polymerized. Clinical remounting and occlusal adjustment were done on the articulator. Overlay dentures were delivered and the patient was recalled for relining. The overlay dentures satisfied patient's esthetic and functional requirements and provided a stable occlusion. Overlay dentures in this case were a reversible and relatively inexpensive treatment for this patient, however the potential possibilities of caries and periodontal disease as a result of poor oral hygiene should be prevented with periodic recall.
