소 족지 단중족증의 치료결과 - 합병증을 중심으로 - (The Outcome of the Treatment of Lesser toe Brachymetatarsia - focus on complications -)

  • Purpose: We analysis the outcome and complications of treatment of lesser toe brachymetatarsia. Materials and Methods: We analysed 28 patients 35 cases of lesser toe brachymetatarsia. Mean post operative follow up period was 2 years 8 months. All of the patients were female and mean age at operation was 21 years old. 2 cases of third metatasal bone and 33 cases of fourth metatarsal bone were operated. 8 metatarsal bones were treated using one staged lengthening with tricortical bone graft and 27 metatarsal bones were treated using callotasis with monofixator. Results: The average amount of lengthening was 13.3mm(12mm-15mm) in one staged lengthening, while 14.4mm(4mm-23mm) in callotasis. Average percentile increase was 28.9%(26%-34%) in one staged lengthening and 32%(18%-46%) in callotasis. The average healing index of callotasis was 76 days/cm (41 days/cm-166 days/cm). Satisfied outcomes in 4 cases of 8 cases (50%) after one staged lengthening and 17 cases of 27 cases (63%) after callotasis. 6 complications in 4 cases were occurred after one staged lengthening; insufficient length gain in 3 cases, fracture on the junction of graft bone and metatarsal bone in 1 case, plantar bowing deformity in 1 case and bony fusion of metatarsophalangeal joint in 1 case. 17 complications in 10 patients were occurred after callotasis ; metatarsophalangeal joint stiffness in 8 cases, metatarsophalangeal joint subluxation in 2 cases, overlengthened metatarsal bone in 2 cases, tapering of callus in 1 case, fracture of callus in 1 case, premature consolidation of callus in 1 case, osteomyelitis of metataral head in 1 case and plantar bowing deformity in 1 case. Conclusion: Although one staged lengthening and gradual lengthening using callotasis are effective treatment for lesser toe brachymetatarsia, complications not rarely occured after lengthening. Insufficient lengthening are most common complication after one staged lengthening while metatarsophalangeal joint stiffness lire most common complication after callotasis.

가토에서 하악골 신연 양에 따른 하치조신경의 조직학적 변화 (HISTOLOGIC CHANGES OF THE INFERIOR ALVEOLAR NERVE ACCORDING TO THE AMOUNT OF THE MANDIBULAR LENGTHENING IN RABBITS)

  • The purpose of this study was to evaluate the histologic change of the inferior alveolar nerve according to distraction amount following mandibular lengthening. Seven rabbits weighing about 2 kg were used. Corticotomy was performed on the mandibular body anterior to the right first premolar region and unilateral external fixation device was placed. Every effort was made to preserve the inferior alveolar nerve during the corticotomy. The rabbits were then allowed to heal for 7 days without distraction of the device. The mandible was lengthened 0.36 mm/day, 0.76 mm/day, or 1.0 mm/day. Corticotomy and lengthening of mandible were not performed in control group. After the completion of the lengthening process, a 14-day-consolidation period was allowed. After consolidation, rabbits were sacrificed, and histologic examination of the inferior alveolar nerve was performed. The results obtained were as follows : 1. In the control group, normal trifascicular pattern of inferior alveolar nerve was observed. Epineurium, perineurium, endoneurium, and axon with myelin sheath were observed in normal appearance. 2. In 0.36 mm/day distraction group, the trifascicular pattern was normally shown, and there was no destruction in epineurium, perineurium, and endoneurium. The mild changes including myelin attenuation, axoplasmic swelling and darkening were observed. 3. In 0.72 mm/day distraction group, it was possible to differentiate the epineurium from the perineurium. Two normal fascicles and one injuried fascicle were observed with a partially destructed perineurium. Most of the axons had axoplasmic swelling and darkening. 4. In 1 mm/day distraction group, it was difficult to differentiate the nerve structures such as fascicles, epineurium, perineurium, and endoneurium. The axons were severely destroyed, except few which showed decreases in size and changes in shape. Some collagen matrices were observed around the axons. These results suggest that the higher the distraction amount, the more severe the injury to the inferior alveolar nerve, fascicles, axons. Although distraction osteogenesis may be useful, the amount of distraction should be carefully selected.

단측성 외고정 기구를 이용한 개의 대퇴골 신연에 있어서 골수강내 K-강선 삽입의 효과 (Effects of Intramedullary K-wire Insertion on Femoral Lengthening in Canine with Monolateral External Fixator)

  • During long bone lengthening, there are many disadvantages including axial deviation, malalignment and re-fracture which are commonly encountered inspite of its proven abilities. To study the effects of intramedullary K-wire application on the lengthening of long bone, ten skeletally mature mongrel dogs were separated into two groups(Group I, II). Right femurs of group I(5 dogs) were fixed with only monolateral external fixator after subperiosteal osteotomy. Right femurs of group II(5 dogs) were fixed with mono lateral external fixator and intramedullary K-wire after subperiosteal osteotomy. Lengthening was started at 7 days after the surgery with the rate of 0.5 mm per day for 5 weeks and the dogs were sacrificed after 15 weeks postoperatively to examine histologic differences and evaluate bone mineral density. Radiographic examination at an interval of two weeks was done to evaluate the type of callus formed and to analyze complications including instability of external skeletal fixation and axial deviation. Bone mineral density at the lengthened area and contralateral nonlengthened area were measured using quantitative computerized tomography. Histological examination of regenerated bone was performed using Masson's trichrome stain method. The radiographs demonstrated poor callus formation, higher incidence of axial deviation and screw loosening in the group I compared to the group II. The bone mineral density at the lengthened area in the group II was higher than that of the group I(P<0.05). Histological examination showed that the new bone trabeculae in the group II were greater than that of the group I. In conclusion, the combination of monolateral external fixator and intramedullary K-wire can prevent pin loosening, axial deviation and reduce healing period in dogs.

가토에서 하악골 신연 6주후 신연양에 따른 두개하악관절의 조직병리학적 변화 (HISTOPATHOLOGIC CHANGES OF THE CRANIOMANDIBULAR JOINT ACCORDING TO THE AMOUNT OF DISTRACTION AFTER 6 WEEKS OF DISTRACTION OSTEOGENESIS IN RABBITS)

  • The purpose of this study is to observe histopathologic changes in the bilateral craniomandibular joints after allowing 6 weeks of consolidation by varying the amount of distraction in rabbit mandible. Eight rabbits weighing about 2 to 3 kg were used. After corticotomy was performed on the left mandibular body between the first premolar and the second premolar region, a unilateral fixation device was placed. Then, a 7-day period was allowed without distraction of the device. The mandible was lengthened 0.5 mm/day. Corticotomy and lengthening of mandible were not performed in control group. After the completion of the lengthening process, a 6-week-consolidation period was allowed. Then, the rabbits were sacrificed, and histologic examination of the craniomandibular joints was performed. Proliferative changes were observed in the craniomandibular joints in all groups. With the increasing amount of distraction, hypertrophy of the cartilage layer became more severe, bone formed was dense and enchondral ossification was clearly shown in subchondral bone. Hypertrophy of the cartilage layer was also seen in the non-distracted side as the distracted side in the experimental group. These results indicate that when physical force is applied constantly to joints, the proliferation of articular cartilage and bone formation are present. When more than 6 weeks of consolidation period is allowed at the time of performing distraction for more than 5 mm, articular changes, especially, in the contralateral side should also be noted.

Soil structure interaction effects on structural parameters for stiffness degrading systems built on soft soil sites

  • In this study, strength reduction factors and inelastic displacement ratios are investigated for SDOF systems with period range of 0.1-3.0 s considering soil structure interaction for earthquake motions recorded on soft soil. The effect of stiffness degradation on strength reduction factors and inelastic displacement ratios is investigated. The modified-Clough model is used to represent structures that exhibit significant stiffness degradation when subjected to reverse cyclic loading and the elastoplastic model is used to represent non-degrading structures. The effect of negative strain - hardening on the inelastic displacement and strength of structures is also investigated. Soil structure interacting systems are modeled and analyzed with effective period, effective damping and effective ductility values differing from fixed-base case. For inelastic time history analyses, Newmark method for step by step time integration was adapted in an in-house computer program. New equations are proposed for strength reduction factor and inelastic displacement ratio of interacting system as a function of structural period($\tilde{T}$, T) ductility (${\mu}$) and period lengthening ratio ($\tilde{T}$/T).

Earthquake motions recorded on different site conditions such as rock, stiff soil, soft soil and very soft soil are used in analyses. Soil structure interacting systems are modeled with effective period, effective damping and effective ductility values differing from fixed-base case. For inelastic time history analyses, Newmark method for step by step time integration was adapted in an in-house computer program. Results are compared with those calculated for fixed-base case. A new equation is proposed for inelastic displacement ratio of interacting system ($\tilde{C}_R$) as a function of structural period of interacting system ($\tilde{T}$), strength reduction factor (R) and period lengthening ratio ($\tilde{T}/T$). The proposed equation for $\tilde{C}_R$ which takes the soil-structure interaction into account should be useful in estimating the inelastic deformation of existing structures with known lateral strength.

It is concluded that soil structure interaction reduces the strength reduction factors for soft soils, therefore, using the fixed-base strength reduction factors for interacting systems lead to non-conservative design forces.

Evaluation of local and global ductility relationships for seismic assessment of regular masonry-infilled reinforced concrete frames using a coefficient-based method

  • Soft storey failure mechanism is a common collapse mode for masonry-infilled (MI) reinforced concrete (RC) buildings subjected to severe earthquakes. Simple analytical equations correlating global with local ductility demands are derived from pushover (PO) analyses for seismic assessments of regular MI RC frames, considering the critical interstorey drift ratio, number of storeys and lateral loading configurations. The reliability of the equations is investigated using incremental dynamic analyses for MI RC frames of up to 7 storeys. Using the analytical ductility relationship and a coefficient-based method (CBM), the response spectral accelerations and period shift factors of low-rise MI RC frames are computed. The results are verified through published shake table test results. In general applications, the analytical ductility relationships thus derived can be used to bypass the onerous PO analysis while accurately predicting the local ductility demands for seismic assessment of regular MI RC frames.

  • 1. 파종기에 따른 묘대일수를 연장함에 따라 출수가 늦어졌으며, 품종별 출수기의 변이는 조생종은 묘대일수, 중만생종은 파종기의 영향을 받으며, 또한 동일 이앙기내에서 묘대일수 연장은 영양생장기간의 증대로 출수를 단축시켰다. 2. 파종기와 묘대일수 연장시 출수일수와 영양생장 기간은 짧아졌으며 기본영양 생장성이 작은 품종일수록 묘대일수 연장시 불시출수의 위험이 컸다. 3. 간장, 등숙비율은 묘대일수, 수수와 일수당입수는 파종기의 조만에 영향을 받으며 이들은 서로 상관을 보였다. 4. 등숙온도와 수량과도 상관을 보였으며 등숙기를 $23^{\circ}C$ 정도에서 출수개화하도록 조기이앙을 요한다. 5. 안전수량을 얻기 위해서는 조생종 6월 10일(40~50일묘), 중만생종 5월 30일(30~60일묘)이전 이앙을 안전작기로 본다.

Inelastic displacement ratios for evaluation of stiffness degrading structures with soil structure interaction built on soft soil sites

For this purpose, SDOF systems for period range of 0.1-3.0 s with different hysteretic behaviors are considered for a number of 18 earthquake motions recorded on soft soil. The effect of stiffness degradation on inelastic displacement ratios is investigated. The Modified Clough model is used to represent structures that exhibit significant stiffness degradation when subjected to reverse cyclic loading and the elastoplastic model is used to represent non-degrading structures. Soil structure interaction analyses are conducted by means of equivalent fixed base model effective period, effective damping and effective ductility values differing from fixed-base case. For inelastic time history analyses, Newmark method for step by step time integration was adapted in an in-house computer program. A new equation is proposed for inelastic displacement ratio of system with SSI with elastoplastic or degrading behavior as a function of structural period ($\tilde{T}$), strength reduction factor (R) and period lengthening ratio ($\tilde{T}$/T). The proposed equation for $\tilde{C}_R$ which takes the soil-structure interaction into account should be useful in estimating the inelastic deformation of existing structures with known lateral strength.
