만성폐쇄성폐질환 환자의 폐기능 향상을 위한 호흡중재기법의 유병기간별 비교 (Comparison of Effectiveness of Breathing Intervention Program for Improvement of Pulmonary Functions according to Prevalence Period in Patients with COPD)

  • Purpose: The purpose of this study was to evaluate the effectiveness of pulmonary rehabilitation on lung function and fatigue in persons with chronic obstructive pulmonary disease (COPD). Methods: Thirty one persons with COPD participated in this study. Four groups were allocated as follows: experimental group 1 (under 10 years of the post-disease period), control group 1 (under 10 years of the post-disease period), experimental group 2 (over 10 years of the post-disease period), and control group 2 (over 10 years of the post-disease period). Results: Forced expired volume in one second (FEV1) % pred and lactic acid showed improvement in experimental group 1 and experimental group 2 after training. Control group 1 and control group 2 did not show improvement of FEV1% pred and lactic acid after training. However, FEV1% pred and lactic acid in experimental group 1 were not significantly different, compared with the experimental group. Conclusion: Results of our study suggest that implementation of a pulmonary rehabilitation program resulted in improvement of lung function and fatigue in persons with COPD not related to the post-disease period.

  • In this paper, scheduling problem is dealt for the minimization of due date penalty for the customer order. Multiproduct batch processes have been dealt with for their suitability for high value added low volume products. Their scheduling problems take minimization of process operation for objective function, which is not enough to meet the customer satisfaction and the process efficiency simultaneously because of increasing requirement of fast adaptation for rapid changing market condition. So new target function has been suggested by other researches to meet two goals. Penalty function minimization is one of them. To present more precisely production scheduling, we develop new scheduling model with penalty function of earliness and tardiness We can find many real cases that penalty parameters are divergent by the difference between the completion time of operation and due date. That is to say, the penalty parameter values for the product change by the customer demand condition. If the order charges different value for due date, we can solve it with the due date period. The period means the time scope where penalty parameter value is 0. If we make use of the due date period, the optimal sequence of our model is not always same with that of fixed due date point. And if every product have due date period, due date of them are overlapped which needs optimization for the maximum profit and minimum penalty. Due date period extension can be enlarged to makespan minimization if every product has the same abundant due date period and same penalty parameter. We solve this new scheduling model by simulated annealing method. We also develop the program, which can calculate the optimal sequence and display the Gantt chart showing the unit progress and time allocation only with processing data.

The Effect of Dynamic Neuromuscular Stabilization (DNS) on the Respiratory Function of Subjects with Forward Head Posture (FHP)

  • PURPOSE: The purpose of this study was to apply dynamic neuromuscular stabilization (DNS) to subjects with forward head posture (FHP) and to compare its effects on respiratory function as against the conventional neck stabilization exercise and neck stretching and extensor strengthening exercises. METHODS: The whole-body posture measurement system was used to measure the degree of FHP, and a spirometer and a respiratory gas analyzer were used to measure the respiratory function. After the intervention was completed, the changes over time were analyzed in the DNS group, the neck stabilization exercise group, and the neck stretching and extensor strengthening exercise group. The inter-group difference in the changes was also analyzed. A repeated ANOVA was performed to compare the respiratory function according to the period between the three groups, and the least significant difference (LSD) method was used for the post hoc test. RESULTS: After the 6-week exercise period, respiratory functions, such as forced vital capacity (FVC), forced expiratory volume for 1 second (FEV1), forced expiratory volume for 1 sec/forced vital capacity (FEV1/FVC), maximum oxygen intake (VO2max), and the volume of expired gas (VE), significantly improved according to the period (p < .05), but no inter-group differences were found. CONCLUSION: DNS is an effective training method, and can be applied along with neck stabilization exercise and neck stretching and extensor strengthening exercises, which are widely used in clinical practice, to people with FHP who cannot directly perform neck exercises to improve their respiratory function.

열자극을 병행한 가상현실훈련이 만성 뇌졸중 환자의 위팔 능동가동범위와 기능에 미치는 영향 (Effects of Virtual Reality Training Combined with Thermal Stimulation on Upper Extremity AROM and Function in Chronic Stroke Patients)

  • Background: This study was performed to evaluate the effects of Thermal stimulation combined virtual reality training (TV) on improvement of upper extremity AROM and function in patient with chronic stroke. Design: Two groups pre-post randomized controlled design. Methods: A single-blind, randomized controlled trial was conducted with 30 chronic stroke patients. They were randomly allocated two groups; the TV group (n=15) and Virtual Reality training group (VT) (n=15). The TV group received treatment for 30 min - 15 min of Thermal stimulation, and 15 min of VR training. The VT group received 15 min of VR training. Each group performed 30 minutes a day 3 times a week for 8 weeks. The primary outcome upper extremity AROM and function were measured by a active range of motion test, Manual Function Test (MFT) and Jebsen-Taylor hand function Test (JTT). The upper extremity active range of motion was evaluated using a digital dual inclinometer. MFT and JTT were used to evaluate the hand function. The measurement were performed before and after the 8 weeks intervention period. Results: Both groups demonstrated significant improvement of outcome in muscle strength and upper extremity function during intervention period. TV group revealed significant differences in AROM and upper extremity function as compared to the VT groups (p<.05). Our results showed that TV was more effective on upper extremity AROM and function in patients with chronic stroke. Conclusion: Both groups demonstrated significant improvement of outcome in muscle strength and upper extremity function during intervention period. Estimating the Transmittable Prevalence of Infectious Diseases Using a Back-Calculation Approach

  • A new method to calculate the transmittable prevalence of an epidemic disease is proposed based on a back-calculation formula. We calculated the probabilities of reactivation and of parasitemia as well as transmittable prevalence (the number of persons with parasitemia in the incubation period) of malaria in South Korea using incidence of 12 years(2001-2012). For this computation, a new probability function of transmittable condition is obtained. The probability of reactivation is estimated by the least squares method for the back-calculated longterm incubation period. The probability of parasitemia is calculated by a convolution of the survival function of the short-term incubation function and the probability of reactivation. Transmittable prevalence is computed by a convolution of the infected numbers and the probabilities of transmission. Confidence intervals are calculated using the parametric bootstrap method. The method proposed is applicable to other epidemic diseases in other countries where incidence and a long incubation period are available. We found the estimated transmittable prevalence in South Korea was concentrated in the summer with 276 cases on a peak at the $31^{st}$ week and with about a 60% reduction in the peak from the naive prevalence. The statistics of transmittable prevalence can be used for malaria prevention programs and to select blood transfusion donors.

Effect of Korea Red Ginseng on the Symptoms and Hemodynamics in Healthy Elders

  • Clinical study on hemodynamics of healthy elders before and after tread mill. Using doubleblind, placebo-controlled study design. Seventy-five 50-70 years old volunteers without organic disease were divided into two groups, Ginseng group and control group. Each subject was received 3 g Korea Red Ginseng (KRG) capsules or placebo per day for 4 weeks. Before and after administration 4 weeks, the symptoms were asked and hemodynamics parameter such as pump function, systolic function, preload and afterload were recorded before and after tread mill 1,5, 10 minutes by the method of thoracic impedance cardiograph. The result showed that Ginseng could improve quality of life, had obvious effect of increasing PEP (pre-ejection period), PEP/LVET (pre-ejection period/left ventricular ejection period), PCWP (wedged pressure pulmonary capillary). The change rate of HR (heart rate), SV (stroke volume) in KRG group were much lowered, while EF (ejection fraction), LVET (left ventricular ejection period), LVEDP (left ventricular end diastolic pressure) was recovered much quickly. The circulation showed Ginseng could improve the quality of life though its promoting circulation function which are increasing both the systolic function and the preload.

  • This note is concerned with some properties of fixed points and periodic points. First, we have constructed a generalized continuous function to give a proof for the fact that, as the reverse of the Sharkovsky theorem[16], for a given positive integer n, there exists a continuous function with a period-n point but no period-m points wherem is a predecessor of n in the Sharkovsky ordering. Also we show that the composition of two transcendental meromorphic functions, one of which has at least three poles, has infinitely many fixed points.

Fault detection of logic circuit by use of M-sequence correlation method

  • In this paper, the estimation of the structure of a logic circuit under test is made from the observation of the input-output correlation function by use of M-sequence, from which we can estimate whether or not any fault exist in the logic circuit. Especially, investigation was made in case of the 2_stage logic circuit. We checked theoretically the sequence of correlation function, and we have shown that the correlation function is a function of period of M-sequence only, land the appearing number of correlation function in a period is a constant value depending on the logic circuit only. And by computer simulations we have shown that the structure of the circuit under test can be estimated from the observation of sequence of correlation function.

  • Replacement policy of a degradation system is investigated by incorporating the loss function. Loss function is defined by the deviation of the value of quality characteristic from its target value, which determines the loss cost. Cost function is comprised of the inspection cost, replacement cost and loss cost. Two cost minimization problems are formulated : 1)determination of an optimal inspection period given the state for the replacement and 2)determination of an optimal state for replacement under fixed inspection period. Simulation analysis is performed to observe the variation of total cost with respect to the variation of the parameters of loss function and inspection cost, respectively As a result, parameters of loss function are seen to be the most sensitive to the total cost. On the contrary, inspection cost is observed to be insensitive. This study can be applied to the replacement policy of a degradation system which has to produce the quality critical product.

발달성 엉덩관절 이형성증으로 인한 엉덩관절 전치환술 후 로봇치료가 환자의 기능에 미치는 영향: 사례연구 (The effect of robotic therapy on patient function after total hip arthroplasty due to developmental dysplasia of the hip: a case study)

  • Background: The advantages of robotic therapy have recently been attempted several times in the rehabilitation of total hip arthroplasty (THA) patients. Therefore, this study also aims to report a case of how robot therapy affects the function of THA patients due to developmental dysplasia of the hip (DDH). Design: Case Study. Method: This study used the A-B-A' design. Period A is before robotic therapy, period B is robotic therapy, and period A' is after robotic therapy. The subjects performed physical therapy and occupational therapy for five days each during the baseline period A and A'. In intervention period B, robotic therapy was performed for five days along with the baseline intervention. This study was conducted for a total of fifteen days. The subjects' sit to stand (STS), timed up and go (TUG), and 10 metre walk (10MW) were evaluated. Result:: STS and TUG were significantly improved in periods B and A' compared to period A (p<0.05), and STS was significantly improved in period A' compared to period B (p<0.05). 10MW showed no significant improvement in periods B and A' compared to period A. Conclusions: This study confirmed that robot therapy was an effective intervention in improving the function of women in their 30s who underwent THA due to DDH. In the future, a study comparing the control group should be performed.
