Acute Achilles Tendon Rupture : - Isokinetic plantarflexion torque evaluation after Krackow suture technique - (급성 아킬레스건 파열 - Krackow 봉합술 후 등속성 족저 굴곡력의 분석 -)

  • Purpose: We try to evaluate the functional outcomes of the ankle with isokinetic plantaflexion torque for acute achilles tendon rupture cases those treated by primary repair with the Krackow suture technique and early rehabilization. Materials and Method: The authors studied retrospectively, 15 patients of acute achilles tendon rupture treated and followed over six months, from July 1997 to May 2001. There were 12 men and 3 women, and mean age was 39.6year. The repair method of ruptured tendon was single or double Krackow suture technique. One week(5days-2weeks) after operation, early ROM with ankle-foot orthosis was started. We used Arner-Lindholm Scale for the clinical evaluation and analyzed patient's satisfaction and subjective strength deficit. We analyzed the dorsiflexion peak torque and plantarflexion peak torque of the ankle statistically with strength test with Cybex dynamometer. Result: In clinical results, we had 11 excellent cases(73%) and 3 good cases(20%). In patients satisfaction degree, 11 excellent cases(66.6%), 3 good cases(20%) and 2 fair cases(13.4%). And in strength deficit, 3 none deficit (20%), 9 minimal deficit(60%). We evaluated the isokinetic plantar flexion torque in 3months or 6months after operation. After 3 months, isokinetic test showed the mean functional deficits, 32% and 25% at $30^{\circ}$ and $120^{\circ}/sec$, in 7 cases(46.6%) of 15 cases respectively. After 6 months, the mean deficits were in 21%, 24% at 30. and $120^{\circ}/sec$, respectively. At 3 and 6 months' follow up, absolute value of isokinetic test showed increase of 25. 31bs to 421bs and 19.61bs to 271bs at $30^{\circ}$ and $120^{\circ}/sec$, respectively. Conclusion: We had good result for acute achilles tendon rupture treated by Krakow suture technique and early range of motion exercise of the ankle. After 6 months, strength deficit was 21% in all of cases but were able to return pre-injured state. This study shows Krakow suture technique was recommended method for primary repair and early rehabilization of achilles tendon ruptue.

Assessment of Isokinetic Muscle Function in Elite Canadian College Paddlers (대학 엘리트 카나디언 선수들의 어깨·허리·무릎 관절의 등속성 근기능 평가 및 진단)

  • The aim of this study is to evaluate university canadian paddlers' iso-kinetic muscle function and provide training advices. Eight university paddlers with a 8.5 years career participated in this study. Inbody 720, biodex system III, biodex stabilizer system and THP 2 are used to investigate body component, muscle function(peak torque, average power and work fatigue), agility and dynamic balance. Iso-kinetic muscle function around shoulder, thorax and knee are measured. And then bilateral and unilateral muscle imbalance are calculated at each joint. Results of this study are as follows; University canadian paddlers' BMI average is 24.90 $kg/m^2$ and percent body fat is 12.23%. Reaction time average was 324 msec and balance score at each leg are below 2.0. A3, A4, A5, A6, A8 are needed to train shoulder muscle strength and power. A1, A2, A7, A8 are needed to strengthen shoulder muscle endurance. A2, A5, A8 are needed to train thorax muscle strength and power. A1, A8 are needed to strengthen thorax muscle endurance. Most of paddlers' reciprocal muscle balance ratio on thorax and knee are out of normal range. A2, A3, A4, A8 are needed to train abdominal strength and A3, A5, A6, A7, A8 are needed to train hamstring strength.

Effects of Ankle Joint Mobilization With Movement on Lower Extremity Muscle Strength and Spatiotemporal Gait Parameters in Chronic Hemiplegic Patients (만성 편마비 환자의 발목에 적용한 능동운동을 동반한 관절가동술이 하지근력과 보행의 시공간적 변수에 미치는 영향)

  • The purpose of this study was to determine the effect of ankle joint mobilization with movement (MWM) on the range of motion (ROM) in the ankle, on the muscle strength of lower extremities, and on spatiotemporal gait parameters in chronic hemiplegic patients. Fifteen subjects with chronic stroke were divided into two groups: an experimental group (8 subjects) and a control group (7 subjects). Both groups attended two or three sessions of physical therapy each week. The experimental group also attended additional MWM training sessions three times a week for five weeks. For both groups, the ROM of the ankle, the muscle strength of the lower extremities, and the spatiotemporal gait parameters in paretic limbs were evaluated before and after the training period. The results showed that the experimental group experienced more significant increases than did the control group in terms of passive (6.10%) and active (21.96%) ROM of the ankle, gait velocity (12.96%), and peak torque, of the knee flexor (81.39%), the knee extensor (24.88%), and the ankle plantar flexor (41.75%)(p<.05). These results suggest that MWM training in patients with chronic stroke may be beneficial in increasing ROM in the ankle, muscle strength in the lower extremities, and gait speed.

Impact of Concurrent Inspiratory Muscle Training and Tape on Inspiratory Muscle Strength, Endurance and Pulmonary Function (들숨근 훈련과 테이핑 동시적용이 호흡의 근력, 지구력, 폐기능 향상에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effect of a kinesio tape on inspiratory muscle training(IMT) to improve muscle strength, endurance and pulmonary function. Methods: Healthy 20 males were divided into IMT group (control group) and IMT with tape group (experimental group). The same IMT program was applied to both groups using the Respifit S for four weeks, three times a week, a total 12 times. To exprimental group, kinesio tape was applied on the inspiratory agonist diaphragm and the accessory inspiratory muscle scalene, sternocleidomastoid, pectoralis minor. The inspiratory pulmonary muscle strength was measured by the maximal inspiratory pressure (PI max) and minute volume (MV) using the Respifit S and the pulmonary function were measured peak expiratory flow (PEF), forced vital capacity (FVC), forced expiratory volume in 1sec (FEV1), FEV1/FVC using the Spirometer and compared before and after. Results: Results showed that the PI max in the two groups increased significantly and experimental group increased more effectively than that of control group. However, only MV showed a significant increase in experimental group but was not significantly different between the two groups. PEF and FEV1/FVC are significantly increased in both groups, but they did not make much difference between two groups, and the FVC for the two groups did not increase significantly. FEV1 increased significantly only with control group, but did not make a difference with experimental group. Conclusion: These result show that the PI max value for experimental group increased significantly than that of control group. Therefore kinesio tape maximizes inspiratory muscle exercise effect on muscle strength improvement. However, because of the short experimental period and difficulty in subject control, increase values of the others did not show a significant difference. In other words, kinesio tape did not show maximizing the inspiratory muscle exercise effect to improve endurance and pulmonary function.

Response Modification Coefficient Using Natural Period (고유진동주기를 이용한 응답수정계수)

  • In some current procedures, ground motions from different sources have been scaled by their peak ground accelerations and combined to obtain smoothed response spectra for specific regions. As consideration of the inelastic deformation capacity of structure, inelastic deformations are permitted under seismic ground excitation in all codes. In the ATC(Applied Technology Council) and UBC(Uniform Building Code), the inelastic design spectrum is obtained by reducing the elastic design spectrum by a factor that is independent of structural period. In this study, the average of nonlinear response spectra calculated from a sample of 20 records for each event are constructed to obtain the smoothed response spectra. These response spectra are used to examine the effects of structural strength factors such as the yield strength ratio and damping value. Through the regression analysis of nonlinear response of system for a given damping value and yield strength ratio, the required yield strength for seismic design can be estimated for a certain earthquake event. And a response modification coefficient depending on the natural period for current seismic design specifications are proposed.

An Experimental Study on Local Stability of Eco-block (생태축조블록의 국부적 안정성에 관한 실험적 연구)

  • In this study, computation method of tensile load which develops in tie-bar of reinforced earth, connection strength between tie-bar and eco-block and shear strength of the interface between two eco-blocks were verified by experiments. According to the test results of connection strength test, peak tensile load of D13 deformed bar were close to allowable tensile load of it for situation of infill with soil. Connection strengths of D10 and D13 deformed bars were greater than the allowable tensile load of those respectively for situation of infill with concrete. According to the test results of shear strength of the interface between two eco-blocks, shear resistance parameters, ${\alpha}_u$ and ��${\lambda}_u$ were evaluated as 1.7kN/m and 2$27.6^{\circ}$ respectively.

  • Recently, granular soils having a large particle size are frequently used as a filling material in the construction of foundation, harbor, dam, and so on. The shear behavior of this granular soil plays a key role in the stability of structures. For example, soil particle crushing occurring at the interface between structure and soil and/or within soil mass can cause the disturbance of ground characteristics and consequently induce an issues in respect of stability of structures. In order to investigate the shear behavior according to an existence and nonexistence of particle crushing, numerical analyses were conducted by using the DEM(Discrete Element Method)-based software program PFC(Particle Flow Code). Using the crushing model and non-crushing model which were created in this study, numerical analyses of ring shear test were conducted and their results were analyzed and compared. In general, landslide and slope stability are accompanied by a large displacement and consequently not only a peak strength but also a residual strength are very important in the analysis of landslide and slope stability. However the direct shear test which has been commonly used in the determination of shear strength parameters has a limitation on displacement therefore the residual strength parameters can not be obtained. The characteristics of residual shear behavior were investigated through the numerical analyses in this study.

Effects of the Abdominal Drawing-in Maneuver and the Abdominal Expansion Maneuver on Grip Strength, Balance and Pulmonary Function in Stroke Patients

  • Purpose: The purpose was to determine whether the application of the abdominal drawing-in maneuver (ADIM) and abdominal expansion maneuver (AEM) to stroke patients would affects their trunk stability, balance, pulmonary function, and grip strength. Methods: The subjects were 36 stroke patients who were randomly and equally assigned to an ADIM group (n=12), an AEM group (n=12), and a control group (n=12). The intervention was applied to each group three times per week, 30 minutes each time, for four weeks. Outcome measures were grip strength, modified functional reach test (mFRT) and pulmonary function. Pulmonary function were measured force expiratory volume at one second (FEV1), forced vital capacity (FVC), FEV1/FVC and peak expiratory flow (PEF) values. Results: The results of the three groups showed statistically significant improvements in grip strength. The AEM group showed significantly greater differences in grip strength than either the ADIM group or the control group. In the anterior mFRT, the ADIM group showed significantly improvements than the control group. The ADIM and AEM groups were showed statistically significant greater improvements in PEF between the baseline and post-intervention and the post-analysis revealed that the AEM group showed significantly greater improvements than the control group. Conclusion: The results of this study indicate that the ADIM and AEM were effective in improving the PEF of pulmonary function. The ADIM was more effective than AEM in trunk stabilization.

Anisotropy in Strength and Deformation Properties of a Variety of Sands by Plane Strain Compression Tests(I) Strength Anisotropy (평면변형률 압축시험에 의한 각종 모래의 강도.변형특성의 이방성(I) -강도 이방성-)

  • Anisotropy in strength and deformation characteristics of isotropically consolidated sande prepared by pluviating through air was studied by plane strain compression tests. Seven types of sand of the world-wide origins were tested, which have been extensively used for research purposes. The strains for direction of bmazimum principal stress and direction of minimum principal strews were measured continuously from $10^{-6}\; to 10^{-2}$. The following results were obtained for all sands. The behaviour at strains leas than about 0.001% was elastic and isotropic regardless of the angle $\delta\; of\; the\;\sigma$ direction relative to the bedding plane. However, the sands became gradually more anisotropic as the strain increased to the extent exceeding the elastic limit. The peak strength was noticeably anisotropic with a similar trend. Thus, the angle of internal friction $\phi\; decreased \;as\;\delta$ decreased from $90^{\circ}$, and the ratio of the smallest to largest values of was between 0.82 and 0.90. The l has a minimum at $\delta=0^{\circ}~30^{\circ}$ depending on the hypes of sand. The residual strength became isotropic again.

The Effect of Poling Strength on Temperature Dependence of Resonance Frequency of PZT Ceramics Near the Morphotropic Phase Boundary (분극전계가 모포트로픽 상경계 부근의 PZT 세라믹스의 공진주파수의 온도의존성에 미치는 영향)

  • Poling is an important process in fabricating PZT ceramic devices such as filters and resonators and activates piezoelectricity to sintered PZT ceramics. Tolerance of the operating frequency of these devices is tightly required in applications. And a factor to attribute the tolerance is the temperature dependence of the resonance frequency of PZT ceramics. In this paper the relationship of poling strength and temperature dependence of resonance frequency of PZT specimens was studied. The $Pb(Zr_{0.53}Ti_{0.47})O_3$ ceramics were fabricated and the poling strengths were chosen to be 0.5, 1.5, 2.5 and 3.5 [kV/mm]. The dielectric constant of the specimen poled in poling strength 0.5 [kV/mm] was less than that of unpoled specimen and the specimen poled in higher electric field had the higher dielectric constant. (002) peak in X-ray diffraction patterns of the specimens increased as poling strength increased. And the change of resonance frequency of the specimens according to the variation of temperature was measured. Resonance frequency of all specimens increased as the temperature increased. The specimen poled in higher electric field had the smaller positive temperature coefficient of resonance frequency. The effect that temperature coefficient of resonance frequency becomes smaller is obtained when Zr mole in PZT composition equation increase. Controlling the poling strength is believed to be a method to adjust the temperature stability of resonance frequency of the PZT ceramic devices.
