A study on the architectural guideline of the patient room in the geriatric medical center (노인보건의료센터의 다인병실 건축계획에 관한 연구)

  • Nowadays, Korean society is quickly transforming into an aging society. There are lots of problems resulting from old aged society. First of all, in the case of medical service, it is an urgent task to find out what is the most proper care for the elderly in order to reduce medical expenses. It can be said that the geriatric hospital has to be designed to give adequate medical service to the elderly. However, it is hard to find out what the differences is between the general hospital and the geriatric hospital. Especially, the plan of the patient room has to reflect thoughtful consideration relating to characteristics of the elderly due to the fact that there is a close connection between the patient room and residence. Therefore this study aims to suggest the guideline and the plan of the patient room for the elderly.

Mid term experience with CarboMedics Medical Valve (CarboMedics 기계판막의 임상경험)

  • The CarboMedics valve is a bileaflet prosthesis with excellent hemodynamic characteristics, but the long term surgical experience with this valve, its durability and its biocompatibility are unknown. During a 5 year period from october 1988 to July 1993, 748 prostheses [402 mitral, 261 aortic, 58 tricuspid, 27 pulmonic] were inserted in 552 patients [mean age 40.2 years]. The operative mortality was 6.6% [37/560, 13.2% in age group below 15 years and 5.7% above 15 years]. and the main causes of death were complex congenital malformation and left ventricular failure. Follow up was totaled 1182 patient- years and mean follow up was 28.3 months/patient. No structural failure has been observed. Hemorrhage was the most frequent valve related complication[1.78% / Patient-year]. Embolism occurred at a rate of 0.93% / Patient-year. There were 5 cases of valve thrombosis [0.42% / Patient-year, two fatal]. There occurred 11 late deaths[6 valve related] and 42 valve related complications. Actuarial survival at 5 years is 97.18 0.94% and actuarial complication free survival at 5 years is 89.07 1.54%. In summary, the CarboMedics valve stands for a durable valve substitute, with low valve related complications.

A Study on Gender Difference in Assessing Emotional Design Characteristics of General Hospital's Wards (사용자 성별에 따른 종합병원 병동부 감성디자인 특성 평가에 관한 연구)

  • Purpose: This research was for understanding the attitudinal difference by gender towards emotional design through questionnaire survey with female and male inpatients on environmental characteristics of wards in general hospitals. Methods: The survey was conducted by questioning inpatients at two general hospitals on the importance rating on emotional design elements of patient rooms, lounges, and hallways. Eighty questionnaires were returned and used for data analyses through SPSS windows version 15.0 statistic package program. Results: 1) In general, female patients considered emotional design of wards as more important than male patients and the most outstanding difference was indicated for patient rooms among patient rooms, lounges, and hallways. For patient rooms the comfortability index was rated as the most important to both female and male inpatient groups, and for lounges and hallways the safety index was evaluated as the most important to both gender groups. 2)For lounges, while male patients rated 'prevention of infection' important among safety relating items, women considered 'accident prevention' more important. It is inferred that female patients have more safety needs and anxiety about physical injury or accidents than male ones do. Implications: It is considered that there need to be further succeeding in-depth studies, e.g. research interviews with inpatient;s family members or other caregivers as well as patients themselves.

The Study on the Development of Fasteners for Senior Patient Wear (패스너를 활용한 고령 환자복 디자인개발에 관한 연구)

  • The purpose of this study is the improvement of the senior patients' life quality by providing aesthetic and emotional stability by creating and providing a newly designed patient wear for them. Empirical research and analysis were done for the research. Survey from advanced research and clothing design for the senior citizens in Japan and German are analyzed. The result of this research extracted characteristics, which applied to the production of a severe senior patient wear. And the following characteristics emerged: affordability, comfort, aesthetics, and ease of putting on the clothes. To meet these conditions by the details of the clothing design, using fasteners like zippers and Velcro is useful. It is able to solve the problem of affordability making the vertically integrated overalls to be able being separated by zippers in order to reduce the volume of laundry. It was able to overcome the discomfort due to contamination of the feces through the use of the fasteners even though the importance of choosing the comfort material related closely to the comfort. Using material mixed with multiple colors, plaid or bright pink, instead of using neutral colors fulfilled the aesthetic requirement. In Particular, utilizing detachable function fasteners contributed great services.

Patient compliance with supportive periodontal therapy (유지 치주치료에 대한 환자 협조도 분석)

  • Purpose: The purpose of this study was to evaluate the compliance to maintenance schedules recommended supportive periodontal therapy(SPT) and to determine differences in the characteristics of compliant and non-compliant patients. Methods: 414 patients commencing SPT after active periodontal treatment from 2003 to 2005 were included in this study. Based on their compliance with the suggested maintenance schedule, patients were classified as compliant and non-compliant groups. Also patients classified by gender, age, degree of alveolar bone loss and treatment rendered. The association between compliance and patient characteristics was assessed by odds ratio in logistic regression analyses. Results: Only 47% of the initial patient was found to be compliant at the end of August 2008 and 20.8% patients were lost in the first year of SPT. There were significant differences between compliant and non-compliant regard to age, degree of alveolar bone loss and treatment rendered. Conclusions: In conclusion, compliance with SPT generally poor and patients who were older, treated surgical therapy and with mild alveolar bone loss are more compliant to SPT.

A Study on Burden and Adaptation of Caregiver of Industrial Accident Patient (산업재해환자 가족원의 부담감과 적응에 관한 연구)

  • Purpose: This study was designed to examine the basic data for an effective measure for the caregiver's burden and adaptation of industrial accident patient. Method: The data were computerized by the SPSS/pc for Window. Results: 1) The burden felt by the caregiver of the patients who were in hospital was more than 'average', and objective burden was higher than subjective one. 2) The adaptation felt by the caregiver was more than 'average'. 3) The objective burden of the caregiver based on the general characteristics varied in the relationship between the caregiver and the patient, the level of education of the caregiver, the age of the caregiver, nursing period, nursing time per day, and so forth. The subjective burden was significantly related to the relationship between the caregiver and the patient, the level of education of the caregiver, the age of the caregiver, nursing period, nursing time per day, and the religion. 4) The adaptation of the caregiver based on the general characteristics varied in monthly income and nursing period. 5) The relationship between the burden and adaptation of the caregiver were reverse. Conclusion: Even though the caregiver have high burden, they positively adaptation it.

Status analysis of patients on islands and seas of Jeollanam-do (전라남도 섬과 해상에서 발생한 환자의 현황분석)

  • Purpose: By analyzing the health status of emergency patients on islands in the Jeollanam-do province, this study aims to provide basic data for proposed future medical emergency services on the islands. This information includes monthly and hourly statistics on patients' characteristics, patient frequency, an analysis of first aid procedures, response times, and time spent at hospitals. Methods: The study analyzed the monthly and hourly frequency of emergency patients, first aid status and procedures, general patient characteristics, accident types, patient cognitive status and agility, marine police response times, and frequency and percentile of hospital dispatch times. Results: 1Q and #q show the highest patient turnover rate in one year. 50.5% of daily patients are admitted between 4 PM and 12 PM. The hospital dispatch time is within a one- to two-hour window for 54.0% of all patients. Conclusion: The emergency medical service system for the Jeollanam-do province islands and seas is available to all Korean residents, citizens, and visitors. It is necessary to implement an emergency medical system for the relevant organizations.

Nurses' Knowledge and Attitude about Incidence Reporting according to Nursing Organizational Culture and Organizational Characteristics (간호조직특성 및 조직문화에 따른 간호사의 사건보고에 대한 지식과 태도 - 일 대학병원을 중심으로 -)

  • Purpose: This study was designed to describe the nurses' knowledge and attitude about incidence reporting according to nursing organizational culture and organizational characteristics. Methods: The subjects of this study were 783 clinical nurses who were in A university hospital in Gyeonggi-Do. The data were collected from May, 20, 2009 to June, 2, 2009. The collected data were analyzed through descriptive methods, Pearson correlation coefficient, multiple regression in SPSS win(12.0). Results: Nurses' knowledge and attitude about incidence reporting were positively correlated with innovation oriented culture, relation oriented culture, and culture of patient safety. And among characteristics of nursing organization, communication, decision making, centralization were positively correlated with nurses' knowledge and attitude about incidence reporting. But the most correlated factor with nurses' knowledge and attitude about incidence reporting was culture of patient safety. Conclusions: The findings of this study suggest that to encourage reporting incidence, there must be a organizational approach, such as creating a culture of patient safety, active participating decision making, and communication.

Would Wernicke's Aphasic Speech Vary with Auditory Comprehension Abilities and/or Lesion Loci?

  • Speech characteristics of Wernicke's aphasia are characterized by such errors as empty speech, jargon, paraphasia, filler and others. However, not all the errors can be observed in each patient presumably due to diverse auditory comprehension (AC) abilities and/or lesion loci. The purpose of this study was, thus, to clarify the speech characteristics of Wernicke's aphasics according to the AC levels (i.e., better vs. worse) and lesion loci (i.e., Wernicke's area, WA vs. non-Wernicke's area, NWA). The authors divided 21 Wernicke's aphasic patients into four patient groups based on their AC levels and the lesion loci. The results showed that the four groups differed only in CIU (Correct Information Unit) rate. The patient groups with a better AC ability had higher CIU rates than the groups with a worse AC regardless of the lesion loci (e.g., WA or NWA). Therefore, it was concluded that CIU rate, the differentiating speech variable was most likely related to the AC levels, but not to lesion loci.

A Study on the Stress of the Family Caregivers for Patients with Stroke (뇌졸중환자 가족간병원의 스트레스에 관한 연구)

  • Purpose: The purpose of this study was to provide the basic data to decrease the stress of the family caregivers. Methods: The questionnaire was administrated to the family caregivers of 156 patients with stroke who consented to be interviewed. Among 156 caregivers, we analyzed 120 caregivers. We evaluated on the stress of the family caregivers. Results: First, the general characteristics of patient with CVA were high at 82.55% in male, 55.83% in infarction, 53.33% in the left hemiplegia, 47.50% in sixties, 80.83% in 1 recurrence rate, 27.50% in 1-2 years of disease period. Second, the general characteristics of family caregivers were high in above sixties, female in gender, existence in religion, married in marital status, high school in educational level, below 1 million-won in monthly income, spouse in relationship with patient, below 1 years in total caring period. Third, it showed that stress of family caregivers above 3.0 score has 8 items. Forth, the patient's characteristic that have the influence on the stress was significant in the period of disease(p<0.05). Finally, the characteristics of family caregivers that have the influence on the stress were the monthly income, caring period(p<0.05). Conclusion: These findings indicated that the stress of the family caregivers was correlated with the time in hospital, the income and nursing period.
