유아 거대 긴장성 기관지 낭종 -수술치험 1례 보고- (Large Tension Bronchogenic Cyst in an Infant -A Case Report-)

  • A routine chest radiograph in a 10 months old male infant revealed a giant air filled cystic lesion of the left hemithorax under tension. At thoracotomy, a large left lower lobe intraparenchymal cyst required lobectomy and the pathological finding were consistent with a bronchogenic cyst. This kind of parenchymal bronchogenic cyst is uncommon lesion, and we have performed successful surgical resection. After this lobectomy, this patient was complete recovered and postoperative course was uneventful.

  • The author has experienced 8 cases of periapical lesion which were very closed to adjacent root tip. To avoid damage on neighboring nerve and blood supply polyvinyl tube was inserted through buccal window leaving a little pathological changes in involved area. The results are as follows: 1. The purpose to maintain adjacent tooth vitality was achieved by means of intubation in the lesion. 2. Swelling and pain after operation was rather lesser than routine apical surgery, because of drainage. 3. The total treatment period seems longer than that of routine apical amputation.

Primary Osteolytic Intraosseous Atypical Meningioma with Soft Tissue and Dural Invasion : Report of a Case and Review of Literatures

  • Primary intraosseous meningioma is a rare tumor, and atypical pathologic components both osteolytic lesion and dura and soft tissue invasion is extremely rare. A 65-year-old woman presented with a 5-month history of a soft mass on the right frontal area. MR imaging revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the right frontal bone, and CT showed a destructive skull lesion. The mass was adhered tightly to the scalp and dura mater, and it extended to some part of the outer and inner dural layers without brain invasion. The extradural mass and soft tissue mass were totally removed simultaneously and we reconstructed the calvarial defect with artificial bone material. The pathological study revealed an atypical meningioma as World Health Organization grade II. Six months after the operation, brain MR imaging showed that not found recurrence in both cranial and spinal lesion. Here, we report a case of primary osteolytic intraosseous atypical meningioma with soft tissue and dural invasion.

A Solitary Skull Lesion of Syphilitic Osteomyelitis

  • We experienced a rare case of solitary syphilitic osteomyelitis of the skull without any other clinical signs or symptoms of syphilis. A 20-year-old man was referred due to intermittent headache and mild tenderness at the right parietal area of the skull with a palpable coin-sized lesion of softened cortical bone. On radiological studies, the lesion was a radiolucent well enhanced mass (17 mm in diameter). The erythrocyte sedimentation rate (52 mm/h) and C-reactive protein (2.24 mg/dL) were elevated on admission. Serum venereal disease research laboratory (VDRL) and Treponema pallidum haemagglutination assay (TPHA) tests were positive. There were no clinical signs or symptoms of syphilis. After treatment with benzathine penicillin, we removed the lesion and performed cranioplasty. The pathologic finding of the skull lesion was fibrous proliferation with lymphoplasmocytic infiltration forming an osteolytic lesion. In addition, a spirochete was identified using the Warthin-starry stain. The polymerase chain reaction study showed a positive band for Treponema pallidum. Solitary osteomyelitis of the skull can be the initial presenting pathological lesion of syphilis.

자돈 농포성 건선양 피부염 증례 보고 (Porcine juvenile pustular psoriasiform dermatitis in Korea)

  • Porcine juvenile pustular psoriasiform dermatitis (PJPPD) is a disease of young pigs and characterized by nonpruritic round eruption of skin. The cause of this disease is yet undetermined but is presumed to be genetic predisposition. There may be few opportunities for veterinarian to detect this disease compared with actual situation in field because these lesions resolve spontaneously in two months. The authors detected spontaneous PJPPD case and performed clinical and pathological studies on three pigs from one farm. The specific skin lesions were observed in the forty-day old pigs of mixed breed, which were produced by the sows received semen from the same boar, restrictively. However, there was no skin lesion of pigs in suckling or fattening periods. Grossly, lesions were commonly found on the ventral abdominal part as a papule and were spreaded to the skin of whole body. With the spreading of lesions centrifugally, skin was showed as a umbilicated plaques or mosaic pattern with a few pustules or crusts. Microscopically, the most prominent lesion was the psoriasiform hyperplasia with acanthosis, down growth of rete ridges, exocytosis of eosinophils and neutrophils, ballooning degeneration of superficial epidermis, and koilocytic degeneration of keratinocytes. Additionally, there were moderate dermal edema and severe mixed cellular infiltration, especially eosinophils. No infectious agent which can cause the skin lesion, was detected or cultured, and no lesion caused by infectious agents was also observed, pathologically. With pathological results of this study, it is supposed that pathogenesis or severity of PJPPD may be related to the infiltration of eosinophil or hypersensitivity.

The Nevus Lipomatosus Superficialis of Face: A Case Report and Literature Review

  • Nevus lipomatosus superficialis (NLS) is a hamartoma of adipose tissue, rarely reported in the past 100 years. We treated one case, and we conducted a systematic review of the literature. A 41-year-old man presented with a cutaneous multinodular lesion in the posterior region near the right auricle. The lesion was excised and examined histopathologically. To review the literature, we searched PubMed with the keyword "NLS." The search was limited to articles written in English and whose full text was available. We analyzed the following data: year of report, nation of corresponding author, sex of patient, age at onset, duration of disease, location of lesion, type of lesion, associated symptoms, pathological findings, and treatment. Of 158 relevant articles in PubMed, 112 fulfilled our inclusion criteria; these referred to a total of 149 cases (cases with insufficient clinical information were excluded). In rare cases, the diagnosis of NLS was confirmed when the lesion coexisted with sebaceous trichofolliculoma and Demodex infestation. Clinical awareness for NLS has increased recently. NLS is an indolent and asymptomatic benign neoplasm that may exhibit malignant behavior in terms of huge lesion size and specific anatomical location. Early detection and curative treatment should be promoted.

주리개념(腠理槪念)에 근거한 피부과 질병의 진단지표 기초연구 (Researches on Diagnostic Indices of Skin Diseases Based on Concepts of Interstices)

  • In order to establish the diagnostic indices of skin diseases, some physiological and pathological concepts of interstices(腠理) were researched based on , and western dermatology with etymological analysis. As physiological indices for diagnosis of skin diseases, measurement of epidermis and dermis using ultrasonogram in the zhongwan and dachui's location. And for grading looseness and fineness of interstices with 3 unsound groups, measuring numbers and sizes of sweat pores in each point's 1 cm diameter circular area using comparative method and palpation assessment. Another index is superficial temperature. As pathological indices for diagnosis, validating volumes of dead skin cells and grading degrees of atrophy and degeneration of skin lesion. And as supplementary measures, absorptive degrees of cosmetics on face should be recorded according to 3 grades. These diagnostic indices can contribute to establishment of standard pattern identification and prescription of skin diseases through converting anatomical cognizance into classical concepts of interstices objectively.

뇌심근염 바이러스의 실험적 감염자돈에 대한 병리학적 소견과 바이러스 항원의 면역조직화학적 검출 (Immunohistochemical detection of viral antigen and pathological lesion in piglets experimentally infected with encephalomyocarditis virus)

  • Three or 7day old piglets were infected experimentally with different encephalomyocarditis virus isolates to detect the viral antigen by the immunoperoxidase technique and to observe strain difference in their pathogenecity in newborn pigs by comparing clinical signs and pathologic lesions. Clinical signs of the infected pigs were different depending on the virus strain, pig age and infection route. Encephalomyocarditis virus(EMCV) NVSL-PR isolate was more pathogenic than MN-25 and MN-30 isolate. Three day old piglets showed more severe illness than 7 day old piglets. Predominant clinical signs were sudden death without noticeable clinical signs and dyspnea manifested as heavy abdominal breathing. Contact-infection from infected piglets to controls was observed in the oro-nasally infected group but not the intramuscular group. Common necropsy findings of dead piglets in both age groups infected with MN-25 and NVSL-PR were accumulation of excessive fluid in the body cavities and mild to diffuse necrotic areas observed in the hearts and occasionally in the livers. Microscopically, myocarditis with inflammatory cell infiltration, necrosis of the myocardial muscle fibers and occasional mineralization were observed along with interstitial pneumonia and centrolobular necrosis in the liver. Using an immunoperoxidase technique, viral antigen was detected in myocardial muscle fibers of piglets infected with EMCV.

Case report of a solitary fibrofolliculoma on the alar rim

  • Fibrofolliculoma is a benign tumor characterized by a smooth, dome-shaped papule of size 2-4 mm. Most fibrofolliculomas occur as multiple lesions, and very rarely, they are solitary. Herein, we report a case of solitary fibrofolliculoma found in the alar rim, without the typical characteristics of a fibrofolliculoma. A 42-year-old man visited the hospital with a protruding lesion that had occurred 1 year previously. A mass of size 5×7 mm was observed on the left alar rim. The tumor was dome-shaped and palpable. The patient did not have any similar lesions elsewhere. No family member was known to have such a lesion. An incisional biopsy was performed before surgery, and pathological examination revealed hyperkeratosis and dyskeratosis; however, an accurate diagnosis was not made. Complete resection was planned for the mass on the alar rim. The resected mass was subjected to permanent biopsy, and the pathological examination results led to the diagnosis of fibrofolliculoma. Therefore, when diagnosing a dome-shaped mass in the alar rim, despite the suspicion of a very rare disease, it is necessary to suspect fibrofolliculoma and consider the process from diagnostic examination to treatment.

낭종을 함유한 전 종격동 종괴 (Anterior Mediastinal Mass with Cavitation)

  • Radiological criteria such as smooth, sharply defined interface, obtuse angles between lesion and lung and intimate effect on mediastinal contents were usually used to differentiate mediastinal lesion from parenchymal lung lesion. Recently, we experienced a 60-year-old female presenting with anterior mediastinal mass with cavitation. Grossly it was proven to be peripheral lung cancer adjacent to mediastinum and microscopically it was squamous cell carcinoma. The gross pathological findings of surgical specimen were very well correlated with radiological findings. The unique location such as lung periphery and attachment to mediastinum led us to misdiagnosis of anterior mediastinal mass such as germ-cell tumor and neurogenic tumor.
