Contralateral recurrence of necrotizing sialometaplasia of the hard palate after five months: a case report

  • Necrotizing sialometaplasia usually heals within 4 to 10 weeks with conservative treatment, and rarely recurs. When necrotizing sialometaplasia is present on the hard palate it may occur unilaterally or bilaterally. In this case, necrotizing ulceration occurred on the left hard palate of a 36-year-old woman after root canal treatment of the upper left first premolar under local anesthesia. After only saline irrigation the defect of the lesion completely healed and filled with soft tissue. After 5 months, however, a similar focal necrosis was found on the contralateral hard palate without any dental treatment having been performed on that side and progressed in similar fashion as the former lesion. We conducted an incisional biopsy and obtained a final pathological diagnosis for the palatal mass of necrotizing sialometaplasia. At the 3-year follow-up, the patient's oral mucosa of the hard palate was normal, without any signs and symptoms of the condition. We report a case of a second occurrence of necrotizing sialometaplasia on the contralateral side from the first, with a time lapse between the first and second occurrence.

A Case of Lung Carcinoma with Rhabdoid Phenotype Mimicking an Aspergilloma in Patient with Recurrent Hemoptysis

  • Malignant rhabdoid tumor was first discovered in the kidney, and rhabdoid tumor of the lung was first reported in 1995. These were included as the variants of large-cell carcinoma, according to the 1999 World Health Organization classification of lung tumors. The rhabdoid tumor of the lung exhibits aggressive biological behavior and has a poor prognosis, and only a few reports of this tumor exist. We report a case of lung carcinoma with a rhabdoid phenotype, initially misdiagnosed as an aspergilloma, in a 48-year-old man who presented with recurrent hemoptysis. The chest computed tomography scans showed a huge consolidative lesion with an air crescent sign in the left upper lung and no contrast-enhancing lesion. An aspergilloma was diagnosed by the radiologist. However, after surgical excision and pathological examination, rhabdoid carcinoma was diagnosed. A surgical resection helps to make it possible to pathologically distinguish a malignancy from an aspergilloma.

  • Purpose: To evaluate clinical characteristics of soft-tissue masses around the foot Materials and Methods: Sixty seven cases of soft tissue masses around the foot, excised at Kang Dong Sacred Heart hospital from Jan. 1987 to Oct. 2000, were included in the study. The age and sex of the patient, location and size of the lesion, history of trauma, presence or absence of pain and neurological symptoms as wellas final pathological diagnosis were obtained from retrospective analysis. Results: For type of lesion, all lesions were benign. Ganglion was 23 cases, epidermal inclusion cyst was 10 cases, lipoma was 8 cases, hemangioma was 5 cases and fibroma was 4 cases. For age, 63 percent of the patients were either between the ages of twenty and thirty nine or between the ages of fifty and fifty nine. For sex, the male-to-female ratio was 1 to 1.4. For zones of the foot, 18 cases were in Zone 1, 17 in Zone 4, 12 in Zone 2, 12 in Zone 3 and 8 in Zone 5. For clinical finding, 18 cases had pain. Conclusion: Ganglion was the most common lesions, followed in order of frequency by epidermal inclusion cyst, lipoma, hemangioma and fibroma. Lesions occurred frequently at Edward and Michael Zone 1, 4 and pain was the most common symptom.

Toxicity Assessment of Photorhabdus temperata Isolated from Heterorhabditis megidis Gwangju Strain (Nematoda: Heterorhabditidae) in Fish and Rat (Heterorhabditis megidis Gwangju Strain (Nematoda: Heterorhabditidae)으로부터 분리한 Photorhabdus temperata의 어류 및 쥐 독성평가)

  • Photorhabdus is a bacterial symbiont of entomopathogenic nematodes of the genus Heterorhabditis in the family Heterorhabditidae. Photorhabdus is known to have nematicidal activity in addition to insecticidal activity. P. temperata isolated from Korean indigenous H. megidis Gwangju strain also produced high control efficacy against root-knot nematode Meloidogyne incognita and root-lesion nematode Pratylenchus penetrans. P. temperata has drawn interest as a potential bionematicide for the control of root-knot nematodes thereby. For the registration as an organic agricultural material, the toxicity of P. temperata was assessed by the acute toxicity test in carp (Cyprinus carpio) and acute oral and dermal toxicity tests in Sprague-Dawley rat (Rattus norvegicus) in compliance with the guidelines of the Rural Development Administration (RDA). In the acute toxicity test in fish, neither lethality nor abnormal responses of carp were observed. Body length and weight of carp and changes in DO concentrations and pH values were not significantly different between the treated group and the untreated control. In the acute oral and dermal toxicity tests, clinical signs, abnormal behavior, mortality, and pathological findings were not observed in all the experimental rats. The weight increment of all rats was normal. Acute toxicity results of P. temperata in fish and rats belonged to categories III, IV, and IV of RDA, respectively. Toxicity results of the present study indicated that P. temperata could be a safe and promising bionematicide against root-knot nematodes and root lesion nematode.

The Prognostic Value of 18F-Fluorodeoxyglucose PET/CT in the Initial Assessment of Primary Tracheal Malignant Tumor: A Retrospective Study

  • Objective: To investigate the potential value of 18F-fluorodeoxyglucose (FDG) PET/CT in predicting the survival of patients with primary tracheal malignant tumors. Materials and Methods: An analysis of FDG PET/CT findings in 37 primary tracheal malignant tumor patients with a median follow-up period of 43.2 months (range, 10.8-143.2 months) was performed. Cox proportional hazards regression analyses were used to assess the associations between quantitative 18F-FDG PET/CT parameters, other clinic-pathological factors, and overall survival (OS). A risk prognosis model was established according to the independent prognostic factors identified on multivariate analysis. A survival curve determined by the Kaplan-Meier method was used to assess whether the prognosis prediction model could effectively stratify patients with different risks factors. Results: The median survival time of the 37 patients with tracheal tumors was 38.0 months, with a 95% confidence interval of 10.8 to 65.2 months. The 3-year, 5-year and 10-year survival rate were 54.1%, 43.2%, and 16.2%, respectively. The metabolic tumor volume (MTV), total lesion glycolysis (TLG), maximum standardized uptake value, age, pathological type, extension categories, and lymph node stage were included in multivariate analyses. Multivariate analysis showed MTV (p = 0.011), TLG (p = 0.020), pathological type (p = 0.037), and extension categories (p = 0.038) were independent prognostic factors for OS. Additionally, assessment of the survival curve using the Kaplan-Meier method showed that our prognosis prediction model can effectively stratify patients with different risks factors (p < 0.001). Conclusion: This study shows that 18F-FDG PET/CT can predict the survival of patients with primary tracheal malignant tumors. Patients with an MTV > 5.19, a TLG > 16.94 on PET/CT scans, squamous cell carcinoma, and non-E1 were more likely to have a reduced OS.

Congenital Fibroepithelial Polyp Presenting as a Cutaneous Horn on the Nasal Tip: A Case Report (코에서 선천성 섬유상피폴립이 피부뿔로 발현한 환자 증례보고)

  • Cutaneous horn is the clinical entity, which is circumscribed, conical, markedly hyperkeratotic lesion in which the height of the keratotic mass amounts to at least half of its largest diameter. It may be associated with many different pathological lesions. It is a relatively rare and a kind of epidermal tumor that generally appears as a conical projection. Here, we report rare case of congenital cutaneous horn. A 39-month-old female Korean patient presented at our clinic with a mass at the tip of her nose present since birth. Under general anesthesia, cutaneous horn of nasal tip was completely excised without any complications. The operation site was small enough to perform a primary closure, without any nasal deformity. Histopathologically, it was reported as a fibroepithelial polyps. After operation, there is no evidence of recurrence at 16 months of follow-up.

  • Desmoplastic fibroma of bone is an extremely rare tumor that was first described by Jaffe in 1958. It histologically resembles the desmoid tumor of soft tissue. It is known as locally aggressive tumor but we experienced definitely benign and resembling simple bone cyst radiographically. We report a case of desmoplastic fibroma of bone and it should be included in the differential diagnosis list of any lytic bone lesion. The radiograph, MR imaging features, radiological and pathological differential diagnosis of the case are described, and literatures are reviewed.

A Case of Pulmonary Sequestration Diagnosed by Doppler Sonography (도플러초음파로 진단한 폐분리증 1례)

  • Pulmonary Sequestration is a rare congenital malformation during embryouic development which results in a cystic mass of nonfunctioning lung tissue. A diagnosis of a pulmonary sequestration has traditionally relied upon identifying the pathological arterial vessels by arteriography, computed tomography, and magnetic resonance imaging. We reported a case of pulmonary sequestration diagnosed by Doppler sonography and subsequent CT angiography. A 21-year-old-woman admitted to hospital for an investigation of recurrent pneumonia with left lower chest pain. A doppler ultrasound sonography showed an aberrant blood supply from the descending thoracic aorta to the left lower chest lesion. The pulmonary sequestration was confirmed by the subsequent CT angiograph.

A Study on the Safety of Korean Domestic Fermented Meju (한국산 재래식 발효메주의 안전성에 관한 연구)

  • For this experiment frist the fermented Soybean Meju was prepared by domestic ordinary method of Korean and concentrated Meju suspensoidal extract was made for toxicological test sample on mice and rat. Even the oral dose of 10g extract per kg of mice had not any acute toxicity. So the median lethal dose(LOD50) is lilely to be much greatr or not to be. In comparison of the average weight gain rate for 4 weeks the control and test groups of 0.308g Ex/rat kg and 9.24g Ex/rat kg daily were the much the same as about 19% but 3.08g Ex/rat kg group waas exceptionally 2.7% higher than control For this period all test groups did not show any abnormality in appearance and anatomic findings. In serum GPT GOT alkaline phosphatase and blood urea nitrogen value there were no difference significantly between control and the test groups of 0.308g/rat kg and 3.08g/rat kg but 9.24g/rat kg group was some what higher than control. Especially in cholesterol alkaline phosphatase and blood urea nitrogen value 9.24g/rat kg group or 28days had 100mg/이 37.5u 32mg/이 respectively which were increased significantly. The pathological findings of all test groups especially 9.24g/kg group did not showed any characterized sign of acute or subacute liver damage or intestine lesion. So it is supposed that there is not any tocixants originated from some molds in fermented Korean domestic Meju which is prepared traditionally in winter.

Atypical β-Catenin Activated Child Hepatocellular Tumor

  • Hepatocellular adenomas are a benign, focal, hepatic neoplasm that have been divided into four subtypes according to the genetic and pathological features. The ${\beta}$-catenin activated subtype accounts for 10-15% of all hepatocellular adenomas and specific magnetic resonance imaging features have been defined for different hepatocellular adenomas subtypes. The current study aimed to report the magnetic resonance imaging features of a well differentiated hepatocellular carcinoma that developed on the basis of ${\beta}$-catenin activated hepatocellular adenomas in a child. In this case, atypical diffuse steatosis was determined in the lesion. In the literature, diffuse steatosis, which is defined as a feature of the hepatocyte nuclear factor-$1{\alpha}$-inactivated hepatocellular adenomas subtype, has not been previously reported in any ${\beta}$-catenin activated hepatocellular adenomas case. Interlacing magnetic resonance imaging findings between subtypes show that there are still many mysteries about this topic and larger studies are warranted.
