Design the Expert Systems for the Stroke Early Diagnosis based in Web Environment (Web환경기반의 뇌졸중 초기진단 전문가시스템 설계)

  • In this study, we designed the expert system for the diagnosis of stroke. The causes of stroke in central nervous systems are very diverse, so a doctor who treats the patients with stroke must have the expert knowledge for the quick and correct diagnosis and for the adequate medical management. But the primary physician who engaged in the primary care of the patient with stroke does not have the export knowledge for the stroke. So, we need to develop the expert system for assisting the diagnosis of stroke. Also the diagnosis system can be used as simulator for the medical students who study the neurology. In this study, we developed the diagnosis expert system that offer a pathological name provided by artificial neural networks. And we designed the inference engine and interfaces. The artificial neural network is a system that provide a possible diagnosis of stroke. We implemented the system using Windows2000 Server, IIS5.0 and ASP.

Discrimination of Pathological Speech Using Hidden Markov Models

  • Diagnosis of pathological voice is one of the important issues in biomedical applications of speech technology. This study focuses on the discrimination of voice disorder using HMM (Hidden Markov Model) for automatic detection between normal voice and vocal fold disorder voice. This is a non-intrusive, non-expensive and fully automated method using only a speech sample of the subject. Speech data from normal people and patients were collected. Mel-frequency filter cepstral coefficients (MFCCs) were modeled by HMM classifier. Different states (3 states, 5 states and 7 states), 3 mixtures and left to right HMMs were formed. This method gives an accuracy of 93.8% for train data and 91.7% for test data in the discrimination of normal and vocal fold disorder voice for sustained /a/.

Impact of Intraoperative Macroscopic Diagnosis of Serosal Invasion in Pathological Subserosal (pT3) Gastric Cancer

  • Purpose: The macroscopic diagnosis of tumor invasion through the serosa during surgery is not always distinct in patients with gastric cancer. The prognostic impact of the difference between macroscopic findings and pathological diagnosis of serosal invasion is not fully elucidated and needs to be re-evaluated. Materials and Methods: A total of 370 patients with locally advanced pT2 to pT4a gastric cancer who underwent curative surgery were enrolled in this study. Among them, 155 patients with pT3 were divided into three groups according to the intraoperative macroscopic diagnosis of serosal invasion, as follows: serosa exposure (SE)(-) (no invasion, 72 patients), SE(${\pm}$) (ambiguous, 47 patients), and SE(+) (definite invasion, 36 patients), and the clinicopathological features, surgical outcomes, and disease-free survival (DFS) were analyzed. Results: A comparison of the 5-year DFS between pT3_SE(-) and pT2 groups and between pT3_SE(+) and pT4a groups revealed that the differences were not statistically significant. In addition, in a subgroup analysis of pT3 patients, the 5-year DFS was 75.1% in SE(-), 68.5% in SE(${\pm}$), and 39.4% in SE(+) patients (P<0.05). In a multivariate analysis to evaluate risk factors for tumor recurrence, macroscopic diagnosis (hazard ratio [HR], SE(-) : SE(${\pm}$) : SE(+)=1 : 1.01 : 2.45, P=0.019) and lymph node metastasis (HR, N0 : N1 : N2 : N3=1 : 1.45 : 2.20 : 9.82, P<0.001) were independent risk factors for recurrence. Conclusions: Gross inspection of serosal invasion by the surgeon had a strong impact on tumor recurrence in gastric cancer patients. Consequently, the gross appearance of serosal invasion should be considered as a factor for predicting patients' prognosis.

  • Objective: To explore the clinical manifestations and imaging characteristics of gliomatosis cerebri to raise the awareness and improve its diagnostic accuracy for patients. Materials and Methods: Clinical data, imaging characteristics and pathological examination of 12 patients with GC from Jan., 2008 to Jan., 2012 were analyzed retrospectively. Results: Patients with GC were clinically manifested with headache, vomiting, repeated seizures, fatigue and unstable walking, most of whom had more than 2 lesions involving in parietal lobe, followed by temporal lobe, frontal lobe, periventricular white matter and corpus callosum. Magnetic resonance imaging (MRI) showed diffuse distribution, T1-weighted images (T1WI) with equal and low signals and T2-weighted images (T2WI) with bilateral symmetrical high diffuse signals. There was no reinforcement by enhancement scanning and signals were different in diffusion-weighted images (DWI). The higher the tumor staging, the stronger the signals. Pathological examination showed neuroastrocytoma in which tumor tissues were manifested by infiltrative growth in blood vessels and around neurons. Conclusions: In clinical diagnosis of GC, much attention should be paid to the diffuse distribution of imaging characteristics, incomplete matching between clinical and imaging characteristics and confirmation by combining with histopathological examination.

Leiomyoma of The Esophagus - Report of two cases - (식도 평활근종;2례 보고)

  • Intramural leiomyoma is the most common benign esophageal tumor, but it is rare compared with carcinoma. Although the most common symptom is dysphagia, this tumor may not cause symptoms before attaining large size. Radiographic and endoscopic findings allow an accurate diagnosis to be made before operation in most patients. The treatment of choice is enucleation without mucosal encroachment. Recently, we experienced two cases of esophageal leiomyoma on the mid portion of esophagus and enucleation of tumors were done by blunt dissection with caution. In one case, postoperative chylothorax was complicated, but cured with conservative treatment. The tissue diagnosis was confirmed with pathological finding.

  • There are various diseases of the gallbladder and cystic duct, and imaging diagnosis is challenging for the rare among them. However, some rare diseases show characteristic imaging findings or patient history; therefore, familiarity with the imaging presentation of rare diseases may improve diagnostic accuracy and patient management. The purpose of this article is to describe the imaging findings of rare diseases of the gallbladder and cystic duct and identify their pathological correlations with these diseases.

Clinical Usefulness of PET-MRI in Lymph Node Metastasis Evaluation of Head and Neck Cancer (두경부암 림프절 전이 평가에서 PET-MRI의 임상적 유용성)

  • Purpose: As PET-MRI which has excellent soft tissue contrast is developed as integration system, many researches about clinical application are being conducted by comparing with existing display equipments. Because PET-MRI is actively used for head and neck cancer diagnosis in our hospital, lymph node metastasis before the patient's surgery was diagnosed and clinical usefulness of head and neck cancer PET-MRI scan was evaluated using pathological opinions and idiopathy surrounding tissue metastasis evaluation method. Materials and Methods: Targeting 100 head and neck cancer patients in SNUH from January to August in 2013. $^{18}F-FDG$ (5.18 MBq/kg) was intravenous injected and after 60 min of rest, torso (body TIM coil, Vibe-Dixon) and dedication (head-neck TIM coil, UTE, Dotarem injection) scans were conducted using $Bio-graph^{TM}$ mMR 3T (SIEMENS, Munich). Data were reorganized using iterative reconstruction and lymph node metastasis was read with Syngo.Via workstation. Subsequently, pathological observations and diagnosis before-and-after surgery were examined with integrated medical information system (EMR, best-care) in SNUH. Patient's diagnostic information was entered in each category of $2{\times}2$ decision matrix and was classified into true positive (TP), true negative (TN), false positive (FP) and false negative (FN). Based on these classified test results, sensitivity, specificity, accuracy, false negative and false positive rate were calculated. Results: In PET-MRI scan results of head and neck cancer patients, positive and negative cases of lymph node metastasis were 49 and 51 cases respectively and positive and negative lymph node metastasis through before-and-after surgery pathological results were 46 and 54 cases respectively. In both tests, TP which received positive lymph node metastasis were analyzed as 34 cases, FP which received positive lymph node metastasis in PET-MRI scan but received negative lymph node metastasis in pathological test were 4 cases, FN which received negative lymph node metastasis but received positive lymph node metastasis in pathological test was 1 case, and TN which received negative lymph node metastasis in both two tests were 50 cases. Based on these data, sensitivity in PET-MRI scan of head and neck cancer patient was identified to be 97.8%, specificity was 92.5%, accuracy was 95%, FN rate was 2.1% and FP rate was 7.00% respectively. Conclusion: PET-MRI which can apply the acquired functional information using high tissue contrast and various sequences was considered to be useful in determining the weapons before-and-after surgery in head and neck cancer diagnosis or in the evaluation of recurrence and remote detection of metastasis and uncertain idiopathy cervical lymph node metastasis. Additionally, clinical usefulness of PET-MRI through pathological test and integrated diagnosis and follow-up scan was considered to be sufficient as a standard diagnosis scan of head and neck cancer, and additional researches about the development of optimum MR sequence and clinical application are required.

  • It is impossible to overestimate the importance of the medical examination. The medical examination and treatment method is composed of Mang(inspection)-Moon(listening)-Moon(anammesis & question)-Jeol(pulse feeling, precussion etc.). Among these 4 methods, the Diagnosis of the Abdominal Region, which is one of the JeolJin, is regarded as the most important method along with pulse feeling. The Diagnosis of the Abdominal Region, which includes the examination of the symptoms and their changes in stomach area to understand the pathological progress of the JangFu, Meridian and Qi-Blood, has been highly emphasized in Western and Eastern Medical Science. External trouble, for instance a cold, can be detected by examining pulse, Internal trouble, for instance indigestion, by Diagnosis of the Abdominal Region. Though the Diagnosis of the Abdominal Region was the important part of the JeolJin, it was often devaluated. The Diagnosis of the Abdominal Region will also be composed of 4 kinds of method on Mang-Moon-Moon-Jeol. We thought that the first of the Abdominal Region Diagnosis is a Mangjin(inspection). So we present the new viewpoint of the abdomen of a diagnosis through emphasizing the importance of Mangjin(inspection).

Clinical and Pathological Findings of Renal Biopsy in Children: Outcomes from a Single Center Over 27 Years

  • Purpose: To classify the results of renal biopsy in pediatric patients and to compare pathological findings with clinical features. Methods: This study included data of 318 children who underwent renal biopsy at our hospital between December 1987 and November 2014. Biopsy specimens were examined histopathologically using light, immunofluorescence, and electron microscopy. Results: Asymptomatic urinary abnormalities was the most common clinical diagnosis (35.9%), followed by nephrotic syndrome (29.3%), and acute glomerulonephritis (18.0%). Glomerular disease was identified in 98.1% of the renal biopsy specimens. The most common primary cause of glomerulonephritis was IgA nephropathy, with gross hematuria in 61.9% of the patients, hypertension in 14.2%, proteinuria >1.0 gm/24-hr in 33.3%, and impaired renal function in 3.6% patients. Conclusion: The most common clinical diagnosis was asymptomatic urinary abnormalities, with primary glomerular disease being the most common renal biopsy finding, and IgA nephropathy the most common histopathological lesion. This study provides a 27-year overview of pediatric renal disease at our center and underlines the importance of renal biopsy for accurate diagnosis and proper management.

The Difference of Digestion, Sweat, Stool, Urination, Drinking, Coldness and Hotness Characteristics according to Sasang Constitutional Exterior and Interior Diagnosis (체질별 표리에 따른 소화, 땀, 대변, 소변, 음수, 한열 특성 차이)

  • 1. Objective : It is well-known that even if some people are born with same constitution, their symptoms can be different according to Exterior and Interior diagnosis. This study aimed to suggest different clinical symptoms according to Exterior and Interior group in individual Sasang constitution. 2. Methods : We collected 706 physiological and pathological data of subjects from August 2009 to July 2011 using case report form of Questionnaire. The Sasang constitutional diagnosis and Exterior and Interior diagnosis were conducted by Sasang constitutional experts. All data were analyzed with Chi-square test and significant p value was 0.05. 3. Results : There are different symptoms between Exterior and Interior group in Taeeumin as followed, the frequence of famine and burp, the part of sweat, brown or not in color of stool, painful or not in evacuating, the frequence of loose feces, the frequence of abdominal inflating, the costive feeling frequence after evacuating, foam in urine or not, urination times, amount of Coldness and Hotness in belly, the frequence of turning fale in face. Soeumin as followed, digestion well or not, amount of sweat in exercise, the brown or not in color of stool, the frequence of turning fale in face, athe frequence of clearness in urine and feeling hot. Soyangin as followed, amount of appetite, amount of sweat in hot weather, sweat in neck or not, the red color in urine or not, Coldness and Hotness in foot or not, amount of drinking. 3. Conclusions : We may suggest that physiological and pathological symptoms are different between exterior and interior group in individual Sasang constitution respectively.
