In spite of the development of scientific and medical technology, the rate of prevalence and death of the chronic disease gets higher. Therefore people come to know prevention is more important than treatment in disease. In Western, it is nearly after 1970's when it is revealed that the major factor of chronic disease comes from habitual behaviors in everyday life. But that is common idea in Oriental Medicine since thousand years. In Oriental Medicine, the main cause of disease is composed of natural factor including climate condition, living-life factor, psycho-social factor, constitutional factor, inbody-pathologic-substance factor, and other factors including habits of using drug and medical service, injury, environmental pollutions, and occupational etiologic factors. Among these, the major factors of disease in modern society are lifestyle factors like the tendency of choosing and intaking food, the level of physical activity and labor, smoking and drinking alcohol, psychological tendency, the habits of using drug and medical service and so on. This is a practical aspect of oriental medicine where the most important etiologic factor comes from human behaviors, not from external factors like germ or virus. So that etiology in Oriental Medicine where improvement in the way of life is needed to prevent disease would play an important role in modern society.

Objectives : Internet addiction or pathologic internet use is one of the major mental health problems in children and adolescents in Korea. Internet addiction is defined as uncontrollable, markedly time-consuming internet use, which lasts for a period of at least six months. Internet addiction results in poor academic performance and negative parent-child relationships. By using $^{18}F$-fluorodeoxyglucose-positron emission tomography (FDG-PET), we investigated the effects of internet addiction on functional changes occurring in the adolescent brain. Methods : Adolescent patients with an internet addiction (4 boys and 2 girls; $15.6{\pm}1.2$ years) participated in this study. Eight healthy young adults (5 males and 3 females; 18-30 years old) with no previous history of psychiatric illness also participated as normal controls. Brain FDG-PET data was obtained with the participants in the resting condition and with no addictive stimuli. Results : Statistic parametric mapping analysis of the brain FDG-PET data revealed hypometabolic changes in the visual information processing circuits and hypermetabolic changes in the prefrontal areas in the adolescents with internet addiction, as compared with normal controls (p<.001). Conclusion : These results suggest a neuronal adaptation to excessive visual stimulation and synaptic plasticity due to internet addiction.
Eleven patients with calcaneal fracture had 2 acute compartment syndrome of the feet and 9 late complication of the compartment syndrome of the feet. An interstitial pressure of more than 30 mmHg in either the central or interosseous compartment was considered pathologic and was treated by fasciotomy performed medially. Rigid claw toe deformity was treated by excision of the head and neck of the proximal phalanx, dorsal capsular release of the metatarsophalangeal joint, and lengthening of the extensor tendon. Patients were evaluated at a mean of 35 months(range, 21 - 44 months) after operation, and the examination was directed specifically toward symptoms and signs of myoneural ischemia, and walking pain. At follow -up, 2 patients with acute compartment syndrome of the foot had no evidence of myoneural ischemla, 9 patients with claw toe deformity had no pain with walking. Based on our clinical observations, we concluded that compartment syndrome of the foot may occur after the calcaneal fracture and fasciotomy is effective treatment for the prevention of the long term sequelae of this debilitating condition.
I got the following result from the bibliographic study about the meaning of Hwa(火) and Hwa- byung(火病) at the concept and the aspect of pathological and etiological factors.The results are as follows.1. The pathological characteristics of Hwa(火) mean the inflammed state, the excited mental condition and perinervous hyperfunction caused by exopathy produced by any of the six extermal etiological factors(外感六淫) and Hwa caused by interior injury(內傷火熱).2. Hwa caused by the fire emotions(五志之火); anger, gladness, contemplation, sadress, fear by menta complications brings to Hwa caused by the five viscera(五臟之火) and Hwa caused by the stagnation of the liver - energy(肝鬱之火) is the rpresentive Hwa caused by the five viscera and the six hollow organs(贓腑之火).3. A symptom caused by a pathogenic Hwa(火證) is understood in a pathological sense involved an inflammed disease and is considered as a pathological state of excess syndrome(實證) caused by exterior Hwa( 外火).4. The pathologic process of Hwa-byung(火病) as the mental disease is based upon our people's emotion; Han(恨) to be pathologized without being overcome and can be considered as the process of Hwa caused by long-term(鬱久而火化).5. Hwa-byung(火病) means a psychosomatic disorder resulting from stress reaction caused by personal relations in the climacterium.Form the above mentioned study, the meaning of Hwa(火) and Hwa-byung(火病) is set theoretically and it also needs to be maintained reserching and studying about Hwa-byung(火病) consistantly.
The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.
An amenorrhoea means the pathologic condition that menstruation stops before the post menopausal period. Clinically, this is classified to both primary amenorrhoea and secondary amenorrhoea. Primary amenorrhoea indicate what has no first menstruation until 14 without secondary sexual sign or what has no first menstruation until 16 with secondary sexual sign before the time. Secondary amenorrhoea is diagnosised when a women with normal menstrual cycle before the onset doesn't have a menstruation over 6 months or appeals amenorrhoea for three times of her normal menstrual cycle. This clinical study was done on the patients who had come to gynecology department of oriental Hospital of Kyung Hee Medical Center from August 1, 1994 to July 31, 1995, complaining of amenorrhoea. The results obtained were as follows: 1. The amenorrhoea patient rate among outpatients who came to the deptment of gynecology was 3.4%. 2. The ratio between primary amenorrhoea and secondary amenorrhoea was 1 : 6.5. 33. The patient rate via other hospitals was 75% and Unremarkable finding(47.6%) was most numerous according to other hospital's diagnosis and hyperprolactinemia(11.9%), premature menopause(11.9%) were the second numerous diagnosis. 4. Unremarkable history(56.7%) was most numerous and among history, the fast(weight loss; 30%) was most numerous. 5. The most general symptom of amenorrhoea patient was indigestion(51.7%). 6. The most frequently used prescribtion for non-insurance was Onpojongoktang(溫胞種玉湯 ; 55%), for insurance was Gamisoyosan(加味逍遙散 ; 16.7%). 7. 25% patient show menstruation in their therapy and among this, 80% patient show menstruation within 40 days. 8. 26.9% secondary amenorrhoea patient show menstruation in their therapy and only one primary amenorrhoea patent(12.5%) shows the same result. 9. Among the effective used prescribtion, Onpojongoktang(溫胞種玉湯 ; 46.7%) is most numerous.
Objectives : The somatization is various physical symptoms that have no pathologic manifestation. It is very common to Korean children and adolescents as well as adults, because cultural background and immature verbal expression. DSM-Ⅳ(Diagnostic and Statistical Manual of Mental Disorders Fourth Edition) presented diagnostic criteria for Somatization disorder by grouping some symptoms out of somatization. This report is on a case of somatization disorder patient. The patient is 13-year-old female. She has been suffered from various physical symptoms those are coincided with diagnostic criteria for Somatization disorder by DSM-Ⅳ. Methods : The authors treated her by acupuncture and 3 kinds of herb medicine-Bunsimgium, Hyungbangdojuksan, Yangguksanwhatang. Results : Bunsimgium was showed a little improvement but could not solved chief complaint. Hyungbangdojuksan was not given a help. After her taking Yangguksanwhatang, most symptoms were subsided. Conclusions : A patient with the somatization disorder was improved by herbal medication and acupuncture. But the Somatization disorder repeats to take a good turn and a bad turn and continues for long time, so we need to follow up her condition.
Objectives: The development of thyroid tumor has a relationship with carcinogen, oncogene and tumor suppressor gene. With aminotriazole, radioactive iodine and nitrosomethylurea as carcinogens in rat, authors investigate the incidence in type of the thyroid tumors, p21 and p53 protein expression pattern by immunohistochemical stain and the relationship between the tumors and p21-p53 protein expressions. Materials and Methods: 80 experimental animals were divided into four groups; group 1(control, no carcinogen, n=20), group 2(oral administration of aminotriazole for 36 weeks, n=20), group 3(intraperitoneal injection of 131I for one time and oral administration of aminotriazole for 36 weeks, n=20), group 4(oral administration of nitrosomethylurea for 3 days and aminotriazole for 36 weeks, n=20). After 40 weeks they were sacrificed with pathologic examination and we performed immunohistochemical staining with pan-ras monoclonal antibody for p21 protein and CMI polyclonal antibody for p53 protein with paraffin-embedded specimens. Results: 1) No tumors were observed in group I, but 38.3% of nodular goiters, 11.7% of adenomas and 50.0% of carcinomas were observed in carcinogen treated groups(group 2, 3, 4). 2) The incidence of nodular goiter, adenoma and carcinoma were 70%, 20% and 10% in group 2, 40%, 15% and 45% in group 3 and 5%, 0% and 95% in group 4. 3) p21 protein was not expressed in normal thyroid tissues but was expressed in 26.1% of nodular goiters, 42.9% of adenomas and 6.7% of carcinomas. On the other hands, p53 protein was not expressed in normal thyroid tissues, nodular goiters, adenomas and in well differentiated thyroid carcinomas by immunohistochemical stain. Conclusion: The authors suggest that aminotrizole, 131I, nitrosomethylurea can be etiologic agents in the development of thyroid tumor and the p21 protein can be expressed in the early stage and in benign condition of thyroid tumor but p53 protein is not expressed in all conditions of development in rats.
Osteoporosis is defined as a pathologic condition in which there occurs no change in the chemical composition of the bone, while bone resorption is abnormally increased. This compares with osteogenesis, which leads to a decrease in the amount of bone. Though many varieties of therapies have been tried, no fully effective method has been found. Korean red ginseng is an important variety of Korean ginsengs and many studies have been performed to investigate its effectiveness. But there has been no report on red ginseng's effects on osteoporosis. The purpose of this study was to determine the clinical effects of Korean red ginseng on postmenopausal osteoporosis patients. The study was designed as a double-blind study. Group I consisted of 30 postmenopausal osteoporosis patients who were administered red ginseng extracts and 15 patients who were administered maltose capsules, were included in group II. The following results were obtained after one-year administration to both groups. 1. There were no significant differences in the degree of wedging, number of compression fractures in the vertebral body, trabeculations, and Cobb's angles between the experimental and control groups. 2. Even though bone mineral density increased in the lumbar spine, and decreased in the femur, there was no significant statistical difference. 3. The serum levels of parathyroid hormone (PTH), calcium, phosphate, and alkaline phosphates showed no difference between the two groups regardless of treatment. 4. Urine Deoxy-pyridinoline (DPYD) decreased in the red ginseng group while it increased in the maltose group, but statistically there was no significant difference between the two groups. 5. The clinical presentations showed no significant differences using Visual Analog Scale. 6. The specific complications related with the long-term use of red ginseng did not occur in all patients. In conclusion, there were no statistically significant differences in clinical presentations, biochemical, and radiological studies between the red gins eng and maltose groups. No definite effectiveness of red ginseng on patients with osteoporosis was found.
