Study on classification of diseases in oriental medicine (한의학(韓醫學)의 질병분류(疾病分類)에 관한(關) 소고(小考))

  • By studying disease classifications of oriental medicine from Nei-Ching, Chao's-Bing-Yuan, Dong-Yi-Bao-Jian and Korea-standard classification of causes of disease & death. The results were obtained as follows : 1. In Nei-Ching 181 kinds, Chao's-Bing-Yuan 1729 kinds, Dong-Yi-Bao-Jian 966 kinds, and Korea-standard classification of causes of disease & death 2519 kinds of diseases, which suggested more diseases as time flew. 2. In classical books such as Nei-Ching, Chao's-Bing-Yuan, and Dong-Yi-Bao-Jian most of diseases and their names were originated from six kinds of pathogenic factors, Zang-Fu, Jung-Qi-Blood-Fluid, soul, and outer-body-signs, while Korea-standard classification of causes of disease & death classified diseases according to oriental medical departments. 3. Symptoms of Cold-Heat-Excess-Deficiency and pathogenic factors, body parts, Zang-Fu were applied to names of diseases in oriental medicine. 4. In oriental medicine, some symtoms, many intermal diseases were used as disease name, but it is necessary for us to select exact name of diseases in modem clinical treatment. 5. We should consider disease names in Korea-standard classification of causes of disease & death in relations with western medical terms of diseases.

Pharmacological treatment options for acne (여드름의 약물치료요법)

  • Acne vulgaris is a very common condition affecting up of about 80% to 90% of adolescents. The patients with acne have been shown to be adversely impacted by the effect of acne on their quality of life. Four factors are believed to play a key role in the development of acne lesions: excess sebum production, disturbed keratinization within the follicle, colonization of the pilosebaceous duct by Propionibacterium acnes, and the release of inflammatory mediators into the skin. Consequently, the target for acne therapy is these well-known pathogenic factors responsible for this disease state. Topical retinoids correct abnormal keratinization, but it should be applied cautiously because of irritation. Benzoyl peroxide is an effective bactericidal agent against P. acnes. Main topical antibiotics are erythromycin and clindamycin. Fixed combination topical products with retinoids, benzoyl peroxide and antibiotics have been introduced. Use of systemic antibiotics, including tetracyclines and macrolides rapidly improves inflammatory acne lesions. Oral isotretinoin is effective against all of the main pathogenic features of acne but is contraindicated in pregnant women and has been associated with cheilitis and dry skin. Hormonal therapy has been found to improve acne in some selective patients and should be considered for appropriate candidates. This review will present the general aspects of the pharmacological treatments for acne.

Molecular cloning, Expression and purification of Anthrax toxin from Bacillus anthracis

  • Bacillus Anthracis is the causative agent of anthrax. The major virulence factors are a poly-D glutamic acid capsule and three-protein component exotoxin, which is collectively known as anthrax toxin, protective antigen (PA, 83 kDa), lethal factor (LF, 90 kDa), and edema factor (EF, 89 kDa). These three proteins individually have no known toxic activities, but in combination with PA form two toxins (lethal toxin and edema toxin), causing different pathogenic responses in animals and cultured cells. However, it remains to be elucidated for pathogenic mechanism of anthrax toxin. In this study, we constructed toxin component in bacterial overexpression system and purified the native toxin from Bacillus anthracis delta sterne F32 using FPLC system. Recombinant toxin showed high homogeneity and rapid purification processes. Also, this recombinant toxin was comparable to B. anthracis native toxin in terms of cytotoxic effects on cultured cell lines.

An Investigation of Undescribed Black Root Rot Disease of Soybean Caused by Cylindrocladium(Calonectria) crotalariae in Korea (콩의 미기녹(未記錄) 병(病)인 Cylindrocladium(Calonectria) crotalaria에 의한 흑색(黑色) 뿌리썩음병)

  • An undescribed black root rot of soybean, caused by Cylindrocladium crotalariae, was observed in Suweon area. The diseased plants showed yellowing at the top and dry rot at the root. Lesions of roots and stems in the soil were red to brown and main roots were cracked. Although not observed the disease in the field, leaves of inoculated test plants in the greenhouse exhibited circular, brown lesion surrounded by chloratic halos. The fungus was recovered in culture from the infected stem and root, and the perithecia of Calonectria crotalariae were demonstrated to be present as well as the cylindrocladium state. The fungus was pathogenic to the root, stem, petioles and leaves of soybean. The probable source of primary inoculum was microsclerotia formed in infected soybean root and stem from the previous season's soybean debris. Black root rot by this fungus was considered to be one of detrimental factors to the maximum yield of soybean. From the morphological and physiological characteristics and pathogenic behaviors, this fungus was identified as Cylindrocladium(Calonectria) crotalariae.

  • Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder with numerous causes that include genetic factors. Efforts to reveal the genetics of ALS have identified several candidate genes that are associated with familial and sporadic ALS. Here we report a Korean ALS patient who showed prominent upper motor-neuron-related symptoms with marked brain atrophy and neuropsychological deficits. The findings were highly suggestive of ALS in a patient with a likely pathogenic FIG4 variant.

  • The pathogenesis and mechanism of obstructive sleep apnea (OSA) has been under investigation for over 25 years, but its etiology and mechanism remains elusive. Skeletal (maxillary and/or mandibular hypoplasia or retrodisplacement, inferior displacement of hyoid) and soft tissue (increased volume of soft tissue, adenotonsillar hypertrophy, macroglossia, thickened lateral pharyngeal walls) factors, pharyngeal compliance (increased), pharyngeal muscle factors (impaired strength and endurance of pharyngeal dilators and fixators), sensory factors (impaired mechanoreceptor sensitivity, impaired pharyngeal dilator reflexes), respiratory control system factors (unstable respiratory control) and so on facilitate collapse upper airway. Therefore, OSA may be a heterogeneous disorder, rather than a single disease entity and various pathogenic factors contribute to the OSA varies person to person. As a result, patients may respond to different therapeutic approaches based on the predominant abnormality leading to the sleep-disordered breathing.

Antimicrobial Resistance and Distribution of Virulence Factors of Vibrio parahaemolyticus Isolated from Shellfish Farms on the Southern Coast of Korea (남해안 패류양식장에서 분리한 Vibrio parahaemolyticus의 병원인자 분포 및 항균제 내성)

  • From 2013 through 2015, we investigated the contamination status and antimicrobial resistance patterns of pathogenic Vibrio parahaemolyticus in commercially valuable seawater and shellfish (Oyster Crassostrea gigas, short-neck clam Venerupis philippinarum, ark shell Scapharca broughtonii and mussel Mytilus galloprovinciallis) from the southern coast of Korea. The detection rate of V. parahaemolyticus was highest in short-neck clams (23.7%), followed by ark shells (19.2%), oysters (15.9%), mussels (13.6%), and seawater (8.6%). The following percentages of PCR assays of shellfish were positive for the thermostable direct hemolysin-related hemolysin gene (trh) : oysters (12.8%), short-neck clams(11.8%), and ark shells (3.4%). Similar assays for the thermostable direct hemolysin gene (tdh) resulted in positive results for short-neck clams (5.9%) and ark shells (3.4%). Antimicrobial resistance was present in 100% of 8 tdh (+) and 2 trh (+) V. parahaemolyticus isolates challenged with ampicillin. However, all pathogenic V. parahaemolyticus were sensitive to 14 other antibiotics. To ensure the safety of shellfish consumption, the continuous monitoring of the prevalence and distribution of virulence factors of V. parahaemolyticus in shellfish farms is needed.

Study on Comparison and Mutual Complementary Development between Sasang Constitutional Medicine and Syndrome Identification and Treatment Medicine (사상체질의학(四象體質醫學)과 증치의학(證治醫學)의 비교(比較) 및 상호(相互) 보완(補完) 방안(方案) 연구(硏究))

  • Objectives : Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about the theory of diagnosis, the theory of herbal medicine, the theory of prescription, the theory of acupuncture and constitutional medicine to put forward a complementary perfect solution which has good clinical utility. Methods : We compared and sorted out the textbook of Korean Sasang constitutional medicine which named Sasang medicine, and also the textbook of Chinese TCM which was including Basic theory of TCM, Diagnostics of TCM, Science of Chinese materia medica, Pharmacology of traditional Chinese medical formulae, Science of acupuncture and moxibustion, Constitution of TCM, and also related papers. Results : (1) Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about diagnosis, herbal medicine, prescription and acupuncture, Sasang constitutional medicine is a medical system which gives priority to balance of organs, when Syndrome identification and treatment medicine is a medical system that gives priority to improve symptom. (2) The prescriptions of Sasang constitutional medicine are used for the treatment of basic pathogenesis in the chronic stage of the disease, which can recover vital qi, when Syndrome identification and treatment medicine are used for the treatment of stage pathogenesis in the acute stage of the disease, which can get rid of pathogenic factors. (3) There are two kind of complementary perfect solution between Sasang constitutional medicine and Syndrome identification and treatment medicine. One perfect complementary medicine is that Sasang constitutional medicine plays a major role when Syndrome identification and treatment medicine plays a supporting role. The other one is that Syndrome identification and treatment medicine plays a major role when Sasang constitutional medicine plays a supporting role. It determined by the usual symptoms and the symptoms now, vital qi and pathogenic factors, symptom and the root cause, acute stage and chronic stage, and the state of pathogenesis. Conclusion : A new perfect complementary medicine can be created which is based on the setting that Sasang constitutional medicine primarily treat the usual symptoms, vital qi, the root cause, chronic stage and basic pathogenesis when Syndrome identification and treatment medicine primarily treat the symptoms now, pathogenic factors, symptom, acute stage and stage pathogenesis.

Risk Factors for Associated with Latent Tuberculosis Infection among Former Mine Workers (광업 이직근로자에서 잠복결핵감염 위험요인 분석)

  • Objectives: To identify the positive rate of and the risk factors associated with latent tuberculosis infection(LTBI) in mine workers, the objectives of the present study evaluated those among former mine workers. Methods: Between January 2015 and May 2017, former male mine workers who had been subjects for epidemiology research for work-related chronic obstructive pulmonary disease(COPD) and had received QuantiFERON-$TB^{(R)}$ Gold In-Tube(QFT-GIT) from the Institute of Occupation and Environment(IOE) under Korea Workers' Compensation and Welfare Service(KCOMWEL) were selected as the study subjects. To identify significant variables for increased risk of LTBI, logistic regression analysis was performed. Results: A total of 736 male former mine workers were selected as study subjects. The positive rate of LTBI among subjects was 69.2%(509/736). The current smoking[odds ratio(OR), 2.3; 95% confidence interval(CI), 1.1-4.9], COPD(OR, 1.4; 95% CI, 0.9-2.3), department loading(OR, 1.8; 95% CI, 0.9-3.4) and mining(OR, 1.5; 95% CI, 0.9-2.5), and working duration of over 20(OR, 1.6; 95% CI, 0.9-3.1) and over 30 years(OR, 2.2; 95% CI, 0.9-4.9) were associated with increased risk of LTBI. The interferon-gamma(IFN-${\gamma}$) level after stimulation with Mycobacterium tuberculosis(MTB)-specific antigens showed a significantly negative correlation with age(r=-0.126). Conclusions: The present study determined that the high positive rate of LTBI among mine workers was associated with not only the host factors but also the occupational exposure to mine dust.

Study on the Regulation Methods of Meridian ki Depending on the Pathological State of 'Discordance between Water and Fire'

  • This study reviews and suggests the followings: 1) The balance and harmony between water and fire (ki and blood, yin and yang) is regarded as the basic condition of maintaining health in Oriental medicine. 2) Oriental medicine defines disease as the imbalance or disharmony of body. So the discordance between water and fire is in ill state, the coordination between water and fire is in healthy state. 3) Etiological factors and pathology are summarized by the conflict between vital ki and pathogenic factors, six exogenous factors, internal injury by seven emotions and the imbalance between yin and yang. 4) The regulation of meridian ki recovers the balance between water and fire, ki and blood using acupuncture, moxibustion, cupping therapy, herbal medicine and physical and breathing exercise and massage which is based on the principle of controlling ki and spirit.
