Augmentation Strategies in Obsessive Compulsive Disorder (강박장애의 약물효과 증대 전략)

  • The serotonin reuptake inhibitors(SRIs) and the serotonin selective reuptake inhibitors(SSRIs) are considered the first choice agents for pharmacologic treatment of obsessive-compulsive disordr(OCD). However, many patients with OCD experience little or no improvement in their symptoms when treated with SRIs or SSRIs. Patients who have experienced a partial or no response to an SRI/SSRI at 10 to 12 weeks are often considered for augmentation strategies. Nearly every class of psychotropic medications has been tried in an open fashion, though augmentation strategies have been somewhat disappointing.

Response of forced Euler-Bernoulli beams using differential transform method

  • In this paper, forced vibration differential equations of motion of Euler-Bernoulli beams with different boundary conditions and dynamic loads are solved using differential transform method (DTM), analytical solutions. Then, the modal deflections of these beams are obtained. The calculated modal deflections using DTM are represented in tables and depicted in graphs and compared with the results of the analytical solutions where a very good agreement is observed.

  • Background : To compare the efficacies and side effects of etoposide, cisplatin/cyclophosphamide, adriamycin, vincristine(VPP/CAV) with those of carboplatin etoposide(CE) in extensive stage small cell lung cancer patients. Method : Patients with extensive stage small lung cancer who has measurable disease were eligible. VPP/CAV group(n=22) was treated with cisplatin(60mg/$m^2$ iv. D1) etoposide(100mg/$m^2$ iv. D1-3), and 3 weeks later cyclophosphamide(1000mg/$m^2$ iv. D1), adriamycin( 40mg/$m^2$ iv. D1), and vincristine(1.4mg/$m^2$ iv. D1), were administered alternatively. CE group(n=22) was treated with carboplatin(325mg/$m^2$ iv. D1) and etoposide (100mg/$m^2$ iv. D1-3) ; repeated treatment was performed every 3 weeks. Result : Forty four patients were eligible for the study. The overall response rate was 61.4% (complete remission rate 0%, partial response rate 61.4%, stable disease rate 25%, progressive disease rate 13.6%), and median survival was 10.8 months. In VPP/CAV group, response rate was 54.5% (complete remission rate 0%, partial response rate 54.4%, stable disease rate 27.3%, progressive disease rate 18.2%), and, in carboplatin/etoposide group, the response rate was 68.2%(complete remission rate 0%, partial response rate 68.2%, stable disease rate 22.7%, progressive disease rate 9.1%). The median survival time was 9.5 months in the VPP/CAV group and 11 months in CE group. The toxicity of both group was moderate, and anemia was more frequent in the CE group. Conclusion : VPP/CAV regimen and CE regimen produced similar response rates and survival times in extensive stage small cell lung cancer patients. CE regimen may be effective as part of the initial therapy for extensive stage small cell lung cancer.

  • In this study, the dynamic behaviour of a rotating Timoshenko beam when under the actions of a variable magnitude load moving at non-uniform speed is carried out. The effect of cross-sectional dimension and damping on the flexural motions of the elastic beam was neglected. The coupled second order partial differential equations incorporating the effects of rotary and gyroscopic moment describing the motions of the beam was scrutinized in order to obtain the expression for the dynamic deflection and rotation of the vibrating system using an elegant technique called Galerkin's Method. Analyses of the solutions obtained were carried out and various results were displayed in plotted curve. It was found that the response amplitude of the simply supported beam increases with an increase in the value of the foundation reaction modulus. Effects of other vital structural parameters were also established.

  • In order to examine the validity of an asymptotic solution obtained from the method of multiple scales, we investigate a third-order subharmonic resonance response of a bar constrained by a nonlinear spring to a harmonic excitation. The motion of the bar is governed by a linear partial differential equation with a nonlinear boundary condition. The nonlinear boundary value problem is solved by using the finite difference method. The numerical solution is compared with the asymptotic solution.

  • This paper is concerned with the analysis of structural equation model(SEM) with the ordinal data such as Likert scale. The SEM is misused when the arbitrary scores allocated to the Likert scale are treated as quantitative data. The underlying distribution approaches have been studied to solve this problem, and the partial least squares(PLS) Is also tried. In this paper the quantification methods for the Likert scale are proposed to analyze the SEM. We assume that the Likert scale is an observation of the interval of the continuous underlying distribution, and the respondents have their own patterns in the response of some questions. Normal and beta distributions as the response patterns are considered to quantify the Likert scale. To compare the efficiency of the proposed method the bootstrap simulations are tried.

  • In this paper, the optimum design of the glass net shape was studied to minimize the electric field of mold transformer. The glass net is used for reinforcement in structure between coils and epoxy, but it can cause to increase the electric field and the partial discharge. Therefore, the optimum design of glass net is required to minimize the electric field. In this paper, the objective function was approximated by using response surface method and then Zoom-in method was used for optimal design to minimize the electric field. The electric field was analysed using finite element method for each shape of glass nets.

Optimization of Cholesterol Removal Conditions from Homogenized Milk by Treatment with Saponin

  • This study was carried out to determine the optimum conditions for cholesterol removal from homogenized milk by treatment with saponin using a response surface methodology (RSM). The effects of temperature, reaction time, and amounts of celite or saponin added on cholesterol removal from milk were investigated. The level of cholesterol removal from milk increased with saponin concentration and varied from 57.4 to 73.3%. The optimum reaction time, amount of celite addition determined by a partial differentiation of the model equation, and amount of saponin addition were 30min, 0.95% and 1.5%, respectively. Under these conditions, the predicted cholesterol removal by RSM was estimated to be 73.4%. The experimental removal value was 73.7%. Thus, there was no appreciable difference between the experimental value and the predicted value based on RSM.

  • This paper introduces the equivalent frequency response method(EFRM) into runoff analysis. This EFRM originally had been developed to analyze dynamic behavior of nonlinear elements such as threshold and saturation in control engineering. Many runoff models are described by nonlinear ordinary of partial differential equations This paper presents that these nonlinear differential equations can be converted into semi-linear ones based on EFRM. The word of "a semi-linear equation" means that the coefficients of derived equations depend on average rainfall.

Two-Dimensional Modelling of the Cochlear Basilar Membrane (달팽이관 기저막의 이차원적 모델링)

  • Two-Dimensional modelling of the Cochlear biomechanics is presented in this paper. The Laplace partial differential equation which represents the fluld mechanics of the Cochlea has been transformed into two-dimensional electrical transmission line. The procedure of this transformation is explained in detail. The comparison between one and two dimensional models is also presented. This electrical modelling of the basilar membrane (BM) is clearly useful for the next approach to the further development of active elements which are essenclal in the producing of the sharp tuning of the BM. This paper shows that two-dimension model is qualitatively better than one-dimensional model both in amplitude and phase responses of the BM displacement. The present model is only for frequency response. However because the model is electrical, the two-dimensional transmission line model can be extended to time response without any difficult.
