골관절염 환자의 통증 감소를 위한 심리적 중재의 효과 : 메타분석 (The Effect of Psychological Intervention for Pain Reduction in Osteoarthritis Patients : A Meta-analysis)

  • 본 연구는 골관절염 환자의 통증 감소를 위한 심리적 중재의 효과를 메타 분석한 연구로 심리적 중재 개발을 위한 기초자료를 제공하기 위해 수행되었다. 연구방법으로는 PubMed, EMBASE, Ovid-MEDLINE, CINAHL, Korean database에서 2018년 12월 30일까지 발표된 국내 외 문헌을 검색하였고, 심리적 중재의 효과 크기를 파악하기 위해 R version 3.5.1 프로그램을 이용하였다. 그 결과 총 1,822편의 문헌에서 총 1,847명의 참가자를 가진 12편의 연구가 최종 분석되었다. 분석결과로는 골관절염 환자에게 통증 감소를 위한 심리적 중재의 전체 효과크기는 -0.31(95% CI:-0.50, -0.11)로 작은 정도의 유효한 효과를 보였다(p<.001). 조절효과 분석에서는 중재 종류, 중재 기간, 중재 제공자에 따른 효과크기가 유효한 차이를 나타냈다. funnel plot 분석으로 출판오류를 분석하였고, 시각적으로 비대칭이었으나 egger's regression test 후 비대칭이 아님을 확인하였다(bias= -3.24, p=.077). 본 연구의 결과는 메타분석에 포함된 연구의 수가 적기는 하나 골관절염 환자의 주요 증상인 통증 문제를 개선하기 위한 심리적 중재의 표준을 제시하였고 중재 종류, 중재 기간, 중재 제공자에 대한 임상적 근거를 마련하였다.

Effects of Posteroanterior Mobilization on the Cervical Spine in Patient with Chronic Whiplash-Associated Disorders

  • The purpose of this case study was to identify the effects of posteroanterior (PA) mobilization on the cervical spine in a patient with chronic whiplash-associated disorder (WAD). The subject of this study was a 58-year-old woman who sustained a chronic WAD as a result of a motor vehicle accident two years prior. The subject has progressively worsening neck pain and stiffness. The subject was determined to have a grade IIb WAD the use of the Modified Quebec Classification. The intervention was central and unilateral PA mobilization on the spinous process of C4 and C5. The PA mobilizations were performed at the end of range to Maitland grade IV. The PA mobilization was conducted once daily for a total of eight days. Two sets of measurements were done one before and one after the intervention. Neck pain, cervical stiffness, range of motion and lordosis of the cervical spine were measured. Experimental intervention decreased the neck pain, and increased the neck stiffness and cervical ROM (range of motion) such as flexion, extension, lateral flexion and rotation. X-ray photographs also represented that cervical curvature increased from $35^{\circ}$ to $40^{\circ}$. This study suggested that PA cervical mobilization applied to hronic WAD is effective in decreasing pain, increasing cervical ROM and cervical curvature.

고관절 가동술이 슬개대퇴통증증후군 환자의 통증, 균형 및 보행에 미치는 영향 (Effects of Hip Joint Mobilization on Pain, Balance, and Gait in Patients with Patellofemoral Pain Syndrome)

  • Background: Patellofemoral pain syndrome (PFPS) is common knee disorder encountered in clinical: notably, altered hip biomechanic may contribute to PFPS. In this study, We investigated the effects of hip joint mobilization on pain, balance, and gait in patients with PFPS. Methods: Patients were randomly assigned to a control group (n=18) or an experimental group (n=20). Both groups received exercise therapy thrice a week for 4 weeks. The experimental group performed additional hip joint mobilization thrice a week for 4 weeks. Measurement were obtained in each patient pre-intervention and post-intervention (after 4 weeks). Results: The assessed items included the visual analog scale (VAS), one leg standing test (OLS), timed up and go test (TUG), and the 10m walk test (10MWT). Post-intervention assessment showed significantly improved results in both groups (p<.01). A significant intergroup difference was observed only in the results of the 10MWT (p<.05). Conclusion: Our results indicate that hip joint mobilization with exercise therapy may be useful to improve PFPS.

  • PURPOSE: The purpose of this study was to investigate the effect of taping method using elastic tape and non-elastic tape on pain, grip strength and muscle activity in middle-aged female patients with lateral epicondylitis. METHODS: The subjects were 40- to 50-year-old female patients with lateral epicondylitis. A total of 22 subjects were divided into two groups, and each group had 11 patients. The intervention was carried out three times a week for 4 weeks. One group applied tape using elastic tape (ETG) while the other group applied tape using non-elastic tape (NETG). The measurement factors were pain, grip strength, and muscle activity. Pain was measured using VAS. Grip strength was measured using a grip dynamometer, and muscle activity was measured using EMG in the affected extensor carpi radialis brevis. Changes in interventions were compared among the groups using a paired t-test. The comparison between groups compared delta values (Changed values) between pre- and post-intervention using an independent t-test. RESULTS: In both groups, there was a statistically significant decrease in VAS. However, there was no statistically significant decrease in the delta value of VAS between ETG and NETG. In both groups, grip strength and muscle activity increased after intervention. There were statistically significant differences in delta values between ETG and NETG. CONCLUSION: Taping is a good intervention method for patients with lateral epicondylitis. Both elastic tape and non-elastic tape were effective. In particular, taping using elastic tape is an effective method for improving muscle function with pain reduction.

The Effects of Cervical Manipulation on Blood Flow Velocity of Cranial Artery and Pain Level in Cervicogenic Headache Patients

  • The purpose of this study was to identify the effects of manipulation on the velocity of cerebral blood flow and level of pain in cervicogeinc headache patients. The velocity of cerebral blood flow of 30 cervicogeinc headache patients(male=15, female=15, age=$24.00{\pm}3.60$) and 33 normal subjects(male=15, female=18, age=$23.27{\pm}3.00$) was compared. The 30 cervicogeinc headache patients were divided into suboccipitalis relaxation group, cervical manipulation group, and placebo group, and each were given different interventions. The velocity of cerebral blood flow and pain level was measured before intervention, and 1, 2, 3 weeks after intervention. The velocity of cerebral blood flow was measured with the Transcranial Doppler(TCD), and pain level was measured with visual analog scale(VAS). Blood flow velocity of middle cerebral artery in cervicogeinc headache patients was slower than those in healthy subjects. Physical therapy intervention did not have significant effect on velocity of cerebral blood flow, but slowly decreased at intervention for pain level increased. The suboccipitalis relaxation group and cervical manipulation group showed significant effect in decreasing pain level compared to the placebo group(p<.05). Directly applied manipulation therapy in the neck area not only has effect on joint of cervical and soft tissue but also on blood vessels and nerves which pass the neck area, and because of those results of manual therapy seems to help recovery.

Immediate Effects of Myofascial Release Using Vibration Foam Rolling Methods on Hamstrings Range of Motion, Flexibility, Pressure Pain Thresholds and Dynamic Balance

  • Background: Many trials have been conducted the methods and types of intervention of form rollers, but no research has been done yet that mixes the methods and types of intervention. Objectives: To analyze the effects of myofascial release on the improvement of range of motion (ROM), flexibility, pain pressure threshold, and balance. Design: Randomized controlled trial. Methods: All subjects measured ROM, flexibility, pressure pain threshold, and dynamic balance by pre-test. After pre-test, subjects were randomized that static-vibration foam rolling group (n=12), dynamic-vibration foam rolling group (n=12), general foam rolling group (n=12). For the intervention, 3 sets of 90 seconds were applied to each group, and rest time was set to 60 seconds between sets. In the post-test and follow-up test after 10 minutes, all three groups were measured the ROM, flexibility, pressure pain threshold, and dynamic balance. Results: The results of comparing ROM, flexibility, pressure pain thresholds, dynamic balance ability appeared higher significant difference in the pre-post-10 minutes follow up test in comparison between time in the intragroup (P<.001). As a result of comparing the change of pre-post-10 minutes follow up, static vibration foam rolling showed higher significant difference compared to control groups (P<.001). Conclusion: Through this study, when foam rolling is applied within the same intervention time, static foam rolling can be expected to have a better effect than the existing dynamic foam rolling as well as vibration foam roller can expect better effect than general foam rolling.

내장 통증에 사용된 침 중재 분석 및 보고의 질 평가 (A Review on Reporting Quality of Acupuncture Intervention for Visceral Pain: Assessment with STRICTA)

  • Objectives : The purpose of this study is to review which acupoints and meridians are selected to treat visceral pain in articles published in international journals and to investigate quality of reporting acupuncture intervention according to the STRICTA guidelines. Methods : Electrical and hand search were conducted in PubMed, EMBASE, MEDLINE, and Cochrane Library for acupuncture studies on visceral pain in human and animals. Reporting quality of intervention was evaluated using the revised STRICTA guidelines. Results : We included 51 articles (7 clinical studies and 44 animal studies) in this study. The most frequently adopted meridians and acupoints were Stomach meridian, Spleen meridian, Conception Vessel meridian, and ST36, CV4, SP6, LI4, PC6, SP9, ST25, ST37 in clinical studies. In animal studies, Stomach meridian, Conception Vessel meridian, Pericardium meridian and ST36, ST37, CV12, PC6 were most frequently used. Animal studies showed significantly lower quality of reporting compared to clinical studies. According to the analysis on the effect of year of publication of articles, announcement of STRICTA guidelines did not improve the quality of reporting for studies on visceral pain. Conclusions : These results suggest that both proximal and distal acupoints based on meridian theory were used to study the underlying mechanisms of visceral pain both in human and animal studies. Guidelines of acupuncture intervention reporting for animal study is necessary to improve the quality of evidence, and it will also allow us the integrative understanding of the mechanisms and clinical effects of acupuncture treatment in human and animal.

모션테이핑을 적용한 재활운동 프로그램이 무릎 골관절염을 가진 50대 중년여성의 통증, 근력, WOMAC index에 미치는 영향 (Effect of Rehabilitation Exercise Program with Motion Taping on Pain, Muscle Strength, and WOMAC Index in 50s Middle Aged Women with Osteoarthritis of the Knee Joint )

  • PURPOSE: This study examined the effects of motion taping during combined lower extremity exercise application on pain, ROM, muscle strength, and WOMAC index in middle-aged women with osteoarthritis of the knee joint METHODS: The subjects were 26 middle-aged women with osteoarthritis in the knee joint. Twenty-six subjects were divided into two groups. Each group contained 13 subjects. Three subjects dropped out in the middle of the experiment. The intervention was six weeks, three times a week. The control group performed traditional physical therapy and lower extremity exercise. The experimental group was motion taping applied to the lower extremity exercise. The measurement factors were knee joint pain, quadriceps muscle strength, and WOMAC index. The pain was measured using the numeric pain scale instrument (NPSI). The quadriceps strength was measured using the 1RM method. RESULTS: In both groups, pain, muscle strength, and WOMAC index showed significant improvement according to the intervention (p < .05). In comparison between groups of the change values pre-post intervention, pain and WOMAC index showed statistically significant differences between the two groups. On the other hand, there was no difference in strength between the two groups. CONCLUSION: When performing a rehabilitation exercise program for middle-aged women with osteoarthritis of the knee, motion taping has a positive effect on pain and the WOMAC index.

자기장 치료법이 무릎 뼈관절염 환자의 통증 역치, 혈류량 및 균형능력에 미치는 영향 (Effects of Magnetic Therapy to Improve on Pain Threshold, Blood Flow, and Balance in Patient with Knee Osteoarthritis)

  • Purpose : This study was performed to evaluate the effects of magnetic therapy (MT) on pain threshold, blood flow, and balance in patients with knee osteoarthritis. Methods : A single-blind, randomized controlled trial (RCT) was conducted with 30 patient with knee osteoarthritis. They were randomly allocated 2 groups; magnetic therapy group (MTG; n=15) and placebo magnetic therapy group (PG; n=15). The MTG group received 30 minutes magnetic therapy and 20 minute conservative physical therapy (Hotpack, ICT), magnetic therapy was conducted in magnetic therapy device (OM-100, NUGA, Korea). In the placebo magnetic group received 30 minutes placebo magnetic therapy and 20 minute conservative physical therapy. Each group performed 50 minutes a day 3 times a week for 8 weeks. The primary outcome pressure pain threshold test, blood flow, balance ability were measured by a pressure threshold meter (Commander algometer, JTECH medical, USA), laser dofler image (Moor LDI2-IR, Moor instruments, USA), balance measurement system (BioRescue, Marseille, France). The measurement were performed before and after the 8 weeks intervention period. Results : Both groups demonstrated significant improvement of outcome in pain threshold, blood flow, and balance ability during intervention period. magnetic therapy group revealed significant differences in pain threshold, blood flow, and balance as compared to the placebo magnetic therapy group groups (p<.05). Our results showed that magnetic therapy was more effective than placebo therapy on pain threshold, blood flow, and balance in patients with knee osteoarthritis. Conclusion : Our findings indicate that magnetic therapy can improve pain threshold, blood flow, and Balance, highlight the benefits of magnetic therapy. This study will be able to be used as an intervention data for recovering pain threshold, blood flow, and balance in patients with knee osteoarthritis.

호흡 재교육 훈련이 만성 목통증 환자의 통증과 기능장애 수준, 자세, 삶의 질, 폐 기능에 미치는 영향: 무작위 대조군 실험 (Effects of Breathing Re-education Training on Pain and Dysfunction Levels, Posture, Quality of Life, Pulmonary Function in Patients with Chronic Neck Pain: A Randomized Controlled Trial)

  • Background: This study aimed to determine the effects of training on breathing re-education on pain and dysfunction levels, posture, quality of life (QoL), and pulmonary function in patients. Methods: This study included 34 patients with chronic neck pain and upper thoracic breathing pattern were included. The participants were assigned to either the routine physical therapy program (RPTP) (CG; n=17) or RPTP and breathing pattern re-education training (EG; n=17). The CG and EG groups performed RPTP for 40 minutes, and only the EG groups performed breathing re-education training for 10 minutes. Exercises were performed thrice weekly for both groups. Level of pain and dysfunction, posture, QoL and pulmonary function status were assessed before and after the intervention. Results: After four weeks of intervention, numeric pain rating scale (NPRS), Korean version of neck disability index, cranio-vertebral angle, cranial rotation angle, and 12-item short form health survey-physical and mental component summaries had significant differences before and after intervention in both groups (p<.01). However, only NPRS, forced vital capacity, forced expiratory volume in 1s, and maximum voluntary ventilation showed significant interactions between the two groups and measurement time (p<.01). Conclusion: Breathing re-education training and RPTP may be optimal for patients with chronic neck pain and may be more effective in improving neck pain and pulmonary function.
