요통기간에 따른 손상, 장애, 심리적 요인들의 상관성 비교 (Comparison of the Relationship Between Impairment, Disability and Psychological Factors According to the Difference of Duration of Low Back Pain)

  • The purpose of this study was to investigate the correlations between pain intensity, physical impairments, disability, and psychological factors according to the difference in duration of low back pain. This study was a cross-sectional survey of 102 participants with low back pain, divided into two groups equal in number: The first group consisted of patients with acute and subacute low back pain, while the second group consisted of patients suffering from chronic low back pain. The results showed that gender, age, pain intensity, physical impairment, disability and Fear-Avoidance Beliefs (FABs) for work activities were not significantly different between two groups. FABs for physical activities of the first group were significantly more prevalent than in the second group. More than moderate correlations were found between pain intensity, physical impairment, and disability in the first group. Less than moderate correlations were found between pain intensity, physical impairment, disability, FABs, and depression in the second group. These findings suggest that we must consider psychological factors in the treatment of patients with chronic low back pain. Regression analyses revealed that pain intensity and FABs for work activities significantly contributed to the prediction of disability in the first group. Also, pain intensity and FABs for physical activities significantly contributed to the prediction of disability in the second group. Pain intensity was most important predictor of disability in two groups.

간호학생의 통증관리에 관한 지식 (Nursing students관 Knowledge on Pain Management)

  • Pain management of postoperative patients or patients suffering from terminal cancer became one of the most important task of nurses in recent years. This study analyzed basic knowledge of 175 student nurses in Taegu on the pain assessment and management. The questionnaires included five areas : Assessment, pharmacological actions, medication, classification of opioids, and psychological dependence of analgesics. The results indicated that nursing students, in general, lacked knowledge on pain management : Correct responses on pain assessment varied between 9.7 and 100% , classification of opioids was in ranges of 61.7 to 95.4% ; Pharmacological actions was 16.6 to 80% : and medication was 14.9 to 85.7%. Less than 12.6% of students correctly identified the frequency of psychological dependence. These results suggested that the need of reinforcement on the pain management in formal nursing education.

뇌 종양 및 뇌 혈관 질환에 의해 유발된 삼차신경통 환자의 임상 고찰 (Trigeminal Neuralgia which Caused by Brain Tumor or Cerebrovascular Disease)

  • A retrospective analysis of 175 patients who were suffering from trigeminal neuralgia was done. We found 21 cases (12.0%) of abnormal findings including brain tumors and cerebrovascular disease on brain MRI. All patients were transferred to department of neurosurgery for operation. Among them, 7 patients refused or gave up operation and received nerve blocks with pure alcohol. Their MRI findings were meningioma, arachnoid cyst, arteriovenous malformation, venous angioma, and frontal sinus cancer This study demonstrates that peripheral nerve block or trigeminal nerve block with pure alcohol would be possible in case of elderly patients, patients who have poor general condition, patients who refuse operation, and brain tumor or cerebrovascular disease which located in dangerous area to be operated.

Pain in amyotrophic lateral sclerosis: a narrative review

  • Amyotrophic lateral sclerosis (ALS) is a rapidly progressive neurodegenerative condition characterized by loss of motor neurons, resulting in motor weakness of the limbs and/or bulbar muscles. Pain is a prevalent but neglected symptom of ALS, and it has a significant negative impact on the quality of life of patients and their caregivers. This review outlines the epidemiology, clinical characteristics, underlying mechanisms, and management strategies of pain in ALS to improve clinical practice and patient outcomes related to pain. Pain is a prevalent symptom among patients with ALS, with a variable reported prevalence. It may occur at any stage of the disease and can involve any part of the body without a specific pattern. Primary pain includes neuropathic pain and pain from spasticity or cramps, while secondary pain is mainly nociceptive, occurring with the progression of muscle weakness and atrophy, prolonged immobility causing degenerative changes in joints and connective tissue, and long-term home mechanical ventilation. Prior to treatment, the exact patterns and causes of pain must first be identified, and the treatment should be tailored to each patient. Treatment options can be classified into pharmacological treatments, including nonsteroidal anti-inflammatory drugs, antiepileptic drugs, drugs for cramps or spasticity, and opioid; and nonpharmacological treatments, including positioning, splints, joint injections, and physical therapy. The development of standardized and specific assessment tools for pain-specific to ALS is required, as are further studies on treatments to reduce pain, diminish suffering, and improve the quality of life of patients with ALS.

  • Chronic pain is a frequent complication after spinal cord injury. Various medical and surgical approaches have been applied for management of spinal cord injury pain but none of them are definitive. The N-methyl-D-Aspartate (NMDA) receptor antagonist, ketamine has been reported to have a significant effect in the management of neuropathic pain. We used small divided doses of intravenous ketamine (30 mg divided by 6 equals 5 mg, 5 min interval) in spinal cord injury patients suffering from chronic pain, and accomplished significant pain relief without side effects.

고통 속에서 발견하는 희망에 대한 신학적 성찰 : 욥기와 몰트만 신학을 중심으로 (Theological Reflection on the Hope Found in Suffering: Focusing on the Book of Job and the Theology of J. Moltmann)

  • 본 연구는 인간학 주제 중의 하나인 고통에 대하여 기존 철학적 접근을 넘어 그리스도교 신학적 입장에서 성찰하여, 고통이 부정적 경험으로만 남지 않고, 삶의 긍정적인 원동력으로서 인격적 성숙에 도달할 수 있는 희망의 매개체가 될 수 있음을 제시한다. '고통과 희망의 관계 안에서 조화'라는 새로운 의미 지평을 발견함으로써 고통의 존재 이유와 의미에 관한 인간학 콘텐츠를 심화시키는 것을 목표로 한다. 욥기는 고통을 인간이 지은 죄에 대한 벌로써 정당화하는 신정론적 개념을 부정하고, 하느님과의 인격적 관계 속에서 하느님 사랑을 체험할 수 있는 희망의 공간으로 이해하는 새로운 지평을 연다. 신학자 몰트만은 고통의 신학에서 욥기가 발견한 지평을 두 가지 측면에서 심화시킨다. 첫째, 부활한 예수 그리스도가 악과 고통에 대한 최종적인 승리를 가져올 종말론적 미래의 희망 안에서 현재 역사를 해석하고, 이 해석을 통해 드러나는 부조리와 고통에 실천적으로 저항하도록 초대한다. 둘째, 고통의 문제를 그리스도의 십자가 사건 안에서 발견되는 인간을 위한 사랑으로 인해 능동적인 수난까지 감수하는 삼위일체론 속에서 성찰하고, 그 하느님 사랑의 힘으로 고통을 극복할 수 있다는 또 다른 희망을 제시한다. 그러므로 그리스도 신앙은 고통과 희망이 대립적인 단어가 아니라, 고통 속에서 희망할 수 있음을 고백한다.

어혈, 담음으로 변증된 요추부 추간판 탈출증 환자들의 특성에 관한 연구 (A Study on the Characteristics of Lumbar Disc Herniation Being Classified Blood Stasis and Phlegm-Retained Fluid)

  • Objectives In this study, we wanted to find out the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of lumbar disc herniation being classified as blood stasis and phlegm-retained fluid. Methods We surveyed 42 patients suffering from lumbar disc herniation using the diagnosis blood stasis syndrome, water retention syndrome of the comprehensive diagnosis of QI blood water. Blood stasis and phlegm-retained fluid are identical with blood stasis syndrome, water retention syndrome. Then we analyzed the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of the patients suffering from lumbar disc herniation who were diagnosed as blood stasis and phlegm-retained fluid. Results Patients were sorted into two groups: 18 blood stasis patients and 30 phlegm-retained fluid patients. Gender (Woman), acute phase and night pain are related to blood stasis. Acute phase, positive of SLR test is related to phlegm-retained fluid. There was no correlation between blood stasis and SLR test, valsalva test, duration, ESR, CRP. Also no correlation between phlegm-fluid and gender, valsalva test, duration, ESR, CRP and night pain. Conclusions By Considering the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of lumbar disc herniation patients, it can help to analyze the pattern of its symptoms.

Spinal Cord Stimulation for the Neuropathic Pain Caused by Traumatic Lumbosacral Plexopathy after Extensive Pelvic Fracture

  • The neuropathic pain caused by lumbosacral plexopathy as a sequela to extensive pelvic and sacral fractures is rare because many posttraumatic cases remain undiagnosed as a result of the high mortality associated with these types of injury and because of the survivors of multiple trauma, including pelvic fractures, frequently have an incomplete work-up. Although surgical treatments for medically refractory lumbosacral plexus avulsion pain have been reported, an effective surgical technique for pain relief in lumbosacral plexopathy has not been well documented. We describe the effectiveness of spinal cord stimulation [SCS] in a patient suffering from severe neuropathic pain caused by lumbosacral plexopathy after an extensive pelvic fracture.

삼차신경통 환자의 알코올 신경차단 효과 (Alcohol Block in the Treatment of Trigeminal Neuralgia: A Retrospective Study to Assess its Efficacy)

  • This is a retrospective analysis of 158 patients who visted our Neuro-Pain clinic, April 1992 to March 1996, suffering from trigeminal neuralgia. Most patients received nerve blocks in its triggering peripheral branches of pain. All patients experienced pain relief for 3 months after initial successful nerve block. Twenty two patients complained of recurring pain within 4 to 32 months. Mean duration of pain relief was as follows: infraorbital nerve block 15.2 months, maxillary nerve block 16.8 months, and mandibular nerve block 23.4 months. Demographic and clinical characteristics of all patients were also evaluated. This study demonstrates that alcohol block is a safe and effective method of treating trigeminal neuralgia.

어지럼 발작 병력환자에서 성상신경절블록 후 다시 발생한 어지럼 발작 -증례 보고- (Meniere's Attack after Stellate Ganglion Block -A case report-)

  • Stellate ganglion block (SGB) is one of the most widely used treatment modalities for a broad range of disorders, including otolaryngologic indications such as Meniere's disease and sudden hearing loss. We present a case of a vertiginous attack following SGB for the management of Meniere's disease. A 31-year-old female, suffering from Meniere's disease, underwent repeated right side SGBs with 6 ml of 1% mepivacaine after negative aspiration tests for blood. The eleventh block was performed in the usual manner. Several seconds after injection, she showed agitation, anxiety, nystagmus, and left-sided tinnitus. Two minutes later, her tinnitus and nystagmus were resolved. Fifteen minutes after injection, she experienced acute onset of severe vertigo, nausea, and vomiting. However, her symptoms were gradually alleviated within two hours.
