The fate of overfilling in root canal treatments with long-term follow-up: a case series

  • This study describes 6 cases of endodontic overfilling with successful clinical outcomes during long-term (up to 35 years) radiographic follow-up. Successful endodontic treatment depends on proper shaping, disinfection, and obturation of root canals. Filling materials should completely fill the root canal space without exceeding the anatomical apex. Overfilling may occur when the filling material extrudes into the periapical tissues beyond the apex. The present case series describes 6 root canal treatments in which overfilling of root canal sealer and gutta-percha accidentally occurred. Patients' teeth were periodically checked with periapical radiographs in order to evaluate the outcomes during long-term follow-up. All cases showed healing and progressive resorption of the extruded materials in the periapex. The present cases showed that if a 3-dimensional seal was present at the apical level, overfilling did not negatively affect the long-term outcomes of root canal treatment.

  • Damage to the inferior alveolar nerve(IAN) is a relatively infrequent complication in endodontic treatment. However, endodontic overfilling involving the mandibular canal may cause an injury of the inferior alveolar nerve resulting in sensory disturbances such as pain, dysesthesia, paresthesia or anesthesia. Two mechanism(chemical neurotoxicity and mechanical compression) are responsible for the IAN injury. When absorbent materials overfilled, it can be treated as a non-surgical procedure. But early surgical intervention required when mechanical, chemical nerve damage expected. We report surgical removal of overfilled gutta-percha and IAN decompression through sagittal split osteotomy in case of dysesthesia after overfilling of endodontic material into the mandibular canal. Dysesthesia recovered 3 months after surgical treatment.

  • This paper introduces the genetic programming algorithm(GP), which can approximate highly nonlinear functions, as a tool for the modeling of response surfaces. When the response surfaces is approximated, the very small or minimal teaming set should be used, and thus it is almost certain that GP trees will show overfilling that must be avoided at all costs. We present a novel method, calledDDBS(DirectionalDerivative-Based Smoothering), which very effectively eliminates the unwanted behaviors of GP trees such as large peaks, oscillations, and also overfitting. Four illustrative numerical examples are given to demonstrate the performance of the genetic programming algorithm that adopts DDBS.

  • This paper quantitatively analyzes the causes of ammonia-filled- cylinder rupture based on Tait equation and the safety guidelines, focusing on liquid expansion, internal temperature, and overfilling. When there exists a safety volume, i.e., gas-occupied volume within the ammonia cylinder, the internal pressure due to temperature rise corresponds to the vapor pressure at that temperature, with an approximate circumferential stress increase of 1.43 MPa/℃. In the absence of the safety volume, the internal pressure due to temperature rise matches the pressure of the compressed liquid ammonia at that temperature, and the resulting circumferential stress gradient in the cylinder shell is approximately 55.94 MPa/℃.

APICAL SEALING OF THERMAFIL OBTURATORS IN CURVED CANALS (만곡근관에서 Thermafil의 근단폐쇄성에 관한 실험적 연구)

  • The purpose of this study was to compare the apical sealing ability, overfilling and obturation of lateral canals using two gutta-percha techniques: Thermafil obturators and cold lateral condensation. Eighty numbered epoxide blocks with one major and five lateral canals were divided into four experimental groups and obturated according to experimental groups. Four experimental groups were as follows: Group 1 : Filling with Thermafil plastic obturators and sealer(ThermaSeal) Group 2 : Filling with Thermafil plastic obturators only, without sealer(ThermaSeal) Group 3 : Fillng using lateral condensation with sealer(AH-26) Group 4 : Fillng using lateral condensation without sealer(AH-26) All the blocks were stored in 100% relative humidor at room temperature for 7 days. Each block was placed in centrifuge tube filled with India ink, and then centrifuged for 20 minutes at 3,000 rpm. Apical leakage was measured from the apical foramen to the most coronal level of dye penetration in millimeter under a stereoscope. The length of gutta-percha and sealer in each of the lateral canals was measured, too. The presence or absence of overfilling of gutta-percha and sealer was recorded. The data was analyzed by one-way ANOVA. The obtained results were as follows: l. Apical dye leakage was almost: not occurred in Group Willed with Thermafil and sealer) and Group 3(filled using lateral condensation with sealer), and there was no significant difference in linear leakage between two groups(p>0.01). 2. In both Thermafil and lateral condensation groups, linear leakage of Group I, 3(filled with sealer) was less than that of Group 2, 4(filled without sealer), and there was no significant difference in linear leakage between Group 1, 2, 3(p> 0.01). 3. Overfilling during obturation of Group 1, 2(filled with Thermafil) was more than Group 3, 4(filled using lateral condensation), and there was no significant difference between groups(p> 0.05). 4. Groups filled with Thermafil had significantly more gutta-percha than groups filled using lateral condensation in all lateral canals(p <0.01), the total length of gutta-percha and sealer found in all lateral canals were similar in Group 1 and Group 3.

  • Probabilities of overfilling for model selection criteria are derived for several different situations. First, one candidate model with one extra variable is compared to the current model. This is expanded to m candidate models. We show that these comparisons are not independent and discuss ovefitting probabilities. Correlation between two F-tests is derived. Finally, probabilities are computed using the dependent F distributions and F distributions based on order statistics of independent Chi-squares.

  • An evolutionary approach for the design of Fuzzy Logic Systems(FLSs) is proposed. Membership functions(MFs) in Takagi-Sugeno type fuzzy logic system is optimized through evolutionary process. Output singleton values are obtained through pseudo-inverse method. The proposed technique is unique for that, to prevent overfilling phenomenon, limited-level RBF membership functions are used and the new fitness function is invented. To show the effectiveness of the proposed method, some simulations results on model identification are given.

  • Hierarchical models are widely used for inference on correlated parameters as a compromise between underfitting and overfilling problems. In this paper, we take a Bayesian approach to analyzing hierarchical models and suggest a Markov chain Monte Carlo methods to get around computational difficulties in Bayesian analysis of the hierarchical models. We apply the method to a real data on smoking and lung cancer which are collected from cities in China.

Chronic maxillary sinusitis caused by root canal overfilling of Calcipex II

  • This is a case report of chronic maxillary sinusitis caused by root canal overfilling of Calcipex II (Techno-Dent). A 60 year-old male complained of dull pain in the right maxillary molar area after complicated endodontic treatment using Calcipex II paste and was finally diagnosed with a chronic maxillary sinusitis through a clinical and radiological observation. In the biopsy examination, the periapical granuloma contained a lot of dark and translucent Calcipex II granules which were not stained with hematoxylin and eosin. They were usually engulfed by macrophages but rarely resorbed, resulting in scattering and migrating into antral mucosa. Most of the Calcipex II granules were also accumulated in the cytoplasms of secretory columnar epithelial cells, and small amount of Calcipex II granules were gradually secreted into sinus lumen by exocytosis. However, chronic granulomatous inflammation occurred without the additional recruitment of polymorphonuclear leukocytes (PMNs) and lymphocytes, and many macrophages which engulfed the Calcipex II granules were finally destroyed in the processes of cellular apoptosis. It is presumed that Calcipex II granules are likely to have a causative role to induce the granulomatous foreign body inflammation in the periapical region, and subsequently to exacerbate the chronic maxillary sinusitis in this study.

  • This paper deals with smooth curve fitting of data corrupt by noise. Most research efforts have been concentrated on employing the smoothness penalty function with the estimation of its optimal parameter in order to avoid the 'overfilling and underfitting' dilemma in noisy data fitting problems. Our approach, called DBSF(Differentiation-Based Smooth Fitting), is different from the above-mentioned method. The main idea is that optimal functions approximately estimating the derivative of noisy curve data are generated first using genetic programming, and then their integral values are evaluated and used to recover the original curve form. To show the effectiveness of this approach, DBSP is demonstrated by presenting two illustrative examples and the application of estimating the principal dimensions of bulk cargo ships in the conceptual design stage.
