A Comparative Study of Surgical Treatment in the Ruptured Achilles Tendon: Minimal incision and Open repair (아킬레스건 파열의 수술적 치료 비교: 최소 절개 봉합술과 관혈적 봉합술)

  • Purpose: The purpose of this study was to compare and analyse the clinical outcomes of minimal incision repair and open repair in ruptured Achilles tendon. Materials and Methods: We retrospectively analyzed the outcomes of 10 patients with minimal incision repair (group 1) and 19 patients with open repair (group 2) from February 2007 to June 2011. The postoperative clinical evaluations were done by Arner-Lindholm scale, AOFAS score, overall patient's satisfaction and cosmetic satisfaction of scar. Results: There was no statistical difference between two groups in Arner-Lindholm scale, AOFAS score, overall patient's satisfaction (p=1.21, 0.87, 1.07). There was statistically high rate of cosmetic satisfaction in group 1(p<0.001). There were no complications in group 1. Complications occurred in three patients (deep infection, rerupture, deep vein thrombosis) of group 2. Conclusion: Treatment of minimal incision repair in Achilles's tendon ruptures showed high rate of cosmetic satisfaction and low rate of complication's, but there were no significant differences with open repair in other clinical outcomes. The minimal incision repair could be recommended as one of the effective treatment for the Achilles's tendon ruptures.

  • A traumatic sternal segment dislocation in children is very rare. Of the few cases reported, some recommended surgical treatment, while others reported good results by conservative treatment. Therefore, the treatment remains unclear. This paper reports a case of completely remodeled traumatic sternal segment dislocation in a 13-year-old boy treated by the restriction of activity and periodic observations.

  • Intramuscular myxoma is a rare benign myxoid tumor that is difficult to differentiate from other benign soft tissue tumors and sarcoma, and as a result, intramuscular myxoma is commonly misdiagnosed as another type of soft tissue tumor. Accordingly, awareness of the existence of this condition is a fundamental requirement for treatment decision-making. Furthermore, although intramuscular myxoma appears grossly to be well-circumscribed, it can infiltrate adjacent soft tissue microscopically. Tumor resection is the recommended treatment, but appropriate surgical margin sizes remain controversial. To the best of our knowledge, this is the first South Korean report to be issued on the treatment of intramuscular myxoma of the foot.

  • As the population grows older and medical treatments are advancing, the number of spine surgeries in elderly patients has been increasing. To obtain a successful outcomes of spine surgery in elderly patients, surgeons should prepare meticulously because elderly patients can have osteoporosis and surgery can be more extensive than in younger patients. Therefore, this study reviewed the perioperative medical treatment, particularly for osteoporosis, to improve the surgical outcomes in elderly patients.

  • Brachial plexus injury is regarded as one of the most devastating injuries of the upper extremity. Accurate diagnosis is important to obtain the successful results. Basic preoperative evaluation includes simple radiography, cervical myelography. Magnetic resonance imaging, angiography, electrophysiologic studies and intraoperative studies. Furthermore, proper timing of surgery, surgical indication, plan and sufficient understanding of patients about the prognosis are the key for the satisfactory outcomes. This article provides an overview of the evaluation, diagnosis, intraoperative monitoring, and proper surgical planning for the treatment of posttraumatic brachial plexus injuries.

Operative Treatment of Fractures of the Midshaft Clavicle using Locking Compression Plate (Locking Compression Plate를 이용한 전위성 쇄골 간부 골절의 수술적 치료)

  • Purpose: To assess the effectiveness of the Locking compression plate (LCP) after open reduction for the treatment of the displaced clavicular shaft fracture, the clinical and radiologic outcome of the patients who were managed with the LCP for internal fixation after open reduction has been analyzed. Materials and Methods: We reviewed 26 cases with a displaced clavicular shaft fracture treated by internal fixation using Locking compression plate after open reduction between May 2003 and November 2004. The patients were followed up for at least six months period, and final postoperative outcome was evaluated using clinical results based on Kang's criteria, radiologic signs of fusion. Results: All fractures united by an average of 9.3 weeks without delayed union and showed fast recovery of motion fraction and shoulder function. In addition,24 cases without the fractures of proximal humerus recovered to normal range of shoulder notion within 2.9 weeks. Clinically, according to Kang's criteria, the outcome was good or better in 22 patients. The complications included shoulder joint dysfunction in two cases and keloid formation in one case, and no other complications were observed. Conclusion: The internal fixation using LCP for the treatment of displaced clavicular shaft fracture is a safe, reliable method of treatment, with few complications, and offers rapid recovery of shoulder joint function and bone union.

Treatment of Infection after Anterior Cruciate Ligament Reconstruction: Graft Retention (전방 십자 인대 재건술 후 감염의 치료: 이식건의 보존)

  • Purpose: To report the results of graft retention treatment for infection cases after anterior cruciate ligament(ACL) reconstruction. Materials and Methods: From a group of 511 consecutive ACL reconstruction patients, we report 5 who sustained septic arthritis. All patients underwent arthroscopic lavage, debridement, synovectomy with graft retention, and treatment with intravenous antibiotics. Results: All patients were evaluated at an average of 34 months after operation. The infection was successfully eradicated. No patient showed instability with KT-2000 arthrometer testing. The clinical outcome was inferior to normal ACL reconstruction patients. Two patients were graded as nearly normal and 3 patients were graded as abnormal with International Knee Documentation Committee evaluation form. Conclusion: There were no recurrences of septic arthritis or bone infection with graft retention treatment for infection patients after ACL reconstruction.

Leiomyosarcoma of the Somatic Soft Tissue: Clinical Manifestation and Treatment Results (연부조직에 발생한 평활 근육종의 임상양상 및 치료 결과)

  • Purpose: The purpose of this study was to evaluate the treatment outcome of leiomyosarcoma (LMS) of the somatic soft tissue and investigate the prognostic factors. Materials and Methods: Seventy-two patients with soft tissue LMS (extremities and trunk) between 2001 and 2017 were reviewed retrospectively. The local recurrence and metastasis after treatment were examined. The survival rates and prognostic factors affecting survival were investigated. Only patients followed-up more than one year were included. The mean follow-up time was 41 months (range, 7-163 months). Results: The overall survival rates at five-year and 10-year were 73.6% and 66.2%, respectively. The tumor size (p=0.018), grade (p=0.017), and local recurrence (p=0.04) affected survival. Metastases developed in 31.9% (23/72) and the median post metastasis survival was 15.6 months (range, 3-78 months). Conclusion: LMS of the soft tissue developed mainly at the lower extremity. The tumor size, grade, and local recurrence were associated with survival. Post metastatic survival was generally poor despite treatment.

  • Object: The aim of this study is to descrive the effect of neurodynamic technique and self management exercise for carpal tunnel syndrome patients(CTS) Method: 13 patients with CTS participated in this study. They were from 18 to 70 years old and mean age was 25.3. In the evalutaion, gripping with grip dynamometer, strength of gripping with precision pinchmeter, pain level with visual analogue scale. All measurement of each subject were measure at pre- treatment and post treatment(after 1week) and post treattment(after 2 weeks) stage. The physical therapy program consisted of neurodynamic technique and self management exercise. SPSS 12.0 program was used to compile result. Result: The grip, pinch, VAS-P were significantly difference between pre-treatment and post treatment(after 2weeks)(p<.05) Conclusion: This study suggest that 3weeks neurodynamic technique and self management exercise improved grip and pinch strength and decrease pain score.

Treatment of Flatfoot Deformity (편평족의 치료)

  • Flatfoot deformity, defined as loss of medial longitudinal arch, sometimes involves symptoms such as medial arch pain or Achilles tendon tightening, etc. Whether the etiology of deformity is congenital or acquired, i.e., posterior tibial tendon dysfunction, symptoms are largely resolved with conservative treatment including medication, orthoses, and activity modification. Surgery should be considered in cases of failure of conservative treatment and clinicians can select an appropriate technique among many surgical options including calcaneal osteotomy or flexor digitorum longus tendon transfer. Principles of corrective surgery include the recovery of alignment and the preservation of joint motion.
