Mini-implant를 이용한 III급 부정교합 환자의 협진 치료: 문제중심의 교정치료 (Interdisciplinary treatment of Class III malocclusion using mini-implant: problem-oriented orthodontic treatment)

  • 상악 우측 견치의 선천 결손과 전치부 반대교합을 보이는 III급 부정교합 환자의 문제중심의 치료계획 수립과 치료과정, 치료결과를 보이고자 한다. 상악 우측 견치와 상악 측절치 보철 공간을 위해 상악 우측 구치부 원심 이동을, 전치부 반대교합 해소를 위해 mini-implant를 적용하여 하악 전 치열의 원심 이동을 빠르고 쉽게 이루었다. 치료기간은 17개월이 소요되었으며, 치료 1년 후에도 교합은 안정적으로 유지되었다.

  • 하악 좌측 부위 부종을 주소로 내원한 10세 남환이 낭종성 법랑아세포종으로 진단되었다. 외과적 술식으로 조대술을 시행한 후 근심 매복되었던 제1대구치는 helical spring으로 인위적 맹출을 시행한 반면, 심하게 하방 변위되었던 제2소구치는 자연적 맹출을 기대하였다. 비교적 간단한 교정치료와 교합유도에 의해 양호한 영구치 교합이 확립되었다.

  • 하악 이부 확장은 해부학적으로 가동성인 하악골에서 시행하는 견인골 신장술의 한 형태로 곡면화된 표면과 하악 과두, 좁은 치열궁 형태로 많은 제약을 받게 된다. 본 연구는 이러한 한계를 보완하기 위해 적용되는 다양한 형태의 이부 확장을 이해하기 위해 시도되었다 골 절단선의 위치와 견인 장치의 위치를 다르게 하여. 각 8가지 군으로 구분한 후 이부 확장을 시행하였다. 이부 확장은 견인기의 위치와 골 절단선의 형태에 따라 다르게 이루어지는 결과를 얻었다.

  • The short-term effects of plaque control instruction were studied in 42 adolescent orthodontic patients under active fixed treatment. At first visit, all the patients were received plaque control instruction using materials such as oral hygiene education slides, dentiform, disclosing agents, tooth brush and interdental brush. After that, Pateints were asked to perform the tooth brushing according to instructions. Such a procedure was repeated every week lot 3 weeks. Plaque index and bleeding index were scored once a week for 4 weeks and were compared according to ages, sex and duration with fixed appliance. The results were as follows: 1. Plaque control instruction was effective in reducing plaque accumulation and gingival inflammation of the orthodontic patients. 2. The effect of plaque control instruction was continued during 3 weeks among all groups of subjects, and it was prominent at the first week. 3. There was no statistically significant difference in the effects of the plaque control of instruction according to sex, ages, or duration with fixed appliances.

Correction of Angle Class II division 1 malocclusion with a mandibular protraction appliances and multiloop edgewise archwire technique

  • A Brazilian girl aged 14 years and 9 months presented with a chief complaint of protrusive teeth. She had a convex facial profile, extreme overjet, deep bite, lack of passive lip seal, acute nasolabial angle, and retrognathic mandible. Intraorally, she showed maxillary diastemas, slight mandibular incisor crowding, a small maxillary arch, 13-mm overjet, and 4-mm overbite. After the diagnosis of severe Angle Class II division 1 malocclusion, a mandibular protraction appliance was placed to correct the Class II relationships and multiloop edgewise archwires were used for finishing. Follow-up examinations revealed an improved facial profile, normal overjet and overbite, and good intercuspation. The patient was satisfied with her occlusion, smile, and facial appearance. The excellent results suggest that orthodontic camouflage by using a mandibular protraction appliance in combination with the multiloop edgewise archwire technique is an effective option for correcting Class II malocclusions in patients who refuse orthognathic surgery.

Patients' and parents' concerns and decisions about orthodontic treatment

  • Objective: Patients' and parents' expectations are important in orthodontic treatment decision making. The literature generally demonstrates the perceived benefits of orthodontic treatment, but patients' and their parents' concerns about orthodontic treatment have not been investigated comprehensively. The aim of this study was to identify patients' and parents' concerns about orthodontic treatment and compare them according to sex, age, and treatment demand level. Methods: One hundred and eighty-nine children and their parents were interviewed about concerns related to orthodontic treatment. Patients and parents were asked about orthodontic treatment decisions. Answers were recorded as "yes," "no," or "don't know." Chi-squared and Fisher's exact tests were used to compare concerns between age groups, sexes, and treatment demand levels. Kappa statistics were used to assess agreement between patients and their parents. Results: Concerns about orthodontic treatment were gathered under 10 items as follows: "feeling pain," "the appearance of braces," "being teased," "avoiding smiling," "speech problems," "dietary changes," "problems with transportation," "economic problems," "long treatment duration," and "missing school." There was no statistically significant difference in concerns between the sexes or age groups. Some concern items and treatment demand were inversely related in patients. Conclusions: The results of this study demonstrate patients' and parents' concerns about orthodontic treatment. Differences between the concerns of patients with different treatment demands imply that children might reject orthodontic treatment because of their concerns. Appropriate consultation of patients addressing their concerns may help reduce anxiety and improve the acceptance of treatment.

Computerized occlusal analysis: correlation with occlusal indexes to assess the outcome of orthodontic treatment or the severity of malocculusion

  • Objective: The aims of our study were to verify the validity of the T-Scan III system (Tekscan) as an objective occlusal evaluation tool, and to assess the differences between two occlusal indexes - the peer assessment rating (PAR) index and the American Board of Orthodontics objective grading system (OGS) - by comparing the scores derived from the T-Scan III system with the two occlusal indexes and analyzing the correlations between them. Methods: The final study sample included 48 adult volunteers (39 men and 9 women, mean age $24.14{\pm}3.16years$), after excluding 29 volunteers whose occlusion could not be evaluated by the T-Scan III system due to severe skeletal or occlusal problems. PAR index and OGS scores were assessed using dental study models, and measurements of centric occlusion, protrusive movement, and lateral excursion movement were obtained via the T-Scan III system. The results were analyzed to determine correlations. Results: Occlusal analysis by the T-Scan III system was clinically reliable (p < 0.05), and the PAR index and OGS scores were significantly correlated with several measurements obtained with the T-Scan III system (p < 0.05). Conclusions: The T-Scan III system is a quantitative and reliable method for occlusal evaluation, and represents a potential substitute for occlusal indexes. Compared to the PAR index, the OGS scores of more variables were significantly correlated with the T-Scan measurements.

  • Pierre Robin sequence (PRS) is characterized by the triad of micrognathia, glossoptosis, and airway obstruction. PRS does not have a single pathogenesis, but rather is associated with multiple syndromes. This report presents the case of a 35-year-old woman with PRS and scoliosis. Among the syndromes related to PRS, cerebro-costo-mandibular syndrome (CCMS), which is characterized by posterior rib gap defects and vertebral anomalies, was suspected in this patient. However, no posterior rib gap defect was detected on radiological examinations. Although over 80 cases of CCMS have been reported to date, few cases of PRS with scoliosis alone have been reported. Therefore, this report demonstrated the clinical, radiological, and cephalometric characteristics of an adult patient with PRS and scoliosis, but without rib anomalies.

  • The movement of teeth and initial stress associated with the treatment of orthodontics have been successfully studied using the finite element method. To reduce the effort in preprocessing of finite element analysis, we developed two types of three-dimensional finite element models based on the standard teeth model. To reduce the effort in preprocessing of finite element analysis, we developed two types of three-dimensional finite element models based on the standard teeth model. Individual malocclusions were incorporated in the finite element models by considering the measuring factors such as angulation, crown inclination, rotation and translations. The finite element analysis for the wire activation with a T-loop arch wire was carried out. Mechanical behavior on the movement and the initial stress for the malocclusion finite element model was shown to agree with the objectives of the actual treatment. Finite element models and procedures of analysis developed in this study would be suitably utilized for the design of initial shape of the wire and determination of activation displacements.

구치부 반대교합이 없는 총생 증례의 급속구개확장에 의한 치료 (Crowding with no posterior crossbite treatment by rapid Palatal expansion)

  • 중등도 내지는 심한 총생과 회전이 있는 12.5세 여자 환자의 증례에 대한 보고이다. 발치를 하지 않고, 상하악 치열궁의 확장에 의해 치료하였다. 구치부의 반대교합이 없었지만, 상악의 경우 급속구개확장을 시행하였다. 양악의 총생과 치아 회전이 해소되었으며, 교합기능과 안모 심미의 개선을 이루었으며, 적절한 수평 및 수직 피개교합을 이룩하였다. 최근에 많이 사용되는 구치부 반대교합이 없는 경우에서의 급속구개확장은, 진단학적 기반 및 적응증에 관한 재평가가 필요하다고 생각된다.
