Quantitative analysis of periodontal pathogens present in the saliva of geriatric subjects

  • Purpose: At present, information regarding periodontal disease in geriatric patients is scarce. The purpose of this study was to quantify the periodontal pathogens present in the saliva of Korean geriatric patients and assess the relationship between the bacterial levels and the periodontal condition. Methods: Six putative periodontal pathogens were quantified by using a real-time polymerase chain reaction assay in geriatric patient groups (>60 years) with mild chronic periodontitis (MCP), moderate chronic periodontitis (MoCP), and severe chronic periodontitis (SCP). The copy numbers of Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Aggregatibacter actinomycetemcomitans, Fusobacterium nucleatum, and Prevotella intermedia were measured. Results: It was found that the bacterial copy numbers increased as the severity of the disease increased from MCP to SCP, except for P. intermedia. For P. intermedia, it was found that samples in the MCP group yielded the largest amount. It was also found that the quantities of P. gingivalis, T. forsythia, and T. denticola, the so-called "red complex" bacteria, were lower than those of F. nucleatum, A. actinomycetemcomitans, and P. intermedia in all of the samples. Conclusions: Collectively, the results of this study suggest that the levels of P. gingivalis, T. forsythia, F. nucleatum, and T. denticola present in saliva are associated with the severity of periodontal disease in geriatric patients.

한국인에서의 혼합치열기 공간분석 (The prediction of the tooth size in the mixed dentition for Korean)

  • 이번 연구의 목적은 혼합치열기 아동에서 미맹출된 견치와 소구치의 크기를 예측하는데 있어서 한국인에 맞는 방정식을 만들기 위함이다. 미맹출 치아의 크기를 예측하는 것은 혼합치열기 교정 진단과 치료계획 수립에 있어서 매우 중요하다. 미맹출된 견치와 소구치 크기를 예측하는 방법은 몇 가지가 있지만 그 중에서도 가장 흔하게 쓰이는 것이 모이어의 예측 표와 다나카와 존스턴의 방정식이 있다. 하지만 그것들은 백인을 위해서 제작된 것이고 치아 크기는 인종에 따라서 다르다고 알려져 있다. 이번 연구에서는 치아크기를 측정하여 하악 영구 절치의 크기 합과 견치 및 소구치의 크기 합 사이의 상관계수를 구하고 회귀방정식을 이용해서 한국인에 맞는 예측표를 만들었다. 연세대학교 치과대학에 재학중인 178명의 한국 학생(남 108명, 여 70명, 평균연령 21.63)을 대상으로 실험하였다. 영구치의 근원심 폭경을 석고모형상에서 calipers를 이용해서 측정하였다. 성별간의 치아 크기는 차이가 있었다 (p<0.05). Correlation coefficient는 0.57에서 0.64의 범위였고, standard errors of the estimates는 여성에서 0.6으로써 남성보다 우수하였다. ${\gamma}^2$값은 0.27에서 0.41의 범위를 나타내었다.

  • 성장기 환자를 다루는 소아치과 분야에서 환아의 성장과 발육에 대한 지식 이 필수적이다. 하지만, 현재 사용되고 있는 소아성장 측정 도표는 현 시대성을 반영하지 못하는 오래된 것이거나 혹은 의과적 용도에 맞추어져 있어서 소아치과 분야에 적용하기에는 연령이 극히 제한적이었다. 이에 따라 본 연구는 2004년 산업자원부 기술표준원에서 시행하고 대한인간공학회에서 주관한 2004년 제5차 한국인 인체치수조사사업보고서 자료를 토대로 현실성 있고 유용한 성장 곡선 및 성장 속도 곡선을 제작하는 과정을 소개할 목적으로 시행되었다. 자료를 처리하고 재배열하는 과정을 통하여 자료의 변이를 사분위수를 기본 골격으로 하고 1, 5, 95, 99 백분위수를 추가하여 남 여별 키 몸무게 성장 곡선을 제작하였고 이와 아울러 기하평균에 기초한 성장 속도 곡선도 제작하여 체성 성장 관찰 도표를 완성하였다. 본 연구 결과 생성된 성장 발육 평가 자료는 성장 발육의 지표로 이용될 수 있으며, 성장 발육과 관련 소아치과 환자 보호자와의 의사소통에 도움이 될 수 있을 것으로 생각되었다.

가구특성과 미성년 자녀의 치과이용 - 모의 특성을 중심으로 - (The Household Characteristics and Underage Children's Dental Clinic Use - Focusing on the Mother's Characteristics -)

  • 본 연구는 <한국의료패널>을 활용하여 어머니의 특성 및 가구특성변수가 아동 청소년기 자녀의 치과이용과 어떠한 관련성을 갖고 있는지를 살펴보고자 하였다. 로지스틱 회귀분석을 이용한 분석결과, 모(母)의 교육수준은 미성년 자녀의 치과이용에 통계적으로 유의한 양(+)의 상관성을 갖는 것으로 나타났으며, 양친가정에 비해 한부모가정에서 양육되는 자녀들의 치과이용 가능성이 낮은 것으로 나타났다. 질환별 치과이용 여부를 살펴본 결과에서는 충치로 인한 치과이용 가능성은 모(母)의 교육수준이 높을수록 높게 나타났으며, 한부모가정 자녀들은 양친가정 자녀들에 비해 낮은 것으로 나타났다. 발치의 경우 모(母)의 특성이나 가구특성에 따른 치과이용 차이를 살필 수 없었는데 이는 유치에서 영구치로 넘어가는 과정에서 연령의 영향이 절대적으로 작용하기 때문인 것으로 판단된다. 마지막으로 교정의 경우 모(母)의 교육수준 및 가구소득이 치과이용에 통계적으로 유의한 양(+)의 상관성을 갖는 것으로 나타났다. 본 연구의 결과는 아동 청소년의 구강건강증진을 위한 개선방안을 강구하는 데 아동의 성장단계 및 가구특성을 반영한 맞춤형 전략이 요구됨을 시사한다.

가토에서 하악골신장술시 신장부에 대한 가골압축의 효과 (EFFECTS OF COMPRESSION ON DISTRACTED AREA DURING MANDIBULAR DISTRACTION IN RABBITS)

  • Purpose: To evaluate the effect of compression on the distracted area in new bone formation during mandibular distraction osteogenesis in rabbits. Materials and method: Sixteen rabbits, weighing approximately 2 Kg, and the prefabricated distraction device were used. With the rabbits under general anesthesia, we performed vertical osteotomies between the anterior and posterior mandibular teeth and then placed the distraction device. After a 5 -day latency period, the mandible was distracted to a length of 10.0 mm at a rate of 1 mm/day and then immediately compressed 4 mm in the experimental group (n=8). In the control group (n=8), the mandible was distracted to a length of 6.0 mm at a rate of 1 mm/day. Rabbits in the control group were killed at 2 and 8 weeks during the consolidation period. The specimens were evaluated with light microscope after H & E stain. Histomorphometric analysis was done at 8 week specimens. Results: All experimental animals showed mandibular elongation on the macroscopical and radiographic evaluations. At 2 week, immature bone formation was observed from the surface of the host bone margins with collagen fibers arranged parallel to the direction of distraction in the control group; in the experimental group, immature bone formation was observed adjacent to the host bone, and the collagen fibers were not arranged uniformly. At 8 week, spindle-shaped new bone formation was seen in the direction of distraction in distracted area of the control group, while in the experimental group, the newly formed bone was arranged in a multidirectional manner, like the pattern of trabeculae. In the histomorphometric analysis of 8 weeks, the area of bone deposition was $2.12{\pm}\;0.75\;inch^2$ in the experimental group and $0.87{\pm}0.51\;inch^2$ in the control group (p<0.01). The bone deposition ratio was $29.60{\pm}10.50%$ in the experimental group and $12.10{\pm}7.17%$ in the control group (p<0.01). Conclusion: These results suggest that compression after over-distraction during the mandibular distraction osteogenesis is an effective method of increasing the amount of newly formed bone in distracted area.

치과 치료를 위한 외래 정주진정 법에 대한 다기관 후향적 임상연구 (A MULTICENTER RETROSPECTIVE STUDY OF OUTPATIENT INTRAVENOUS SEDATION FOR DENTAL TREATMENTS)

  • Purpose : On outpatient facilities, our sedation protocol focuses on the intermittent bolus injections of midazolam intravenously, according to patient's and operator's needs during the dental treatment. This multicenter retrospective study was aimed to prove the efficiency and safety of our sedation protocol. Patients and Methods : In three centers using the same outpatient sedation protocol for dental treatment (Division of Oral and Maxillofacial Surgery/Department of Dentistry in Hanyang University Medical Center, S-plant Dental Hospital. and Grand Oral and Maxillofacial Surgery), total 937 patients had various dental treatments under intravenous conscious sedations with independent patient monitoring from March 2006 to March 2009. By reviewing charts, we analyzed the results of sedation and dental treatment, retrospectively. Results : Our sedation protocol had no severe postoperative complications requiring admission. while showing good compatibility with almost all dental treatments, with acceptable satisfaction of both patients and operators. Conclusion : We assure that our sedation protocol can be used efficiently and safely on routine outpatient basis. We also hope that this study will provide the concrete concepts to common dental practitioners, who desire to perform sedation for dental treatment.

하악골 전돌증환자의 구내 하악골상행지 수직골절단술후 이하두정 계측방사선사진상에서의 근심골편의 형태 및 위치 변화 (MORPHOLOGIC AND POSITIONAL CHANGE OF THE PROXIMAL SEGMENTS AFTER INTRAORALVERTICAL RAMUS OSTEOTOMY OF THE MANDIBULAR PROGNATHISM ON SUBMENTOVERTEX CEPHALOGRAM)

  • Intraoral Vertical Ramus Osteotomy,along with Sagittal Split Ramus Osteotomy,is an popular surgical technique performed on mandibular prognathism. However Intraoral Vertical Ramus Osteotomy has been suspected for an initial mobilization at the healing phase of segment because it does not employ the rigid fixation between segments. To execute a study on the healing phase of segment after Intraoral Vertical Ramus Osteotomy on the horizontal plane, 102 patients (204 parts) who were diagnosed mandibular prognathism and took Intraoral Vertical Ramus Osteotomy at the Yonsei University dental hospital were observed during the period of before operation, immediately postoperation, 1 month, 3 months, 6 months, and 12 months. The change in the width of segment and horizontal angle of proximal segment and condylar head on the Submentovertex Cephalogram taken from those patients represented following results. 1. The width of proximal and distal segment decreased with the lapse of time. It decreased into 84.5% between immediate postoperative and 6M and even continued to decrease till 12M. 2. The horizontal angle of the proximal segment did medial rotation according as the lapse of time and rigorously continued till 3M. The rotation angle of condylar head indicated its tendency of recurrence to the original position but the entire recurrence was not allowed. The bigger an initial angle was, the higher was the tendency of recurrence after the operation while the rotation angle remained still bigger. 3. After grouping into group 1, group 2,and group 3 based on the extent of the variation of rotation angle of condylar head at immediate postoperative, the variation of rotation angle was measures in each group. The result presented that the initial rotation angle of condylar head had correlation with that of proximal segment but had no relation with the extent of setback of the mandible. However a quantitative analysis alone is not a sufficient method for analyzing the healing phase of segment on the horizontal plane.Therefore a multilateral analysis using 3 dimensional data such as CT is recommendable for the future study.

A comparative study of the deviation of the menton on posteroanterior cephalograms and three-dimensional computed tomography

  • Purpose: Facial asymmetry has been measured by the severity of deviation of the menton (Me) on posteroanterior (PA) cephalograms and three-dimensional (3D) computed tomography (CT). This study aimed to compare PA cephalograms and 3D CT regarding the severity of Me deviation and the direction of the Me. Materials and Methods: PA cephalograms and 3D CT images of 35 patients who underwent orthognathic surgery (19 males and 16 females, with an average age of $22.1{\pm}3.3years$) were retrospectively reviewed in this study. By measuring the distance and direction of the Me from the midfacial reference line and the midsagittal plane in the cephalograms and 3D CT, respectively, the x-coordinates ($x_1$ and $x_2$) of the Me were obtained in each image. The difference between the x-coordinates was calculated and statistical analysis was performed to compare the severity of Me deviation and the direction of the Me in the two imaging modalities. Results: A statistically significant difference in the severity of Me deviation was found between the two imaging modalities (${\Delta}x=2.45{\pm}2.03mm$, p<0.05) using the one-sample t-test. Statistically significant agreement was observed in the presence of deviation (k=0.64, p<0.05) and in the severity of Me deviation (k=0.27, p<0.05). A difference in the direction of the Me was detected in three patients (8.6%). The severity of the Me deviation was found to vary according to the imaging modality in 16 patients (45.7%). Conclusion: The measurement of Me deviation may be different between PA cephalograms and 3D CT in some patients.

하악전돌증 환자의 하악지 시상분할 골절단술에서 자가수혈의 유용성 (AVAILABILITY OF AUTOLOGOUS TRANSFUSION IN BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY FOR MANDIBULAR PROGNATHISM)

  • Objective: Autologous transfusion is frequently used with orthognathic surgery. The necessity of autologous transfusion has begun to question alone in bilateral sagittal split ramus osteotomy. The aim of this study was to evaluate the availability of autologous transfusion. Methods: The chart reviews of sixty patients who had undergone bilateral sagittal ramus osteotomy were done. The subjects were divided into two groups: In experimental group, 30 patients who autodonated 320ml in the preoperative 3 weeks actually received a transfusion in the postoperative 6 hours were included. In control group, 30 patients who underwent the same operation without preoperative donation and any other transfusion were included. Hemoglobin, hematocrit, and changes of these values in both groups were evaluated. Results: 1. From postoperative 3 hours to postoperative 1 day, the increases of hemoglobin(0.8g/dL in experimental group, 0.2g/dL in control group) and hematocrit(0.3% in experimental group, 0.6% in control group) were not statistically significant between both groups. 2. From postoperative 1 day to postoperative 1 week, the increase of hemoglobin(0.6g/dL in experimental group, 0.3g/dL in control group) was not statistically significant between both groups. But the increase of hematocrit(2.5% in experimental group, 1.0% in control group) was statistically significant between both groups over the same period(hematocrit p=0.043). 3. On postoperative 1 week, the values of hemoglobin(12.3% in both groups) and hematocrit(35.6% in experimental group, 36.8% in control group) were not statistically significant between both groups. Conclusion: The autologous transfusion in surgery of just a little blood loss was not effective. The most results show that there is little availability of autologous transfusion according to changes of hemoglobin and hematocrit in bilateral sagittal split ramus osteotomy.

  • Purpose : Cyclosporine A (CsA) is a versatile immunosuppresive agent used to prevent graft rejection syndrome and treat autoimmune disease. One of the major side effects associated with CsA is the abnormal gingival hyperplasia. The purpose of this study was to investigate the relationship between the mRNA expression of the MMP-1, TIMP-1, and $TGF-{\beta}_1$ and the concentration of CsA in cultured human gingival keratinocytes. Materials & Methods : Gingival keratocytes were obtained from gingival tissues of 4 healthy donors. The cultured gingival keratocytes were incubated with increasing concentrations of CsA (0-2000 ng/ml) for 24 hours and the expression of MMP-1, TIMP-1, and $TGF-{\beta}_1$ were determined by reverse transcription-polymerase chain reaction (RT-PCR). Results : The expressions of MMP-1 and $TGF-{\beta}_1$ were not significantly different according to the concentrations of CsA. The expression of TIMP-1 was significantly increased at the CsA concentration of 500 ng/ml. Conclusion : We concluded that the gingival hyperplasia induced by CsA was more related with TIMP-1 than MMP-1 or $TGF-{\beta}_1$ on gingival collagen metabolism in patients treated with CsA.
