A study of lower facial change according to facial type when virtually vertical dimension increases (가상적 수직 교합 고경 증가 시 안모의 유형에 따른 하안모 변화에 관한 연구)

  • Purpose: The aim of this study was to evaluate the effect of increased vertical dimension of occlusion on lower facial changes by facial type. Materials and methods: Lateral cephalograms from 261 patients were obtained and classified by sagittal (Class I, II, and III) and vertical (hypodivergent, normodivergent, and hyperdivergent) facial patterns. Retrusive displacement of soft tissue Pogonion and downward displacement of soft tissue Menton were measured in each group after 2 mm of vertical dimension of occlusion was increased at the lower central incisor using a virtual simulation program. The ratio of both displacements was calculated in all groups. The statistical analysis was done by 2-way ANOVA and Post hoc was done by Tukey test (5% level of significance). Results: Retrusive displacement of soft tissue Pogonion in Class III group was statistically different compared to Class I and II, and in vertical facial groups all 3 groups were significantly different (P<.05). Downward displacement of soft tissue Menton showed statistically significant difference between all sagittal groups and vertical groups (P<.05). The ratio of both displacements showed statistically significant difference in all sagittal groups and vertical groups (P<.05), and Class II hyperdivergent group had the highest value. Conclusion: Lower facial change was statically significant according to the facial type when vertical dimension of occlusion increased. Class II hyperdivergent facial type showed the highest ratio after increase in vertical dimension of occlusion.

Effects of implant thread profile on insertion stress generation in cortical bone studied by dynamic finite element simulation (유한요소 모사해석을 통한 임플란트 나사산 형상이 치밀골의 식립응력에 미치는 영향 분석)

  • Purpose: The aim of this study was to investigate the effect of implant thread profile on the marginal bone stresses which develop during implant insertion. Materials and methods: Four experimental implants were created by placing four different thread systems on the body ($4.1mm{\times}10mm$) of the ITI standard implant. The thread types studied in this study included the buttress, v-shape, reverse buttress, and square shape threads. In order to examine the insertion stress generation, 3D dynamic finite element analysis was performed which simulated the insertion process of implants into a 1.2 mm thick cortical bone plate (containing 3.5 mm pilot hole) using a PC-based DEFORM 3D (ver 6.1, SFTC, Columbus, OH, USA) program. Results: Insertion stresses higher than human cortical bone developed around the implants. The level of insertion stresses was much different depending on the thread. Stress level was lowest near the v-shape thread, and highest near the square shaped thread. Difference in the interfacial bone stress level was more noticeable near the valley than the tip of the threads. Conclusion: Among the four threads, the v-shape thread was turned out to minimize the insertion stress level and thereby create better conditions for implant osseointegration.

A COMPARATIVE STUDY OF THE ANGLES BETWEEN CROWN AXIS AND ROOT AXIS IN MESIODISTAL DIRECTION BY USING ORTHOPANTOMOGRAM

  • Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar.   • 1958년(年) Fullmer와 $Lillie^9$에 의(依)하여 최초(最初)로 보고(報告)된 Oxytalan 섬유(纖維)는 산(酸)에 내성(耐性)이 강(强)한 섬유(纖維)로 치근막(齒根膜)에 많이 출현(出現)하고 기계적(機械的) 자극(刺戟)에 의(依)하여 배열(配列), 주행(走行) 및 수적(數的)인 변화(變化)를 가져온다. 저자(著者)는 교정력(矯正力)을 이용(利用)하여 치아(齒牙)를 이동(移動) 시킨후(後) 치근막내(齒根膜內) Oxytalan섬유(纖維)의 수(數), 주행(走行) 및 형태(形態)의 변화(變化)를 실험적(實驗的)으로 관찰(觀察)한바 있어 이를 보고(報告)하는 바이다. 본연구(本硏究)에 사용(使用)된 실험동물(實驗動物)로는 체중(體重) 60gram내외(內外)의 자성백서(雌性白鼠) 15마리를 택(澤)하였다. 각동물(各動物)은 Ether마취후(麻醉後) 교정용(矯正用) 고무줄편(片)을 상악우측(上顎右側) 제1구치(第一臼齒)와 제2구치(第二臼齒) 사이에 삽입(揷入)하여 24, 48, 72시간(時間) 간격(間隔)으로 관찰(觀察)하였다. 동물(動物)을 도살후(屠殺後) 상악골(上顎骨)을 적출(摘出)하여 10%중성(中性) 호루마린에 고정후(固定後) 3%의산(蟻酸)으로 탈회(脫灰)하였다. 파라핀 포매후(包埋後) 근원심적(近遠心的)으로 $4{\sim}6{\mu}$의 절편(切片)을 만들어 Hematoxylin-eosin 및 Aldehyde fuchsin염색(染色)을 시행(施行)하여 경험(鏡險)한 결과(結果)는 하기(下記)와 같다. 1. 치아이동후(齒牙移動後) 교원성섬유(膠原性纖維) 및 Oxytalan섬유(纖維)들의 배열(配列)에 있어 뚜렷한 차이(差異)를 보였다. 2. 치아이동후(齒牙移動後) 사주섬유(斜走纖維)는 염박측(壓迫側)에서는 치아장축(齒牙長軸)에 수직(垂直)되게, 견인측(牽引側)에서는 평행(平行)되게 주행(走行)하고 있었다. 3. 48시간군(時間群)에서 세포증식(細胞增殖)이 심(甚)하였다. 4. Oxytalan섬유(纖維)는 치아(齒牙)들에 교정력(矯正力)을 가(加)한후(後) 견인(牽引) 염박(壓迫) 양측(兩側) 공(共)히 수(數)가 증가(增加)하였다. 5. 염박측(壓迫側)에서 Oxytalan섬유(纖維)는 속(束)을 형성(形成)하며 치아장축(齒牙長軸)에 평행(平行)되게 주행(走行)하였다. 6.견인측(牽引側)에서는 하나 또는 두세개의 섬유(纖維)들이 산발적(散發的)으로 치아장축(齒牙長軸)에 평행(平行)되게 하였다. 7. Oxytalan섬유(纖維)의 수(數)는 염박측(壓迫側)에서 보다 견인측(牽引側)에서 더 변화(變化)가 많았다.

A COMPARATIVE STUDY OF PRE- AND POST-TREATMENT CEPHALOMETRIC MEASUREMENTS: EXTRACTION VS. NON-EXTRACTION GROUPS OF CLASS I MALOCCLUSION (제 I 급 부정교합 환자의 발치와 비발치 교정치료 전.후의 두부방사선 계측학적 비교 연구)

  • 71 Class I malocclusion samples were selected and they were divided into premolar-extraction and non-extraction groups. Vertical and horizontal cephalometric evaluations on dental and soft tissue measurements were done before and after treatment. Also, treatment results in adolescent patients and adult patients were compared. The following conclusions were obtained: 1. In comparison of extraction and non-extraction groups, all the dental and soft tissue measurements, with exception of SN-MP angle, upper lip to E-line, vertical movement of upper first molar, md horizontal movement of lower first molar, showed statistically significant differences. 2. In comparison of extraction and non-extraction groups of adolescent samples, there were statistically significant differences in upper and lower incisor inclinations, horizontal dental movements from vertical reference line, positional changes in upper and lower lips, and mesial movements of upper first molar. 3. In comparison of extraction and non-extraction groups of adult samples, there were statistically significant differences in upper and lower incisor inclinations, horizontal dental movements from vertical reference line, positional changes in upper and lower lips from I-line and vertical reference line, vertical height of upper first molar, and mesial movement of lower first molar. 4. There was no statistically significant difference in SN-MP angle between extraction and non-extraction groups of both adolescent and adult samples.

THE EFFECTS OF HORIZONTAL ORTHODONTIC TOOTH MOVEMENT ON THE PERIODONTALLY DISEASED TISSUE IN DOGS (성견 치주질환 이환치아의 수평이동이 치주조직에 미치는 영향)

  • Most adults, unlike growing children, have some periodontal problems which can influence the outcome of the orthodontic treatment. In cases where periodontal disease progression resulted in marked reduction of periodontium, orthodontic treatment could result in the worsening of the periodontal conditions, and therefore orthodontic treatment planning in such adult patients requires special considerations for the periodontal problems. This study investigates the effects of horizontal orthodontic tooth movement on the changes in the mesial, distal and furcation areas of the disease affected periodontium of adult dogs with advanced bone loss. Six adult dogs with healthy periodontium were selected, and mandibular 2nd premolars were extracted. In the mandibular 3rd premolars, angular bony defects in the mesial and distal sides, and horizontal bony defects in the furcation areas were created. Those that received the flap operation and plaque control were designated as the control, those that had horizontal tooth movement without plaque control after the flap operation as Experimental group I, those that had horizontal tooth movement under plaque control without the flap operation as Experimental group II, and those that had horizontal tooth movement under plaque control after the flap operation as Experimental group III. The control group was sacrificed 2 months postoperatively, and the experimental groups were sacrificed 5 months after the initiation of tooth movement. Specimens were histologically analyzed under light microscope. The results were as follows; 1. After the horizontal tooth movements, Experimental group I and II showed angular bony defects in the mesial sides of the roots and the distal side of the furcation areas, which correspond to the pressure sides. 2. After the horizontal tooth movements, Experimental group I and II showed decreased level of alveolar bone crest in the distal sides of the roots, which correspond to the tension sides. 3. Long junctional epithelium in the control group has not been replaced by periodontal connective tissue after the horizontal tooth movements. 4. Limited formation of new bone was observed in the angular bony defects in the mesial and distal aspects of the roots in the control group. 5. Inflammatory cell infiltration in the connective tissue was most severe in the Experimental group I, followed by Experimental group II, III, and the control group in that order. These results seem to indicate that plaque control was the most influencing factor in the alteration of the periodontal tissue after the horizontal tooth movements in the periodontal tissue with alveolar bone defects.

Cephalometric difference according to the differential treatment methods in Class III malocclusion; (제 III급 부정교합 환자들의 각 치료법에 따른 측모두부방사선사진 계측치의 비교)

  • Class III malocclusion patients can be approached with many different types of treatment methods, and thus, each patient's problems must be accurately evaluated to allow selection of the best possible treatment method. Cephalometric analysis is an essential part of diagnosis and treatment planning of orthodontic patients, and it would certainly be helpful if reliable cephalometric guidelines could be set. The author divided 482 Class III malocclusion patients(253 males and 229 females) into fourgroups according to different types of treatment methods they have received to correct imbalance between upper and lower jaws: 1) orthopedic appliance (face mask & RPE), 2) camouflage treatment with fixed appliance, 3) surgical-orthodontic treatment, 4) cross-bite correction with removable plates/ functional appliance. Cephalometric values at the time of first clinical examination were compare among the four groups. Cephalometric analysis indicates the following results: 1)the amounts of antero-posterior and vertical skeletal discrepancies and dental compensation were greatest in surgery group 2) SNB, Wits, distance from Nasion Perpendicular Plane to point a facial angle, facial convexity, and APDI were greater in orthopedic appliance group than fixed appliance(camouflage) group, but there was no statistical difference 3) removable plates/ functional appliance group showed least amounts of skeletal discrepancies and dental compensation with statistical significance.

THE EFFECT OF LIGHT CURED GLASS IONOMER CEMENT ON THE SHEAR BOND STRENGTH OF ORTHODONTIC BRACKETS (광중합형 글래스 아이오노머 시멘트 교정용 브라켓의 전단결합강도에 미치는 영향)

  • The purpose of this study was to evaluate clinical applicability of light cured glass ionomer cement as a othodontic adhesive. The metal brackets and plastic brackets were bonded with light cured glass ionomer cement(Fuji Ortho $LS^{(R)}$) after polishing with a slurry of pumice, surface conditioning with 10% polyacrylic acid and chemically cured resin(Mono-$Lok2^{(R)}$) after acid etching with 38% phosphoric acid on the extracted human bicuspids. The shear bond strength was tested with a universal testing machine(HGS-100A, Shimadzu Co., Japan) after storage in normal saline at $37^{\circ}C$ or 24 hours and 48 hours. The results were as follows: 1. The shear bond strength of light cured glass ionomer cement group polished with a slurry of pumice was significantly lower than that of chemically cured resin group(P<0.01). 2. The shear bond strength of light cured glass ionomer cement group conditioned with 10% polyacrylic acid was significantly lower than that of chemically cured resin group(P<0.01). 3. The shear bond strength of light cued glass ionorner cement group conditioned with 10% polyacrylic acid was slightly higher than that of light cured glass ionomer cement group polished with a slurry of pumice, but there was no significant difference(P>0.05). 4. There was no significant difference between metal bracket group and plastic bracket group irrelevant off enamel conditioning(P>005). In summary, although the shear bond strength of light cured glass lionomer cement was lower than that of chemically cured resin, it night be clinically applicable.

Treatment effect of face mask therapy for Class III malocclusion patients according to low facial morphology (성장기 골격성 III급 부정교합 환자의 상악골 전방 견인 시 하안모 형태에 따른 치료 효과 비교)

  • Improvements in jaw relationship through clockwise rotation of the mandible may be desirable in some Class III patients with short low facial height. The aim of this study was to examine the treatment effect of face mask for Class III malocclusion patients according to their low facial morphology. Methods: Class III patients in their pubertal growth period were divided into two groups (Group 1, high LFH; Group 2, low LFH) according to lower facial height (LFH) by Ricketts (norm, 47). treatment changes between groups after face mask treatment was compared not only for hard tissue but also for soft tissue. Results: There were no significant differences between the two groups for the skeletal and soft tissues of the maxilla. There were no significant differences between the two groups for the skeletal posterior movement of the mandible, but posterior movement of the mandibular soft tissues in group 2 was larger than group 1. There were no significant differences between the two groups for the vertical hard tissue proportion changes of the mandible, but the vertical soft tissue proportion changes of the mandible in group 2 was larger than group 1. There was a significant correlation between the sagittal hard tissue and soft tissue changes of the maxilla and mandible, but there was no significant difference in the vertical changes. Conclusion: The clockwise rotation of the mandible occurred from use of the face mask, and posterior movement of soft tissues of the mandible was higher in Cl III patients with low LFH than with high LFH.

The relationship between facial asymmetry and maxillary dental arch shape (안면비대칭과 상악치열궁형태의 연관성에 관한 연구)

  • This study was designed to examine the dental asymmetry in person with facial asymmetry and to examine the relationship between the degree of mandibular deviation and asymmetry of maxillary dental arch. The sample!, were divided to asymmetry group and normal group. The asymmetry group consisted of 21 subjects(6 males and 15 females) and their mean age was 23.5 years. The normal group consisted of 20 subjects(10 males and 10 females) and their mean age was 18.6 years. Anteroposterior, transverse position of all maxillary teeth except 3rd molars, vortical position of maxillary 1st molars, and angulation of central incisors were measured. The anterioposterior and transverse positions of teeth were measured on the maxillary dental casts, the vertical position of maxillary 1st molars and angulation of maxillary incisors were measured on posteroanterior cephalometric radiographs. The data were analyzed to examine whether significant asymmetries existed in each of the asymmetry and normal groups. The results of this study were as follows : 1. In the asymmetry group, the correlation between the degree of mandibular deviation and that of 3-dimensional dental asymmetry was not so high. 2. In the asymmetry group, the teeth in deviated side were more laterally positioned than that of undeviated side. There were differences in the anteroposterior position of maxillary 1st and 2nd molars and the angulation of maxillary central incisors. 3. In the asymmetry group, the transverse asymmetry was larger in the posterior teeth rather than in the anterior teeth and larger than the anteroposterior asymmetry.
