  • The new type of orthodontic mini-implant named C-implant can be an effective alternative to conventional one-component mini-implant in the intermaxillary fixation (IMF) cases because of its particular design. The small size, two-part design, efficiency, and low cost of the C-implant make it applicable to various types of IMF cases easily such as fracture reduction and orthognathic surgery. The two part design resists highly to the fracture or deformation during implantation and removal. The long span head allows the patient to easily attach intermaxillary elastics, so that the patient can apply intermaxillary elastics for traction easily. Through this article, we tried to show the possibility of this appliance as a good adjunct for the IMF screw.

The treatment of an impacted canine related with a miniplate inserted for mandibular fracture (하악골 골절 치료를 위한 miniplate와 관련된 매복견치의 치험례)

  • This case report describes the treatment of an impacted tooth in relation to pediatric mandibular fracture. A 10-rear-old girl presented with au impacted mandibular canine related with a miniplate in the left mandibular body area. An ordinary surgical traction ended in failure. Good occlusion was established by removal of the miniplate. It would be considered important to make an exact diagnosis of the cause and remove the mechanical obstruction instantly for successful orthodontic treatment of an impacted tooth.

Three dimensional finite element analysis of continuous and segmented arches with use of orthodontic miniscrews (교정용 미니스크류를 이용한 연속호선과 분절호선의 유한요소분석)

  • Objective: The purpose of this study was to compare the displacement patterns shown by finite element analysis when the maxillary anterior segment was retracted from different orthodontic miniscrew positions and different lengths of lever arms in lingual continuous and segmented arch techniques. Methods: A three dimensional model was produced, the translation of teeth in both models was measured and individual displacement was calculated. Results: When traction was carried out from miniscrews in the palatal slope, lingual tipping of crowns and extrusion of the maxillary anterior segment were found in both continuous and segmented arches as the lever arms were made shorter. With miniscrews in the midpalatal suture area, the displacement patterns were similar to the palatal slope, but bodily movement of the upper incisors was observed in both continuous and segmented arches with the lever arm at 20 mm. When lever arms were longer, there was less extrusion of the incisors and more buccal displacement of the canines. Such displacement was shown less in the continuous arch than the segmented arch. The second premolar showed crown mesial tipping and intrusion, and the molars showed distal tipping in the continuous arch. The posterior segment was displaced three dimensionally in the segmented arch, but the amount of displacement was less than the continuous arch. Conclusions: It is recommended that lever arms of 20 mm in length be used for bodily movement of the anterior segment. Use of continuous or segmented arches affect the displacement patterns and induce differences in the amount of displacement.

AUTOTRANSPLANTATION OF A MALPOSITIONED MANDIBULAR SECOND PREMOLAR : A CASE REPORT (이소매복된 하악 제2소구치의 자가치아이식을 이용한 치험례)

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, orthodontic or surgical relocation and extraction. Autotransplantation may be defined as the transplantation of embedded, impacted or erupted teeth, from one site to another in the same individual into extraction site or surgically prepared sockets. Autotransplantation ensures preservation of natural tooth, induction of alveolar bone growth and root development, offers one of the fastest and most economically feasible means in the replacement of young patients' missing teeth. This case presents a malpositioned impacted mandibular premolar of an 11-year-old girl. It was thought that orthodontic traction was difficult because of its unfavorable impacted position. Therefore the tooth was treated by autotransplantation, we can observe good healing pattern during 12 months.

  • Maxillary canines are the most commonly impacted or ectopically erupted teeth. If we find the abnormality of maxillary canines early, we can manage it reasonably and systematically. If we cannot see the spontaneous normalization at the periodic recall, primary canine extraction will be the next treatment choice. However, if the primary canine is extracted too early, the extraction socket will be filled with hard bone and then the eruption pathway can be locked. So it is more beneficial to extract the primary canine at the period about 6 months before the normal canine eruption time. The next treatment plan can be surgical and orthodontic approaches before the root apical closure of the impacted canine. Sometimes, surgical extraction and further prosthetic procedure can be needed for a severely malposed impacted canine or badly resorpted incisor. This is the case of the idiopathic mesial movement of impacted maxillary canines.

  • Transverse skeletal deficiency is a common clincal problem associated with narrow basal and dentoalveolar bone. The clinical characteristics of transverse deficiency presents with anterior crowding and posterior buccal crossbite. Orthodontic expansion, using lip bumper and functional devices, was recommanded for younger ages. However, expansion of lower anterior area in older Patients is unstable and tends to relapse toward the original dimension. Distraction osteogenesis is a unique form of clincal tissue engineering and biologic process of new bone formation between bone segments that are gradually separately by incremental traction. Distraction osteogenesis was considered that great potential for correcting transverse mandibular deficiencies. In this Paper, a case of treated transverse deficiency patients with distraction osteogenesis using tooth-borne and tooth & bone-borne distractor is presented.

  • Tooth eruption involves a complex developmental process of tooth migration from the dental follicular origin to the final occlusion position in the oral cavity via the alveolar process. Disturbance of tooth eruption can occur at any point in a series of eruption stages; however, horizontal impaction of the mandibular first molar and primary retention of the primary mandibular second molar are rarely observed simultaneously. This study describes the treatment for two cases of horizontally impacted first molar with primary retention of primary molar. The primary retention of the primary mandibular second molar was extracted, and orthodontic traction was applied to the horizontally impacted primary mandibular first molar. Subsequently, displacement of the premolar tooth bud was improved and space regaining for eruption was achieved, guiding to normal eruption of the first molar.

  • Inverted Impaction of the permanent maxillary central incisor is rare. The causes of impaction are trauma and periapical inflammation of primary maxillary incisor teeth. Treatment options for a inverted incisor is extraction, surgery and orthodontic traction, transplantation, and spontaneous eruption guidance. Treatment depends on the incisor's root development and the space available for eruption. If root development is immature, prognosis would be good. We reported successful treatment for inverted maxially central incisor of proper eruption and normal root development by correction of a eruption route. But further observation will be required to evaluate the final root development state and amount of at tachment gingiva.

THE SPONTANEOUS ERUPTION OF DISPLACED PERMANENT TOOTH BY PERIAPICAL LESION OF PRIMARY TEETH (유치의 치근단 병소에 의해 변위된 영구치의 자발적 맹출)

  • The infection of primary teeth occurs by caries or trauma. But, if it is not treated on time, the complication may occur such as enamel hypoplasia, discoloration, impaction, displacement and dilaceration of permanent successors. The periapical lesion on primary tooth could displace the permanent successors in any direction. Treatment options of displaced tooth are observation after extracting the primary teeth, surgical exposure orthodontic traction, transplantation and extraction. In these cases, displacement of permanent central incisor caused by the periapical lesion of primary teeth was observed. The spontaneous eruption after extraction of primary teeth was expected considering degree of displacement, development of root and direction of eruption. This case report shows spontaneous eruption of displaced permanent tooth germ was achieved with periodic examination after extraction of primary teeth.

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.
