Challenges in the Management of Breast Cancer in a Low Resource Setting in South East Asia

  • Background: Breast cancer is the second most common cancer in women in Cambodia, a low income country in South-East Asia. The Sihanouk Hospital Centre of Hope (SHCH) is a charity hospital set up by an international non-governmental organisation, HOPE Worldwide. In 2008, SHCH partnered with AmeriCares, a global health organisation to set up and deliver a breast cancer programme to provide education, diagnosis and treatment for women with breast cancer. The objective of this study is to characterise the presentation, diagnosis, treatment and outcomes of women treated under this program. Materials and Methods: A total of 215 women newly diagnosed with breast cancer from 1 March 2008 until 31 March 2011 were studied. Age at diagnosis, tumour size, histological type, tumour grade, ER, lymph node involvement, treatment modalities (surgery, radiotherapy, chemotherapy, hormone therapy) were recorded. Data on mortality at 3 years were obtained whenever possible. Results: The median age was 47 years old. Some 77.8% were diagnosed with stage 3 and 4 lesions, and 78.5% underwent mastectomy, of which 28.4% the intent was palliative. Of those whose ER status were known, only 48.3% were ER positive. Only 6 patients could afford chemotherapy while only 1 patient had radiotherapy. Hormone therapy was provided free for those who were ER positive. The overall survival rate at 3 years was 39.1%. Conclusions: Breast cancer presents at a late stage, and because treatment is suboptimal, survival is poor in Cambodia. A more aggressive approach to early detection and treatment needs to be developed to improve outcome from this potentially curable disease.

Background Level of Atmospheric Radon-222 Concentrations at Gosan Station, Jeju Island, Korea in 2011

  • Real-time monitoring of hourly atmospheric radon (Rn-222) concentration was performed throughout 2011 at Gosan station, Jeju Island, one of the least polluted regions in Korea, in order to characterize the background levels, and temporal variations on diurnal to seasonal time-scales. The annual mean radon concentration for 2011 was $2527{\pm}1356$ mBq $m^{-3}$, and the seasonal cycle was characterized by a broad winter maximum, and narrow summer minimum. Mean monthly radon concentrations, in descending order of magnitude, were Oct > Sep > Feb > Nov > Jan > Dec > Mar > Aug > Apr > Jun > May > Jul. The maximum monthly mean value (3595 mBq $m^{-3}$, October), exceeded the minimum value (1243 mBq $m^{-3}$, July), by almost a factor of three. Diurnal composite hourly concentrations increased throughout the night to reach their maximum (2956 mBq $m^{-3}$) at around 7 a.m., after which they decreased to their minimum value (2259 mBq $m^{-3}$) at around 3 p.m. Back trajectory analyses indicated that the highest radon events typically exhibited long-term continental fetch over Asia before arriving at Jeju. In contrast, low radon events were generally correlated with air mass fetch over the North Pacific Ocean. Radon concentrations typical of predominantly continental, and predominantly oceanic fetch, differed by a factor of 3.8.

  • 글로벌 위성 기반의 강수량 관측에 대한 역사는 1979년에 Arkin의 의해 제안된 IR방법에 의해 위성으로 부터 강우자료를 유도하는 개념이 도입된 이후 1987년 해양에서의 비교적 정확한 강수량 추정이 가능한 다중 채널의 마이크로파(MW) 복사계를 이용한 방법으로 위성강수 추정에 대한 연구가 활발히 진행되었다. 이 후 두 IR과 MW를 혼합한 방법에서, 또다시 1997년 TRMM위성의 PR(Precpipitation Radar)의 레이더를 이용하는 방법, 그리고 2014년 GPM 핵심 위성(GPM Core Observatory)에 탑재된 Dual PR에 의한 방법으로 위성강수의 정확도를 매우 높여가고 있다. 전지구강수관측위성(GPM, Global Precipitation Measurement Mission) 사업은 미국우주항공국(NASA)과 일본우주항공국(JAXA)의 주도로 전 지구 규모의 강수관측을 목적으로 시작되었으며, 추가 파트너로 프랑스의 CNES(French Centre National d'?tudes Spatiales), 인도의 ISRO(Indian Space Research Organisation), 미국 NOAA, 그리고 유럽연합의 EUMETSAT(European Organisation for the Exploitation of Meteorological Satellites)가 참여하고 있다. 본 연구는 2014년 4월 발사된 GPM핵심 위성의 발사에 따라 제공되는 GPM 위성강우 자료의 정확도 평가를 목적으로 하고 있다. GPM 데이터는 Level-1에서 Level-3까지 다양한 데이터를 제공하고 있으며 본 연구에서는 Level-3의 IMERG 데이터를 이용하여 위성강우의 정확도를 평가하였다. IMERG 위성강우자료는 GPM 위성군의 모든 수동 MW 데이터를 조합하여 강우량을 추정하는 데이터이다. 자료의 시간적 범위는 2015년 8월 18일~8월 20일이고 공간적 범위는 한반도 영역으로 하였으며, 자료의 정확도 평가를 위한 지상계측자료는 기상청 ASOS(Automated Synoptic Observing System)의 강수량 자료를 이용하였다. 자료분석 결과 GPM에서 제공되는 IMERG 데이터의 공간적 분포는 그림 1과 같이 전라도 지역에 많은 강수가 분포하는 것을 확인할 수 있다. 이 기간 동안의 기상청 ASOS 관측 강수량 기록은 전국적으로 1순위가 고창군 25.5mm, 2순위가 부안군 21.9mm, 3순위가 정읍 및 영광군이 19.0mm로 위성으로부터 관측된 값과 지상계측값의 공간적 분포가 매우 유사한 경향을 보이는 것으로 분석되었다. 향후 위성강우 및 지상계측강우의 시계열적 정확도와 총강우량 등의 정확도 평가를 수행할 계획이다.

Enhancing Business Continuity in the Oil and Gas Industry through Electronic Records Management System Usage to Improve Off-Site Working: A Narrative Review

  • The primary function of an electronic records management system (ERMS) is to support organisations in providing effective records management services by enabling efficient remote access to the organisations' records. This helps the organisation to continue running during emergency events, such as the COVID-19 pandemic. The need to study ERMS for accessing records remotely has increased dramatically, due to the increase in daily use. The situation arising from the COVID-19 pandemic has increased the need for implementing proper digital systems, such as ERMS, to enable efficient work processes and enhance business continuity. An ERMS has the potential to allow organisations to create records and workflows off-site. During a pandemic, the ability to structure processes digitally helps in maintaining operations remotely. This study aims to provide a narrative review of the ERMS literature with an emphasis on explaining the primary components of ERMS that act as enablers for the implementation of the system in the oil and gas sector of developing countries. The current study proposes ERMS roles and responsibilities that could enhance business continuity. The authors use a qualitative narrative review and analyse the literature related to this study and its findings. The results show that, in cases of risk or crises, staff members need to have easy access to their records and documents to remain productive. An ERMS allows professionals to remain active and work off-site. Thus, ERMS play a significant role in protecting an organisation's content through the monitoring and control over who has authorisation to access its records.

  • This paper summarizes progress on work under the Asian Pacific FRIEND(Flow Regimes from International Experimental and Network Data - APFRIEND) initiative of the International Hydrological Programme (IHP) of the United Nations Educational, Scientific and Cultural Organisation (UNESCO) from 2005 to 2009. The results of initial work on Rainfall Intensity Duration Frequency (IFD) have just been published as a volume by UNESCO. The results of work to date is concisely presented and observations made about lessons learnt on how to successfully integrate work from nine diverse countries with differing approaches to both hydrology and water resource management structures and on some possible directions for future work.

THE OPAL (OPEN POOL AUSTRALIAN LIGHT-WATER) REACTOR IN AUSTRALIA

  • The OPAL (Open Pool Australian Light-water) reactor is currently being constructed to replace HIFAR (HI-Flux Australian Reactor, commissioned in 1958) in mid-2006. HIFAR will be shutdown for decommissioning after several months of simultaneous operation with OPAL for smooth transition of operating systems and business. OPAL is a 20 MW multipurpose research reactor for radioisotope production, irradiation services and neutron beam research. The OPAL reactor uses low enriched uranium fuel in a compact core, cooled by light water and moderated by heavy water, yielding maximum thermal flux not less than $4{\times}10^{14}ncm^{-2}s^{-1}$. The reactor containment building is constructed of reinforced concrete and has been designed to protect the reactor from all external events such as seismic occurrences and impact from a hypothetical light aircraft crash. This paper describes the main elements of the reactor design and its applications.

  • In order to find out the appropriate parents for the breeding programme, twelve bivoltine and three multivoltine silkworm breeds were evaluated on the basis of multivariate selection index and isozyme analysis. Of which, four [CSR2, D6 (P), SK3, SK4] bivoltine and two multivoltine (Nistari, Cambodge) breeds were selected and breeding initiated to develop higher survival bivoltine silkworm breed suitable for tropical conditions. Among two isozyme (Esterase and acid phosphatase) analyzed, only esterase exhibited polymorphism among the bivoltine breeds. No polymorphism was observed among multivoltine in respect of esterase as well as acid phosphatase.

  • This paper discusses the potential of system dynamics modelling to support balanced scorecard. The balanced scorecard is a conceptual framework for translating an organisation's strategy into a set of performance indicators. These performance indicators are distributed across the 'classic'model's four perspective: Customers, Internal Business Processes, Financial, and Learning and Growth. This balanced scorecard, whilst having significant strength, suffers from the limitation of all performance indicator systems, namely that the interrelationships between indicators are overlooked and there is no way of taking into account the impact of delayed feedback which flows from introduction of new policy and legislative changes. System Dynamics is a methodology for understanding complex problems where there is dynamic behaviour and where feedback impacts significantly on system outcomes. System dynamics provides a rigorous basis for qualitative testing of the effects of performance indicators in complex environments such as health or social security. This can be supplemented with quantitative system dynamics simulation tools that further test the validity of indicators and the business rules implicit in them. System dynamics modelling has an important role to play in extending feedback cycle in performance measurements to a full systems approach.

Eurocode 4: A modern code for the design of composite structures

  • The European Standards Organisation (CEN) has planned to develop a complete set of harmonized European building standards. The Eurocodes, being the design standards, form part of this total system of European standards, together with standards for fabrication and erection and product standards. After a period of experimental use of the ENV(European Pre Standard)-versions of the Eurocodes, these are now converted into official EN's (European Standards). Design of composite steel and concrete buildings and bridges is covered by Eurocode 4. An overview will be given of the historic development of Eurocode 4, the structure and contents of the EN version and the present status and planning for completion. The Eurocode treatment of some selected technical items will be presented in more detail.

  • The purpose of this paper is to establish the policy for the abroad promotion of small-medium and venture businesses. The core strategy is enhancing the total sales trough the low management cost and low advertisement cost. The Partnership Management by the Export Cooperation Organisation is a model which is maximize the outputs and profits at the same time minimize the fixed costs, advertisement costs etc. The effect of synergy from the grand exhibition and partnership management will be a critical momentum to the national business that concrete and diversify the export.
