Lead Poisoning: Historical Aspects of a Paradigmatic "Occupational and Environmental Disease"

  • Lead poisoning is one of the earliest identified and most known occupational disease. Its acute effects have been recognized from antiquity when this condition principally afflicted manual workers and slaves, actually scarcely considered by the medicine of that time. The Industrial Revolution caused an epidemic of metal intoxication, urging scientists and physician of that period to study and identify specific symptoms and organ alterations related to chronic lead poisoning. During the 20th century, the acknowledgment of occupational and environmental toxicity of lead fostered public awareness and legislation to protect health. More recently, the identification of sub-clinical effects have greatly modified the concept of lead poisoning and the approaches of medicine towards this condition. Nowadays, lead poisoning is rarely seen in developed countries, but it still represents a major environmental problem in certain areas. Consequently, it may appear as a paradigm of "occupational and environmental disease," and the history of this condition seems to parallel the historical development of modern "Occupational and Environmental Health" as a more complete medical discipline.

Effects of Lead and Particulate Montmorillonite on Growth Performance, Hormone and Organ Weight in Pigs

  • Seventy-two crossbred gilts of approximately 33 kg initial weight were used in this study. The gilts were randomly assigned into three groups. The three dietary treatments were basal diet only (control group), basal diet+10 mg/kg lead, and basal diet+10 mg/kg lead+0.5% particulate montmorillonite (PM). The results showed that the addition of lead to the diet decreased significantly the body weight and feed efficiency, but PM could restore body weight and feed efficiency of gilts compared to the Pb exposure group. There were no significant differences in weights of ovaries and uteri with addition of either lead or PM to the diet. Supplementing the lead in the diet of gilts also significantly increased the concentration of lead in blood, decreased circulating lutenizing hormone (LH) and estradiol (E$_2$) levels in serum, the addition of PM to the diet effectively adsorbed and lowered lead concentration in the blood. These data suggested that lead disrupts the signals between the hypothalamus and pituitary gland in gilts, and possibly suppressed the secretion of relative growth hormone and sex hormone. On the other hand, PM may ameliorate Pb toxicity in pigs.

노인 암 환자의 접근과 관리 원칙 (How we should approach and manage older patients with cancer)

  • Cancer is the disease of aging and Korea is one of the fastest aging country in the world. Older patients have decreased organ function and stress tolerance, therefore are at high risk of functional decline and developing complication from cancer and cancer treatment. Before beginning cancer treatment, it is important to assess patients' life expectancy, whether the patient is likely to die of cancer or of other comorbidity, and also the risks with cancer treatment. In order to estimate patient's physiologic age, it is recommended to perform geriatric assessment and implement appropriate geriatric intervention together with meticulous supportive care, when planning cancer treatment for older patients. In a resource limited country such as Korea, two step approach of applying screening tool followed by geriatric assessment can be more efficient. Geriatric assessment is used to predict toxicity from cancer treatment such as surgery, radiotherapy, and chemotherapy, predict survival, and also to aid treatment decision. Number of randomized trials are ongoing to compare usual care versus oncogeriatric care, and with these results we expect to improve outcome of older patients with cancer.

  • The carcinogenic potential of steviol, a metabolite of/ stevioside (a compound that is used as a sweetener for food and drink), was examined in hamsters of both sexes. Groups of 55 male and 55 female hamsters were given diets containing steviol at 0, 100 and 500 mg/kg diet for 22 months in males and 18 months in females. After 6, 12 and 22 months in males and 18 months in females. hamsters from each group were sacrificed for hematological and biochemical tests. Growth food utilization and consumption, general appearance and mortality were similar in treated and control groups. The mean life span of hamsters given steviol was not significantly different from that of the controls. No treatment-related changes were observed in hematological, urinary and biochemical values at any stage of the study. There was no significantly altered development of neoplastic or non-neoplastic lesions attributable to steviol treatment in any organ or tissue. The highest level oj steviol in the diet which still causes no effects in hamsters was 500 mg/kg diet, under the experimental conditions used.

Di-(2-ethylhexyl) Phthalate (DEHP) and Uterine Histological Characteristics

  • Phthalates and those metabolites have long history in industry and suspected to have deficient effects in development and reproduction. These are well-known anti-androgenic chemicals and many studies have examined the effects of these compounds on male reproduction as toxins and endocrine disruptors. Uterus is a key organ for proper embryo development, successful reproduction, and health of eutherian mammals including women. To understand the effects of the phthalate, the horizontal approach with a whole group of phthalate is best but the known phthalates are huge and all is not uncovered. Di-(2-ethylhexyl) phthalate (DEHP) is the most common product of plasticizers in polymer products and studied many groups. Although, there is limited studies on the effects of phthalates on the female, a few studies have proved the endocrine disrupting characters of DEHP or phthalate mixture in female. An acute and high dose of DEHP has adverse effects on uterine histological characters. Recently, it has been revealed that a chronical low-dose exposing of DEHP works as endocrine disrupting chemicals (EDC). DEHP can induce various cellular responses including the expression regulation of steroid hormone receptors, transcription factors, and paracrine factors. Interestingly, the response of uterus to DEHP is not monotonous and the exposed female has various phenotypes in fertility. These suggest that the exposing of DEHP may causes of histological modification in uterus and of disease in female such as endometriosis, hyperplasia, and myoma in addition to developmental and reproductive toxicity.

대체의학으로서 백화사설초 급여가 돼지 생산성에 미치는 효과 (Effects of Oldenlandidae Herba as an Alternative Medicine on the Swine Productivity)

  • As control of swine consumptive disease is the key of success in pig farming business, the effect of Oldenlandidae Herba on prevention of swine diseases as an alternative medicine was investigated in-vivo. As it contains substances such as ${\beta}$-sitisterol, ursolic acid and 3-O-${\beta}$-D-giucoside, it is effective on suppression of colonic tumor in rats and also, is widely used as an preventative medicine of cancer and inflammation in Chinese medicine. In this experiment, sows and piglets on two different farms were treated with Oldenlandidae Herba, and they recoded weaning rate greater than 80%, which were higher than national average 70% (PSY 68%). Meanwhile, neither organ accumulation nor toxicity has been detected on the basis of hematology and serum chemistry. In conclusion, the extracts of Oldenlandidae Herba is expected to be an good alternative medicines for swine productivity.

  • Objectives : The fresh young leaves and dried fruits of Zanthoxylum piperitum (Korean name: Chopi) are used as diuretics, stomachies, anthelmintic and for the treatments of disorders of the digestive organ in Asia. We investigated inhibitory effects of Zanthoxylum piperitum extract on lipopolysaccharide(LPS)-induced production of nitric oxide(NO) and pro-inflammatory cytokines including $TNF-{\alpha}$ and $IL-1{\beta}$ from RAW264.7 mouse macrophage cells. Methods : After methanol extract of Zanthoxylum Fructus (Zanthoxylum extract) was pretreated in RAW264.7 cells, the cells were stimulated with LPS. Cell toxicity of Zanthoxylum extract was assayed bv MTT assay. The production of NO from the cells was measured in culture medium by Griess reaction. The production of $TNF-{\alpha}$ and $IL-1 \;{\beta}$ from the cells was measured in culture medium by ELISA. Results : Zanthoxylum Fructus extract greatly inhibited the production of inflammatory mediators such as NO, $TNF-{\alpha}$ and $IL-1{\beta}$ from LPS-stimulated RAW264.7 cells. Conclusion : This result suggests that Zanthoxylum extract may have an anti-inflammatory effect through the inhibition of inflammatory mediators.

Sprague-Dawley 랫드를 이용한 수태능 및 초기배 발생시험의 기초자료연구 (Background Data for Fertility and Early Embryonic Development Study in Sprague-Dawley Rats)

  • Historical control data have been shown to be valuable in the proper interpretation and validation of reproductive toxicology studies. The present data were compiled from rat fertility and early embryonic development studies conducted at Korea Institute of Toxicology during the 1994∼2001 period. These data were assembled in order to provide background information for the general and reproductive data collected in 11 fertility and early embryonic development studies using Sprague-Dawley rats obtain-ing from the Breeding Facility, Korea Institute of Toxicology, Korea. A total of 274 males and 274 females were used in these studies during the eight-year period. Parameters of fertility and early embryonic development included clinical sign, body weights, food consumption, organ weights, estrus cycle, copulation index, precoital time, fertility index, pregnancy index, sperm parameters, and early embryonic development parameters. Most of the values were comparable to the previous historical control data reported by other investigators. These data can be wed not only as a historical data base for the meaningful interpretation of data from reproductive and developmental toxicity studies, but also as a contribution to biological characterization of Sprague-Dawley rats.

Cisplatin에 의한 이독성(耳毒性)에서 영계출감탕(苓桂朮甘湯)의 보호 효과 (Protective Effects of Younggyechulgam-tang in Cisplatin-induced Ototoxicity)

  • The water extract of Younggyechulgam-tang (YGCGT) has been traditionally used in treatment of tinnitus in Oriental Medicine. However, little is known about the mechanism by which YGCGT rescues ototoxicity. The purpose of the present study is to investigate the protective effect of YGCGT against cisplatin-induced toxicity in HEI-OC1 auditory cells, organotypic cultures of cochlear explants from three-day postnatal rats (P3). Pretreatment with YGCGT ameliorated apoptotic death induced by cisplatin in HEI-OC1 cells and organotypic cultures of Corti's organ. YGCGT pretreatment also significantly suppressed cisplatin-induced increases in intracellular reactive oxygen species (ROS). Treatment with YGCGT resulted in an increased expression of HO-1 and Bcl-2. These results suggest that YGCGT induced HO-1 signaling pathway, which ultimately prevents free radical stresses from cisplatin and further contributes to protect auditory sensory hair cells from free radicals produced by cisplatin.

Three Weekly Versus Weekly Cisplatin as Radiosensitizer in Head and Neck Cancer: a Decision Dilemma

  • Cisplatin-based concurrent chemoradiation plays an undisputed key role as definitive treatment in unresectable patients with locally advanced squamous cell carcinoma head and neck or as an organ preservation strategy. Treatment with 100 mg/m2 3-weekly cisplatin is considered the standard of care but is often associated with several adverse events. The optimum drug schedule of administration remains to be defined and presently, there is insufficient data limiting conclusions about the relative tolerability of one regimen over the other. This review addresses regarding the optimal dose schedule of cisplatin focusing mainly on three-weekly and weekly dose of cisplatin based concurrent chemoradiotherapy in locally advanced head and neck cancer with an emphasis on mucositis, dermatitis, systemic toxicity, compliance, and treatment interruptions. To derive a definitive conclusion, large prospective randomized trials are needed directly comparing standard 3-weekly cisplatin ($100mg/m^2$) with weekly schedule ($30-40mg/m^2$) of concurrent cisplatin based chemoradiotherapy in locally advanced squamous cell carcinoma head and neck.
